요양보호사의 감정노동과 장기요양기관의 서비스 질의 관계 - 직무관련 삶의 질 매개효과 - (Effect of Long-term Care Worker's Emotional Labor on Service Quality of long term care facility - Mediating Effect of Professional Quality of Life -)

  • 본 연구에서는 요양보호사의 감정노동, 직무관련 삶의 질, 서비스 질의 관계를 분석하였다. 연구대상은 전국의 장기요양시설에 근무하는 요양보호사 211명이며, 자기기입식 설문지를 활용하여 조사 하였다. 연구모형은 감정노동(표면행위, 내면행위)을 독립변수, 직무관련 삶의 질(공감만족, 공감피로)을 매개변수, 서비스 질을 종속변수로 구성하여 검증하였다. 모형의 적합성과 서비스 질에 영향을 미치는 각 요인의 직접효과, 간접효과를 검증한 결과 모형의 적합도는 적합한 수준이었으며, 감정노동 표면행위와 내면행위에서 서비스 질로 향하는 직접효과는 통계적으로 유의미하지 않는 것으로 나타났다. 그러나 직무관련 삶의 질을 매개하여 서비스 질에 영향을 미치는 총 효과는 통계적으로 유의미하였으며, 내면행위와 서비스질의 관계에서는 정적 상관관계, 표면행위와 서비스질의 관계는 부적 상관관계가 있음을 확인할 수 있었다. 서비스 질에 영향력이 가장 큰 변인은 직무관련 삶의 질(공감만족)이었다. 이 연구 결과를 토대로 요양보호사의 감정노동과 삶의 질에 대한 임상적, 학문적 관심을 환기시킬 필요성을 제안하였다.

지역사회 당뇨병 노인의 당뇨병 임파워먼트와 건강인식이 당뇨병 자가간호행위에 미치는 영향 (The Effects of Diabetes Empowerment and Health Perception on Diabetes Self-Care Behavior in Community Diabetic Elderly)

  • 본 연구는 지역사회 당뇨병 노인을 대상으로 당뇨병 임파워먼트, 건강인식, 당뇨병 자가간호행위 간의 상관관계를 파악하고 당뇨병 노인의 당뇨병 자가간호행위에 미치는 영향요인을 파악하고자 시행되었다. 일 지역 보건소에 등록된 65세이상 당뇨병 노인 80명을 대상으로 설문조사하였다. 평균 나이는 71.15세였고 남자가 41명, 여자가 39명이 포함되었다. 당뇨병 자가간호행위는 경제적 상태(r=.245, p=.029), 당뇨병 임파워먼트(r=.406, p<.001)와 유의한 양의 상관관계를 나타내었으나 건강인식(r=.127, p=.263)과는 유의한 상관관계를 나타내지 않았다. 본 연구결과, 당뇨병 노인의 당뇨병 자가간호행위에 미치는 요인으로 당뇨병 노인의 당뇨병 임파워먼트와 경제적 상태가 유의한 영향요인으로 나타났으며 수정된 모형의 설명력은 19.6%(F=10.623, p<.001)로 나타났다. 따라서 지역사회 당뇨병 노인의 당뇨병 임파워먼트를 향상시킬 수 있도록 지역사회 프로그램을 개발하고 공중보건 차원에서 경제적 지원 정책도 함께 제공된다면 지역사회 당뇨병 노인의 자가간호행위의 향상을 도모할 수 있을 것이다.

부모의 온정적 양육행동과 또래 상호작용이 유아의 외현화 문제행동에 미치는 영향 (The Effects of Parenting Behavior and Peer Interaction on Preschoolers' Externalizing Problem Behaviors)

  • The purpose of this study was to examine the effects of parenting behavior and peer interaction on externalizing problem behaviors of preschoolers. The data of 953 49~55 month old preschoolers and their parents were extracted from the Korean Children Panel Survey of Child-Care Policy Research Institute(2012). This study was conducted with Structural Equation Modeling(SEM). The results of this study were as follows. First, parenting behavior had a direct influence on externalizing problem behaviors of preschoolers. Second, parenting behavior had a direct influence on preschoolers' peer interaction. Third, preschoolers' peer interaction had a direct influence on externalizing problem behaviors of preschoolers. Fourth, preschoolers' peer interaction had mediating effects on the relationship between parenting behavior and externalizing problem behaviors of preschoolers.

행동조절을 통한 장애인 환자 치료 (Behavior management for dental patients with special needs)

  • Individuals with special needs include those with behavioral issues, developmental disorders, cognitive disorders, congenital or genetic disorders, or systemic disease. These conditions may place them at increased risk for oral diseases. Dental management of patients with special needs require in-depth understanding of the background of disability and available behavioral guidance theories. Therefore dental team members need more training in the theory and practice of behavior management principles, which might lead to a clinical experience that is more respectful of the dignity and independence of patients with special needs. The dental professional should be flexible to modify the behavior management approach according to the individual patients needs. Also a family/care-giver centered approach based on their preferences and concerns, the patient's challenging behaviors, and related medical problem can serve to improve the treatment planning and oral health management of dental patients with special needs. This article focuses on uncooperative behavior and behavior management, which help practicing dentists to understand their role in the care of patients with special needs.

남성들의 외모관리행동에 관한 연구(I) - 연령을 중심으로 - (A Study on Appearance Management Behavior of Male Consumers(I) - Focusing on the Age -)

  • The purpose of this study was to examine the appearance management behavior and its motives according to male consumers' age. A set of questionnaire was administered to 398 male consumers. Data were analyzed by utilizing frequency, factor analysis, ANONA, and Crosstabs. The results showed that factors of the appearance management behavior were classified into such as fashion oriented, figure management, figure improvement, skin care, cosmetic surgery, hair care, and health oriented. The male consumers age in the 20s, were more conscious of the appearance management. The male consumers aged over 40s were more aware of figure management and hair care for better health oriented. Appearance management behavior motives were classified into individuality, sociality, and interest arousal. Most of the male consumers showed higher interests in sociality among appearance management motives for better personal relations at workplace and increasing competitive edge.

Educational Status and Needs of Premature Birth Prevention and Its Association with Preconception Health Behavior among Women of Childbearing Age in Korea

  • Purpose: The aim of this study was to investigate the educational status and needs of premature birth prevention, and to identify factors associated with preconception health behaviors. Methods: The study design was a crosssectional descriptive study. Data were collected through an online questionnaire survey, and the subjects were 192 women of childbearing age in Korea. Data were analyzed using the Mann-Whitney U test, Kruskal-Wallis test, Spearman's correlation coefficients, and multiple regression. Results: The proportion of subjects who received education on premature birth prevention was 8.9%, and 75.5% of subjects answered that they needed education on premature birth prevention. They demanded education through online media, small groups, cases, cartoons (webtoon) with stories, pictures, and videos. A related factor of preconception health behavior was self-efficacy for high-risk pregnancy health care (β=.20, p=.012), which accounted for 8.2% of the total variance related to preconception health behavior. Conclusion: There was a need for more development of education programs to prevent premature birth for women of childbearing age. Its education programs should be applied with online, small group activities using various educational media. It is also required to promote preconception health behavior through self-efficacy for high-risk pregnancy health care.

대학병원 직원의 환자정보보호행동 분석 (An Analysis of the Behavior of Tertiary Care Hospital Employee in ensuring the Confidentiality of Patient Records)

  • Ensuring the confidentiality of patient records is critical requirement for quality of care and in fulfilling legal obligation of healthcare organizations. This study analyzed the behavior of hospital employees who are dealing with confidential patient information in a hospital. Theory of Planned Behavior(TPB) model and TPB expanded models that add habit concept to TPB are tested for the validity in explaining the predisposing factors that affect the behavior of hospital employee in ensuring the confidentiality of patient records. Data were collected by administrating a survey to the 350 employee of a tertiary care hospital. Of the 350 questionaries distributed, 321 were responded resulting 92% of response rate. The mean differences among the groups classified by age, years of experience, gender, and occupation were analysis using ANOVA. The relationships among the concepts suggested in the models were analysed by applying the Structural Equations Modeling method. The results of ANOVA indicated significant mean differences in the frequency of confidentiality ensuing behavior. Administrative staff and medical technicians show higher frequency of ensuing behavior compared to the physicians and the nurses. And more experienced employee show more confidentiality ensuring behavior. The results of Structural Equations analysis showed that the strong effect of habit and attitude in predicting the behavior. However, the effect of perceived behavioral control was not significant. Based on the results the theoretical and practical implications are discussed.

치료놀이를 활용한 집단프로그램이 만 2세 영아의 친사회적 행동에 미치는 영향과 영아의 친사회적 행동 변화 과정 (The Effect of a Group Program Using Theraplay on Prosocial Behavior of 2-year-old Infants and Process of Infants' Prosocial Behavior Change)

  • 본 연구는 치료놀이를 활용한 집단프로그램이 만 2세 영아의 친사회적 행동에 미치는 영향과 프로그램 과정에서 영아의 친사회적 행동 변화 양상을 알아보기 위해 실시되었다. W시의 한 어린이집에 재원 중인 영아 12명을 실험집단과 통제집단에 배정하였고 실험집단에 주 2회씩, 총 11회의 집단프로그램을 실시하였다. 적응적 사회적 행동 척도(Hogan et al., 1992)로 사전-사후검사를 실시하고 Wilcoxon rank-sum test로자료를 분석해 프로그램의 효과를 알아보았다. 실험집단을 대상으로 한 프로그램의 전 회기를 녹화하고 Padgett(2001)의 제안을 따라 분석해 친사회적 행동이 어떻게 변화하는지를 살펴보았다. 연구결과, 실험집단의 친사회적 행동이 유의미하게 향상되었다. 또한 친사회적 행동은 프로그램의 전 과정에 걸쳐 점진적으로 개선되었다. 본 연구의 결과는 치료놀이를 활용한 집단프로그램이 만 2세 영아의 친사회적 행동 증진에 효과적임을 의미한다.

자가간호증진을 위한 건강계약이 고혈압자의 건강행위 이행에 미치는 영향 (A Study of the Effects of Health Contracting on Compliance with Health Behaviors in Clients with Hypertension)

  • It is generally accepted that the delivery of health care is undergoing many changes specially those related to acute, contagious disease care and to the increase of chronic illnesses which can not be cured but are controlable. The health care practitioner can not be soley responsible for the control of their clients' care. Because the clients will play a vital role in controlling their illnesses, long term participation by both the health care provider and the client is necessary. Since most individuals with hypertension do not experience signs or symptoms, the disease is difficult to detect and even when diagnosed, clients do not comply well with their hypertension regimens. The noncompliant client is at increased risk for compliants involving the heart, brain, kidney and other organs. In an effort to explore methods of increasing patient participation in and adherence to treatment programs for hypertension, the researcher used health contracting to promote self care. The research questions are; 1) Will the health contracting increase compliance in health behavior and reduce the blood pressure\ulcorner 2) If clients comply with their regimens will this reduce their blood pressure\ulcorner The research design utilized in this study was a quasi-experimental design. A purposive sample, was abtained from two churches in the 1. area, consisting of 64 clients with hypertension. The data was collected from the middle of January to the 1st of September 1985. Randomization was only of the two church groups into experimental and control groups. Compliance with health behavior related to the hypertensive regimen, blood pressure and body weight were measured, compared and analyzed. In the experimental group measurements were made 6 times; one month before the education program after education program when health contracting was done and 4 more times once a month for 4 months. In the control group measurements were made 3 times; one month before the education program after the education program, and once 4 months later. There was no health contracting. The data were analyzed by t-test, Pearson correlation and ANOVA according to purpose of the study. The result of this study may be summarized as follows: The result related to the hypothesis on the effect of health contracting are as follows: H$_1$; “The hypothesis that the experimental group, with a health contractual agreement will demonstrate increased compliance levels for health behavior than the control group” was supported(t=-5.29, df=62, p=.000). H$_2$; “The hypothesis that the experimental group, with a health contractual agreement, will demonstrate a greater reduction in blood pressure than the control group” was supported (for systolic blood pressure t=2.72, df=62, p=.009, for diastolic blood pressure t=1.95, df=62, p=.050). H$_3$; The hypothesis that the greater the compliance of the client with health behavior the lower the client's blood pressure will be was partially supported (for systolic pressure r=-.2981, p=.008, for diastolic pressure r=-.1720, p=.087). From the examination of the results of this study it can be concluded that the interaction between the nurse and the client, contracting to define goals and reinforcing compliant behavior, leads to improved compliance with health care behaviors and thus to an increase in the effectiveness of nursing care. Further consideration need to be given to the inclusion of the concept of health contracting in primary nursing and to further research in this area.

경피적 관상동맥 중재술을 받은 노인환자의 자가간호 교육프로그램 개발 및 평가 (Development and Evaluation of a Self Care Education Program for Elderly Patients with Percutaneous Coronary Intervention)

  • Purpose: The purpose of this study was to identify the effects of the education program to promote self care for elderly patients with percutaneous coronary intervention. Method: The research was a quasi-experimental study using a nonequivalent control group non-synchronized design. The participants was 56 patients, 28 in the experimental group and 28 in the control group. A pretest and a posttest were conducted to measure main variables. The self care education program, consisting of flash animation, leaflet and telephone counseling, was given for the experimental group. Data were analyzed using ${\chi}^2$, Fisher's exact test, and independent t-test with SAS statistics program. Results: There were statistically significant increase in knowledge and self care behavior, and decrease in anxiety by difference between a pretest and a posttest in the experimental group compared to that in the control group. Conclusion: The result of this study indicate that the self care education program is effective in increasing knowledge and self care behavior, and in decreasing anxiety for elderly patients with percutaneous coronary intervention. Therefore, it can be usefully utilized in the field of nursing for elderly patients with percutaneous coronary intervention.
