Diffuse Alveolar Hemorrhage

  • Diffuse alveolar hemorrhage (DAH) is a life-threatening and medical emergency that can be caused by numerous disorders and presents with hemoptysis, anemia, and diffuse alveolar infiltrates. Early bronchoscopy with bronchoalveolar lavage is usually required to confirm the diagnosis and rule out infection. Most cases of DAH are caused by capillaritis associated with systemic autoimmune diseases such as anti-neutrophil cytoplasmic antibody-associated vasculitis, anti-glomerular basement membrane disease, and systemic lupus erythematosus, but DAH may also result from coagulation disorders, drugs, inhaled toxins, or transplantation. The diagnosis of DAH relies on clinical suspicion combined with laboratory, radiologic, and pathologic findings. Early recognition is crucial, because prompt diagnosis and treatment is necessary for survival. Corticosteroids and immunosuppressive agents remain the gold standard. In patients with DAH, biopsy of involved sites can help to identify the cause and to direct therapy. This article aims to provide a general review of the causes and clinical presentation of DAH and to recommend a diagnostic approach and a management plan for the most common causes.

  • In this paper, diagnosis for 85 high voltage induction motors which have operated for more than 20 years in 18 wide area water supply offices were applied and the results of diagnosis were analysed. Furthermore, main factors that would be affecting rotor defects were selected and correlations between dependent variables which was magnitude for sideband frequency on current during operation and independent variables such as starting characteristic, operating time, number of operation, load factor, maker, rotation speed, capacity were analysed. It was clear that factors including starting characteristic, number of operation, maker, rotation speed caused break by correlation analysis. From this, regression equation was deduced through regression analysis. Based on suggested regression equation, it is applied usefully that we can estimate the condition of rotor without onsite diagnosis and plan the schedule of diagnosis.

  • The metal processing system usually consists of various components such as motors, work rolls, backup rolls, idle rolls, sensors, etc. Even a simple fault in a single component in the system may cause a serious damage on the final product. It is, therefore, necessary to diagnose the faults of the components to detect and prevent a system failure. Especially, the defects in a work roll are critical to the quality of strip. In this study, a new 3-D diagnosis method was developed for roll shape defects in rolling processes. The new method was induced from analyzing the rolling mechanism by using a rolling force model, a tension model, the Hitchcock's equation, and measurement of the strip thickness, etc. Computer simulation shows that the proposed method is very useful in the diagnosis of the 3-D roll shape.

  • Plantar fasciitis is the most common cause of heel pain. The diagnosis of plantar fasciitis is primarily based on the presentation of symptoms and physical examination. Patients usually complain of heel pain at the medial calcaneal tubercle when taking their first step in the morning or when walking after resting. Diagnostic imaging is rarely required for the initial diagnosis of plantar fasciitis; however, it can be used for differential diagnosis. Conservative treatments, such as stretching, rest, ice massage, oral analgesics, foot orthotics, use of night splint, and corticosteroid injection, may be effective. The majority of patients report improvement with conservative treatments, and those who show no response from conservative treatments for a duration of six months or longer can consider extracorporeal shock wave therapy or surgery.

  • Sjogren syndrome is a chronic systemic autoimmune disorder that chiefly involves the salivary gland and the lacrimal gland, resulting in xerostomia and xerophthalmia. Although the exact cause of the disease is not early diagnosis, treatment and observation must be emphasized because of its poor prognosis, such as the high occurrence of malignant lymphoma and other autoimmune disease that may be accompanied. In the present case, a twenty-year-old woman whose chief complaint was multiple dental hard tissue loss and xerostomia, which was misdiagnosed as iron deficiency anemia at first, but through re-evaluation and differential diagnosis it was Sjogren syndrome. the diagnosis approach was discussed in this report, suggesting that Sjogren syndrome should be considered as a differential diagnosis in a with xerostomia.

Pathological interpretation of connective tissue disease-associated lung diseases

  • Connective tissue diseases (CTDs) can affect all compartments of the lungs, including airways, alveoli, interstitium, vessels, and pleura. CTD-associated lung diseases (CTD-LDs) may present as diffuse lung disease or as focal lesions, and there is significant heterogeneity between the individual CTDs in their clinical and pathological manifestations. CTD-LDs may presage the clinical diagnosis a primary CTD, or it may develop in the context of an established CTD diagnosis. CTD-LDs reveal acute, chronic or mixed pattern of lung and pleural manifestations. Histopathological findings of diverse morphological changes can be present in CTD-LDs airway lesions (chronic bronchitis/bronchiolitis, follicular bronchiolitis, etc.), interstitial lung diseases (nonspecific interstitial pneumonia/fibrosis, usual interstitial pneumonia, lymphocytic interstitial pneumonia, diffuse alveolar damage, and organizing pneumonia), pleural changes (acute fibrinous or chronic fibrous pleuritis), and vascular changes (vasculitis, capillaritis, pulmonary hemorrhage, etc.). CTD patients can be exposed to various infectious diseases when taking immunosuppressive drugs. Histopathological patterns of CTD-LDs are generally nonspecific, and other diseases that can cause similar lesions in the lungs must be considered before the diagnosis of CTD-LDs. A multidisciplinary team involving pathologists, clinicians, and radiologists can adequately make a proper diagnosis of CTD-LDs.

  • The RCP is one of the main components in nuclear power plants and plays an important role in circulating coolant to the RCS system. Currently, nuclear plants are monitored using various monitoring systems. However, since they operate independently according to their functional purpose, it is not able to analyze vibration and operation/performance information comprehensively, and thus failure diagnosis accuracy is limited. In addition, these systems do not provide some important information (such as fault type, parts and cause) necessary for emergency actions, but provide only alarm information. To improve these technical problems, this study proposes a diagnosis technique (M/L, Rule-based model, Data-driven model, Narrow band model) and methodology for comprehensive analysis.

  • Benign monomelic amyotrophy(BMA) is an uncommon cause of progressive mildly disabling atrophy and weakness of a limb. It predominantly affects the distal upper limb of young men. I present two men with BMA of the lower extremities. Although the disorder seemed clinically confined to a arm or leg, I confirmed by electromyography evidence of denervation in the other extremities. I reviewed the literatures and discuss the differential diagnosis. BMA is a diagnosis of exclusion that requires consideration in men with unilateral arm or leg atrophy and weakness.

  • NPHP is the most common monogenic cause of CKD in children or adolescents. Extra-renal symptoms often accompany, therefore examination of retina, hearing, and skeleton is necessary in patients with CKD with insidious onset. Genes involved in NPHP-RC are mostly related in primary cilia. While genetic diagnosis is necessary for definitive diagnosis, there is no curative treatment.
