만성 흡인을 유발하는 위 식도 역류 모델 (Evidence of Aspiration Gastric contents in Induce Gastroesophageal Reflux in Rats)

  • Background : Anti-reflux procedures treat gastroesophageal reflux (GER) disease. It is known that gastroesophageal reflux is likelyrelated to the increased incidence of chronic rejection in lung transplantation recipients. Because experimental animal studies areto verify this, we have tried to make an animal model of GER in a rat. Material and Methods : Using the SD rats weighing 250-300 g, we surgically induced gastroesophageal reflux and measured the gastrostomy time under anesthesia. Of three groups, Group I was the control, Group II had lower esophageal and anterior myotomy, and Group III had lower esophageal and anterior myotomy plusdiaphragmatic crural myotomy.The animals were scarified, and lung biopsies and histological examinations were performed 1 week, 2 weeks, 4 weeks, 8 weeks and 3 months after gastroesophageal reflux surgery. Results : Baseline animals (n=5) had no GER after charcoal instillation through a gastrostomy tube in Group I. Charcoal-laden macrophages were observed in GroupsII and III. To determine evidence of GER evidence, charcoal was instillated through the gastrostomy tube in group III. In contrast, Group II demonstrated severe neurophil infiltration in the bronchioles and alveolar walls after procedure. After 12 weeks, we observed the disappearance of neurophil, lymphocyte and histiocyte infiltration, and also occasional focal bronchopneumonia and bronchitis. Group III demonstrated neurophil and basophil infiltration in the bronchioles and alveolar walls which was more severe than that in Group II. Interstitial fibrotic changes were observed in Group III.Conclusion : The purpose of our gastroesophageal reflux model was to find evidence of aspiration. There was more evidence of aspiration in Group II than in either of theother two groups.

Xenograft Failure of Pulmonary Valved Conduit Cross-linked with Glutaraldehyde or Not Cross-linked in a Pig to Goat Implantation Model

  • Background: Biologic valved grafts are important in cardiac surgery, and although several types of graft are currently available, most commercial xenografts tend to cause early disfiguration due to intimal proliferation and calcification. We studied the graft failure patterns on non-fixed and glutaraldehyde-fixed pulmonary xenograft in vivo animal experiment. Materials and Methods: Pulmonary valved conduits were obtained from the right ventricular outflow tract of eleven miniature pigs. The grafts were subjected to 2 different preservation methods; with or without glutaraldehyde fixation: glutaraldehyde fixation (n=7) and non-glutaraldehyde fixation (n=4). The processed explanted pulmonary valved grafts of miniature pig were then transplanted into eleven goats. Calcium quantization was achieved in all of the explanted xenograft, hemodynamic, histopathologic and radiologic evaluations were performed in the graft which the transplantation period was over 300 days (n=7). Results: Grafts treated with glutaraldehyde fixation had more calcification and conduit obstruction in mid-term period. Calcium deposition also appeared much higher in the glutaraldehyde treated graft compared to the non-glutaraldehyde treated graft (p<0.05). Conclusion: The present study suggests that xenografts prepared using glutaraldehyde fixation alone appeared to have severe calcification compared to the findings of non-glutaraldehyde treated xenografts and to be managed with proper anticalcification treatment and novel preservation methods. This experiment gives the useful basic chemical, histologic data of xenograft failure model with calcification for further animal study.

Effect of Resveratrol on Coxsackie Virus B3m-induced Myocarditis in Mice

  • To observe the intervening effect of resveratrol on coxsackie virus B3m-induced myocarditis in Balb/c mice and explore the mechanism of intervening effect. Using an animal model of viral myocarditis induced by coxsackie virus B3m (CVB3m), with Ribavirin and Astragalan as comparison, to examine the changes of general condition, mortality, the weights of heart, liver and spleen, serum MDA and NO levels, and cardiac histology in Balb/c mice. By comparison with Ribavirin and Astragalan, it was found that in the mice model of viral myocarditis induced by coxsackie virus B3m resveratrol significantly improved the changes of general condition, mortality, the weights of heart, liver and spleen, serum MDA and NO levels, and cardiac histology. It suggested that resveratrol may have some chemopreventive and chemotherapeutic effects in the treatment of viral myocarditis.

  • Mortality of domestic people from cardiovascular disease ranked second, which followed that of from cancer last year. Therefore, it is very important and urgent to enhance the reliability of medical examination and treatment for cardiovascular disease. Heart Rate Variability (HRV) is the most commonly used noninvasive methods to evaluate autonomic regulation of heart rate and conditions of a human heart. In this paper, our aim is to extract a quantitative measure for HRV to enhance the reliability of medical examination for cardiovascular disease, and then develop a prediction method for extracting multi-parametric features by analyzing HRV from ECG. In this study, we propose a hybrid Bayesian classifier called FP-based Bayesian. The proposed classifier use frequent patterns for building Bayesian model. Since the volume of patterns produced can be large, we offer a rule cohesion measure that allows a strong push of pruning patterns in the pattern-generating process. We conduct an experiment for the FP-based Bayesian classifier, which utilizes multiple rules and pruning, and biased confidence (or cohesion measure) and dataset consisting of 670 participants distributed into two groups, namely normal and patients with coronary artery disease.

심혈관계 질환과 구강기능제한의 관련성 (Association between cardiovascular disease and limited oral function)

  • Objectives: The purpose of this study was to examine the relationship between limited oral function and cardiovascular disease in adults over 40 years of age. Methods: Data from the Seventh Korean National Health and Nutrition Examination Survey (2016-2018) was used. In this study, 8,766 adults over the age of 40 years were included as study subjects. They completed a health survey which included current prevalence of hypertension, stroke, myocardial infarction, and angina pectoris, as well as information about blood tests, physical measurements, and oral examinations. Statistical analyses were carried out using complex sample cross-tabulation analysis, general linear model, and logistic regression analysis. Results: The study showed that limited oral function was experienced by patients with stroke (61.3%), myocardial infarction (49.1%), cardiovascular disease (38.5%), hypertension (38.1%), and angina (36.4%) (p<0.05). In patients with stroke, the risk of limited oral function was 2.393 times higher than in patients without stroke. Patients with hypertension were 1.233 times more at risk of speaking difficulty than those without hypertension. Conclusions: Limited oral function is associated with cardiovascular disease. To improve oral health, it is necessary to provide integrated, health-based oral care.

Effects of a Proteasome Inhibitor on Cardiomyocytes in a Pressure-Overload Hypertrophy Rat Model: An Animal Study

  • Background: The ubiquitin-proteasome system (UPS) is an important pathway of proteolysis in pathologic hypertrophic cardiomyocytes. We hypothesize that MG132, a proteasome inhibitor, might prevent hypertrophic cardiomyopathy (CMP) by blocking the UPS. Nuclear factor kappa-light-chain-enhancer of activated B cells ($NF-{\kappa}B$) and androgen receptor (AR) have been reported to be mediators of CMP and heart failure. This study drew upon pathophysiologic studies and the analysis of $NF-{\kappa}B$ and AR to assess the cardioprotective effects of MG132 in a left ventricular hypertrophy (LVH) rat model. Methods: We constructed a transverse aortic constriction (TAC)-induced LVH rat model with 3 groups: sham (TAC-sham, n=10), control (TAC-cont, n=10), and MG132 administration (TAC-MG132, n=10). MG-132 (0.1 mg/kg) was injected for 4 weeks in the TAC-MG132 group. Pathophysiologic evaluations were performed and the expression of AR and $NF-{\kappa}B$ was measured in the left ventricle. Results: Fibrosis was prevalent in the pathologic examination of the TAC-cont model, and it was reduced in the TAC-MG132 group, although not significantly. Less expression of AR, but not $NF-{\kappa}B$, was found in the TAC-MG132 group than in the TAC-cont group (p<0.05). Conclusion: MG-132 was found to suppress AR in the TAC-CMP model by blocking the UPS, which reduced fibrosis. However, $NF-{\kappa}B$ expression levels were not related to UPS function.

Cardiovascular Health Metrics and All-cause and Cardiovascular Disease Mortality Among Middle-aged Men in Korea: The Seoul Male Cohort Study

  • Objectives: This study estimated the association of cardiovascular health behaviors with the risk of all-cause and cardiovascular disease (CVD) mortality in middle-aged men in Korea. Methods: In total, 12 538 men aged 40 to 59 years were enrolled in 1993 and followed up through 2011. Cardiovascular health metrics defined the following lifestyle behaviors proposed by the American Heart Association: smoking, physical activity, body mass index, diet habit score, total cholesterol, blood pressure, and fasting blood glucose. The cardiovascular health metrics score was calculated as a single categorical variable, by assigning 1 point to each ideal healthy behavior. A Cox proportional hazards regression model was used to estimate the hazard ratio of cardiovascular health behavior. Population attributable risks (PARs) were calculated from the significant cardiovascular health metrics. Results: There were 1054 total and 171 CVD deaths over 230 690 person-years of follow-up. The prevalence of meeting all 7 cardiovascular health metrics was 0.67%. Current smoking, elevated blood pressure, and high fasting blood glucose were significantly associated with all-cause and CVD mortality. The adjusted PARs for the 3 significant metrics combined were 35.2% (95% confidence interval [CI], 21.7 to 47.4) and 52.8% (95% CI, 22.0 to 74.0) for all-cause and CVD mortality, respectively. The adjusted hazard ratios of the groups with a 6-7 vs. 0-2 cardiovascular health metrics score were 0.42 (95% CI, 0.31 to 0.59) for all-cause mortality and 0.10 (95% CI, 0.03 to 0.29) for CVD mortality. Conclusions: Among cardiovascular health behaviors, not smoking, normal blood pressure, and recommended fasting blood glucose levels were associated with reduced risks of all-cause and CVD mortality. Meeting a greater number of cardiovascular health metrics was associated with a lower risk of all-cause and CVD mortality.

  • In this study, the effects of vascular parameter changes and electrodes on VOP measurement based on IPG were simulated mathematically. For the evaluation of the effects of hemodynamic changes on VOP, a mathematical model, which consists of cardiovascular system model and venous occlusion model, was developed and the model solution representing the blood flow and pressure in measuring point was found by 2nd order Runge-Kutta method. And, with sensitivity coefficients obtained from finite element solution of electric field in measuring point, the effects of electrode system on measurement were evaluated. As increasing the resistance, the venous capacitance was not changed but the venous outflows were decreased and the decreased compliance reduced the venous capacitance. And, for several configurations of round electrodes and band electrodes, the sensitivity coefficients were computed using the electric field distribution along deep vein. In conclusion, the proposed mathematical cardiovascular model could be applied to the simulation study on the effects of hemodynamic parameters on DVT diagnosis with IPG. And, also the sensitivity coefficients could provide effective electrode configuration for exact measurement of VOP.

The Correlation of Serum Osteoprotegerin with Non-Traditional Cardiovascular Risk Factors and Arterial Stiffness in Patients with Pre-Dialysis Chronic Kidney Disease: Results from the KNOW-CKD Study

  • Background: Osteoprotegerin (OPG) plays protective roles against the development of vascular calcification (VC) which greatly contributes to the increased cardiovascular events in patients with chronic kidney disease (CKD). The present study aimed to find the non-traditional, kidney-related cardiovascular risk factors correlated to serum OPG and the effect of serum OPG on the arterial stiffness measured by brachial ankle pulse wave velocity (baPWV) in patients with the pre-dialysis CKD. Methods: We cross-sectionally analyzed the data from the patients in whom baPWV and the serum OPG were measured at the time of enrollment in a prospective pre-dialysis CKD cohort study in Korea. Results: Along with traditional cardiovascular risk factors such as age, diabetes mellitus, pulse pressure, and baPWV, non-traditional, kidney-related factors such as albuminuria, plasma level of hemoglobin, total $CO_2$ content, alkaline phosphatase, and corrected calcium were independent variables for serum OPG in multivariate linear regression. Reciprocally, the serum OPG was positively associated with baPWV in multivariate linear regression. The baPWV in the 3rd and 4th quartile groups of serum OPG were higher than that in the 1st quartile group after adjustments by age, sex and other significant factors for baPWV in linear mixed model. Conclusion: Non-traditional, kidney-related cardiovascular risk factors in addition to traditional cardiovascular risk factors were related to serum level of OPG in CKD. Serum OPG level was significantly related to baPWV. Our study suggests that kidney-related factors involved in CKD-specific pathways for VC play a role in the increased secretion of OPG into circulation in patients with CKD.
