Clinical and Mycological Studies on Tinea cruris (완선(頑癬)의 임상(臨床) 및 균학적(菌學的) 연구(硏究))

  • The authors performed clinical and mycological studies on Tinea cruris of 842 outpatients who had visited the dermatology clinic of Seoul National University Hospital from 1975 to 1979. 1) The incidence of Tinea cruris was 4.4% in maximum and 3.0% in minimum of the total superficial mycotic diseases cases, so there are no remarkable changes of the yearly prevalence rate. 2) Tinea cruris was more frequent among males, and the male cases were 15.2 times higher than those of female cases. 3) By the monthly distribution, Tinea cruris shows most high in summer season (June, July and August). 4) The age distribution group for Tinea cruris varied from the first to the eighth decade, but most of them were in their third decade. 5) Concurrent infection of Tinea cruris with other types of dermatophytoses(Tinea) was noted in 19.0% of the cases. Patients with Tinea cruris and Tinea pedis were most highly observed in 51.3%, and on next. Tinea cruris and Tinea corporis 25.6% in all of the concurrent infection cases. 6) The strains were identified by culture on ordinary Sabouraud's glucose agar media and abtained three species and 99 strains. a) Trichophyton rubrum was most common causative organism of Tinea cruris. Trichophyton rubrum was isolated 64 strains (64.6%). b) Trichophyton mentagrophytes was isolated 27 stains (27.3%) and Epidermophyton floccosum was 8 strains (8.1%).

A Case of Pyogenic Liver Abscess Caused by Staphylococcus epidermidis in a Healthy Child (건강한 소아에서 발생한 Staphylococcus epidermidis에 의한 화농성 간농양 1례)

  • Staphylococcus epidermidis is a normal inhabitant of skin, throat, mouth, vagina, and urethra. It is not usually pathogenic, particularly in immunocompetent hosts. This report describes a case of a pyogenic liver abscess caused by Staphylococcus epidermidis in a healthy 12-year-old boy. He was admitted to Seoul St. Mary's Hospital with intermittent fever for 6 days. Findings on abdominal computed tomography (CT) showed a mass measuring $7.0{\times}6.5$ cm in the right hepatic lobe. Culture of the abscess resulted in growth of Staphylococcus epidermidis as a causative organism. The patient was successfully treated with intravenous administration of antibiotics and percutaneous drainage of the abscess.

  • Anthrax caused by Bacillus anthracis is one of the most important zoonotic diseases in the worldwide. To control and prevent the disease effectively, several methods such as development of a fast and specific diagnostic method and vaccine, education etc, have been carried out. However, it still has a problem in the control and prevention. To control, the most important method is the prevention of direct or indirect contact of the causative agent with susceptible host. Therefore, we developed a fast and specific detection method, polymerase chain reaction, of B anthracis from soil and infected animals because the organism could survive long time in the environment including soil due to formation of spore. With the method, virulence genes of B anthracis were successfully amplified from experimentally infected soil and mice. Up to $4.2{\times}10$ of the organisms per gram could be detected with the PCR method from experimentally infected soil. These results suggested that this PCR method could be effectively used not only to detect B anthracis in soil and infected animal but also to provide the information to prevent the disease.

  • Gambierdiscus toxicus, Adachi et. Fukuyo, is a benthic ciguatoxin-producing armored dinoflagellate, often attached to macroalgae. This organism is the primary causative agent of ciguatera fish poisoning which occurs in tropical and subtropical regions. However, regardless of the fact that the population of Gambierdiscus spp. has expanded to such temperate areas from sub-trophic and trophic areas, monitoring of G. toxicus has been lacking in the Korean coastal waters of temperate areas. This study was performed at the uninhabited Baekdo Islands off the southern coast of Korea and at Seopsom Island in the vicinity of Seogwipo, Jeju Province during April and May, 2011. Cell densities of Gambierdiscus spp. on macroalgae at Baekdo and Jeju Island ranged from zero to 56.4 cells $g^{-1}$. Maximum density was recorded on the brown alga Cladophora japonica at St. 3 of Jeju Island. In particular, the cell densities of Gambierdiscus spp. were influenced by the substrate characteristics of macroalgae. In the future, the continuous monitoring of toxic benthic dinoflagellate is necessary to predict and prevent ciguatera poisoning in Korean coastal waters.

Studies on Immuno-responses of Eel, Anguilla japonica-I. Serological Studies on Edwardsiella tarda (뱀장어의 면역응답에 관한 연구-I 에드와드병 원인균의 혈청형에 관한 연구)

  • Edwardsiella tarda, the causative organism of Edwardsiellosis, was isolated from the water and the eel, Anguilla japonica of the recirculating eel culture ponds in 1988. E. tarda was always isolated from the water of eel culture ponds in the number of log 2.30~log 4.78 CFU/$m{\ell}$ without distinguishable seasonal characteristics. The counts of isolated E. tarda were changed with the health condition of the eel, i. e., a number of E. tarda were isolated from all checked organs, intestine contents, liver, spleen and kidney of the diseased eel, however, it was hardly isolated from spleen and kidney of the healthy eel. The isolates were classified according to o-agglutinating test into three serotypes (I, II, III). There were no distinct differences in the composition of the serotypes between intestine contents of eel and pond water samples(I, 52.4~53.3% ; II, 20.0~23.8% ; III, 4.8~6.7%). Serotype I was ascendant in the spleen(66.7%) and the kidney(73.7%) samples. By experimental challenge, most of serotype I proved highly virulent to eel in comparison with the other serotypes tested.

A Case of Equine Endometritis Caused by Candida albicans (Candida albicans 에 의한 말의 자궁내막염 1예)

  • A seven-year old mare had endometritis after history of abortion and repeated intra-uterine infusion of antibiotics. She showed hyperemia and oedema of the cervical mucosa with grayish white purulent discharge. Candida albicans was determined to be the causative agent of the endometritis. The diagnosis was established by the direct demonstration of the pathogen in the uterine exudate and its isolation in pure, heavy and luxuriant growth. In vitro disc diffusion test showed the organism was sensitive to all the four antifungal drugs tested, which are amphotericin B, clotrimazole, fluconazole and nystatin. The intrauterine nystatin infusion was found very effective as C. albicans could not be recovered 7 days after the last treatment.

  • Acute epiglottitis is one of the common conditions of upper airway obstructions in children. The clinical course is not only fulminant but possibly fatal, resulting from massive swelling of supraglottic structures especially epiglottis caused by Hemophilus influenzae. This is so because early recognition and careful airway management is always desired. Numerous reports in the literature reveals that a majority of cases occurs mainly in young children under six years of age, however, recent reports of acute epiglottitis in adult is increasing. We present a series of acute epiglottitis in adult and a review of the literature. We also discuss the differences between acute epiglottitis in adult and in children in terms of clinical course, causative organism, management and the results of treatment.

Rhinovirus Associated Severe Respiratory Failure in Immunocompetent Adult Patient

  • Rhinovirus infection is typically associated with the common cold and has rarely been reported as a cause of severe pneumonia in immunocompetent adults. A 55-year-old previous healthy woman, who consumed half a bottle of alcohol daily, presented with respiratory failure after one week of upper respiratory infection symptoms. Radiography revealed bilateral, diffuse ground glass opacity with patchy consolidation in the whole lung field; bronchoalveolar lavage fluid analysis indicated that rhinovirus was the causative organism. After five days of conservative support, the symptoms and radiographic findings began to improve. We report this rare case of rhinovirus pneumonia in an otherwise healthy host along with a review of references.

In vivo and in vitro efficacy of florfenicol, terbinafine, and mometasone furoate topical otic solution for the treatment of canine otitis externa

  • This study evaluated the efficacy of a new, single-dose otic solution combining florfenicol, terbinafine, and mometasone furoate for the treatment of canine otitis externa (OE) in vitro and in vivo. Forty-one client-owned dogs with OE were included in the study and divided into a treatment group that received the test solution and a negative control group that received a normal saline solution. On day 0, the dogs were treated either with the test or the control solution and evaluated over 30 days. Clinical efficacy was evaluated by clinical signs and cytological organism counts. In vitro antimicrobial susceptibility was evaluated by the minimum inhibitory concentration (MIC). After treatment with the test solution, clinical signs continuously decreased and cytological scores were significantly reduced. The results of MIC testing showed that the test solution was potent against the common pathogenic causes of canine OE. In this study, the most common causative pathogens were Staphylococcus pseudintermedius, Pseudomonas spp. and Malassezia pachydermatis. No issues related to safety were identified. Based on these results, this new ototopical drug can be used as first line treatment for canine OE.

Comparison of Infective Endocarditis between Children and Adults with Congenital Heart Disease: A 16-Year, Single Tertiary Care Center Review (선천성 심장병을 가진 소아와 성인에서 발생한 감염성 심내막염의 비교: 1개 3차 병원의 16년간 경험)

  • Purpose: This study aimed to evaluate and compare the characteristics of infective endocarditis (IE) between children and adults with congenital heart disease (CHD) at a single tertiary care center. Methods: In this retrospective medical record review, we extracted the demographic characteristics, diagnostic variables, and outcomes of patients diagnosed with IE and CHD between 2000 and 2016. Results: We identified a total of 14 pediatric patients (nine male; median age at diagnosis, 3 years). Of the 14 patients, six had a history of previous open heart surgery, while four had undergone tetralogy of Fallot repair, with transannular patch or Rastelli procedure. Among the 10 children with positive blood cultures, the most common isolated organism was Staphylococcus spp. (8/10, 80%). Eleven adult patients had IE and CHD. Among the adult patients, only four were diagnosed with CHD before IE, and ventricular septal defect was the most common CHD. The most common isolated organism was Streptococcus spp. (6/11, 55%). Compared with adult patients, pediatric patients had a higher incidence of previously diagnosed CHD (P=0.001), with Staphylococcus spp. as the causative organism (P=0.027). The median duration between the onset of symptoms and diagnosis of IE was 9 days in children and 42 days in adults (P=0.012). Conclusions: Significant differences with regard to the diagnosis and progress of IE were observed between children and adults. Age-adjusted and systematic reassessment may be necessary for the diagnosis and management of IE.
