Development of Case Review Form for Detecting Adverse Events (위해사건 확인을 위한 증례검토지 개발)

  • Objectives: The purpose of this study is to develop a case review form for detecting adverse events through a medical records review in hospitalized patients in South Korea. Methods: To develop the case review form, several literatures were reviewed, first. Through the clinical expert meeting, screening criteria were selected and case review form was developed. Result: The Korean version of case review form consisted of the review form-1 for adverse event screening and form-2 for adverse event identification. The applied methodology for the case review form is determined according to the previous studies. For example, the method used in the first stage review is nurse review. Furthermore, the National Coordinating Council for Medication Errors Reporting and Prevention index is used to categorize disability, and a scale of 1 to 6 was used in the causation scores and preventability scores, respectively. Through the clinical expert meeting, a total of 41 screening criteria were selected. Conclusion: The Korean specific case review form was developed for detecting adverse events in hospitalized patients. The results from this study can be used in a large-scale study regarding the nationwide incidence of adverse events.

  • The main purpose of this study is to examine the overall procedure of hospital's internal review of health insurance reimbursement, to present the case of protest against reimbursement cut, and hence to provide some information on hospital's management of medical revenue. The object of the case study is 'P' university medical center, possessing 5 different hospitals under its system. Presentation of the case of protest against reimbursement cut has following meanings: Firstly, to the hospitals that already have internal review departments, information on the details of the protest process and results can be exchanged. Secondly, to the Government and National Health Insurance Corporation, useful data are provided for the improvement of the rules and procedures of health insurance reimbursement. Thirdly, to the hospitals without internal review departments, fundamental materials on the internal review process are provided for the effective management of medical revenue.

  • This paper shows the case study of risk analysis in an weapon system research and development project. For risk analysis, an advanced stochastic networking technique-VERT (Venture Evaluation and Review Technique) is used. Assumptions for activities of network diagram and conversion methods from PERT ( Program Evaluation and Review Technique) to VERT are discussed. Also, simulation result is presented and discussed.

  • We report a case of tenosynovial giant cell tumor with severe bone erosion in the right fifth finger of a 46-year-old man. Throughout this case review, we describe the imaging findings of tenosynovial giant cell tumor with severe bone erosion and review the literatures regarding osseous lesions caused by tenosynovial giant cell tumor and their significance related to the differential diagnosis and patient treatment.

  • This study aims to review the research trends concerning environmental psychology. The scope of review is limited to the papers that dealt with theoretical and case-directed investigation research about the relationship between socio-physical environments and human beings. The relationship has been investigated by diverse human reactions such as cognitive, emotional, analytical, behavioral, and comprehensive. 219 papers were reviewed in two steps. In the first step, the ratio of the papers in the field of environmental psychology to the total papers published in the Journal from 1973 to 2004 was analyzed. Second, all the 219 papers were classified by such three criteria as the type of the paper(theoretical, review, case investigation), the methodology taken, and the main theme of the paper. The main findings from the review are summarized as follows; 1. Out of the 219 papers, the number of the theoretical papers was 4 (1.8%), whereas the number of the case investigation papers was 185 (84.5%). 2. Out of the 30 review papers, the main methodology used was simple introduction (15 papers, 50%) and the main theme was research methodology (36%), whereas analytic comparison was used only in 10 papers (33%) and theory review was only found in 6 papers (20%). 3. Out of the 185 case investigation papers, experimental method was found in 34 papers (18%), whereas 99 papers used correlational method (54%). Based on the findings, it was suggested that both theoretical and case-directed research should be performed in order to enhance the state of art in environmental psychology. Theoretical research as a framework to understand the empirical findings are as important as the case-investigation research peformed by scientific experimental method.

Job Satisfaction and Organizational Commitment of Health Insurance Review Team Leader and Member (진료비 심사부서장 및 부서원의 직무만족도와 조직몰입도)

  • The purpose of this study was to investigate the factors that affect the performance of health insurance review department workers of Korean tertiary hospitals, general hospitals and hospitals. A survey was conducted through structured questionnaire for chiefs and members of health insurance review departments of hospitals, and data from 1,064 respondents were utilized in the final analysis. Survey items included general characteristics of the hospitals and health insurance review departments, job satisfaction and organizational commitment as organizational effectiveness. As multiple linear regression results, in the case of chiefs, the most significantly related factor to job satisfaction was a monthly salary. The other related factor was job stress. In the case of members, the most significantly related factor was job stress. The other related factors were more monthly salary, religious believer, and less complex workplace, in order. As organizational commitment, in the case of chiefs, the most significantly related factor was a monthly salary. The other related factor was more clinical experience. In the case of members, the most significantly related factor was job stress. The other related factors were more monthly salary, tertiary hospitals, more age, and less complex workplace, in order.

Literature Review of Korean Medicine Studies for Fibromyalgia Syndrome (FMS) (섬유근통 증후군의 한의학적 연구에 대한 문헌 조사: 국내 논문을 대상으로)

  • Objectives This study aimed to review studies in Korean medicine for fibromyalgia syndrome (FMS). Methods We conducted search across 3 electronic databases (Korean traditional knowledge portal, RISS, Oasis) to find studies for FMS in Korean medicine. Studies of Korean medicine for FMS were included and analysed. Results 15 studies were included. 2 studies were reviews and 13 studies were case reports. Total numbers of patients in case studies were 15. In case studies, acupuncture (100%), herbal medicine (76.9%), Korean physical therapy (69.2%), pharmacopuncture (53.8%), Chuna (46.1%) and moxibustion (30.7%) etc. were used. BL23 in acupuncture and CV12 in moxibustion were most frequently used. Gamisoyo-san in herbal medicine and visual analogue scale (VAS) in outcome measure were most frequently used. Conclusions This systematic review found encouraging but limited evidence of Korean medicine for FMS. I expect various trials such as randomized clinical trial or animal experimentation and more case reports. And systemic review for FMS in other national journals is needed.

Systematic Review of Case Reports about Korean Medicine for Lung Cancer

  • Objectives: This systematic review aimed to put the case reports of lung cancer on Korean medicine (KM) together and adopt the results in clinical practice. Methods: Researches were searched using the PubMed, EMBASE, OASIS, KoreanTK, KISTI, RISS, KISS, and NDSL. The search term were 'lung cancer' and KM. There was no restriction in year. Results: 1. Among the 48 studies, 68 patients were reported in total. The types of lung cancer were non-small-cell lung cancer (n=41) and small-cell lung cancer (n=6). 2. The number of patients who received KM therapy alone was 40. On the other hand, 25 patients were treated with KM and chemotherapy simultaneously. All case reports used herbal medicine except 2 studies. Other types of treatment were acupuncture, moxibustion, pharmacopuncture, cupping, meditation, etc. 3. Several efficacy evaluation variables were used such as tumor size, changes of symptoms, duration of survival, the quality of life, and so on. The safety was evaluated by checking adverse effects using blood test. 4. Regarding the tumor response, partial response was reported in 12 cases, stable disease was in 22 cases, 50% of the total cases, which is a high level of tumor response. Furthermore, all 11 cases with the evaluation on the length of survival showed prolonged survival than the expectancy of corresponding stage, with the stable quality of life. Conclusion: We have found that the applicability of KM for treatment of lung cancer through this review. Evidence based medicine can be realized by checking cases and applying them in clinical practice.
