An Analysis on Actual Condition of Health Promotion Program through Oriental Medicine in Health Center (한방건강증진HUB보건소사업 실태분석)

  • This study was carried out to examine the actual condition of health promotion program through oriental medicine in the health center and to provide basic data to develop proper policy of oriental medical health promotion program for the community people. The data were collected from 26 health centers which have been implementing the oriental medical health promotion program, using selfadministered questionnaire for two weeks from 1 October to 15 October 2006. The results are as follows : Generally, the respondents have the positive views on the level of budget and facilities/equipments of the oriental medical health promotion program in health center. However, they have the negative views on the level of manpower and education/training of the program. And also more than 70% of the respondents have the negative opinion on capabilities of formulating and evaluating the oriental medical health promotion program. The respondents indicated that there was the lack of coordination between the oriental medical health promotion program and existing health promotion in health center, and that low rate of utilizing community resources. With regard to the method of selecting the target group for the program, there are differences according to the each program. Many programs tended to select the target group not through the criteria of life-course and illness group but through the efficiency of selecting group. And many programs such as stroke prevention program, constitutional medicine program, oriental medical prenatal program, oriental medical prenatal and postnatal program, oriental medical child care program are mainly composed of the development of educational program and lecture. Regarding the number of the present oriental medical health promotion programs, around 65% of respondents answered that the number of the programs was many and thus they needed to decrease to the proper level. And with regard to the priority of the need, effectiveness and the satisfaction for each programs, on the whole, Qui gong program, stroke prevention program, area-specialty program and oriental medical home visiting program have high score. In particular, oriental medical quit-smoking program has lowest score. From these results, it requires to develop and improve the oriental medical health promotion program in health center considering the need and characteristics of community.

Problems with Transferring Major Trauma Patients to Emergency Medical Center of a University Hospital from Another Medical Center (대학병원 응급의료센터로 전원되는 중증 외상환자의 현황 및 문제점)

  • Purpose: The incidence of multiple trauma is increasing nowadays and is the leading cause of death among young adults. Initial treatment is well known to be crucial in multiple trauma victims. However, many indiscriminate transfers occur due to the lack of a well-organized trauma system in Korea. The objective of this study is to demonstrate the current serious state in which major trauma patients are transferred to the Emergency Medical Center of a university hospital from another medical center. Methods: From November 2009 to October 2010, we performed a retrospective study to analyze the characteristics of patients who visited the Ajou University Medical Center located in Gyeonggi-do. We evaluated the ISS (injury severity score), and a score over 15 point was identified as major trauma. The major trauma patients were separated into two groups according to the visit route, and the characteristics of each group were analyzed. Results: Among the 88,862 patients who visited to the Emergency Medical Center, trauma patients accounted for 19,950, and 343 of them were evaluated as major trauma patients. Among the 343 patients, 170 patients had been transferred from other medical centers. The proportion of males to females was 3.3:1, and the mean ISS was 22.7. The leading cause of trauma was motor vehicle accidents. Of the total 170 patients, 77.6% were admitted to the Intensive care unit and 36.3% underwent surgery. The 170 patients that had been transferred to our medical center, 78.8% were transferred from Gyeonggi-do, 15.3% were transferred from other regions, and 5.9% were miscellaneous. Conclusion: Almost half of the major trauma victims treat at our medical center had been transferred from other medical centers. Establishing a traumatic system, supported by well-organized trauma centers and emergency medical services, that can reduce inappropriate transfers among medical facilities is essential.

  • The purpose of this study is to analyze supporters' community formation motives/ Process/consumption experiences and community characteristics. For this purpose, this study collected the data using ethnographic interview. participant observation, documents and media reports. The results of this study show that supporters communities' formation and diffusion process were influenced by individual characteristics(e.g., personality, hobby and etc.), community characteristics(e.g.,team performance, star player, facilities and etc.) and external factors(ex: media movement etc.) and supporters have experienced various emotions such as intimacy. cohesion, pride and so on through various activities at on-line and off-line site. Community characteristics were classified into we-ness, rituals/traditions, moral responsibility. We found that we-ness influenced emotional dimensions such as joy, pleasure, fun and excitement. rituals and traditions made members feel passion. hope. love and vitality. and moral responsibility provided satisfaction. enthusiasm anxiety. regret and so on. Also, emotional attachment and brand loyalty were increased by these experiential aspects of community consumption.

Effects of the College Financial Support Program on Development of the Department (전문대학 재정지원사업이 학과발전에 미치는 영향연구)

  • The purposes of this study which was done by questionnaire survey and case study of First Aid Certification Program in Seoul Health College were to analyze results and effects of the college financial support program from Ministry of Education & Human Resources Development. The significant 164 data were collected from 300 professors in 8 colleges having the Department of Emergency Care from Oct. 1, 2002 to Dec. 31, 2002 and analyzed using SPSS in terms of t-test, ANOVA, Pearson's correlation coefficient and so on. The conclusions and suggestions from this study were summarized as follows; (1) Degree of acknowledgement about aims, contents, results of the college financial support program was above the middle(participants 4.21, non-participants 3.63) and higher relation than the others in case of participants(r:0.32 p<0.001), planning team(r:0.22 p<0.05). (2) Degree of acknowledgement about improvement of curriculum & educational facilities through the program was above the middle and higher relation than the others in case of participants. (3) Degree of acknowledgement about positive influence on acquirement rate of certification(licence) & job through the program was above the middle and performance & adaptation at work place was higher relation than the others in case of participants. (4) Degree of acknowledgement about positive influence on improvement of learning of student's major through the program was above the middle and higher relation than the others in case of participants. (5) The respondents thought that the program had a negative influence on professor's educational activities & research such as study 40.4%, lecture 23.1%, student guidance 12.5%. (6) When the program was administered by planning team, there were difficulties such as execution of budget 42.1%, cooperation with companies 17.1%, cooperation among program teams 11.8%, when it was administered by non-planning team there were difficulties such as execution of budget 45.5%, cooperation among departments 22.7%, cooperation with companies 13.6%. (7) Because it was too short to execute the budget, the ministry have to select proposed programs before the next school year. (8) This program should be changed temporary, insufficient support into consecutive, sufficient support for more characterization & specialization of the college which has a long-term developmental plan & vision.

Requirements for Cerebrovascular Surgery in Comprehensive Stroke Centers in South Korea

  • Objective : Cerebrovascular disease (CVD) was the third most common cause of death in South Korea in 2014. Evidence from abroad suggests that comprehensive stroke centers play an important role in improving the mortality rate of stroke. However, surgical treatment for CVD is currently slightly neglected by national policy, and there is still regional imbalance in this regard. For this reason, we conducted a survey on the necessity of, and the requirements for, establishing regional comprehensive cerebrovascular surgery centers (CCVSCs). Methods : This investigation was performed using the questionnaire survey method. The questionnaire was consisted with two sections. The first concerned the respondent's opinion regarding the current status of demand and the regional imbalance of cerebrovascular surgery in South Korea. The second section asked about the requirements for establishing regional CCVSCs. We sent the questionnaire to 100 board members of the Korean Society of Cerebrovascular Surgeons. Results : Most experts agreed that cerebrovascular surgery patients were concentrated in large hospitals in the capital area, and 83.6% of respondents agreed that it was necessary to alleviate the regional imbalance of cerebrovascular surgery. With regards to personnel, over 90% of respondents answered that at least two neuro-vascular surgeons and two neuro-interventionists are necessary to establish a CCVSC. Regarding facilities, almost all respondents stated that each CCVSC would require a neuro-intensive care unit and hybrid operating room. The survey asked the respondents about 13 specific neurovascular surgical procedures and whether they were necessary for a regional CCVSC. In the questions about the necessity of cerebrovascular surgical equipment, all seven pieces of equipment were considered essential by all respondents. A further five pieces of equipment were considered necessary on site: computed tomographic angiography, magnetic resonance angiography, conventional angiography, surgical microscope, and surgical navigation. Our results may provide a basis for future policy regarding treatment of cerebrovascular disease, including surgery. Conclusion : Raising the comprehensiveness of treatment at a regional level would lower the national disease burden. Policies should be drafted regarding comprehensive treatment including surgery for cerebrovascular disease, and related support plans should be implemented.

Effects of Depression, Expectations Regarding Aging on the Health Promoting Lifestyle among Health Checkup Examinee (건강검진 수검자의 노화에 대한 기대, 우울이 건강증진 생활양식에 미치는 영향)

  • This is a correlational study to investigate the degree of expectation regarding aging, depression and health promotion lifestyle and to identify the factors influencing health promotion lifestyle among health checkup examinee. The subjects of the study were 143 health checkup examinee who visited the M medical center located in G Metropolitan city. Data were collected from July 5 to 21, 2016 using the structured questionnaire of self-report form. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation, and hierarchical regression. As a result, health promoting lifestyle were correlated with expectation regarding aging positively, and with depression negatively. The factors influencing the health promotion lifestyle were education level, perceived health status, time to move to health care facilities, expectation regarding aging, and depression. With controlled general characteristics, the level of health promotion lifestyle was better explained by the addition of the independent variables of expectation regarding aging, and depression, and the explanatory power of the final model was calculated as 27.0%. Therefore, in order to improve health-promoting lifestyle, it is necessary to consider cognitive factors (knowledge, expectation regarding aging) and psychological factor (depression) besides physical approach.

Correlation between scope of practice and clinical paramedic properties (임상 1급응급구조사의 특성과 업무범위 간의 상관관계)

  • The aim of this study was to reveal correlation between scope of practice and clinical paramedic properties. from April 9 to May 12, 2012, one hundred and eighty-one clinical paramedics filled out anonymously to the questionnaire which includes gender, academic background, clinical career, clinical practice and medical direction. To analyse the collected data, Pearson's r at ${\alpha}$=0.05 (two-tailed) was performed using IBM SPSS 19 (Chicago, IL, USA). One hundred and forty (77.4%) working at medical facilities, one hundred and four (57.5%) graduated from four-year colleges, seventy seven (42.5%) acquired paramedic certificate in 2010-2012, one hundred and twelve (61.9%) charted on nursing records, one hundred and sixteen (69.6%) had a good knowledge in scope of practice, one hundred and six (58.5%) thought of legal restriction & absence of direct medical direction leading to narrow clinical practice. Implementing practices and practices needed were thirty two. There were significant relationships in Likert scale 1-5 between paramedic competency and legal practice (p=0.039); implementing practice (p=0.006); practice needed (p=0.049); and overall (p=0.001). Also, between knowledge on scope of practice and implementing practice (p=0.003); overall (p=0.047); clinical career and legal practice (p=0.019); practice needed (p=0.002); and overall (p=0.002). However, these correlations were relatively low (r $$\leq_-$$ 0.238). The working condition of clinical paramedics was restricted by the Emergency Medical Services Systems Act, which requires a narrow scope of practice. This condition leads to the poor quality of emergency care, therefore the scope of practice in the act will be revised from specificism to generalism, from direct medical direction to indirect medical direction by means of quality management.

A Dynamic Analysis of the Women's Labor Market Transition: With a Focus on the Relationship between Productive and Reproductive Labor (여성의 생산노동과 재생산노동의 상호연관성이 취업에 미치는 영향에 관한 경험적 연구)

  • Wornen's lahor market participation as well as the policy concern for wider utilization of married women, have continuously grown up. However, research efforts on the determinants of women's labor market participation, in the context of the relationship hetween life courses and active entry into lahor market, has been far behind the growing interest in this field. This study has conducted an event histoiry analysis of women's labor market transition utilizing personal occupational history data collected by the Korea Institute for Women's Development in 1991. The analysis is divided into tow parts: First part introduces logit regression to analyze the determinants of women's labor market participation and exit. The second part employs Cox regression to see the variation of transition rate between employment and non-employment. The result shows that there is a wide variation in women's labor market participation according to age, cohort, and family formation. Special note is needed for the significantly negative effect of marriage and child birth on labor market participation. The transition pattern of lower class women with less education fits well to the prediction of neo-classical economics; but the tendency of highly educated women's regression to non-employment reveals the strong influence of the unfavorable labor market structure, which can be better explained by the neo-structuralist perspective. There is a strong trade-off between productive and reproductive labor of women, which can only be corrected by strong policy implementation, such as extended child care facilities, abolition of discriminatory employment practices, and expansion of flexible part-time employment.

A Study on the Environmental Hazard Factors within Children's Play Facilities in Gyeonggi-do Province (경기도 내 어린이놀이터 환경유해인자 오염실태 연구)

  • Objectives: This study was performed to determine environmental hazard factors and provide more eco-friendly child activity spaces within children's playgrounds installed in Gyeonggi-do Province. Methods: Basic (XRF) and precise (ICP, UV) inspections were conducted. The test items examined were heavy metals and parasite eggs. As a sampling point, painted finish materials (180), synthetic rubber (50), and sand (50) were selected. Results: The total excess rate of heavy metals in the XRF was found to be 7.4% (17/230 points). In a comparison between the basic and precise inspections with 17 excess points, the concentration deviation between the two tests was found to be from 0.01 to 7.7 times, resulting in a large difference. Furthermore, all the excess samples were dual samples. However, the contribution rates of Pb and $Cr^{6+}$ to combined concentration were found to be 85.1 and 14.9% for basic inspection and 91.9 and 8.1% for precise inspections, so there is a similar tendency between the two tests. The excess rate of parasite eggs in sand was expressed at 6%. The excess rate of heavy metals in synthetic rubber was found to be 0%. Conclusion: The reliability of the XRF is low. However, considering the contribution rate of Pb and $Cr^{6+}$ between the two tests, it is likely to be applicable for screening. Dual samples provided high concentrations and excess samples and care should be taken when managing them.

Comparison of the Effects of Virtual Reality and Task-Oriented Programs of Balance for the Elderly (가상현실 프로그램과 현실화된 과제 지향 프로그램이 노인의 균형에 미치는 효과 비교)

  • Objective : The purpose of this research is to discuss on comparative assessments between virtual reality and task-oriented programs of balance for elderly. Method : 12 healthy elderly for each program are received twice a week for 6 week period. Changes in balance ability were assessed using Berg Balance Scale (BBS), Timed Up and Go(TUG), One-Legged Stance Test(OLST) and 5 - Repetition Sit-To-Stand test(5R-STS test). Result : All groups were not statistically meaningful differences between pre- and post-program. However, comparative outcomes among three groups were statistically significant in opening the eyes of One-Legged Stance Test(OLST) from the left and right side, and Berg Balance Scale(BBS). In post hoc, shows a statistically significant in both programs with opening the eyes of One-Legged Stance Test(OLST) from left side. Conclusion : Task-oriented programs is more useful and effective clinical methodology for balance excise programs for elderly. In addition, this clinical trial may be easily implemented by elder care facilities.
