Surgical Treatment of Endobronchial Hamartoma -A case report- (기관지내 과오종의 외과적 치료 -1례 보고-)

  • Endobronchial hamartoma is a extremely rare lesion. The patient was 47 years old female and complained of cough and dyspnea for several years. On bronchoscopy, a finger tip size mass, nearly occluding the left main bronchus and located 4cm from the carina, was found. Bronchoscopic biopsy showed the chronic inflammatory findings.We performed bronchotomy and removed the mass through left thoracotomy. The endobronchial tumor was confirmed hamartoma histopathologically.

Initial Mass Functions of Massive Stars in OB Associations

  • We derived initial mass functions (IMF) of massive stars in three different regions of spiral arms within 2.5kpc from the sun. The derived IMF slope $\beta$ of Local arm stars is found to be $-2.09{\sim}-2.06$, very close to that of Bisiacchi et al. (1983). For Sagittarius-Carina arm stars $\beta$ ranges from -1.77 to - 1.72 which is close to that of overall stars given by Germany et al. (1982). Possible causes inducing the regional difference in IMFs are discussed.

  • Congenital bronchobiliary fistual is a rare disease with unclear etiology. An abnormal tract communicates the tracheobronchial junction to a hepatic segment, usually the left lobe. Billous sputum, a positive HIDA(o-Dimethyliminodiacetic acid) scan, and a trification at the level of the carina lead to the diagnosis, which can be confirmed by bronchoscopic contrast injection. We experienced a case of congenital bronchobiliary fistual in a 27-day-old girl. Our case is reported with literature reviews.

  • In patient with lung cancer, the resection margin of right main bronchus was invaded by tumor intraoperatively. So we performed tracheal reconstruction with autologous pericardium after resection of lower trachea including carina. Postoperatively, the patient discharged well and followed up for 5 months without any evidence of tumor recurrence or restenosis.

  • We present [Fe II] 1.64 ${\mu}m$ imaging observations for jets and outflows from young stellar objects over the northern part (${\sim}24^{\prime}{\times}45^{\prime}$) of the Carina Nebula, a massive star forming region. The observations were performed with IRIS2 of Anglo-Australian Telescope and the seeing was ~1.5". Eleven jets and outflows features are detected at eight different regions, and are termed as Ionized Fe Objects (IFOs). The [Fe II] features have knotty or elongated shapes, and the detection rate of IFOs against previously identified YSOs is 1.4%. Four IFOs show anti-correlated peak intensities in [Fe II] and $H{\alpha}$, where the ratio I([Fe II])/I($H{\alpha}$) is higher for longish IFOs than for knotty IFOs. We estimate the outflow mass loss rate from the [Fe II] flux using two different methods. The jet-driving objects are identified for three IFOs (IFO-2, -4, and -7). The ratios of the outflow mass loss rate over the disk accretion rate for IFO-4 and -7 are consistent with the previously reported values ($10^{-2}-10^{+1}$), while the ratio is higher for IFO-2. This excess may result from underestimating the disk accretion rate. Other YSO physical parameters show reasonable relations or trends.

  • A 25 years old male patient was experienced Trough method operation to correct of Tracheal stenosis following Tracheostomy. After admission, a No.3 Magill plastic portex tube had been placed in the previous tracheostomy opening. Stenotic narrowing portion was noted low trachea (about 3.5cm above carina) Despite of narrowing of the lesion, anesthetic course was uneventful with oxygen, halothane and non depolarized muscle relaxant by controlled respiration. Postoperative coures were also favorable With adequate respiratory cares and blood gas analyzed.

  • Specimens of Austinogebia wuhsienweni (Yu, 1931) previously recorded from China to Japan were collected at a manila clam farm of 0.5 m depth in Hongseong, Yellow Sea. They were briefly described and illustrations included color photographs, since this was the first record of this rare species from Korean waters. Three species of Upogebiidae are known from Korean waters: Upogebia major (De Haan, 1839), U. issaeffi (Balss, 1913), and A. wuhsienweni (Yu, 1931). They are similar to each other, however, the presence of the ventral spines of the rostrum, a proximal knob on the lateral margin of the uropodal endopod, and a longitudinal carina on the inner surface of the palm of the male first pereiopod can easily distinguish the present species from the two species of Upogebia.

  • Rupture of a bronchus is an unusual result of nonpenetrating trauma to the chest. This case was a 19 year old male who was a worker in the mine. The patient had sustained a compression chest injury without external wound or rib fracture. At five days after trauma, he was suffered from dyspnea and the symptom was progressively increased. Two weeks after the chest trauma, a diagnosis of left main bronchial obstruction due to traumatic bronchial rupture was made by means of bronchoscopy and bronchography. He was operated upon. The left lung was completely atelectatic and the left main bronchus was found to be transected at the level of 3cm from carina. End-to-end anastomosis of the bronchus was performed and the left lung was aerated very well. postoperative course was uneventful.

  • For the treatment of bronchial stenosis due to trauma, inflammatory and neoplastic lesion, bronchoplastic procedure in the interest of preservation of lung tissue are relatively new developments in the field of thoracic surgery. We reported on case of bronchoplasty using to pericardial patch for the treatment of bronchial stenosis due to chronic inflammation. The patient was 26 years old female and chief complaint was respiratory difficulty. Bronchogram revealed diffuse stenosis of left main bronchus about 4cm and especially, at just below the carina marked narrowing of lumen and fine serration in the wall. At the time of operation, longitudinal incision was made at left main bronchus about 5cm and reconstructed bronchus using to pericadial patch at membranous compartment of bronchus. The postoperative course was uneventful and post-operative follow up bronchography showed that improvement of bronchoplastic segmented region.

  • Tracheopathia osteoplastica is a rare, benign pathologic condition chahacterized the occurrence of numerous cartilage and bony nodules within the tissue of mucosa. We report a case of tracheopathia osteoplastica that involved both larynx and trachea. The patient was 73 years old male and had hoarseness. The neck CT showed thickened and irregular wall of the trachea extending to the level of the carina with multiple calcified nodules and plaques of bony density projecting into the lumen.
