Analytic studies on self-care activities and social activities of physically disabled person (지체장애자의 자기간호수행정도 및 사회활동에 관한 분석적 연구)

  • The main purpose of this study was to find out variables relevant to self-care activities of physically disabled person. The subjects of this analysis were 1277 person which is between 15~64 years, the data came from the 1985 National Interview Survey on Disabled Person in Korea. For this analysis, Breakdown, Oneway and Discriminant Analysis were used. The finding of the analysis can be summarized as follows: First, the mean of self care activities was 2.57 (SD: 0.69, range: 1-3). The relevance for the self-care activities by several variables is as follows. 1. The relevance for the self-care activities by socioeconomic status is significant at age, education level, occupation of household members variables. Especially, in the case of high age, low education level, the self-care activities are shown low score. 2. The relevance by impairment characteristics is shown high significance at all input variables. When disabled person have double impairment. paralysis, late occurance age, and is due to diseases the self-care activities score is lowered. 3. The relevance by health care services variables. is not shown significant at all input variables. Second, the relevance for social activities by sev-eral variables was conducted by discriminant analysis. The relative importance of social activities discriminant function is 0.344 of eigenvalue. The-canonical correlation between the social activities discriminant function and 9 dummy variables is 0.51, total variance of dummy variables for social activities is shown 26 persent. The self-care activities variable represents the highest contribution of its associated variable to the function (canonical coefficient: -.56). The occurance age, the occupation of household members, the education level variables are shown comparatively high contribution to the function. To sum up, this analysis suggests that the self-care activities variable is the highest contributed to the social activities. In relation to self-care concept, this finding will be useful in rehabilitation nursing care.

Evaluation of Child Care Activities in Day Care Centers (시설보육원의 보육활동 실태와 평가)

  • The purpose of this study was to investigate child care activities in day care centers and to evaluate the conditions in order to clarify the phases of our child care activities and to present future directions for desirable child care. The subjects of investigation were 110 day care centers in Seoul authorized by the city administrators until July, 1990. Child care activities were evaluated focusing on the whole development of the child based on developmental, open, interactional and ecological approach that are emphasized in quality child care. By evaluating the actual conditions of child care activities, it was found that child care activities could not be provided at the high quality program because of limitations of personal and physical resources.

The Influence of Health Literacy and Diabetes Knowledge on Diabetes Self-care Activities in Korean Low-income Elders with Diabetes

  • Purpose: This study aimed to assess the levels of and relationships among health literacy, knowledge about diabetes, and self-care activities in the Korean low-income elderly with diabetes and to identify factors influencing the self-care activities of this vulnerable population. Methods: This study surveyed a total of 151 low-income elderly patients with diabetes registered at 16 Visiting Health Care Services in B City, Korea. Health literacy was measured with the Newest Vital Sign. Diabetes knowledge was measured with the Diabetes Knowledge Test. The Summary of Diabetes Self-care Activities Questionnaire was used to assess diabetes self-care activities. A stepwise multiple regression analysis was conducted to identify significant factors influencing diabetes self-care activities in these patients. Results: In the regression model, diabetes knowledge (${\beta}=.322$, p<.001), exercise (${\beta}=.337$, p<.001), and experiences of diabetes education (${\beta}=.241$, p=.001) were significantly associated with increased diabetes self-care activities in low-income elderly patients with diabetes when gender, education, health literacy, and subjective health state were controlled. Conclusion: To improve diabetes self-care activities in the low-income elderly with diabetes, it is important to develop a customized program considering their knowledge, exercise, and diabetes education experience.

Time Spent for Child Care among Employed and Non-Employed Mothers with Young Children (취업 여부에 따른 어머니의 미취학자녀 돌봄시간 사용 양태 : 단독주행동, 동시주행동, 동시부행동을 기준으로)

  • This study aims to better estimate the amount of time spent for child care and to understand the characteristics of time spent for child care among employed and non-employed mothers with children under age 6 by taking concurrent activities into account. To assess time spent for child care, I differentiate child care activities into three types: primary activity without concurrent activities, primary activity with concurrent activities, and secondary activity with concurrent activities. The Major results of this study are as follows: First, employed mothers spent 145 minutes less than non-employed mothers in the total amount of time spent for child care during weekdays and this difference diminished to 62 minutes on weekends. Specifically there were differences in all types of time spent for child care among employed and non-employed mothers during weekdays, while the difference was only significant in the average amount of primary time spent for child care without concurrent activities on weekends. Second, there was no difference in the average ratio of time spent for child care with concurrent activities among employed and non-employed mothers during weekdays. By contrast, the average ratio of time spent for child care with concurrent activities of employed mothers was 5% higher than non-employed mothers on weekends. Lastly, the highest concurrent activity with child care activities was leisure followed by housework. This means that child care, leisure, and housework are somewhat related to each other among mothers.

Association between caregiving activities and care burden among caregivers of people with dementia

  • The purpose of this study was to examine the difference in the association between caregiver's activities and caregiving burden according to gender and family relationship of caregivers of older people with dementia. This study used data from the Caregivers of Alzheimer's Disease Research survey (n=476). The association between caregiving activities and care burden was analyzed by multiple regression. In this study, the caregivers were predominantly spouses, followed by daughters. The care burden, especially personal burden, and depression were significantly higher in women than men. The spouses (either male or female), compared with the sons and daughters, spent significantly more time providing care. Care time and depression of caregivers and physical disability of the patient were significantly correlated with care burden. Among the caregiving activities, using transportation, dressing, eating, looking after appearance, and supervising were significantly associated with care burden. The daughters and daughters-in-law presented more care burden with higher number of care days, and the female spouse who were younger tended to experience higher care burden. Daughters who provided longer time looking after appearance exhibited higher care burden. For female spouse, eating time was significantly associated with care burden. The association between caregiving activities and care burden of caregivers of people with dementia differed by gender and family relationship with the patient. This study was characterized by analyzing the effect of caregiving activities on caregiving burden by gender and family relationship of caregivers.

Measuring Workload of Home Visit Care Activities Using Relative Values (방문요양 행위 업무량의 상대적 가치 측정)

  • Objectives : The purpose of this study was to measure the workload of home visit care activities and their relative values. This study examined also factors that affect the workload of home visit care activities. Methods : The participants of this study were 126 home-helpers of 50 home visit care agencies at the 2nd Long-term Care Insurance Demonstration Project. The workload of home visit care activities was divided into total work and four dimensions ; physical efforts, mental efforts, stress and time. Home visit care activities consisted of four categories with 24 items. We used magnitude estimation method to measure their relative values of the four dimensions. The participants answered the relative values of each activities based on the reference service. We used the activity for supporting their elderly's evacuation as the reference service. Results : Most of the respondents were over 40 years old female. They consumed most their time supporting elderly's going out. They consumed their highest physical, mental efforts, and stress for activities of coping with emergency situation. The Pearson correlation coefficients showed significant relationships between workload and each dimensions. This study showed that all four dimensions are statistically significant predictors of workload of home visit care activities. Also, we found that the home-helper's career affects the workload of home visit care activities. Conclusions : The workload of home visit care activities could be explained by physical efforts, mental efforts, stress and time.

Development of the Nursing Record Forms for Effective Home Care Nursing -Focused on Postpartum Women following a Cesarean Section and Newborns- (가정간호업무 효율성을 위한 간호활동 기록도구 개발 -제왕절개 산욕부와 신생아를 중심으로-)

  • Purpose: The purpose of this study was to develope nursing activities record to improve home care nursing for postpartum women following a Cesarean section and newborns. Method: This study for instrument development had three phases: first, selection of nursing activities according to intervention, second, validation of the preliminary home care nursing activities, and third. application of the home care nursing activities. The subjects for validaton were 137 home care nurses and clinical nurses in department of maternity. Result: By Fehring's method, 116 nursing activities according to 19 interventions were included in the preliminary nursing activities record. Among them, 51 critical nursing activities and 65 supporting nursing activities were chosen. During the final process of validation, 121 nursing activities were included. Conclusion: In order to have systemic standardization of this record forms, replication and application in the various home nursing area is need in the future.

  • Background: This study investigates improvements in non-value-added nursing activities in clinical work, in order to improve the efficiency and quality of nursing activities. Methods: The study was performed as a quality improvement project at a tertiary general hospital. The nursing activities that needed improvements were categories into admission care, discharge care, supply management, diagnostic work-up related activities, and others. The nursing time and frequency of non-value-added activities were compared across nine nursing units before and after implementation of the quality improvement program. Post-implementation patient and nurse satisfaction were subsequently analyzed. Result: Post-implementation, the time spent on non-value-added nursing activities was reduced and patients and nurses were satisfied with the improvements. Discussion: Reducing non-value-added activities in nursing can increase the work efficiency and ensure time for patient care, thus improving the quality of nursing care. For further study, accurate surveys on nursing activities based on nursing time are required.

The Impact of Psychological Insulin Resistance on Self-Care Activities in Patients with Type 2 Diabetes Mellitus Undergoing Insulin Therapy (인슐린요법을 받는 제2형 당뇨병 환자의 심리적 인슐린저항성이 자가관리에 미치는 효과)

  • Purpose: The purpose of this study was to identify the level of psychological insulin resistance and self-care activities and to evaluate the factors affecting self-care activities in patients with type 2 diabetes mellitus undergoing insulin therapy to provide basic data for the development of educational programs. Methods: Data were collected through the interviews using a structured questionnaire from August 29 to October 20, 2017, from the patients with type 2 diabetes mellitus visiting the Diabetes Mellitus Center at H-General Hospital in J-city. The subjects were 168 patients who had been being treated via self-injection for at least three months after the start of insulin therapy. Data analyses were conducted using t-test, ANOVA, Pearson's correlation coefficients, and stepwise multiple regression using the SAS WIN 9.2 program. Results: The mean score of psychological insulin resistance was 61.25 (range 19-95) and the mean score of self-care activities was 53.19 (range 18-90). Self-care activities were significantly different by gender (t = -2.94, p= .004), perceived health status (F= 7.00, p< .001), and hypoglycemia during the last three months (t= -2.47, p= .015). Negative correlation was observed between psychological insulin resistance and self-care activities (r= -.33, p< .001). Self-care activities were significantly predicted by psychological insulin resistance, perceived health status, gender, and hypoglycemia during the last three months, and 19.0% of the variance in self-care activities was explained (F= 9.01, p< .001). Conclusion: Psychological insulin resistance in patients undergoing insulin therapy and its effects on self-care activities identified in this study will be useful in starting and maintaining insulin therapy in the future.

Analysis of Clinical Nursing Activities Using Home Hospice Nursing Intervention Standards (가정호스피스 간호중재 표준서에 의한 임상간호활동 분석)

  • Purpose: The purpose of this study was to validate the nursing intervention standards of home care to home care setting for a guide in giving quality care to home hospice patients. Methods: The developed nursing standards were applied to 79 home hospice patients at K hospital, C university. Data were collected through the developed nursing intervention standards of home care with 19 selected nursing interventions and 418 associated nursing activities from January to June in 2006. Results: The performance frequency was 509 times for total nursing interventions and 7,815 times for total associated nursing activities. The most frequently used nursing activity was 722 times(9.24%) on teaching prescribed medication, followed by surveillance(718 times, 9.18%), and vital signs & neurologic monitoring nursing intervention(701 times, 8.97%). The highest mean nursing activity performance rate showed on Intravenous therapy(82%), followed by dying care(81%), and vital signs & neurologic monitoring (80%). Among 418 nursing activities, there were three classifications: 168 as core activities, 165 as major, and 85 as accessory activities. Conclusion: The final validated nursing intervention standards can guide home care nurses to perform quality care and contribute to computerized nursing services and request of nursing fees.
