다기관 코호트 연구에서 경동맥 내막-중막 두께 측정의 측정자간 신뢰도 평가 (Inter-Rater Reliability of Carotid Intima-Media Thickness Measurements in a Multicenter Cohort Study)

  • 목적: 경동맥 내막-중막 두께와 경동맥 경화반의 존재유무는 죽상동맥경화증의 임상 전단계를 나타내는 지표로 널리 사용되고 있다. 경동맥 내막-중막 두께를 측정할 때의 측정자 의존성 때문에, 다기관 연구에서는 경동맥 내막-중막 두께와 경화반 측정의 기관간 신뢰도를 확인하는 것이 중요하다. 이 연구의 목적은 심뇌혈관 및 대사질환원인 연구센터에 속해 있는 세 임상기관 사이의 경동맥 내막-중막 두께와 경화반 측정의 측정자간 신뢰도를 평가하는 것이다. 방법: 심뇌혈관 질환 과거력이 없는 사람 20명이 2014-2015년 사이에 이 연구에 참여하였고(연령 37-64세), 미리 정해진 프로토콜에 따라 연구참여자의 좌, 우 경동맥을 세 임상기관에서 반복적으로 측정하였다. 총 경동맥의 원위부에서 측정한 경동맥 내막-중막 두께의 최대값과 평균값을 기록하였다. 경동맥에서의 경화반 존재유무는 측정자에 의해 확인되었다. 경동맥 내막-중막 두께와 경화반 존재유무의 신뢰도를 급내상관계수와 카파 통계량을 통해 각각 평가하였다. 결과: 계산된 급내상관계수는 최대 경동맥 내막-중막 두께를 평가하였을 때 0.647이었고 (95% CI: 0.487-0.779), 평균 경동맥 내막-중막 두께를 평가하였을 때 0.758 (95% CI: 0.632-0.854) 이었다. Bland Altman plot에서, 관측치의 대부분은 평균의 차이에서 ${\pm}1.96$ 표준편차 사이에 분포하였다. 각 기관 사이의 경화반 존재유무에 대한 카파 통계량은 0.304 (기관 1과 2), 0.507 (기관 1과 3), 0.606 (기관 2와 3)이었다. 전반적인 일치를 평가하는 Fleiss카파값은 0.445였다. 결론: 세 임상기관 사이의 경동맥 내막-중막 두께의 측정자간 신뢰도는 훌륭하였으며, 경화반 존재유무에 대한 신뢰도는 적정하였다.

Neurological Improvement after Cranioplasty in Patients with Surgical Bony Defects : The Usefulness of Acetazolamide Activated $^{99m}Tc-HMPAO$ SPECT

  • Objective : The purpose of this study is to examine the influence of cranioplasty on dynamics of cerebral blood flow[CBF] and cerebrovascular reserve capacity[CVRC], and to investigate the usefulness of single photon emission computed tomography [SPECT] as a prognostic factor for neurological improvement after cranioplasty. Methods : Between March 2003 and December 2005, a prospective study was performed on 24 patients who had undergone total 25 cranioplasty operations. Cerebral blood flow velocities in the middle cerebral artery[MCA] and internal carotid artery[ICA] were obtained by transcranial Doppler ultrasonography[TCD]. The CVRC was assessed by SPECT in the natural state and after stimulation with 1g of acetazolamide. Neurological improvement after cranioplasty was compared between patients who showed hyperactivity to acetazolamide-activated SPECT [Group 1, n=7] and hypoactivity to acetazolamide-activated SPECT [Group 2, n=17]. These measurements were obtained two weeks prior to and two weeks after cranioplasty. Results : The blood flow velocities at the opposite site to the cranioplasty as well as at the cranioplasty site were significantly increased [P<0.05]. Compared with Group 2, there was significant increase in CBF and neurological improvement after cranioplasty in Group 1. Conclusion : Among patients with surgical bony defects, the patients who had normal reactivity of the CVRC showed a significant increase in CBF and neurological improvement after cranioplasty. The authors suggest that CVRC measurement prior to surgery may be an important prognostic factor for neurological improvement after cranioplasty.

일개 농촌지역 저소득층 당뇨병환자의 관리 상태 (Management of Diabetic Mellitus in Low-income Rural Patients)

  • Objectives : Knowledge about the management status of diabetic melitus (DM) is essential to improve diabetic management. Moreover, low income is associated with poor adherence to treatment and increased mortality. This study was performed to evaluate the management status of DM in low-income patients in a rural area. Methods : We enrolled 370 patients with type 2 DM living in Gokseong county, JeollaNamdo. A well-trained examiner measured the height, weight, waist circumference, blood pressure, total cholesterol, triglyceride, high density lipoprotein cholesterol, fasting blood sugar and glycosylated hemoglobin (HbA1c) levels. Carotid ultrasonography was used to measure carotid artery carotid artery intima media thickness (IMT) and plaque. anklebrachial index (ABI) was used to evaluate peripheral artery disease. A fundoscopic examination was performed to evaluate diabetic retinopathy. A history of diabetes complications and health-related questionnaires were also completed. Results : The age of diabetic subjects was 68.7$\pm$8.7 years and the duration of diabetes was 8.9$\pm$8.2 years. Most (63.5%) had hypertension, and 45.7% had triglycerides below 150 mg/dl, 38.1% had low density lipoprotein cholesterol (LDL) cholesterol below 100 mg/dl, 48.7% had urine albumin to creatinine ratio (UACR) below 30 mg/g. Less than half (45.9%) achieved the goal of HbA1c less than 7% suggested by the American Diabetes Association (ADA). 10.6% had peripheral vascular disease, 11.9% had retinopathy, and 60.8% had chronic kidney disease. Conclusions : DM management in low income patients is very poor and requires further work to improve.

뇌경색 환자의 경동맥 내막 두께와 사속도맥파 검사의 관련성 연구 (Study on the Relationship between Carotid Intima-Media Thickness and The Second Derivative of Photoplethysmogram Waveforms in Ischemic Stroke Patients)

  • The purpose of this study was to investigate the correlation of Intima-Media Thickness of common carotid artery(CCA-IMT) and The Second Derivative of Photoplethysmogram Waveforms(SDPTG). 38 subjects with acute ischemic stroke were recruited from the patients admitted to Dong-Eui Medical Center from the June 2013 to January 2014. We assessed 38 patient's SDPTG data and CCA-IMT data by B Mode ultrasonography. then 38 subjects were divided into two groups by the CCA-IMT difference; CCA-IMT 〈 0.8 mm group(n=11), CCA-IMT $$\geq_-$$ 0.8 mm group(n=27). We analyzed their characteristics, risk factor, blood test result, life style by CCA-IMT difference. As a result, Age, Hypertension were significantly higher in the CCA-IMT $$\geq_-$$ 0.8 mm group then in the CCA-IMT < 0.8 mm group. Regular exercise were significantly higher in the CCA-IMT < 0.8 mm group then in the CCA-IMT $$\geq_-$$ 0.8 mm group. The result of Multiple regression analysis on the factors affecting the CCA-IMT was hypertension, total cholesterol, age. and The result of Pearson's Correlation analysis on CCA-IMT and SDPTG is that e/a ratio, SDPTG AI were respectively and significantly correlated with CCA-IMT. According to the analysis, the Relationship between CCA-IMT and SDPTG in Ischemic Stroke Patients were founded. We suggest that further investigation with larger and better controlled trials of the Relationship between CCA-IMT and SDPTG could contribute to better understand the effects of risk factors on atherosclerosis.

TCD를 이용한 정상군과 고지혈증군의 혈류측정에 관한 비교연구 (Cerebral Blood Flow as Measured by TCD in Hyperlipidemic Group)

  • The purpose of this study was to compare cerebral blood flow between hyperlipidemic patient group and normal healthy aldult group using transcranial doppler ultrasonography(TCD). I investigated cerebral blood flow of 63 hyperlipidemic patients and 42 healthy adults. To evaluate the cerebral blood flow, I measured the systolic peak velocity(Vs) and mean flow velocity(Vm) of the milddle cerebral artery(MCA), anterior cerebral artery(ACA), posterior cerebral artery(PCA), basilar artery(BA), internal carotid artery(ICA) in the two groups using TCD. In normal healthy adults, subjects showed a decerease in Vs and Vm with advancing in age. There was a significant difference in the Vm of ICA. There was no significant differences in the Vm of ACA and ICA. In normal healthy adults, females showed high velocities of all examined vessel. There was a significant difference in the Vs of ICA. There was a significant differences in the Vm of MCA and ICA. Normal healthy adults higher than hyperlipidemic patients in the Vs of MCA, ACA, PCA. There was no significant difference in the Vs of all exmined vessels. Hyperlipidemic patientsincrease higher than normal healthy adults in the Vm of ACA. There was a significant difference in the Vs of BA. In the patients aged under 50's, normal healthy adults higher than hyperlipidemic patients in the Vs and Vm except BA. But there was no significant difference in all exmined vessels. In the patients aged over 50's, hyperlipidemic patients higher than normal healthy adults in all examined vessel expect PCA. There was a significant difference in the Vs of BA and ICA. And hyperlipidemic patients higher than that normal healthy adults in all exmined vessels. There was a significant difference in BA and ICA. In this study, visible differences in blood flow between hyperlipidemic patient group and normal healthy aldult group were shown. However, these results do not come up to the previous values that were reported and known worldwide.

  • 1. Objectives : This study is a pilot study to construct of metabolic syndrome model with constitutional type. We focused on the correlation of Metabolic syndrome(MetS) and ordinary symptoms with Taeumin at 40. 2. Methods : 32 subjects who had taken health examinations and diagnosis of Sasang constitution from Oct 1, 2008 to Dec 31, 2008 at Kyung-Hee medical center were included and underwent B-mode ultrasonography for measurement of intima-media thickness of common carotid arteries. MetS was defined by the criteria of the criteria of the National Cholesterol Education Program Adult Treatment Panel III. 3. Results and Conclusions : Mean intima-media thickness of common carotid artery was higher in subjects with MetS compared with control subjects(0.56$\pm$0.12 vs 0.54$\pm$0.08mm) however. The ordinary symptoms shown to differentiate the MetS group and the control group were related to "dark urine color," "dryness of eyes," and "night sweat". Therefore, these ordinary symptoms can be said to be indicators of the Taeumin with Mets syndrome.

비만 소아에서 경동맥 내중막 두께와 심혈관질환 위험인자와의 상관관계 (Carotid artery intima media thickness in obese children : relation with cardiovascular risk factors)

  • 목 적 : 총경동맥의 IMT 측정은 초기 죽상 경화증 변화에 대한 비침습적 marker로 인정되고 있고, 임상에서 실행 가능한 유용한 검사 방법이다. 본 연구는 비만아와 정상 대조군의 경동맥 IMT를 측정하여 두 군 사이에 차이가 있는지 비교해 보고, 체질량지수, 혈압, 혈중 지질치와 같은 심혈관질환 위험인자와의 상관관계를 알아보고자 하였다. 방 법 : 9-15세의 비만한 소아 및 청소년 49명(남 41명, 여 32명)과 정상 대조군 24명(남 14명, 여 10명)을 대상으로 신체 계측하고 8시간 공복 상태에서 채혈하여 혈당과 혈중 지질치, adiponectin을 측정하였다. 대상자는 누운 상태에서 7.5MHz 선형 탐촉자를 이용하여 고해상도 B형 초음파로 양측 경동맥 IMT를 측정하였다. 결 과 : 비만군의 mean IMT의 평균은 $0.34{\pm}0.03mm$, 정상 체중군은 $0.31{\pm}0.01mm$로 비만군이 유의하게 높았다(P<0.01). 비만군의 peak IMT의 평균은 $0.42{\pm}0.06mm$, 정상 체중군은 $0.37{\pm}0.05mm$로 비만군이 유의하게 높았다(P<0.01). Mean IMT는 연령(r=0.317, P<0.01), 체질량지수(r=0.431, P<0.01), 총 콜레스테롤(r=0.377, P<0.01), 중성지방(r=0.253, P<0.05), 저밀도 지단백 콜레스테롤(r=0.289, P<0.05)과 유의한 상관관계를 보였다. 비만군의 혈중 adiponectin의 평균은 $11.2{\pm}5.7{\mu}g/mL$, 정상 체중군은 $14.7{\pm}7.9{\mu}g/mL$로 비만군이 유의하게 낮았고(P<0.05), mean IMT는 adiponectin(r=-0.267, P<0.05)과 유의한 음의 상관관계를 보였다. 결 론 : 비만아에서 경동맥 내중막 두께는 정상 체중아에 비해 유의하게 증가되어 있었으며, 심혈관질환 위험인자와 연관성을 보였다. 따라서 이러한 비만아의 혈관 변화는 소아기 때부터 시작되는 것으로 생각되며, 비만 소아를 대상으로 심혈관질환 위험인자에 대한 조기 검사가 필요하다고 생각된다.

  • 1995년 3월 56세 여자환자가 계속 확장하는 만성 Debakey 1형 대동맥 박리증으로 대동맥궁 치환술을 시행하였다. 계속되는 박리의 전후 확장으로 대동맥궁의 혈류가 분리되었고, 가강이 복부대동맥까지 확장되어 있었다. 수술후 3시간 뒤에 갑작스러운 무뇨증이 발생하였고, 도플러 초음파와 대동맥촬영에서 신동맥으로의 혈류가 감소된 것이 발견되었다. 내막피판에 의한 급성 신부전으로 판단하고 대동맥 풍선확장술을 시행하였으나 실패하였다. 응급으로 복부대동맥 개창술을 시행하여 내막피판과 혈전을 제거하였다. 이후 환자는 경한 만성신부전 상태로 투석 치료없이 지내고 있다.

Immunoglobulin G4-Related Disease Masquerading Anaplastic Thyroid Carcinoma

  • Immunoglobulin G4-related disease (IgG4RD) is a chronic inflammatory condition characterized by tissue infiltration with lymphocytes and IgG4-secreting plasma cells, as well as varying degrees of fibrosis. We report a case of a 70-year-old man with a rapid-growing cervical mass for several months. Computed tomography and positron emission tomography showed a huge, ill-defined mass involving left thyroid lobe and encasing the common carotid artery, which was clinically and radiologically suspicious for anaplastic thyroid carcinoma. Ultrasonography-guided core needle biopsy was performed and histopathology examination revealed to be consistent with IgG4RD, and the IgG4/IgG ratio was 0.6. After oral corticosteroid was administered, the mass was dramatically resolved. Because IgG4RD often presents as a single localized and infiltrated mass lesion, it can be confused and misdiagnosed as a malignancy. Thus, clinicians should consider IgG4RD as a differential diagnosis in a rapid-growing neck mass to prevent unnecessary and excessive treatments.

전기치료가 긴장형 두통환자의 뇌 혈류 속도에 미치는 영향 (Effects of Electrotherapy on Blood Velocity of Cranial Artery in Tension-Type Headache subjects)

  • The aim of study was to evaluated the possible role of cranial artery velocity in headache pathogenesis. The present study was studied of five headache(F=5. Mean $age=29.80\pm6.76yrs$) were compared to 4 controls(F=4, Mean $age=29.00\pm5.48yrs$). Transcranial doppler ultrasonography(TCD) is a new non-invasive and easily applicable method to evaluate flow velocities of the intracranial and extracranial cerebral arteries. TCD was performed with standard method to measure the mean Flow Velocity(MFV) of the middle and posterior cerebral arteries, the internal carotid artery, the vertebral and the basilar artery. We reviewed the whole TCD results performed at Taejon Veterans Hospital from October. 11. 2000 to November. 10. 2000. Mean flow velocities in headaches and controls at their 6 decades are $28.00\pm3.61cm/sec$ and $41.25pm1.71cm/sec$ in lent PCA (P<0.01), $50,000\pm23.07cm/sec$ and $82.75\pm15.59cm/sec$ in right MCA(P<0.05), $26.20\pm4.82cm/sec$ and $45.50\pm4.51cm/sec$ in fight PCA(P<0.01). $26.60\pm4.56cm/sec$ and $38.25\pm4.92cm/sec$ in right VAC(P<0.01). After treatment for 2 weeks, mean of velocity on pre treatment and post treatment and post treatment are $28.00\pm3.61cm/sec$ and $38.20\pm5.81cm/sec$ in left PCA (P<0.05), $26.20\pm4.827cm/sec$ and $39.20\pm5.54cm/sec$ in right PCA(P<0.05), $40.60\pm9.18cm/sec$ in right VA(P<0.01). It is concluded that Electrical Therapy for two weeks was effected to promote Mean Flow of Velocity in cranial artery. Mean of velocity in cranial artery with headaches observed in this study was lower than controls, but MFV was promote after treatment for 2 weeks.
