Ischemic Infarcion Model by Middle Cerebral Artery Occlusion using Allogenic Blood Clot in Beagle Dogs (비글견에서 동종혈전 색전술을 이용한 중간대뇌동맥의 허혈성 뇌경색 모델)

  • The purpose of this study was to establish reproducible ischemic infarction model using allogenic blood clot in beagle dogs and identify induced ischemic lesion after middle cerebral artery occlusion using magnetic resonance imaging (MRI) and histopathologic findings. Twenty eight male beagle dogs with no evidence of neurologic disease were experimented. Allogenic embolus was made using a healthy beagle dog. After internal carotid artery (ICA) was exposure, 16G catheter was introduced through the ICA. The dog was administered 0.3 ml blood clot for 15 seconds followed by 3 ml of saline for 15 seconds. MRI scans were performed with 1.5T to evaluate ischemic lesion at 7 days after middle cerebral artery occlusion procedure. Evaluation parameters of MRI include location, distribution, infarction type, margin, shape, mass effect and intensity of T1-weighted imaging (T1WI), T2-weighted imaging (T2WI), fluid attenuated inversion recovery (FLAIR) sequence, diffusion weighted imaging (DWI) and apparent diffusion coefficient (ADC). On MRI, all dogs (28/28) showed focal or multifocal lesion including telencephalon and thalamus lesions, especially caudate nucleus (24/28). These lesions had well-defined margin from adjacent brain parenchyma, none or mild mass effect and various shape. Most of dogs appeared hyperintensity on T1WI, T2WI, FLAIR, and DWI/ADC, corresponding to chronic infarction. These lesions were histopathologically confirmed atrophic changes and unstained lesion. In conclusion, MRI is the useful method to provide information about ischemic infarction in dogs and the best reproducible ischemic infarction model was developed by using allogenic blood clot.

Influence of Caffeine on Dopamine D$_1$and D$_2$Receptor mRNAs Expression in Rat Brain (도파민 D$_1$과 D$_2$수용체 mRNAs의 발현에 미치는 카페인의 영향)

  • The caffeine intake cause a local or wide ranges of convulsion and it is associated with release of dopamine (DA) receptors into the brain striatum. However, the effect of caffeine addiction on expression of DA receptors gene in the rat caudate-putamen (CPu), nucleus accumbens (NAc), and olfactory tubercle (OTu) has not been elucidated. In this study, we examined the influence of caffeine addiction on DA D $_1$and D$_2$receptor mRNAs after the treatment of caffeine for four weeks. Using the specific antisense ribo-probes for DA D$_1$and D$_2$receptor cDNAs, in situ hybridization was performed on the CPu, NAc, and OTu of the adult male Sprague Dawely rats. In caffeine-treated group, DA D$_1$and D$_2$receptor mRNAs were highly increased in CPu, NAc, and OTu. The expression density of DA D$_1$receptor mRNAs were 2.52${\pm}$1.40 (CPu), 2.78${\pm}$1.69 (NAc), and 3.91${\pm}$1.28 (OTu) in control group and 7.76${\pm}$2.09 (CPu), 4.2 ${\pm}$1.85 (NAc), and 8.21${\pm}$1.72 (OTu) in caffeine-treated group. The expression density of DA D$_2$receptor mRNA was 2.32${\pm}$1.52 (CPu), 2.63${\pm}$2.11 (NAc), and 3.61${\pm}$1.43 (OTu) in control group, and 6.41${\pm}$1.82 (CPu), 6.89${\pm}$1.32 (NAc), and 6.82${\pm}$1.18 (OTu) in caffeine-treated group. DA D$_1$receptor mRNA was higher expressed than DA D$_2$ receptor mRNA in CPu and NAc. These results suggest that caffeine reacts as a upregulator of the expression of DA D$_1$and D$_2$receptor mRNA among the neurotransmitters.

Effects of Hyulboochucke-tang on the Collagenase-Induced Intracerebral Hemorrhage in Rats (혈부축어탕이 교원효소로 유발된 흰쥐의 뇌출혈에 미치는 영향)

  • Objectives The purpose of this study was to investigate the effect of Hyulboochucke-tang on the collagenase induced intracerebral hemorrhage in white rats. Methods To identify the effect of the Hyulboochucke-tang on intracerebral hemorrhage, intracerebral hemorrhage was induced in the right caudate nuclei of white rats. For normal group (n=12) and comparative group (n=12), saline was dosed, and vaccum evaporated Hyulboochucke-tang extract was dosed to treatment group (n=12), 3 and 10 days after the collagenase injection, the body weight, the brain weight, the size of hematoma, the size of the area of malacia, the number of apoptotic cell and the change in pathological histology were observed. Results 3 days after the injection, the brain weight(g) was considerably decreased in treatment group (n=12) compared to comparative group (n=12). The brain weight after 10 days of the injection was also considerably decreased in treatment group (n=6) against comparative group (n=6). The cross section(mm) of cerebral malacia after 10 days of the injection was considerably decreased in treatment group (n=6) compared to comparative group (n=6). The number of apoptotic cell in normal intracerebral around the area of malacia did not show considerable change between treatment group and comparative group. 12 days after the injection, the multiplication of gitter cells, astrocyte and newly formed capillaries around the area of malacia was distinct. Conclusions On the basis of these results, We sugggest that Hyulboochucke-tang controls swelling caused by hemorrhage and contributes to absorption of hematoma by multiplication of newly formed capillaries and recovery of damaged cerebral tissue by multiplication of gitter cells and astrocyte.

Digital subtraction angiography(DSA) of hepatic artery using selective catheterization technique in beagle dogs (비글견에서 선택적 카테터 삽관법을 이용한 간동맥의 디지털 감산 혈관조영술)

  • The aim of this study was to establish selective hepatic artery catheterization technique through percutaneous femoral artery puncture and to offer digital subtraction angiography (DSA) of hepatic artery in beagle dogs. Percutaneous femoral artery puncture was performed with Sheldinger's method. Microferret$^{TM}$-18 Infusion catheter(William, Cook, Europe) was introduced into abdominal aorta. Then, under fluoroscopy, iopamidol 370(Bracco, Italy) was injected to identify celiac artery and 'J' shaped guide wire was introduced into celiac artery. Catheter could be introduced into celiac artery through guide wire. In this manner, catheter was located at the insertion of hepatic artery and DSA was performed. In DSA of beagle dogs, hepatic artery which was divided into lateral branch, right-medial branch, right-lateral branch of hepatic artery, cystic artery and gastroduodenal artery was opacified without superimposition of any other body structure and so was the parenchyme of liver afterward. In autopsy angiographic finding of resected liver, cystic artery, caudate branch, lateral branch, right-medial branch, right-lateral branch and quadrate branch of hepatic artery were identified. It was concluded that selective hepatic artery catheterization technique was a minimally invasive method that facillitated the approach of hepatic artery and DSA was an excellent tool to visualize the vessle of liver in dogs.

Taxonomy of tribe Neillieae (Rosaceae): Neillia (나도국수나무족(장미과)의 분류: 나도국수나무속)

  • Neillieae was traditionally recognized as a small tribe in Rosaceae, which consists of Neillia, Stephanandra, and Physocarpus. Recently, Stephanandra was merged into Neillia based on molecular phylogenetic analyses, meaning that Neillieae now contains Neillia and Physocarpus. The genus Neillia is distinguished from Physocarpus by ovate to lanceolate leaves with acuminate to caudate apices, racemose or paniculate inflorescences, and a unicarpellate (rarely bicarpellate) gynoecium. Plants of Neillia are distributed from the Himalayas across China and Korea to Japan in the east, and south to Indonesia. This study provides a taxonomic treatment of Neillia based on a morphological examination of herbarium specimens, including types, and field observations, as the second part of the taxonomic revision of the tribe Neillieae. A summary of the phylogeny of Neillia, keys to the species, nomenclatural reviews, descriptions, distribution maps, and lists of specimens examined are provided. Twelve species with ten varieties are recognized in Neillia. A lectotype was designated here for the following species: N. sinensis f. glanduligera and N. thyrsiflora.

A Study on the Effects of Position Change on the Liver Shape on Radioisotope Scan (간(肝)스캔상(上) 체위변동(體位變動)에 따른 간형태(肝形態) 변화(變化)에 대(對)한 고찰(考察))

  • For this study, the authors obtained and examined anterior views of the liver and spleen in two different positions, upright and supine, of 39 normal subjects and 20 patients with hepaticpathology by means of $^{99m}Tc-colloid$ and gamma camera. This examination confirms the following findings. In general, it is shown, left lobe of the liver in upright position becomes longer in height than in supine position, while the distance between lateral margin of the liver and that of the spleen becomes father in with in supine position than in upright position. The upper margin of liver moves more downward in upright position than in supine. It is noticeable that as for 5 cases with severe chronic liver disease, there is minimal alteration of the liver shape between in two positions. The comparison of the both positions for the better liver scan shows the following finding. Prominent caudate and/or left lobe are marked in the upright position in 16 cases out of the total 59, while none is found in the supine. The false cold area in lower part of the liver disappears in 7 cases in the upright position, while only one shows the disappeance of the false cold area in the supine. Left liver margin due to close contact of spleen is blurred in 3 cases in the upright position. In total 23 cases out of 59 support that the upright position is better for the liver scan, while only 4 cases support the supine position is better. These findings support two assumptions. One is that upright view can bo expected more improved resoluton than supine view for liver scan. Second is that minimal change of the liver shape in both views indicate the serious abnormality in the liver.

Effects of Dodam-tang on Cerebral Ischemic Damage of Hyperlipidemic Rats (도담탕(導痰湯)이 고지혈증 흰쥐의 뇌허혈 손상에 미치는 영향)

  • Objectives : Dodam-tang has been reported to have a control effect against the hyperlipidemia and thrombosis. Based upon these previous reports, this study investigates the effects of Dodam-tang on the cerebral ischemic damage of the hyperlipidemic rats. Methods : Hyperlipidemia was induced by the beef tallow 30% diet for 14 days on Sprague-Dawley rats. Ischemic damage was induced by the middle cerebral artery occlusion (MCAO) for 2 hours with the intraluminal thread method. Then water extract of Dodam-tang was administered daily for 5 days. The effect of Dodam-tang was evaluated with the serum lipids, infarct volume and edema percentage, and immunohistochemical expressions of iNOS, MMP-9, and GFAP in the brain tissue. Results : The obtained results were as follows; Dodam-tang reduced significantly the infarct size in a TTC-stained 5th brain section of the hyperlipidemic MCAO rats. Dodam-tang suppressed the infarct volume of the hyperlipidemic MCAO rats, but not significant statistically. Dodam-tang suppressed the edema percentage of the hyperlipidemic MCAO rats significantly in the brain tissue. Dodam-tang suppressed significantly the iNOS expression in the cerebral penumbra and caudate putamen of the hyperlipidemic MCAO rats. Dodam-tang suppressed significantly the MMP-9 expression in the cerebral penumbra of the hyperlipidemic MCAO rats. Dodam-tang suppressed significantly the GFAP-expressed atrocytes in the cerebral penumbra of the hyperlipidemic MCAO rats. Conclusions : These results suggest that Dodam-tang suppresses the brain edema formation through the suppression of the iNOS, MMP-9 and GFAP, but the neuroprotective effect against the cerebral infarct are not distinct.

Effects of Salviae Miltiorrhizae Radix on Blood-Brain Barrier Impairment of ICH-Induced Rats (단삼(丹蔘)이 뇌조직출혈 흰쥐의 혈액뇌관문 손상에 미치는 영향)

  • Objectives : This study was performed in order to evaluate the effects of Salviae Miltiorrhizae Radix (SMR) water extract against the cerebral hemorrhage and the blood-brain barrier (BBB) impairment in the intracerebral hemorrhage (ICH). Method : ICH was induced by the stereotaxic intrastriatal injection of bacterial collagenase type IV in Sprague-Dawley rats. SMR was orally given three times every 20 hours during 3 days after the ICH induction. Hematoma volume, water content of brain tissue and volume of evans blue leakage were examined. Myeloperoxidase (MPO) positive neutrophils and tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$) were observed with immunofluorescence labeling and confocal microscope. Results : SMR significantly reduced the hematoma volume of the ICH-induced rat brain. SMR significantly reduced the water content of brain tissue of the ICH-induced rat brain. SMR reduced the percentage of the evans blue leakage around the hematoma on the caudate putamen compared to the ICH group, especially on the cerebral cortex. SMR significantly reduced the volume of the evans blue leakage level in the peri-hematoma regions of the ICH-induced rat brain. SMR significantly reduced MPO positive neutrophils in the peri-hematoma regions of the ICH-induced rat brain. SMR reduced the TNF-${\alpha}$ expression in peri-hematoma regions of the ICH-induced rat brain. TNF-${\alpha}$ immuno-labeled cells were coincided with MPO immuno-labeled neutrophils in peri-hematoma regions of the ICH-induced rat brain. Conclusion : These results suggest that SMR plays a protective role against the blood-brain barrier impairment in the ICH through suppression of inflammation in the rat brain tissues.

Application of T1 Map Information Based on Synthetic MRI for Dynamic Contrast-Enhanced Imaging: A Comparison Study with the Fixed Baseline T1 Value Method

  • Objective: For an accurate dynamic contrast-enhanced (DCE) MRI analysis, exact baseline T1 mapping is critical. The purpose of this study was to compare the pharmacokinetic parameters of DCE MRI using synthetic MRI with those using fixed baseline T1 values. Materials and Methods: This retrospective study included 102 patients who underwent both DCE and synthetic brain MRI. Two methods were set for the baseline T1: one using the fixed value and the other using the T1 map from synthetic MRI. The volume transfer constant (Ktrans), volume of the vascular plasma space (vp), and the volume of the extravascular extracellular space (ve) were compared between the two methods. The interclass correlation coefficients and the Bland-Altman method were used to assess the reliability. Results: In normal-appearing frontal white matter (WM), the mean values of Ktrans, ve, and vp were significantly higher in the fixed value method than in the T1 map method. In the normal-appearing occipital WM, the mean values of ve and vp were significantly higher in the fixed value method. In the putamen and head of the caudate nucleus, the mean values of Ktrans, ve, and vp were significantly lower in the fixed value method. In addition, the T1 map method showed comparable interobserver agreements with the fixed baseline T1 value method. Conclusion: The T1 map method using synthetic MRI may be useful for reflecting individual differences and reliable measurements in clinical applications of DCE MRI.

Left lobe living donor liver transplantation using the resection and partial liver segment 2-3 transplantation with delayed total hepatectomy (RAPID) procedure in cirrhotic patients: First case report in Korea

  • In liver transplantation, the primary concern is to ensure an adequate future liver remnant (FLR) volume for the donor, while selecting a graft of sufficient size for the recipient. The living donor-resection and partial liver segment 2-3 transplantation with delayed total hepatectomy (LD-RAPID) procedure offers a potential solution to expand the donor pool for living donor liver transplantation (LDLT). We report the first case involving a cirrhotic patient with autoimmune hepatitis and hepatocellular carcinoma, who underwent left lobe LDLT using the LD-RAPID procedure. The living liver donor (LLD) underwent a laparoscopic left hepatectomy, including middle hepatic vein. The resection on the recipient side was an extended left hepatectomy, including the middle hepatic vein orifice and caudate lobe. At postoperative day 7, a computed tomography scan showed hypertrophy of the left graft from 320 g to 465 mL (i.e., a 45.3% increase in graft volume body weight ratio from 0.60% to 0.77%). After a 7-day interval, the diseased right lobe was removed in the second stage surgery. The LD-RAPID procedure using left lobe graft allows for the use of a small liver graft or small FLR volume in LLD in LDLT, which expands the donor pool to minimize the risk to LLD by enabling the donation of a smaller liver portion.
