Tankers have somewhat different shapes in construction and working condition compared with normal merchant ship. If an accident occurs, normal merchant ship's damages will mostly be confined to ship and cargo, but those of tankers will result in oil spills and catastrophic loss beyond our imagination. So, first we must understand risk factors, pre-loss control and post-loss control, legal regulations about its indemnification and marine insurance for oil tanker operation. When unexpected accidents happen, despite pre-loss control, it is possible to cover those kinds of losses by insurance. To control these losses, however, it is important to establish compensation for oil pollution and arrange for oil pollution preventing system. In spite of these oil pollution preventing systems, we have rarely seen that pollution from oil tankers could be solved. So this paper was studied more fundamental and overall control measures for the risk management of oil tanker operation.
The aorta is the most common major thoracic artery injured by blunt chest trauma. Injuries to major aortic arch branch arteries can also occur but are much less common than aortic injuries in the setting of blunt trauma. Although internal mammary artery (IMA) injury is uncommon and rarely diagnosed in cases of blunt chest trauma, it is one of the important sources of bleeding in chest trauma. IMA bleeding can cause ongoing blood loss and may lead to serious conditions such as extensive hemothorax, anterior mediastinal hematoma or its catastrophic complication, cardiac tamponade. However such arotic and branch artery injuries are not easily detected by plain radiograph, and are detected indirectly because of associated mediastinal hematoma. Herein, we report a case of IMA injury caused by blunt chest trauma secondary to pedestrian traffic accident. The injured patient was successfully treated by transcatheter arterial embolization (TAE).

So previous studies have focused on design for reliability, and recently many studies have dealt with a maintenance system to prevent unexpected failures. Nowadays due to the emerging ICTs (Information Communication Technologies) and sensor technologies, it is possible to send health monitoring information of important equipment to administrator of an offshore plant in real time, which leads to having much concern on condition based maintenance policy or predictive maintenance. In this study, we have reviewed previous studies associated with condition-based maintenance of offshore plants, and introduced the approaches predicting failures of the compressor which is one of essential mechanical devices in LNG FPSO.

In this study, we have reviewed previous studies associated with CBM (Condition-Based Maintenance) of offshore plants, and introduced an algorithm predicting the next failure time of the compressor which is one of essential mechanical devices in LNG FPSO (Liquefied Natural Gas Floating Production Storage and Offloading vessel). To develop the algorithm, continuous time Markov model is applied based on gathered vibration data.
To cope with the change of railway safety environment and to prevent the catastrophic accident, the railway safety management system was established through the legislation of railway safety rules. And to audit and evaluate the accomplishment of railway safety rules by the railway operators, the Integrated Railway Safety Audit System(IRSAS) has been conducting. This study find out the strategy to stabilize the IRSAS by applying Theory of Constraints(TOC) Thinking Process. For meeting the IRSAS's goal of effective levelling up of railway safety, the two necessary conditions, 1)the secure of substantial safety through the IRSAS and 2)the execution of efficient IRSAS, should be fulfilled. Estimated undesirable effects(UDEs) from the IRSAS were identified, and 3 of them were selected for creating the requisite conflict clouds. Entities from these conflict clouds were synthesized into a core conflict cloud that foamed the base of Current Reality Tree. The strategic direction for change extracted from the conflict cloud is the reinforcement of IRSAS preparation system including the level up of operator's self audit, the deepening of preliminary survey, the establishment of complementing system of audit check list and the build up of auditor's specialization. These injection were logically validated via a Future Reality Tree and expected to be confirmed by further progressing of IRSAS.
This paper provides an integrated view on human and system interaction in advanced and automated systems, which adopting computerized multi-functional artifacts and complicated organizations, such as nuclear power plants, chemical plants, steel and semi-conduct manufacturing system. As current systems have advanced with various automated equipments but human operators from various organizations are involved in the systems, system safety still remains uncertain. Especially, a human operator plays an important role at the time of critical conditions that can lead to catastrophic accidents. The knowledge on human error helps a risk manager as well as a designer to create and control a more credible system. Several human error theories were reviewed and adopted for forming the integrated perspective: gulf of execution and evaluation; risk homeostasis; the ironies of automation; trust in automation; design affordance; distributed cognition; situation awareness; and plan delegation theory. The integrated perspective embraces human error theories within three levels of human-system interactions such as affordance level, psychological logic level and trust level. This paper argued that risk management process should dealt with human errors by providing (1) reasoning improvement; (2) support to situation awareness of operators; and (3) continuous monitoring on harmonization of human system interaction. This approach may help people to understand risk of human-system interaction failure characteristics and their countermeasures.
A leak of fuel gas in partially confined area creates a flammable atmosphere and give rise to an explosion, which is one of the most common accident in domestic. Observations from accident in domestic suggest that some explosions are caused by a quantify of fuel significantly less than lower explosion limit(LEL) amount required to fill the room, which is attributed to inhomogeneous mixing of leaked gas. The minimum amount of leaked gas for explosion is highly dependent on the mixing degree in the area. For lighter gas, such as methane, a high concentration tends to build up in the space from ceiling of room. But heavy gas, such as propane, a high concentration tends to build up in the space from bottom of room. This paper presents a method for analysing the explosion hazard in a room with very small amount of leaked gas. Based on explosion limit concentration, the gaussian distribution model is used to estimate the minimum amount of leak which yields a specified explosion pressure. The results demonstrate that catastrophic structural damage can be achieved with a volume of fuel gas which is less than 0.5 percent of the total enclosed volume in domestic. The method will help analyzing hazard to develop new safe device as well as investigating accident.
In 2021, the Joint Group of Experts on the Scientific Aspects of Marine Environmental Protection (GESAMP), a U.N. advisory research institute, cited container loss as one of six sources of marine litters in shipping. The sinking of the X-P ress Pearl in May 2021 caused a catastrophic environmental pollution accident in which the loaded containers were moved to the shore, and the plastic pellets were loaded inside covered the coast of Sri Lanka. With this history, the International Maritime Organization (IMO) will discuss prevention and follow-up measures for container loss during ship voyages, as an agenda at the 8th Sub Committee on Carriage of Cargoes and Containers meeting in September 2022. To establish Korea's response direction at the IMO meeting, this study identified major causes of container loss accidents, and considered the response through analysis based on the accident investigation report and related professional data. As a result, it was found that the major cause of container loss during voyages was the enlargement of container ships, bad weather, and poor loading of containers. In particular, the need to prepare countermeasures for the deterioration of the operational safety of large container ships due to bad weather was identified. Additionally, integrated monitoring of the implementation of international conventions is required, for the safe sea transportation of container cargo. In particular, in terms of preservation of the marine environment, it is necessary to supplement the system for the recovery of lost containers. Finally, it was found that it is necessary to establish systems that can complement each other in the shipbuilding and shipping industries, in terms of shipbuilding as well as ship operation, to fundamentally prevent container loss accidents at sea. It is judged that it is difficult to resolve the various factors of container loss at sea during voyages, by responding from an individual perspective.
For a methanol separation column of the BPA (Bisphenol A) plant, HAZOP (hazard and operability) assessment was performed and damage ranges were predicted from the accident scenarios for the fire and the explosion. As a result, the damage range of the jet fire was 20 m in the case of rupture of the discharge pipe (50 mm diameter) of safety valve, and that of the flash fire was 267 m in the case of catastrophic rupture. Also, the damage ranges of the unconfined vapor cloud explosion (UVCE) for the rupture of the discharge pipe and for the catastrophic rupture were 22 m and 542 m, respectively. For the worst case of release scenarios, safety measures were suggested as follows: the pressure instruments, which can detect abnormal rise of the internal pressure in the methanol separation column, should be installed by the 2 out of 3 voting method in the top section of the column. Through the detection, the instruments should simultaneously shut down the control and the emergency shut-off valves.

Tension viscerothorax (gastrothorax) is rare life-threatening disease which is caused by air trapped in viscera. A distended viscera in the hemi-thorax shifts the mediastinal structures and causes extra-cardiac obstructive shock. A defective diaphragm is caused by abdominal trauma or a congenital anomaly. Traumatic diaphragmatic injury can be missed until herniation develops several years after blunt trauma. In our case, a 10-year old boy developed hemodynamic compromise in the emergency department. Three years earlier, he had suffered blunt abdominal trauma during a pedestrian traffic accident, but there was no evidence of diaphragmatic injury at that time. He was successfully resuscitated by gastric decompression and an emergent thoracic operation. The operation finding revealed a traumatic diaphragmatic injury. Tension viscerothorax is a rare, but catastrophic, condition, so we suggest that addition of tension viscerothorax to the Advanced Trauma and Life Support (ATLS) guidelines may be helpful.
