Triple tooth is rare in primary dentition; it is the abnormal fusion of three teeth. There are a few reports of double teeth, but triple teeth are rare. These multiple teeth create several clinical problems, not only esthetic problems but also a high caries susceptibility, congenital missing permanent tooth germ(s), orthodontic problems, and periodontal problems. They also make it difficult to perform pulp treatment because of the complex tooth structure. A 1-year 7-month-old male underwent a partial pulpotomy with mineral trioxide aggregate (MTA) when pulp exposure caused by deep caries occurred at a maxillary anterior triple tooth in the first case. The second case was a 1-year 9-month-old male presenting with a fracture line at a mandibular anterior triple tooth. After removing the fractured fragment, a pulpectomy was performed at the remaining primary lateral incisors. Specific complications were not observed during 24 and 6 months of follow-up, respectively.

Metachromatic leukodystrophy (MLD) is a progressive and degenerative neurological disease caused by a deficiency of the catabolic enzyme arylsulfatase A. Deficiency of arylsulfatase A results in accumulation of sulfatide in the white matter of the peripheral and central nervous system and it occurs demyelination as a result. The patient gradually goes through mental and motor failure. General symptoms of MLD include gait disturbance, mental deterioration, muscle rigidity and impaired swallowing. Inheritance of the disease is autosomal recessive. We report a dental caries treatment of a 3-year old boy with MLD. The patient underwent hematopoietic stem cell transplantation (HSCT) to slow the progression of the disease. He was suffered from difficulties of mastication and swallowing from the degenerative neurological symptom. He was ingesting food by both oral feeding and tubal feeding after he took percutaneous endoscopic gastrostomy (PEG). The cause of multiple caries was mainly presumed as patient's prolonged time of meal. The treatment was performed under general anesthesia considering patient's incompliance. Severely affected lower primary molars were treated with pulp treatment and restored with stainless steel crown. Others were restored with composite resin. There were no postoperative complications. MLD is life threatening progressive disease and also has an impact on unfavorable condition for oral health. Routine home oral care and periodic professional dental care should be emphasized to the caregiver of patient considering the susceptibility of dental caries. Not only the medical care, but periodic dental office visit would benefit the quality of life of the patient.
Halitosis is caused by consumption of certain foods or drinks and production of volatile sulfur compounds (VSCs) by periodontopathogens. VSCs-related halitosis is not easily removed using mechanical or chemical therapies such as dental floss, plaque control and mouth rinse. Lactobacillus are known to be probiotics and stimulate immune systems of human. Furthermore, L. casei ATCC 334 and L. rhamnosus GG have an effect on protection of dental caries in vitro studies. The aim of this study was to investigate effect of Lactobacillus on halitosis by Fusobacterium nucleatum- and Porphyromonas gingivalis-producing VSCs and to analyze inhibitory mechanism. The periodontopathogens were cultivated in the presence or the absence Lactobacillus, and the level of VSCs was measured by gas chromatograph. For analysis of inhibitory mechanisms, the susceptibility assay of the spent culture medium of Lactobacillus against F. nucleatum and P. gingivalis was investigated. Also, the spent culture medium of Lactobacillus and periodontopathogens were mixed, and the emission of VSCs from the spent culture medium was measured by gas chromatograph. L. casei and L. rhamnosus significantly reduced production of VSCs. L. casei and L. rhamnosus exhibited strong antibacterial activity against F. nucleatum and P. gingivalis. The spent culture medium of L. casei inhibited to emit gaseous hydrogen sulfide, methyl mercaptan and dimethyl sulfide from the spent culture medium of periodontopathogens. However, the spent medium of L. rhamnosus repressed only dimethyl sulfide. L. casei ATCC 334 may improve halitosis by growth inhibition of periodontopathogens and reduction of VSCs emission.
The aim of this study was to analyze the dental caries of chilren's deciduous teeth in relation to their mothers' socioeconomic factors and their oral health beliefs to increase the effect on children's oral health. One hundred seventy four children and their mothers were selected for the study. The children were 5 and 6 years old in children house in Kun-san, Korea. Research was conducted by oral examination, their mothers' socioeconomic factors and dental health belief model. The finding of analysis can be summaried as follows; In regard to the caries in deciduous teeth status, the five-year-old boy children's dft index was 3.39 and that of the girls of the same age was 2.76. The dft index of the boys of six was 3.86 and that of the girls of the same age was 3.27. The five-year-old boy children had the highest df rate(81.8%), and their dt rate was more than their ft rate. But for the six-year-old girl children, ft rate was higher than dt rate. For the five years old, the boy children's ft rate was significantly higher than that of the girls(p<0.05). Concerning the caries in deciduous teeth states by socioeconomic factors, their oral health status was significantly different according to mother employment and householder age. The df rate of the preschoolers with working mothers(84.7%) was significantly higher than that of those with nonworking mothers(66.7%)(p<0.05). The dft index of the group whose households were over forty was highest with 4.07, followed by the children of the households aged 35 to 39 with 3.83, those of the households aged 25 to 29 with 3.33, and those of the households aged 30 to 34 with 2.15. The difference between the groups was significant(p<0.05). As to the difference of maternal dental health beliefs by maternal socioeconomic factors, households educational background and whether to live with parents or parents-in-law under the same roof made a significant difference to maternal oral health beliefs. The mothers whose households were college graduates or better-educated were stronger in oral health susceptibility and seriousness(p<0.01, p<0.05). The salience was recognized more by those who lived with parents or parents-in-law than by the others who didn't(p<0.05). The dft index of the preschoolers was related to maternal dental health beliefs. That was lower when their mothers perceived dental health benefit better.

Osteopetrosis is characterized by impaired osteoclast function and increased bone density. Infantile osteopetrosis is a severe form of the disease and has characteristics such as diffusely sclerotic skeleton, pancytopenia, cranial nerve entrapment, infection susceptibility, and abnormal craniofacial appearance. Patients with infantile osteopetrosis often experience developmental delay, and may have a short life span. A 14-month-old girl with osteopetrosis presented to the department of pediatric dentistry. Incipient caries on deciduous incisors were observed. The patient revisited 4 years of age. Besides medical problems, oral complications such as growth retardation, narrow upper arch, crowding, dental caries, and abnormal tooth development were observed. After consultation with her pediatrician, dental treatments were performed on the deciduous molars under sedation after a prophylactic antibiotic injection. At a periodic follow-up, multiple deciduous teeth were treated and extracted, and oralrehabilitation with a removable partial denture was initiated. Patient with osteopetrosis are highly susceptible to infection because of their compromised immune system and problems associated with wound healing that lead to osteomyelitis or sepsis development. Active participation in dental care for sugar intake management and proper oral hygiene are obligatory.
Statement of problem. Among the physical properties of adhesion luting cement, the aspect that requires the most important factor is the degree of solubility and water sorption. Dissolution or an inadequate due to excessive water sorption inside the oral cavity compromises the while concurrently increasing the susceptibility to secondary dental caries. Susceptibility to dissolution and difficulty of removing remnant cement from the gingival sulcus have hindered the use of dental resin cement in the clinical practice, but the improved characteristics of newer generation resin cements have interest in and enabled resin cements to be widely used in adhesion of fixed prosthesis, such as laminate veneers and all-ceramic crowns. Purpose. The purpose of this study is to compare and analyze the degrees of solubility and water sorption of a variety of resin cements widely used for clinical purposes with different curing methods. Material and methods. Self-curing resin cements, $Avanto^{(R)}$, $C&B^{TM}$ CEMENT and Superbond C&B cements comprised group 1, 2 and 3. The dual-curing resin cements $Panavian^{TM}$ F, $Calibra^{(R)}$ and $Variolink^{(R)}$ II were divided into groups 4, 5, and 6, respectively. The investigation was carried out using disc-shaped specimens as specified by ANSI/ADA Specification No. 27. The degree of water sorption, water solubility and lactic acid solubility of each test group was analyzed statistically leading to the following conclusion. Results. The degree of water sorption was shown to increase in the following order : group 6, 5, 4, 2, 1 and 3. There were significant differences between the water sorption of each group. Results of the degree of water solubility were shown to increase in the following order : group 6, 5, 4, 2, 1 and 3. Statistically significant differences were found between each group, with the exception of groups 1 and 3. Finally, the degree of lactic acid solubility was found to increase in the following order : group 6,5,4,2,3 and 1. Significant differences were found between each group. In general dual-curing resin cements displayed substantially lower values than self-curing resin cements with regard to water sorption, water solubility, and lactic acid solubility. Conclusions. From the results of this study, dual-curing resin cements show a significantly lower degree of water sorption and solubility than their self-curing counterparts. Clinically, when selecting resin cements, the product with a lower degree of water sorption and solubility are preferred. The results of this study indicate that the use-of dual-curing resin cements is preferable to self-curing cements.

An infraoccluded tooth is a tooth that has failed to erupt to be in line with adjacent teeth in the vertical plane of occlusion. Multiple complications can occur as a result of an infraoccluded tooth. Tipping of neighboring teeth, loss of space opposing teeth elongation, increased susceptibility to dental caries and abnormal eruption path, impaction and rotation of permanent successor are the consequences of infraocclusion of primary molar. Therefore, early diagnosis and treatment is the key to prevent the complications. Treatment options can be periodic follow-up, temporary restoration or extraction of the infraoccluded tooth depending on the presence of the successor, the extent of infraocclusion and the extent of tilting of the neighboring teeth. The infraoccluded primary molars with permanent successors present tend to exfoliate normally. However, failure to do periodic check up of the infraoccluded teeth may lead to serious complications. In these cases, surgical extractions are often necessary after space regaining and space maintainers should be placed until the eruption of the permanent successors are completed.

McCune-Albright syndrome (MAS) is a rare disease characterized by fibrous dysplasia (FD), Cafe-au-lait spots, and endocrine disorder. A 4-year-old girl with MAS visited the clinic with a chief complaint of facial asymmetry and bruxism without any pain. Facial asymmetry and many dental problems such as midline deviation, "ground glass appearance" on the entire jaw, thinned cortical bone, loss of lamina dura and ectopic germs were found. Because of severely displaced tooth germs and FD affected jaw, there is a high possibility of malocclusion during mixed/permanent dentition. It is necessary to observe the eruption pattern periodically. If there are clinical symptoms like an abnormal eruption pattern, facial asymmetry or high caries susceptibility, appropriate interventions of dentist are required.
Purpose : To observe direct effect of irradiation on cariogenic Streptooccus mutans. Materials and Methods : S. mutans GS5 was exposed to irradiation with a single absorbed dose of 10, 20, 30, and 40Gy. Viability and changes in antibiotic sensitivity, morphology, transcription of virulence factors, and protein profile of bacterium after irradiation were examined by pour plate, disc diffusion method, transmission electron microscopy, RT-PCR, and SDS-PAGE, respectively. Results : After irradiation with 10 and 20Gy, viability of S. mutans was reduced. Further increase in irradiation dose, however, did not affect the viability of the remaining cells of S. mutans. Irradiated 5. mutans was found to have become sensitive to antibiotics. In particular, the bacterium irradiated with 40Gy increased its susceptibility to cefotaxime, penicillin, and tetracycline. Under the transmission electron microscope, number of morphologically abnormal cells was increased as the irradiation dose was increased. S. mutans irradiated with 10 Gy revealed a change in the cell wall and cell membrane. As irradiation dose was increased, a higher number of cells showed thickened cell wall and cell membrane and Iysis, and appearance of ghost cells was noticeable. In RT-PCR, no difference was detected in expression of gtfB and spap between cells with and without irradiation of 40Gy. In SDS-PAGE, proteins with higher molecular masses were gradually diminished as irradiation dose was increased. Conclusion : These results suggest that irradiation affects the cell Integrity of S. mutans, as observed by SDS-PAGE, and as manifested by the change in cell morphology, antibiotic sensitivity, and eventually viability of the bacterium.
The purpose of this study was to provide the basic data for preventing dental caries, and maintaining and enhancing oral health. The subjects of this study were 138 male and female students who were in the 6th grade of elementary schools in Gimpo city. This study investigated the number of decayed tooth and the factors related to the number of decayed tooth, by using the results of questionnaire and oral health survey over such subjects. So, this study obtained the following conclusions. 1. For the number of decayed tooth, 'nothing(D = 0)' was the highest as 37.7%, and 'from two to three' was 23.9%, 'more than four' was 21%, and 'one' was 17.4% in order. 2. For the frequency of visiting dental clinics within the last one year, 'one to two times' was the highest as 52.2%, and the children who have not visited dental clinics even one time during one year were 23.9%. 3. For average daily tooth brushing frequency, 'two times' was the highest as 71.7%. For the use of oral health devices besides toothbrushes, 'I don't use' was the highest as 54.3%. For the experience of sealant and fluoride application, 'nothing' was the highest as 86.9% and 71.3% respectively. For the recognition on the use of fluoride dentifrice, 'I don't know' was the highest as 66.9%. 4. The socio-demographic factors related to the number of decayed tooth were mother's age, mother's background, mother's employment, and after-school fosterer(p > 0.05). 5. The oral health care factors related to the number of decayed tooth were average daily tooth brushing frequency, the use of oral health devices besides toothbrushes, and the experience of fluoride application.(p > 0.05) 6. The snack intake factors related to the number of decayed tooth were tooth care foods intake frequency and decaying foods intake frequency(p > 0.05). 7. The oral health belief item related to the number of decayed tooth was susceptibility(p > 0.05). 8. According to the results of regression analysis, the less mother was employed, the more the average daily tooth brushing frequency was, the more the tooth care foods intake frequency was, the less the decaying foods intake frequency was, and the higher susceptibility was, the lower the number of decayed tooth. 9. In order to prevent and cure early the dental caries which occur frequently in elementary school students, the establishment of oral health centers within schools should be expanded to promote tooth brushing instruction, fluoride solution rinsing, diet control, periodic oral examination.
