A Case of Lung Injury Caused by Ammonia-Gas Inhalation (암모니아 가스 흡입에 의한 폐 합병증 1예)

  • We have experienced a case of lung injury caused by accidental inhalation of ammonia gas in a 34 year-old-man. By the explosion of ammonia tank in a refrigerator boat he inhaled ammonia gas. Several minutes later, he suffered from severe dyspnea and visual loss. On arrival at emergency room, analysis of arterial blood gas revealed severe hypoxemia and his chest film showed bilateral pulmonary infiltrates. Under the impression of adult respiratory distress syndrome, mechanical ventilator was applied to the patient. After recovery from ARDS and tracheal edema, he complained of some hemoptysis and productive sputum during the admission. So we checked bronchoscopy and bronchograpy which showed tracheal bullae just above carina and tubular bronchiectatic change in the right lower lobe. We report a case of lung injuries-ARDS, tracheal bullae, and bronchiectasis-caused by inhalation of ammonia gas with the review of the relevant literatures.

Delayed Repair of Completely Transected Left Main Bronchus-A report of one case- (좌측 주기관지 외상성 단절의 지연복원 -1례 보고-)

  • A complete transection of left main bronchus was repaired by end to end anastomosis 5 months after the chest trauma in the Yeungnam University Hospital. The patient was a 36 years old male who had been injured bluntly by a heavy package on the left chest. The initial symptoms and signs were dyspnea, chest pain and subcutaneous emphysema on the left neck, but on admission at our hospital his chief complaint was only mild left chest discomfort. The preoperative chest X-ray findings 5 months after the trauma revealed total collapse of the left lung, deviation of trachea to the left, elevation of left diaphragm, abrupt discontinuation in the course of an air-filled left main bronchus and bronchoscopy showed that the left main bronchus was completely occluded, without any signs of inflammation, approximately 4 cm from the carina. The operation was performed through standard posterolateral thoracotomy incision at the fifth intercostal space. There was not any suppuration within the transected lung but plenty of white mucus which was removed by forceful suction. The transected bronchial edges were debrided and anastomosed primarily by end to end with interrupted nonabsorbable sutures. The suture line was reinforced with a pleural flap. The postoperative course was uneventful and pulmonary function following operation improved progressively and proved the delayed repair to have a reasonable decision.

Resection of Intrapericardial Schwannoma Co-Existing with Thymic Follicular Hyperplasia through Sternotomy without Cardiopulmonary Bypass

  • A 35-year-old man was admitted to Korea University Anam Hospital for evaluation of intermittent chest pain. Computed tomography of the chest showed enlargement of a previously identified anterior mediastinal mass and also a well-defined, circumscribed mass in the subcarinal area, surrounded by the roof of the left atrium, right pulmonary artery, and the carina. Complete resection of the intrapericardial tumor was performed through median sternotomy without cardiopulmonary bypass. Pathologic examination identified the tumor as schwannoma, of an ancient type, diffusely positive for the S-100 antigen. Unlike other reported cases, grossly, the tumor did not seem to be involved with any nerve.

  • Tracheal collapse is a common respiratory disease in dogs. There are many ways to treat tracheal collapse, one of which is the use of an intraluminal stent. In this study, we divided 21 dogs into two groups and implant conventional stents and new nitinol stents. Comparison of two groups was based on following, feature of stent fracture, form of stent migration, clinical sign improvement, complication and prognosis. Approaching was established via C-arm under spontaneous breathing and placing a stent at the site of collapse. Using radiographic images, determine stent size accurately. For a comparison of identical condition, all intraluminal stents were placed 10 mm caudal from larynx to 10 mm cranial from carina. In this study, new nitinol stents improve the problems of conventional stents and may be effective in the treatment of tracheal collapse in small dogs.

  • A foreign body in the airway can be a potentially life-threatening event. The diagnosis and treatment of foreign bodies in the airway are a challenge for otolaryngologists. Despite the improvements in medical care and public awareness, approximately 3,000 deaths occur each year from foreign body aspiration. A high degree of vigilance is required to ensure prompt treatment and avoid the complications of foreign body aspiration. The author encountered a case of a 77-year-old female patient who had aspirated an unknown foreign body that was fixed in her main bronchus. An initial attempt was made to remove it with a flexible bronchoscope but failed due to the patient's hypoxemic state during the procedure. Under general anesthesia, a rigid bronchoscopic examination was performed, but it was difficult to approach the object due to the bronchus curvature. Instead, a cryotherapy instrument of bronchoscopy was applied. The foreign body was frozen and removed to the carina, where a laryngoscope and laryngeal forceps were used to remove it.

  • The use of digital systems in radiation science has been increased last years in the different knowledge areas, as a detectors, spectrometry, spectroscopy, simulation, etc. This manuscript presents the design and implementation of a low-cost, fully portable multi-channel analyzer for nuclear spectrometry (in situ). The development is based on a 32-bit microcontroller with ARM Cortex-M7, this design is able to digitize and analyze pulses from a radiation detector without the need to transform the input signal with some filter, obtains the maximum height of each of the digitized pulses, segmenting the information into channels to form a histogram and visualizing the LCD screen incorporated in the system. A continuous digitization methodology was used, which is in charge of the DMA and an ADC with a resolution of 12 bits at a speed of 3.6 MSPS. The system has a compact design and can open and save spectra in an SD memory built into the system. The MCA in MCU was tested with a NaI(Tl) Scintillation radiation detector, which allowed us to determine that the spectra obtained are similar compared to commercial MCA's. The results obtained show that the MCA in MCU is efficient for nuclear spectrometry, in addition to being very economical and low power consumption.

Endobronchial Metastasis from Renal Cell Carcinoma -A case report- (신장세포암의 기관지내 전이 - 1예 보고 -)

  • Lung parenchyma is a common organ for metastases of extrathoracic tumors, but endobronchial metastasis is very rare. In this report, we present a case of endobronchial metastases from renal cell carcinoma (RCC), and this was managed by performing operative resection. A 63-year-old man presented with frequent dry cough; he had previously undergone left nephrectomy and postoperative chemotherapy for grade 2 RCC eight years ago. Computed tomography and bronchoscopy showed an endobronchial tumor from the left lower lobe bronchus to the second carina, and this mass was diagnosed as a necrotic tissue with chronic inflammation at biopsy. During the operation, the mass was revealed to be a metastatic renal cell carcinoma on the frozen section diagnosis and there was no mucosal invasion on the resection margin of the left lower lobe bronchus. We performed lobectomy of the left lower lobe with systemic dissection of the mediastinal lymph nodes. The final histopathologic diagnosis of the endobrochial mass was metastatic RCC and any mediastinal lymph node metastasis was not found. The patient was discharged on postoperative day 10 without any postoperative complications.

Left Sleeve Pneumonectomy Via Sequential Bilateral Thoracotomy in Carinal Squamous Cell Carcinoma -One case report- (기관분기부 편평상피 세포암에서 순차적 양측 개흉술을 통한 좌측 소매 전폐 절제술 치험 1예)

  • Sleeve pneumonectomy can be a method of treatment in a selected patient with bronchogenic carcinoma involving carina. A 64 years old male with a history of mitral valve replacement via midsternotomy 13 years ago and resection of papilloma of the vocal cord 2 years ago. The patient was admitted due to blood-tinged sputum. Bronchoscopy and computerized tomogram of the chest revealed 3.5 cm mass at lower margin of the trachea and totally obstructing the left main bronchus. A biopsy revealed squamous cell carcinoma. He underwent left sleeve pneumonectomy through sequential bilateral thoracotomy without cardiopulmonary bypass, and the pathologic stage was T4N0M0 stage IIIB. The patient is being followed through the outpatient clinic in good general condition.

Clinical Value of Bronchoscopic Examination in the Diagnosis of Cancer of the Lung (기관지경검사의 폐암진단에 관한 임상적 의의)

  • Two-hundred and eighteen patients were taken bronchoscopy to evaluate the value of bronchoscopic diagnosis in the lung cancer patient at the Department of Thoracic Surgery of the Kyung-pook University Hospital. Ninety-eight eases of these patients were studied for possible lung cancer by bronchoscopic examination, biopsy and bronchial cytologv. The results were obtained as follows: Two-hundred and eighteen patients were taken bronchoscopy to evaluate the value of bronchoscopic diagnosis in the lung cancer patient at the Department of Thoracic Surgery of the Kyung-pook University Hospital. Ninety-eight eases of these patients were studied for possible lung cancer by bronchoscopic examination, biopsy and bronchial cytologv. The results were obtained as follows: 1) Forty-five (55.5%) of 81 cases impressed as definitive, and six (35. 3%) of 17 cases impressed as probable lung cancer were subsequently proved to have lung cancer. This represents a total of 51 cases (52. 0%) of 98 patients. 2) Twenty-six (51.0%) of 51 patients were proved by bronchoscopic biopsy and cytology. 3) The positive rates of bronchoscopic biopsy were higher in the lesions on tile intermediate bronchus, carina and main stem bronchi by bronchoscopy, and also in the hilar than in peripheral lesions by X-ray finding and in histologically undifferentiated cell carcinoma than in adenocarcinoma. 4) The positive diagnosis rates of bronchoscopic aspirate were not correlated to the locations of cancer lesions. 5) Thirty- nine percent of 51 case3 were visualized definitive tumor mass and obstructive lesions under bronchoscopy. 6) Patient of lung cancer varied in age from 27 to 77 years, with highest concentration lying in the 5th decade (43.0%). Forty-seven patients were men and 4 were women giving a proportion of men to women, 12: 1 7) Sixty-five percent of proved lung cancer patients were diagnosed as operable cases. Fifty-three percent of explored patients were resected. These results conclude that bronchoscopy is of a considerable value as a diagnostic procedure in these lung cancer patients.

A Study on the Measurement of the Normal Tracheal Length in Korea adults (한국성인의 기관 길이 측정에 관한 연구)

  • The trachea is defined as the airway from the inferior border of the cricoid cartilage to the top of the carinal spur. This paper would confirm the normal tracheal length of Korean adults through the actual measurement using the fiberoptic bronchoscopy. The subjects of this study were 25 patients, 13 males and 12 females between the age of 20 to 69 without abnormality on the neck, trachea, mediastinum and lung pharenchyme on the preoperative chest X-ray, who received the operations from the period of July to September, 1994. For those patients who had heart diseases, the cardiothoracic ratio was below 50%. The measurement was performed on the patients with endotracheal intubation under the general anesthesia in supine and neutral position. The tracheal length was calculated by the difference between the length from the tip of the endotracheal tube [E-tube to carina and to the needle which was inserted into the E-tube at the lower border of the palpated cricoid cartilage, by inserting the broncoscopy through the E-tube. The result was as follow : 1 The measured tracheal length for men was 11.8 0.2 cm[mean standard deviation and women was 10.5 0.3 cm, and that was longer than this [p<0.01 . The average was 11.2 1.0 cm and the standard error was 0.20 cm. 2 According to the correlation between the tracheal length to weight, height[Ht , age, and body surface area[BSA respectively, the Ht [p=0.003 , age [p=0.055 , and the BSA[p=0.017 were significant, while weight was not [p=0.314 . 3 From the regression analysis of the tracheal length[T.L. to the Ht, Age, and the BSA which were significant, the following equation was derived.i Ht : T.L.= -1.29 + 0.076 x Ht [P=0.003 ii Age: T.L.= 10.04 + 0.028 x Age [P=0.055 iii BSA : T.L.= 5.60 + 3.48 x BSA [P=0.017 iv In multi-regression : T.L. = -4.15 + 0.034 x Age + 0.085 x Ht [P=0.0002]
