Dry mouth is common among older adults and significantly affects the oral health-related quality of life. It is a significant risk factor for dental caries, particularly root caries, in older adults due to concurrent periodontal disease and age-related comorbidities. Clinicians managing patients with dry mouth must be aware of preventive measures against dental caries. This clinical review aims to update our knowledge on the use of fluoride for caries prevention in order to establish better strategies for the management of dry mouth in older adults.
Objectives: The present study aimed to evaluate the usability, quality, and usefulness of the mobile application, CAMBRA-kids, for caries management in preschoolers. Methods: Ninety-one caregivers and preschoolers participated in the user evaluation of CAMBRA-kids. The evaluators assessed the usability, quality, and usefulness of CAMBRA-kids. The collected data were analyzed using R studio 1.2.1335 for Windows (RStudio Inc. 2018, Boston, MA, USA). The evaluations for usability, quality, and usefulness were analyzed with descriptive statistics, Kruskal-Wallis rank sum test, and Wilcoxon rank sum test. Results: On user evaluation, the quality and usefulness of the application scored >3 points, and majority of the users rated the quality and usefulness of the application as high. The quality of the application's system was the highest at 3.68 points, and the actual usage of the application was the lowest at 3.10 points. The application had the highest technological aspect score of 3.20, whereas the content related to oral health information had the lowest score of 3.10/5. Although the application showed no statistically significant differences in the quality and usefulness according to general characteristics, there was a statistically significant difference (p<0.05) between the two when compared between the caries risk group. For all items, the protector of the high-risk group evaluated the quality and usefulness of the application highly. The moderate risk group gave the lowest evaluation score for the application's quality and usefulness. The usability score of the application, according to the user version of the mobile application rating scale (uMARS), was 3.25, and the average engagement score was 2.82 (56.31%). The highest subscale score was 3.63. Conclusions: In this study, we found suitable quality, usability, and usefulness of the mobile application, CAMBRA-kids. In future, the use of this application will contribute to the prevention of dental caries among preschoolers.
Purpose: The present study confirms the content validity and understanding level of the questions of the Korean version of caries management by risk assessment (CAMBRA). Methods: To develop the Korean version of CAMBRA for children aged 0-5 years, a series of steps were followed: first, translation and reverse-translation processes were conducted by an expert committee. To establish content validity, CAMBRA was modified to adapt to the conditions in Korea by 17 dental hygiene professionals with a master's or a higher degree. The responses were analyzed using analysis of frequency and descriptive statistics. Content validity was verified by calculating the content validity index (CVI). The survey was administered to 64 mothers and caregivers to assess the understanding level of questions of the Korean version of CAMBRA. Results: Content validity of each of the items showed an S-CVI/AVE value of 0.79, but the content validities varied from 0.63 to 1.0. The understanding level of each question was between 3.56 and 4.46, thus showing favorable results. Conclusion: It was concluded that the test items verified for content validity were suitable for assessing caries risk for children of ages 0-5 years. This study would contribute to improving children's oral health and disease prevention by providing timely intervention.
치아우식증은 치아, 바이오필름(biofilm), 식이 요인의 상호작용을 기본으로 타액, 불소 등의 구강 환경요인과 생물학적, 행동적, 사회문화적, 유전적 요인이 관여하는 복잡한 다인성질환이다. 최근 치아우식증은 외부 병원체의 감염에 의한 것이 아닌 구강 미생물군의 생태적 변화에 따른 불균형(dysbiosis)의 결과로 이해되면서 전염성질병에서 비전염성 질병(non-communicable diseases, NCD)으로 전환되었다. 치아우식증은 심혈관질환이나 당뇨병과 같이 만성적으로 진행되는 비전염성 질환(NCD) 특성을 가지며, 식이섭취와 생활 습관과 환경적 요소들이 관여한다는 점에서 유사성이 있다. 높은 유병률과 함께 사람들의 건강과 삶의 질에 미치는 영향을 감안할 때 효과적인 구강 건강관리를 목표로 비전염성질환(NCD)로서 치아우식증에 대한 이해가 필요하며, 구강 미생물군의 생태적 균형을 이루기 위한 적절한 예방법과 효율적인 공중보건 정책들이 제공되어야 할 것이다.
본 연구는 2018년 아동 구강건강실태조사자료를 활용하여 아동의 치아우식증에 영향을 미치는 위험 요인을 알아보고자 시행하였다. 연구 대상은 만 12세 아동 20,235명을 대상으로 시행하였다. 조사항목으로 일반적인 특성, 치아우식 식이 행태, 구강 상태, 구강건강 관련 행태 요인을 조사하였다. 그 결과 성별, 지역, 경제수준, 주관적 구강건강 상태, 치아우식 식이 행태, 구강상태, 구강건강 관련 행태 모두 위험요인으로 나타났다. 특히 구강 상태는 치석, 치은 출혈, 치아 통증, 반점치 증상을 나타내는 학생에서 치아우식증 위험도가 더 높게 나타났다. 구강건강 관련 행태는 잇솔질 횟수 2회 이하, 치실과 손잡이 치실 미사용 학생에서 치아우식증 위험도가 높게 나타났다. 이상의 결과로 볼 때 치아우식증의 감수성이 가장 높은 아동을 대상으로 국가나 지역사회는 치아우식을 초기에 진단하여 관리할 수 있고 계속 구강건강관리를 위한 치아우식 관리사업체계를 구축함이 필요하다. 또한 아동·청소년기의 구강건강관리 습관을 개선시킬 수 있는 구강보건교육을 확대하고 치아우식증 예방 프로그램 개선 및 지역사회 활용의 구강보건정책의 체계가 필요하다.
Objectives : The purpose of this study was to provide basic data for the development of general and oral health improvement programs for the elderly by analyzing the effect of health risk factors on the remaining teeth of the elderly in day care centers. Methods : The subjects were 294 elderly people who used day care centers in Seoul. Results : In terms of health risk factors, the elderly who did not have diabetes or smoke had more remaining teeth. In terms of oral health risk factors, the elderly with no dental caries, periodontal disease and dental pain had more remaining teeth. Conclusions : It is necessary to activate systematic welfare services for the elderly that fit their lifestyle in the light of the health and oral health risk factors of the elderly who use day care centers.
Objectives : The aim of the study si to investigate correlation between metabolic syndrome risk factors and oral health status in workers and to propose the integrated and effective management measures. Methods : Subjects were 4,600 workers in industrial company. Data by medical checkup and oral examination were collected from July 13, 2010 to September 12, 2010. Self-reported questionnaire included general characteristics, job description, smoking, drinking, and metabolic syndrome risk factor indicators. Gingivitis, dental calculus and missing tooth tended to have more metabolic syndrome risk factors. Data were analyzed by descriptive statistics, chi-square test, correlation analysis, logistic regression using SPSS version 18.0. Results : Dental caries revealed 1.146 times higher than the control group. Risk group showed the following results; gingivitis was 1.317 times higher in risk group and 1.612 times in metabolic syndrome. Dental calculus was 1.532 times higher in risk group and 1.557 times in metabolic syndrome. Mssing teeth were 1.976 higher in metabolic syndrome. Conclusions : Metabolic syndrome risk factors had close correlation to poor raol health status. It is necessary to establish the effective plan for the oral health in workers.
본 연구는 2009 국민건강영양조사자료를 근거로 6세 이상 인구의 DMFT index(9,271명)를 구하고 그 중 상위 1/3인 고위험군을 분류하여 SiC index(2,517명)를 산출하였다. 이에 치아우식증 고위험군의 존재를 확인하고 고위험군에 대한 예방전략의 필요성을 강조하고자 하였다. 통계 분석은 STATA 11.0 프로그램을 이용하여 t-test 분석을 하였다. 1. DMFT index의 연구대상자는 남자가 45.0%(4,174명), 여자가 55.0%(5,097명)이었으며, SiC index 대상자를 산출한 결과 전체 2,517명이었으며, 남자는 36.3%(914명), 여자는 63.7%(1,603명)로 SiC index에서 여자가 더 많았다. 2. DMFT index와 SiC index의 평균을 보면 연령이 증가할수록 두 집단 간 평균의 차이가 컸으며, 특히 13, 14세는 평균 4 이상, 15세 이상부터는 평균 2배 이상의 차이를 보였다. 3. 성별에 따라 DMFT index를 비교한 결과 14세부터 남녀 간에 통계적으로 유의하였으나 결과는 모든 연령에서 여자가 남자보다 높게 나타났다. SiC index는 성별에 따른 통계적 유의성이 없었으며, 9세와 14세에서 약 1개 여자가 더 많게 나타났다. 4. 지역별 DMFT index를 비교한 결과 60대와 70세 이상에서 도시보다 전원지역이 더 높았다. SiC index는 60대에서 도시가 15.49, 전원지역이 17.04로 약 1.5개 정도 전원지역이 높았다. 이상의 결과를 볼 때 DMFT index는 연령이 증가할수록 성별의 차이를 보였으나 SiC index는 성별, 지역별 큰 차이를 보이지 않았다. 고위험군은 성별과 지역별 차이 없이 포괄적이고 집중적인 예방관리가 필요하며, 고위험군의 구강건강증진을 위한 구강보건정책이 수립되어야 한다.
본 연구의 목적은 비만 및 다이어트가 치아우식에 미치는 관계를 융합적으로 파악하는 것이다. 본 연구는 국민건강영양조사 자료(2008년, 2018년)를 활용한 2차 분석연구로, 연구대상은 영구치우식경험이 있는 대상자이다. 연구에서 자료는 SPSS /WIN 24.0을 이용하여 t-test, ANOVA, 회귀분석을 하였다. 연구결과 성별에 따른 영구치 우식경험여부는 여성이 높았고(M=.76, W=.83), 교육수준에 따른 영구치 우식경험여부는 고학력에서 높았다(대학 이상=.92). 주관적 체형인식에 따른 차이에서는 비만이 영구치 우식경험과 연관이 있다는 것을 확인하였다. 체중 조절 방법 중 단식이 영구치 우식경험에 영향이 컸다. 체중 변화에 따른 영구치 우식경험여부는 체중의 감소, 증가 모두 영향을 미치는 것으로 나타났다. 향후 연구에서는 비만과 관계있는 당 섭취의 연구뿐만 아니라 현 트렌드에 맞추어 진행되는 다이어트에 치아우식의 위험을 알리는 올바른 구강보건 교육 연구가 계속 수행되어야 할 것이다.
General status of pediatric dentistry in Korea is to conduct vigorous academic activities and specialized medical care centering the Korean Association of Pediatric Dentistry (KAPD) that has about 1,000 pediatric dentists as members, pediatric dentistry departments of 11 Colleges of Dentistry, numbers of pediatric dentistry training institutions and private clinics specialized in children. From 1996, the accredited pediatric dentists were produced by the KAPD and from 2008, the state began to produce the accredited pediatric dentists. Since then, doctors with expertise in pediatric care had opened private clinics in addition to the university hospitals, it became the basis of a momentum to deepen the specialty of pediatric dentistry. The Dentistry community of Korea is going through rapid and profound changes recently, and the underlying reasons for such changes can be classified largely into a few categories: (1) Decreasing population and structural changes in population (2) Increase in numbers of dentists, (3) Changes in the pattern of dental diseases and (4) Changes in medical environment. In Korea, the children population in the age range of 0 ~ 14 years old had been decreased by 2 million in 2010 compared to that of 2000 due to reduction of birth rate. The current population of children in the age range of 0 ~ 4 years old in 2010 takes up 16.2% of the total population, but it is estimated that such percentage would decrease to 8.0% by 2050. Such percentage is largely behind the estimated mean global population of 19.6% by 2050. On the other hand, the number of dentists had been largely increased from 18,000 in 2000 to 25,000 in 2010. And it is estimated that the number will be increased to 41,000 by 2030. In addition, the specialized personnel of Pediatric dentistry had been shown as increased by 2.5 times during past 10 years. For the changes in the pattern of dental diseases, including dental caries, each df rate of 5 years old children and 12 years old children had been decreased by 21.9% and 16.7% respectively in 2010 compared to 2000. Each df Index also had been decreased by 2.5 teeth and 1.2 teeth respectively. The medical expenditure of Korea is less than that of OECD and more specifically, the expenditure from the National Health Plan is less than OECD but the expenditure covered by households is larger than OECD. These facts indicate that it is considered as requiring the coverage of the national health plan to be reinforced more in the future and as such reinforcement needs continuous promotion. In medical examination pattern of Pediatric dentistry, the preventive and corrective treatment were increased whereas the restorative treatment was decreased. It is considered that such change is caused from decrease of dental caries from activation of the prevention project at national level. For the restorative treatment, the restorations in use of dental amalgam, pre-existing gold crowning and endodontic treatment had been decreased in their proportion while the restorative treatment in use of composite resin had been increased. It is considered that such changes is caused by the change of demands from patients and family or guardians as they desired more aesthetic improvement along with socio-economic growth of Korean society. Due to such changes in dentistry, the pediatric dentistry in Korea also attempts to have changes in the patterns of medical examination as follows; It tends to implement early stage treatment through early diagnosis utilizing various diagnostic tools such as FOTI or QLF. The early stage dental caries so called white spot had been included in the subjects for dental care or management and in order to do so, the medical care guidelines essentially accompanied with remineralization treatment as well as minimally invasive treatment is being generalized gradually. Also, centering the Pediatric dentists, the importance of caries risk assessment is being recognized, in addition that the management of dental caries is being changed from surgical approach to internal medicinal approach. Recently, efforts began to emerge in order to increase the target patients to be managed by dentists and to expand the application scope of Pediatric dentistry along with through such changes. The interest and activities of Pediatric dentists which had been limited to the medical examination room so far, is now being expanded externally, as they put efforts for participating in the preventive policy making process of the community or the state, and to support the political theories. And also opinions are being collected into the direction that the future- oriented strategic political tasks shall be selected and researches as well as presentations on the theoretical rationale of such tasks at the association level.
