Purpose: To report a case of a 60-year-old female with rhegmatogenous retinal detachment, presenting with suprachoroidal hemorrhage after vitrectomy. Case summary: A 60-year-old woman visited our clinic complaining of floaters, flashing, and blurred vision. Best-corrected visual acuity was 0.1 in her left eye, and fundus examination of her left eye revealed macula-involved retinal detachment with a retinal break at the superotemporal quadrant. She underwent cataract surgery, 23-gauge transconjunctival sutureless vitrectomy, and 14% C3F8 gas tamponade under general anesthesia. One hour after anesthesia recovery, she suddenly complained of severe pain in her left eye. The intraocular pressure measured after removal of the pressure patch from her left eye was as high as 58 mmHg. Her ocular pain improved spontaneously within 10 minutes, and the intraocular pressure decreased to 8 mmHg. Fundus examination of her left eye revealed a reddish-brown raised lesion, suggesting suprachoroidal hemorrhage. She was placed in a prone position with a pressure patch over her left eye. Bleeding through the sclerotomy site was observed 1 day after surgery. Subsequently, hemorrhagic choroidal detachment of her left eye continued to decrease without deterioration. Three weeks after surgery, the patient received an intravitreal injection of 100% C3F8 gas into her left eye. At 3 months after surgery, best-corrected visual acuity had improved to 0.8 in her left eye, and the retina was stable. Conclusions: Suprachoroidal hemorrhage may be suspected in a patient complaining of severe ocular pain after vitrectomy.
This study was performed to describe the outcome of pars plana retinopexy with perfluoro-n-octane (PFO)-silicone oil (SiO) exchange in dogs with rhegmatogenous retinal detachment in Seoul National University Veterinary Medical Teaching Hospital (SNU VMTH) from 2014 to 2017. Nine eyes of 8 dogs were included in this study. Medical records including signalment, history, duration from onset of blindness to surgical intervention, pre-operative findings, duration from surgery to regaining vision, and post-operative complications were evaluated. No eyes were visual before surgery. Duration from onset of blindness to surgical intervention was 2-30 days (median 8 days); duration from surgery to regain vision was 1-14 days (median 6 days); follow-up time was 15-1088 days (median 69 days). Post-operative complications were divided as temporary vs permanent conditions. Temporary complications were corneal ulcer, uveitis, retinal haemorrhage, glaucoma, subconjunctival leakage of SiO, and vitreal haemorrhage. Permanent complications were anterior chamber migration of SiO, retinal degeneration, corneal degeneration, re-detachment, and cataract. Six of 9 eyes regained functional vision, five of which remained visual throughout the follow-up time while the other one lost vision after 3 months because of uveitic glaucoma. In conclusion, pars plana retinopexy with PFO-SiO exchange provided fair outcome in 66.7% cases described in this study.

To evaluate the effects of spherical aberration (SA) correction on optical quality in pseudophakic eyes, we simulated the optical quality of the human eye by computation of the modulation transfer function (MTF). We reviewed the medical records of patients who underwent cataract surgery in Asan Medical Center, retrospectively. A Zywave aberrometer was used to measure optical aberrations at 1-12 postoperative months in patients with AR40e intraocular lens implants. The MTF was calculated for a 5 mm pupil from measured wavefront aberrations. The area under the MTF curve (aMTF) was analyzed and the maximal aMTF was calculated while changing the SA ($-0.2{\sim}+0.2{\mu}m$) and the defocus (-2.0 ~ +2.0 D). Sixty-four eyes in 51 patients were examined. The maximal aMTF was $6.61{\pm}2.16$ at a defocus of $-0.25{\pm}0.66D$ with innate SA, and $7.64{\pm}2.63$ at a defocus of $0.08{\pm}0.53D$ when the SA was 0 (full correction of SA). With full SA correction, the aMTF increased in 47 eyes (73.4%; Group 1) and decreased in 17 eyes (26.6%; Group 2). There were statistically significant differences in Z(3, -1) (vertical coma; P = 0.01) and Z(4, 4) (tetrafoil; P = 0.04) between the groups. The maximal aMTF was obtained at an SA of $+0.01{\mu}m$ in Group 1 and an SA of $+0.13{\mu}m$ in Group 2. Optical quality can be improved by full correction of SA in most pseudophakic eyes. However, residual SA might provide benefits in eyes with significant radially asymmetric aberrations.
The Korean government has a new system for charging patient care for patients in hospital, on hold for the present(9 / 1995) but to start implementation in certain areas of patient care next year. From the latter half of next year the Ministry of Health and Welfare would like to start demonstration projects for hospitals who want to start using DRGs for frequently seen medical diagnosis and for patients with a course that is predictable and for whom non-insurance costs are minimal : such as the patient who has a delivery, cesarean deliveries, cataract surgery, tonsillectomy or an appendectomy, and apply the DRG system of payment for hospital care for these patients. The purpose of this study was to establish a recording system to give effective home health care to postpartum women and their newborns. Recently the government announced a DRG system to apply to postpartum women for pilot purposes starting next year. This gives impetus to the need to develop home care records that will allow for systematic recording and provide continuity and consistency in care across all health professionals and with in-depth communication between the professions to assure high quality care. There has been a rise in medical costs and a shortage of patient bed space in hospitals, particularly since the introduction of national medical insurance. The study focused on developing client selection criteria, a primary assessment tool, progress notes and nursing diagnoses applicable to postpartum and newborn clients. Selection criteria for home health care, assessment tool content, nurses progress notes and diagnoses were developed through a review of the literature, advice from professionals who are expert in home health care and actual practice in the use of recording tools through workshops. The recommendations based on the research results are as follows : 1) Replication and application of these tools is needed to test the validity of the tools 2) In order to have systematic nursing records standardization of records has to be done after nurses have had experience using them. 3) Reliability and validity of the tools has to be established through applicability to actual care situation.

To evaluate clinical results of silicone oil injection for retinal detachment. The 45 eyes of 44 patients underwent vitrectomy and silicone oil injection from 1999 to 2002. 1.There were no history of previous retinal surgery and ocular trauma. In 24 of the 45 eyes(53%) visual acuity increased and stabilized in 18 of the 45 eyes(40%). 2. 1 of 16 eyes(6.3%) had recurred retinal detachment after silicone oil removal. 3. Final retinal reattachment was achieved in 44 eyes(97%). Postoperative complications were cataract(16 eyes), glaucoma(10 eyes), emulsification of silicone oil(5 eyes), keratopathy(2 eyes) and recurrent retinal detachment(1 eye). These results show that silicone oil injection for primary retinal detachment yields a high rate of anatomic success and the good visual outcome
A 13-year-old neutered female Maltese was referred to Veterinary Medical Teaching Hospital of Seoul National University with 8-month-history of gradually enlarging retrobulbar mass in the right eye. On the ophthalmic examinations, exophthalmos, exposure keratitis, and cataract were observed, and menace response was absent in the eye. The fellow eye was normal except for nuclear sclerosis. On the ocular ultrasonographic evaluation, a mass was identified in retrobulbar and periocular region in the right eye. Exenteration was performed in order to remove the mass. Histopathologic evaluation revealed that most part of the mass was composed of epithelial-like neoplastic cells. Invasion into adipose cells and osseous metaplasia around the mass were also confirmed. Based on the radiologic and histopathologic examination, the mass in the right eye was diagnosed as primary retrobulbar meningioma. Eleven months after the surgery, recurrence was not observed.

Background and Objectives Ocular alkali burns cause severe damage to the ocular tissues and vision loss. Solcoseryl is a standardized calf blood extract that normalizes the metabolic disturbance and aids in maintaining the chemical and hormonal balance and has been used to treat burns in various tissues. This study examined the effects of Solcoseryl on a rat corneal alkali burn model. Materials and Methods Twenty rats were assigned randomly to four equal groups, including alkali burn, hyaluronic acid, Solcoseryl eyedrop, and Solcoseryl gel. A corneal alkali burn was induced by a NaOH-soaked paper disc. The treatments were given twice a day, every day. The wound area was measured after 24 and 48 hours, and the degree of neovascularization and corneal opacity were scored every week. The rats were sacrificed after three weeks for immunohistochemistry (IHC) to compare the level of inflammatory cytokines, IL-1β, IL-6, and TNF-α. The thickness of the retinal layers was compared to observe any changes in the retina. Results The use of Solcoseryl on corneal alkali burn accelerated wound healing with less neovascularization, greater opacity, and less cataract. IHC showed that the inflammation of the cornea was controlled by both the hyaluronic acid and Solcoseryl treatments. On the other hand, the inflammation had spread to the retina. When the dosage forms were compared, eyedrops were more effective on corneal inflammation, while the gel-type had a greater effect on retinal inflammation. Conclusion Solcoseryl was effective in accelerating the wound healing rate on a corneal alkali burn but could not prevent the spread of inflammation from the cornea to the retina. Eyedrops were more effective on inflammation in the cornea, and the gel was more effective in the retina.
Background: Long-term comparative data of phacoemulsification combined with endoscopic cyclophotocoagulation (phaco-ECP) versus phacoemulsification (phaco) alone in dogs are rare. Objectives: To investigate the effects of ECP on postoperative intraocular pressure (IOP) and complications after phaco in dogs with normal IOP. Methods: Medical records of IOP, conjunctival hyperemia, corneal edema, aqueous flare, posterior synechia, intraocular fibrin, and posterior capsule opacification (PCO) formation in 15 canine eyes that underwent phaco-ECP and 36 eyes that underwent phaco alone were evaluated retrospectively. ECP was applied when either the iridocorneal angle or the ciliary cleft was narrow or closed. Results: The IOP of the phaco-ECP group persisted within the normal range postoperatively. The phaco-ECP group had a shorter period of dorzolamide use than did the phaco group. PCO was formed earlier in the phaco-ECP group than in the phaco group. The phaco-ECP group showed more severe corneal edema than the phaco group at every follow-up visit. Posterior synechia was more severe in the phaco-ECP group than in the phaco group from two weeks until the last follow-up. Conclusions: Although ECP might cause more postoperative complications such as corneal edema and posterior synechia, it could effectively reduce the incidence of IOP increase after phaco in dogs with a high risk of postoperative glaucoma.
Background: Long-term comparisons of phacoemulsification with topical medication are limited in canine diabetic cataracts. Objectives: To compare outcomes of eyes submitted to phacoemulsification with those of topical medication for canine diabetic cataracts and identify risk factors for complications. Methods: Through medical records review, 150 eyes (76 dogs) with diabetic cataracts were included; 58 eyes (31 dogs) underwent phacoemulsification (phaco-group) and 92 eyes (48 dogs) received ophthalmic solution alone (medication-group). The medication-group was divided into owner-led and vet-led groups depending on who elected not to perform surgery. Comparisons involved time-to-complications, vision, and the number and type of ophthalmic solutions administered. The association between complications and pretreatment clinical findings was investigated. Results: No difference was found in complication risk between the phaco and owner-led medication groups. Conversely, the vet-led medication-group had a higher complication risk than the other groups. At the last follow-up, 94.8% of the phaco-group had vision, whereas 7.6% of the medication-group restored some visual axis. Poor glycemic control in the medication-group and younger age in the phaco-group increased complication risk. At 1-year post-treatment, the average number of ophthalmic solutions administered was 1.7 and 2.6 in the phaco and medication groups, respectively. The medication-group used anti-inflammatories the most throughout the follow-up, whereas the phaco-group used anti-inflammatories the most until 1-year post-treatment and lacrimostimulants at 1.5-year post-treatment. Conclusions: For canine diabetic cataracts, phacoemulsification is recommended because it is superior to topical management alone in terms of maintaining vision and reducing the number of ophthalmic solutions required in the long term.
