  • Foreign holies in the upper aerodigestive tract are an important cause of morbity and mortality in older adults and children under 3 years of age. Fish and chicken bones are the usual foreign bodies lodged in the hypopharynx, and most of them lodge In the lower pole of a tonsil, at the base of the tongue, at the lateral wall of the pharynx, or in a vallecula. Recently, the authors experienced a case of aspiration pneumonia caused by chronic local inflammatory reaction of the posterior wall of the hypopharynx originated from a special foreign body.

집단 따돌림 현상에 나타난 청년 초기의 의복 행동 (A Study on Clothing Behavior of Early Adolescents in the School Bullying)

  • In this study, we inquire what is the rules which is implicitly used by the adolescent group about bullying phenomenon. We inquire how to interpret the phenomenon from the adolescents'viewpoint and their opinions of how appearance and clothing determine the extent of bullying. After we discuss this problem, we propose an indirect solution to the cause of bullying. We selected a middle school boy by reason that boys have been more experienced than others according to a previous study and are at a keen adolescent stage of development. Moreover, 'ichime'phenomenon is seriously reported at a middle school student stage in the case of Japan. The purpose of the study was to analyze the following : 1. Research on an actual case of bullying in a middle school student. 2. the adolescents'viewpoint on characteristics of school bullying. 3. The cause of violence and injury from school bullying. 4. How appearance and clothing affect bullying. 5. The preference and uses of clothing worn by adolescents. The process of this study brought a substantially between quantitative and qualitative research. Theses questionnaires were administered to 320 middle school students in Seoul from the 14/sup th/ to the 25/sup th/ of June. The data totaled up to 280 subjects, which consist 121 male students and 159 female students except for invalid data.

Acute Myelopathy Caused by a Cervical Synovial Cyst

  • Synovial cysts of the cervical spine, although they occur infrequently, may cause acute radiculopathy or myelopathy. Here, we report a case of a cervical synovial cyst presenting as acute myelopathy after manual stretching. A 68-year-old man presented with gait disturbance, decreased touch senses, and increased sensitivity to pain below T12 level. These symptoms developed after manual stretching 3 days prior. Computed tomography scanning and magnetic resonance imaging revealed a 1-cm, small multilocular cystic lesion in the spinal canal with cord compression at the C7-T1 level. We performed a left partial laminectomy of C7 and T1 using a posterior approach and completely removed the cystic mass. Histological examination of the resected mass revealed fibrous tissue fragments with amorphous materials and granulation tissue compatible with a synovial cyst. The patient's symptoms resolved after surgery. We describe a case of acute myelopathy caused by a cervical synovial cyst that was treated by surgical excision. Although cervical synovial cysts are often associated with degenerative facet joints, clinicians should be aware of the possibility that these cysts can cause acute neurologic symptoms.

Pleuroperitoneal communication-associated pleuritis as an uncommon cause of fever of unknown origin in a child on peritoneal dialysis: a case report

  • Pleuroperitoneal communication (PPC) is a rare mechanical complication of peritoneal dialysis (PD), which causes dialysate to move from the peritoneal cavity to the pleural cavity, resulting in pleural effusion. Typically, PPC is discovered through pleural effusion in PD patients who are not in volume overload status. A unique characteristic of the pleural effusion caused by PPC is that it is not resolved by increasing ultrafiltration by dialysis. In this report, we present a 7-year-old girl with PD after birth with the history of various infectious PD-related complications, presenting with fever ongoing for 6 months. PPC-associated pleuritis was suspected as the cause of fever, which eventually developed after long-term PD and induced complicated pleural effusion, lung inflammation, and prolonged fever for 6 months.

CASE STUDY ON SEVERELY-DAMAGED REINFORCED EARTH WALL WITH GEO-TEXTILE IN HYOGO, JAPAN Part I: Site Investigation into the cause of damage

  • Case study was carried out on the interpretation of the mechanical behavior of a severely damaged reinforced earth wall comprising geotextile with the concrete panel facing. In this part I, the outline of the damaged reinforced earth wall is in detail described. The background and cause of the damage are discussed based on the results of site investigation. The engineering properties of the fill were examined by performing various in-situ and laboratory tests, including the surface wave survey (SWS), PS-logging, RI-logging, soaking test, the direct shear box (DSB) test, bender element (BE) test, etc. The background as well as the cause for the damage of the wall may be described such that i) a considerable amount of settlement took place over a 3m thick weak soil layer in the lower part of the reinforced earth due to seepage of rainfall water, ii) the weight of the upper fill was partially supported by the geo-textile hooked on the concrete panels (n.b., named conveniently "hammock state" in this paper), and iii) the concrete panels to form the hammock were severely damaged by the unexpectedly large downwards compression force triggered by the tension force of the geotextile. The numerical simulation for the hammock state of the wall, together with counter-measures to re- stabilize the wall is subsequently described in Part II.

고의에 의한 승용차폭발 사례의 분석 (A Case Analysis of Intentional Car Explosion)

  • 이 논문에서는 운전자(남)와 동승자(여)가 탑승하여 하천 둔치의 공원에 정차 중 차실 내에서 폭발사고가 발생한 승용차에 대해 폭발사고 원인을 분석하였다. 폭발사고가 발생한 직후 운전자와 동승자가 화상을 입고 119구급대에 의해 병원에 이송될 때는 119구급대원에게 시너를 뿌려놓은 상태에서 운전자와 동승자가 다투는 중에 폭발사고가 발생하였다고 진술하였는데, 며칠 경과한 후부터 운전자와 동승자는 플라스틱 병 속에 들어 있던 휘발유가 엎질러져 시거 잭으로 흘러들어가서 시거 잭을 빼려다가 시거 잭을 실수로 눌러서 폭발사고가 발생하였다고 주장하였다. 실수로 인해 폭발사고가 발생하였다면 자동차보험금을 지급받을 수 있고 고의에 의해 폭발사고가 발생하였다면 보험금 지급이 안되므로 폭발사고의 원인이 고의에 의한 것인가 실수에 의한 것인지가 쟁점이 되었다. 폭발한 승용차에 대한 구체적인 조사 분석과 운전자 및 동승자의 진술에 대한 화재과학적 분석을 통해 이 자동차 폭발사고는 운전자의 고의에 의해 발생한 것임을 입증하였다.

Necrosis of intact premolar caused by an adjacent apical infection: a case report

  • Although periapical inflammatory lesions are usually resulted by infection in the root canal system, this rare case showed that a periapical lesion related to an infected tooth may cause pulpal necrosis in adjacent intact tooth, with no history or clinical signs of caries, disease, trauma or developmental anomaly. This case also suggests that the periapical lesion can be treated conservatively, without surgical intervention. Furthermore, this case highlights the importance of prompt treatment of apical periodontitis before the lesion becomes extensive as well as follows up of large lesions.

  • Whenever Information leak case had been happened, even though IT Infra systems were reinforced, those cases of kind were not decreased. It meant that the causes are not only the fault of IT, but also other things. So that the information leak cases should have been studied by the interdisciplinary way. There was a point of view for the Business Ethics which needed to be studied in the interdisciplinary way. Financial Information Leak Case of three representative Card Companies in Korea which is happened in 2014 was a case which was composed by many problems. These were bad circumstances, half-educated, lower salary, and the most of all, the C-Levels' ignorance of the law. By studying the CLD: Causal Loop Diagram of Business Ethics and those mentioned factors, few meanings were discovered. Firstly, this case was not a accident, but a predeterminate issue at all, because of the structural unethical corruption. Secondly, main reason of this case was not only criminals who leaked information, but also managers, especially CEO who didn't obey the law, forced that to employees. Thirdly, although those companies had moral guide and did CSR activities, it was not help to protect this information leak case. This study shows that the important thing was not the action for showing, but effective action to management for sustainability.

Hemifacial Spasm Caused by Epidermoid Tumor at Cerebello Pontine Angle

  • Hemifacial spasm (HFS) is almost always induced by vascular compression but in some cases the cause of HFS are tumors at cerebellopontine angle (CPA) or vascular malformations. We present a rare case of hemifacial spasm caused by epidermoid tumors and the possible pathogenesis of HFS is discussed. A 36-year-old female patient presented with a 27-month history of progressive involuntary facial twitching and had been treated with acupuncture and herb medication. On imaging study, a mass lesion was seen at right CPA. Microvascular decompression combined with mass removal was undertaken through retrosigmoid approach. The lesion was avascular mass and diagnosed with an epidermoid tumor pathologically. Eventually, we found a offending vessel (AICA : anterior inferior cerebellar artery) compressing facial nerve root exit zone (REZ). In case of HFS caused by tumor compression on the facial nerve REZ, surgeons should try to find an offending vessel under the mass. This case supports the vascular compression theory as a pathogenesis of HFS.
