Objective : Covered stenting is an optional strategy for traumatic carotid pseudoaneurysm, especially in malignant conditions of potential rupture, but the long-term outcomes are not clear. Our aim was to determine if covered stenting is an effective option for traumatic carotid pseudoaneurysm with promising long-term outcomes. Methods : Self-expanding Viabahn and balloon-expandable Willis covered stents were separately implanted for extra- and intracranial traumatic carotid pseudoaneurysm. The covered stent was placed across the distal and proximal pseudoaneurysm leakage under roadmap guidance. Procedural success was defined as technical success (complete exclusion of the pseudoaneurysm and patency of the parent artery) without a primary end point (any stroke or death within 30 days after the procedure). Long-term outcomes were evaluated as ischemic stroke in the territory of the qualifying artery by clinical follow-up through outpatient or telephone consultation and as the exclusion of the pseudoaneurysm and patency of the parent artery by imaging follow-up through angiography. Results : Five patients with traumatic carotid pseudoaneurysm who underwent covered stenting were enrolled. The procedural success rate was 100%. No ischemic stroke in the territory of the qualifying artery was recorded in any of the five patients during a mean clinical follow-up of 44±16 months. Complete exclusion of the pseudoaneurysm and patency of the parent artery were maintained in all five patients during a mean imaging follow-up of 39±16 months. Conclusion : Satisfactory procedural and long-term outcomes were obtained, suggesting that covered stenting is an effective option for traumatic carotid pseudoaneurysm.
During the period from Mar. 1983 to Feb. 1986, 22 patients with vascular trauma were treated at Capital Armed General Hospital. 1] 11 patients had arterial injury alone, 6 patients had venous injury alone, and 5 patients had both arterial and venous injuries. 2] The mechanism of injury in these 22 patients was 7 penetrating non-gun shot wounds, 5 blast fragments injuries, 4 iatrogenic injuries, 4 blunt injuries, 2 gun shot wounds. 3] The method of 16 arterial repair was 5 autogenous saphenous vein graft, 8 synthetic vascular graft, 1 end to end anastomosis, 2 lateral suture. Midterm patency of 16 repairs was 100% within 1 months. 4] The method of 11 venous repair was 1 autogenous saphenous vein graft, 2 onlay vein patch, 2 ligation, 1 lateral suture. The thrombotic obstructions occurred in 4 repairs [36.4%], but they were resolved somewhat with heparinization. 5] Uncommon cases of false aneurysm of internal carotid artery and laceration of retrohepatic inferior vena cava were summarized.
Objective To suggest rotation angles of fluoroscopy that can bypass the carotid sheath according to vertebral levels for cervical transforaminal epidural steroid injection (TFESI). Methods Patients who underwent cervical spine magnetic resonance imaging (MRI) from January 2009 to October 2017 were analyzed. In axial sections of cervical spine MRI, three angles to the vertical line (${\alpha}$, angle not to insult carotid sheath; ${\beta}$, angle for the conventional TFESI; ${\gamma}$, angle not to penetrate carotid artery) were measured. Results Alpha (${\alpha}$) angles tended to increase for upper cervical levels ($53.3^{\circ}$ in C6-7, $65.2^{\circ}$ in C5-6, $75.3^{\circ}$ in C4-5, $82.3^{\circ}$ in C3-4). Beta (${\beta}$) angles for conventional TFESI showed a constant value of $45^{\circ}$ to $47^{\circ}$ ($47.5^{\circ}$ in C6-7, $47.4^{\circ}$ in C5-6, $45.7^{\circ}$ in C4-5, $45.0^{\circ}$ in C3-4). Gamma (${\gamma}$) angles increased at higher cervical levels as did ${\alpha}$ angles ($25.2^{\circ}$ in C6-7, $33.6^{\circ}$ in C5-6, $43.0^{\circ}$ in C4-5, $56.2^{\circ}$ in C3-4). Conclusion The risk of causing injury by penetrating major vessels in the carotid sheath tends to increase at upper cervical levels. Therefore, prior to cervical TFESI, measuring the angle is necessary to avoid carotid vessels in the axial section of CT or MRI, thus contributing to a safer procedure.
무명동맥은 길이가 짧고 가슴 골격에 의해 보호되기 때문에 외상에 의한 무명동맥의 파열은 드문 질환으로 알려져 있다. 본 증례는 자동차 사고로 내원한 25세 남자로 흉부 전산화 단층촬영 및 혈관 조영술로 무명동맥 파열이 진단되어 우측 쇄골하 절개를 동반한 정중 흉골 절개술을 이용하여 응급 수술을 시행하였다. 다른 동반 손상이 많아 인공 심폐기를 사용하지 않고 수술을 하였으며, 손상은 무명동맥의 중위부부터 우측 쇄골하 동맥 및 총경동맥의 기시부까지 약 3 cm 정도로 동맥 내막까지 완전히 찢어져 있었다. 복제정맥을 이용하여 첩포 혈관 성형술을 시행하였으며, 수술 후 환자는 별 다른 이상 없이 외래 추적 관찰 중이다.
두경부 관통성 외상의 기본적 처치는 먼저 초기 단계에서 환자의 정확한 병력 및 이학 검사, 호흡 억제 유무 평가, 중대한 출혈 유무의 평가 등을 시행한다. 중대한 출혈의 평가는 외부 출혈 유무, 급속히 커지는 혈종 유무, 혈류역학적으로 불안정한 생징후를 보이는 경우, 맥박 소실, 저혈량성 쇼크의 소견, 혈종격동이나 혈흉 소견이 있는 경우 등이다. 이러한 경우 즉각적인 외과적 처치를 시행해야 한다. 중대한 출혈의 소견을 보이지는 않지만 혈관 손상이 의심되거나 가능성이 높다고 생각되는 경우로는 혈류 잡음, 삽입 이물질이 큰 혈관에 근접 위치한 경우, 맥박 소실이나 중추 신경학적 문제가 발생한 경우 등이 해당된다. 이때는 혈관조영술과 CT촬영을 시행한 후 보존적으로 관찰을 할지 외과적 처치를 시행할지를 결정한다. 이때도 외과적 처치가 필요한 경우 지체없이 시행한다. 또한 두경부 이몰 삽입에 의해 환자가 자신의 생명에 심각한 위협을 받았다고 느끼기때문에 정신적인 충격을 해소해 주어야 한다.
Objectives : In conditions of brain infarction, irreversible axon damage occurs in the central nerve system (CNS), because gliosis becomes a physical and a mechanical barrier to axonal regeneration. Reactive gliosis induced by ischemic injury such as middle cerebral artery occlusion is involved with up-regulation of GFAP and CD81. This study was undertaken to examine the effect of the Gongjin-dan (GJD) on CD81 and GFAP expression and its pathway in the rat brain following middle cerebral artery occlusion (MCAO). Methods : In order to study ischemic injuries on the brain, infarction was induced by MCAO using insertion of a single nylon thread, through the internal carotid artery, into a middle cerebral artery. Cresyl violet staining, cerebral infarction size measurement, immunohistochemistry and microscopic examination were used to detect the expression of CD81 and GFAP and the effect on the infarct size and pyramidal cell death in the brain of the rat with cerebral infarction induced by MCAO. Also, c-Fos and ERK expression were measured to investigate the signaling pathway after GJD administration in MCAO rats. Results : Measuring the size of cerebral infarction induced by MCAO in the rat after injection of GJD showed the size had decreased. GJD administration showed pyramidal cell death protection in the hippocampus in the MCAO rat. GJD administration decreased GF AP expression in the MCAO rat. GJD administration decreased CD81 expression in the MCAO rat. GJD administration induced up-regulation of c-FOS expression compared with MCAO. GJD administration induced down-regulation of ERK expression compared with MCAO. Conclusion : We observed that GJD could suppress the reactive gliosis, which disturbs the axonal regeneration in the brain of a rat with cerebral infarction after MCAO by controlling the expression of CD81 and GFAP. The effect may be modulated by the regulation of c-Fos and ERK. These results suggest that GJD can be a candidate to regenerate CNS injury.
A 27-year-old man was admitted with a penetrating injury at the mid-manubrium. Computed tomographic (CT) angiography showed a f illing def ect in the aortic arch. This was evaluated as a sign of injury and the patient underwent an emergency operation. No active bleeding or clot was f ound in the mediastinum during the operation. The laceration point was between the innominate and the left carotid artery posteriorly. The injury was approached using hypothermic circulatory arrest. Aortotomy and exploration showed a 2-cm-long full-thickness aortic injury with an overlying clot. A filling defect on angiography as a sign of a penetrating arch injury has never been reported previously, but was the main pathological finding on CT angiography in our case. The aorta is a high-pressure system and injuries to it should be treated aggressively.
The abducens nerve usually travels from the brainstem to the lateral rectus muscle as a single trunk. However, it has been reported that this nerve could split into branches occasionally. We attempted to show the aberrant course of abducens nerve in a specimen with unilateral duplicated abducens nerve and review relevant literatures. The micro-dissections were performed in a head specimen injected with colored latex under the microscope. The abducens nerve was duplicated unilaterally. This nerve emerged from the pontomedullary sulcus as a single trunk and splitted into two branches in the prepontine cistern. These two separate branches were piercing the cerebral dura of the petroclival region respectively. The slender lower branch passed between the petroclinoid and petrosphenoid ligaments and the thick lower one passed under the petrosphenoid ligament. These two branches united just lateral to the ascending segment of internal carotid artery in the cavernous sinus. The fact that there are several types of aberrant abducens nerve is helpful to perform numerous neurosurgical procedures in the petroclival region and cavernous sinus without inadvertent neurovascular injuries.

The administration of water extract of Uncariae Ramulus et Uncus induced up-regulation of c-Fos expression significantly compared with MCAO. 6. The admistration of water extract of Uncariae Ramulus et Uncus increased ERK expression significantly compared with MCAO. Conclusion : We observed that Uncariae Ramulus et Uncus could suppress the reactive gliosis, which disturbs the axonal regeneration in the brain of the rat with cerebral infaction after MCAO by controlling the expression of CD81 and GFAP. 목 적 : 신생아 질식은 신생아 사망과 영구적 신경발달 장애의 중요한 원인이다. 이에 대한 여러 치료 방법이 시도되어 왔으나 현재까지 임상에 적용 가능한 치료 혹은 예방법은 개발되지 못하고 있다. 산모의 고열은 신생뇌에 악영향을 미치는 것으로 알려져 왔으나 최근 동물에서 시행한 고온 전처치가 뇌손상을 예방한다는 상반된 효과가 보고된 바 있다. 이에 저자들은 신생 동물의 저산소 허혈 뇌손상에 있어서의 고온 전처치의 효과를 조사하고자 이 연구를 시행하였다. 방 법 : 신생쥐를 사용하여 생후 6일에 이들을 두 군으로 나누어 전처지군은 $40^{\circ}C$의 고온 환경에, 대조군은 상온에 2시간 노출시켰다. 24시간 후 두 군 모두 편측 온목동맥 결찰 후 산소 농도 8%의 저산소 환경에 2시간 노출시켜 저산소 허혈을 유도하였으며 저산소 허혈후 7일에 뇌를 적출하여 뇌손상 정도를 비교하였다. 결 과 : 고온 전처치군 16례 중 5례(31.2%)에서 뇌손상이 관찰되어 대조군(17례 중 11례, 64.7%)에 비해 유의성 있게 뇌손상 빈도가 낮았다(P<0.05). 뇌피질, 해마, striatum 및 시상에서 관찰한 손상 부위의 분포 양상과 손상 정도는 전처치군과 대조군 사이에 통계적으로 유의한 차이를 보이지 않았다. 결 론 : 신생쥐에 있어서 2시간의 고온 전처치는 저산소 허혈 뇌손상의 빈도를 감소시켰다. 이는 산모의 발열이 저산소 허혈 뇌손상 보호 효과를 나타낼 가능성을 시사하나 임상 적용에 앞서 다양한 환경 온도 및 장시간의 노출 시간으로 그 효과와 안정성에 대한 연구가 계속되어야 할 것으로 생각된다.
