A Study on Development of Root Cause Analysis Map In Laboratory (연구 실험실 사고의 근본원인분석 Map 개발에 관한 연구)

  • The continuous fatal accidents like explosion or fire cause huge losses of both life and property in laboratories even though safety management system has been built and periodical checkups and safety diagnosis have been implementing in universities and enterprise-affiliated research institutes since Act on the Establishment of Safe Laboratory Environment was enacted in 2005 to prevent accidents in research laboratories. Cause analysis and safety management measures to prevent recurrence of accidents are urgently needed because accidents in research laboratories occur repeatedly with similar contents. This study will show results of analysis on incidents and accidents occurred in laboratories in universities and enterprise-affiliated research institutes using Root Cause Analysis Method and propose classified map of cause investigation and improvements so as to improve safety management in research laboratories.

A retrospective analysis of etiology and outcomes of hemophagocytic lymphohistiocytosis in children and adults

  • Background: Hemophagocytic lymphohistiocytosis (HLH) is a rare but severe, life-threatening inflammatory condition if untreated. We aimed to investigate the etiologies, outcomes, and risk factors for death in children and adults with HLH. Methods: The medical records of patients who met the HLH criteria of two regional university hospitals in Korea between January 2001 and December 2019 were retrospectively investigated. Results: Sixty patients with HLH (35 children and 25 adults) were included. The median age at diagnosis was 7.0 years (range, 0.1-83 years), and the median follow-up duration was 8.5 months (range, 0-204 months). Four patients had primary HLH, 48 patients had secondary HLH (20 infection-associated, 18 neoplasm-associated, and 10 autoimmune-associated HLH), and eight patients had HLH of unknown cause. Infection was the most common cause in children (14/35, 40.0%), whereas neoplasia was the most common cause in adults (13/25, 52.0%). Twenty-eight patients were treated with HLH-2004/94 immunochemotherapy. The 5-year overall survival (OS) rate for all HLH patients was 59.9%. The 5-year OS rates for patients with primary, infection-associated, neoplasm-associated, autoimmune-associated, and unknown cause HLH were 25.0%, 85.0%, 26.7%, 87.5%, and 62.5%, respectively. Using multivariate analysis, neoplasm-induced HLH (p=0.001) and a platelet count <50×109/L (p=0.008) were identified as independent risk factors for poor prognosis in patients with HLH. Conclusion: Infection was the most common cause of HLH in children, while it was neoplasia in adults. The 5-year OS rate for all HLH patients was 59.9%. HLH caused by an underlying neoplasm or a low platelet count at the time of diagnosis were risk factors for poor prognosis.

  • Nuclear power plant has many external fire hydrants that have to operate in the state of emergency such as facility fire, forest fire. The valve stem of one among them was broken 3 times for 4 years. It had long valve stem and operated under high water pressure. The elongation and the tensile strength for the broken valve stem was measured to examine the defect of material property. And the vibration level and the natural frequencies was detected to check the resonance. As the result of a diagnosis, the cause of this fault is proven buckling of long valve stem.

Management of failed periodontal surgical intervention for a furcal lesion with a nonsurgical endodontic approach

  • As long as the prognosis of teeth remains a matter of concern, the endodontic-periodontal relationship will be considered a challenge for the clinician. Many etiologic factors, including bacteria, fungi, and viruses, plus other contributing factors, such as trauma, root resorptions/perforations, and dental malformations, play a role in the co-occurrence of endodontic and periodontal lesions. Whatever the cause, a correct diagnosis on which to base the treatment plan is the key to successful maintenance of the tooth. This article reports the successful endodontic management of a furcation lesion in a mandibular molar that was nonresponsive to a previous periodontal surgical graft. The case had presented a diagnostic challenge for the clinicians, and this article reviews the key points that can lead to a correct diagnosis and treatment planning.

  • In power system, substation facilities have become too complex and larger according to an extended power system. Also, customers require the high quality of electrical power system. However, some facilities become old and often break down unexpectedly. The unexpected failure may cause a break in power system and loss of profits. Therefore it is important to prevent abrupt faults by monitoring the condition of power systems. Among the various power facilities, power transformers play an important role in the transmission and distribution systems. In this research, we develop intelligent diagnosis technique for predicting faults of power transformer by FCM(Fuzzy c-means) and Euclidean based distance measure. The proposed technique make it possible to measures the possibility and degree of aging as well as the faults occurred in transformer. To demonstrate the validity of proposed method, various experiments are performed and their results are presented.

  • Tetanus is a fatal disease caused by the infection Clostridium tetani found in animal feces and soil. It is a vaccine-preventable disease and rarely occurs in developed countries. However, approximately 30 cases still occur annually in South Korea. Tetanus, commonly called lockjaw, cause contraction of the masseter muscles in the early stage, resulting in trismus as the first symptom. As it progresses, spasm extends to various muscles in the face, neck, shoulder, and back, leading to distorted facial expression, dysphagia, backward arching of the body, dyspnea, and even death. Early diagnosis of tetanus is critical because it can quickly become fatal if left untreated. We present a case of trismus caused by tetanus and emphasize the importance of early diagnosis of acute trismus.

  • Since the first outbreaks of Newcastle disease ( ND) in Korea was reported in 1927, the disease has been a major problem in Korean poultry industry. Several attempts related ND prevention and controls have been tried and significantly reduced the ND outbreaks. However, recently frequencies of ND outbreaks have been increased throughout the country since 1988. Many assumptions such as variances of ND virus pathogenicity, decreased efficacy of ND vaccine, incorrect vaccination program etc, has been issued and discussed. Therefore, to clearly understand this recent situation of ND outbreaks in Korea, we reviewed all available past informations related with ND outbreaks in Korea and compared the virulence of recently isolated ND viruses with that of a reference one. Also, we discussed about some problems related with serological diagnosis. administration methods of vaccine and vaccination program.

  • Hyuk An, M.D.Atrial myxoma constitutes the most significant of all cardiac neoplasm. About 75% occur in the left atrium and 25% in the right. They may cause severe and progressive disease mimicking valvular heart disease. But recently attention to differential point in the clinical features along with various diagnostic techniques have allowed pre-operative diagnosis in the most cases. From April, `77 to Aug., `78, three cases of left atrial myxoma and one case of right atrial myxoma were operated in Seoul National University Hospital. Pre-operative diagnosis was established with echocardiography and cardiac angiography. In all cases, tumors were resected with cardiopulmonary bypass successfully, and discharged with good results.

  • The huge and complicated plants such as nuclear power stations are likely to cause the operators to make mistakes due to a variety of inexplicable reasons and symptoms in case of emergency. That’s why the prevention system assisting the operators is being developed for. First of all. I suggest an improved fuzzy diagnosis. Secondly, I want to demonstrate that a classification system of nuclear plant’s accident investigating the causes of accidents foresees possible problems, and maintains the reliability of the diagnostic reports in spite of improper working in part. In the event of emergency in a nuclear plant, a lot of operational steps enable the operators to find out what caused the problems based on an emergent operating plan. Our system is able to classify their types within twenty to thirty seconds. As so, we expect the system to put down the accidents right after the rapid detection of the damage control-method concerned.

Sudden Foot Drop Caused by Foraminal Gas Pseudocyst

  • A foraminal gas pseudocyst is a rare cause of lumbar radiculopathy. The association with a sudden foot drop has not been previously reported. Here, a 67-year-old woman with sudden foot drop on the left side is reported. Computed tomography and magnetic resonance imaging identified a foraminal gas containing lesion compressing the left L5 root at the L5-S1 foramen. The foraminal gas containing lesion compressing the L5 ganglion was successfully removed by the posterior approach. The histological diagnosis was a gas pseudocyst. This unique case of surgically proven gas pseudocyst indicates that it should be included in the differential diagnosis of patients presenting with sudden foot drop.
