Pulmonary Artery Embolization of Intravenous Leiomyomatosis Extending into the Right Atrium

  • A 43-year-old woman was diagnosed with an intravenous leiomyomatosis at a previous hospital and transferred to our hospital to undergo surgical treatment. Emergency one-stage operation for coincidental removal of intra-abdominal, right atrial, and intravenous masses were planned. Upon arriving at the operating room, she suffered a sudden onset of severe dyspnea and showed hemodynamic instability. Intraoperative TEE showed pulmonary embolization of a right atrial mass. Removal of the pulmonary artery mass and the intra-abdominal mass, and the cardiopulmonary bypass were performed without any complications.

  • The object of this study is to develop a mathematical model of the hemodialysis system including the mechanism of solute kinetics, water exchange and also cardiovascular dynamics. The cardiovascular system model used in this study simulates the short-term transient and steady-state hemodynamic responses such as hypotension and disequilibrium syndrome (which are main complications to hemodialysis patients) during hemodialysis. It consists of a 12 lumped-parameter representation of the cardiovascular circulation connected to set-point models of the arterial baroreflexes, a kinetic model (hemodialysis system model) with 3 compartmental body fluids and 2 compartmental solutes. We formulate mathematically this model in terms of an electric analog model. All resistors and most capacitors are assumed to be linear. The control mechanisms are mediated by the information detected from arterial pressoreceptors, and they work on systemic arterial resistance, heart rate, and systemic venous unstressed volume. The hemodialysis model includes the dynamics of urea, creatinine, sodium and potassium in the intracellular and extracellular pools as well as fluid balance equations for the intracellular, interstitial, and plasma volumes. Model parameters are largely based on literature values. We have presented the results on the simulations performed by changing some model parameters with respect to their basal values. In each case, the percentage changes of each compartmental pressure, heart rate (HR), total systemic resistance (TSR), ventricular compliance, zero pressure filling volume and solute concentration profiles are represented during hemodialysis.

  • A 43-year-old man experienced chest trauma due to a car accident. Compound sternal fractures with severe dislocation were seen on computed tomography of the chest. Using a SternaLock plating system with manual reduction, fixation of the sternal fracture was successfully performed. There were no complications related to the operation.

  • Blood in congenital or acquired AY fistula(arteriovenous fistula) flows from arteries directly to veins. detouring peripheral micro-circulation. This makes a great effect on the hemodynamics of human cardiovascular system. In this study, a computational method using lumped parameter mode) was proposed to simulate the cardiovascular hemodynamics of patients with acute AV fistula The cardiovascular system model with a fistula compartment in left lower limb was built using 17 standard lumped compartments. Using fourth order Runge-Kutta method. we solved numerically the unsteady linear set of the ordinary differential equations resulting from application of Kirchhoff's law to the lumped parameter hemodynamic model. The baroreceptor reflex system was implemented to explain the auto-regulation effect of the cardiovascular system with acute AV fistula.

  • da $Vinci^{TM}$ Surgical System is an advanced mode of minimal invasive surgery, using 3-dimensional imaging system and robotic arms which can mimic the dexterity of the human hand. Thus, various operations can be performed with minimal incision and limited surgical field and the merits of minimal invasive surgery can be maximized by using it. We report our first experience of robotic open heart surgery using the da $Vinci^{TM}$ Surgical System for repairing atrial septal defect.

Impact of High-Dose Statin Pretreatment in Patients with Stable Angina during Off-Pump Coronary Artery Bypass

  • Background: Periprocedural treatment with high-dose statins is known to have cardioprotective and pleiotropic effects, such as anti-thrombotic and anti-inflammatory actions. We aimed to assess the efficacy of high-dose rosuvastatin loading in patients with stable angina undergoing off-pump coronary artery bypass grafting (OPCAB). Materials and Methods: A total of 142 patients with stable angina who were scheduled to undergo surgical myocardial revascularization were randomized to receive either pre-treatment with 60-mg rosuvastatin (rosuvastatin group, n=71) or no pre-treatment (control group, n=71) before OPCAB. The primary endpoint was the 30-day incidence of major adverse cardiac events (MACEs). The secondary endpoint was the change in the degree of myocardial ischemia as evaluated with creatine kinase-myocardial band (CK-MB) and troponin T (TnT). Results: There were no significant intergroup differences in preoperative risk factors or operative strategy. MACEs within 30 days after OPCAB occurred in one patient (1.4%) in the rosuvastatin group and four patients (5.6%) in the control group, respectively (p=0.37). Preoperative CK-MB and TnT were not different between the groups. After OPCAB, the mean maximum CK-MB was significantly higher in the control group (rosuvastatin group $10.7{\pm}9.75$ ng/mL, control group $14.6{\pm}12.9$ ng/mL, p=0.04). Furthermore, the mean levels of maximum TnT were significantly higher in the control group (rosuvastatin group $0.18{\pm}0.16$ ng/mL, control group $0.39{\pm}0.70$ ng/mL, p=0.02). Conclusion: Our findings suggest that high-dose rosuvastatin loading before OPCAB surgery did not result in a significant reduction of 30-day MACEs. However, high-dose rosuvastatin reduced myocardial ischemia after OPCAB.

  • Objectives: The purpose of this study is to review the simulator for cardiovascular system. Methods & Results: Simulators were classified according to the structure and function of cardiovascular system. Heart and blood vessel were selected as the represent of structure. Blood pressure and blood flow were chose as the functional index. With the view points of four keywords, four kinds of simulators were selected: artificial heart, pressure simulator, flow simulator, and pulse simulator. Conclusions: This paper discussed the state of the art of research and development of the selected four kinds of simulators.

An Experimental Study on Brain Damage and Cardiovascular System Effects of Angelicae Gigantis Radix Extract (당귀가 백서의 뇌손상 및 심혈관계에 미치는 영향)

  • Objective : The purpose of this study was to investigate the effects of Angelicae Gigantis Radix on brain damage and the cardiovascular system. Method : By administration Angelicae Gigantis Radix, we compared treated groups with not treated groups, in view of five points as follows $\circled1$ the intervascular eNOS, $\circled2$ the blood pressure of SHR, $\circled3$ the transcription of the Kir6.2, $\circled4$ the survival time of the rats when common carotid artery was ligated, and $\circled5$ antiplatelet aggregation. Result : In this experiment, Angelicae Gigantis Radix treated group showed significant produced in the intervascular eNOS and decreased the blood pressure of SHR, increased the transcription of the Kir6.2 and SUR significantly in all Treated groups, prolonged the survival time of the rats when common carotid artery, showed the effects on antiplatelet aggregation. Conclusion : According to the above results, Angelicae Gigantis Radix can protect the brain damage and have cardiovascular system effect.

  • Objectives : This study was conducted to assess the effect of Uhwangchungsimwon on the autonomic nervous system. Methods : Eligible subjects were enrolled from healthy people in their 20s-30s. They were divided into two groups, the UC group (n=22) and the control group (n=20). We monitored ECG from 14:00 to 18:00 in both groups. In the UC group, subjects were administered UC at 15:00. HRV analysis was conducted at 14:00, 16:00, and 17:00 for 512 seconds. Results : The degree of LF/HF ratio change was significantly higher in the UC group at 1 hour after administration. Conclusion : We suggest Uhwangchungsimwon may be useful for stabilization of the autonomic nervous system by activating parasympathetic nerve in healthy people.
