  • It is now generally accepted that individuals at increased risk for cardiovascular disease may be identified by certain traits or habbits. The factors such as high blood pressure, elevated blood cholestrol, age, sex and obesity are associated with increseaed frequency of disease. The blood cholesterol level lowering will decrease cardiovascular disease risk. The regression of atherosclerosis can be achieved by lowering the level of circulating cholesterol. Those things are connected with the quantity and quality of protein, fats, carbohydrates, especially soluble and non-soluble fiber, magnesium and calcium. The lipoprotein and lipid metabolism are connected with the lipid transport. The factors on lipid absorption and blood serum lipid pattern of human are exist. The factors have a variety of materials with different chemical and physical properties. The soluble fiber diet make a low blood and liver lipids. Many kind of soluble fiber results in a lowering of blood cholesterol and triglyceride levels. The cholesterol lowering effects of dietery fiber may be a results of alterations of in intestinal handling of fats, hepatic metabolism of fatty acid or triglyceride acid metabolism of lipoprotein. It is investigated that the high density lipoprotein (HDL) is inversely related to coronary artery disease. It has been postulated that HDL may be an important factor in cholesterol efflux from the tissues, therby reducing the amount of cholesterol deposited there. Alternatively, the HDL may pick up cholestyl ester and phospholipid during normal VLDL lipolysis in the plasma. The HDL levels are relatively insensitive to diet. At present time, the cause-and -diet effect of HDL's inverse relation to CHD remains unclear.

Bayesian Network Model to Evaluate the Effectiveness of Continuous Positive Airway Pressure Treatment of Sleep Apnea

  • Objectives: The association between obstructive sleep apnea (OSA) and mortality or serious cardiovascular events over a long period of time is not clearly understood. The aim of this observational study was to estimate the clinical effectiveness of continuous positive airway pressure (CPAP) treatment on an outcome variable combining mortality, acute myocardial infarction (AMI), and cerebrovascular insult (CVI) during a follow-up period of 15.5 years ($186{\pm}58$ months). Methods: The data set consisted of 978 patients with an apnea-hypopnea index (AHI) ${\geq}5.0$. One-third had used CPAP treatment. For the first time, a data-driven causal Bayesian network (DDBN) and a hypothesis-driven causal Bayesian network (HDBN) were used to investigate the effectiveness of CPAP. Results: In the DDBN, coronary heart disease (CHD), congestive heart failure (CHF), and diuretic use were directly associated with the outcome variable. Sleep apnea parameters and CPAP treatment had no direct association with the outcome variable. In the HDBN, CPAP treatment showed an average improvement of 5.3 percentage points in the outcome. The greatest improvement was seen in patients aged ${\leq}55$ years. The effect of CPAP treatment was weaker in older patients (>55 years) and in patients with CHD. In CHF patients, CPAP treatment was associated with an increased risk of mortality, AMI, or CVI. Conclusions: The effectiveness of CPAP is modest in younger patients. Long-term effectiveness is limited in older patients and in patients with heart disease (CHD or CHF).

  • An eight-day-old neonate was diagnosed with dextro-transposition of the great arteries, atrial septal defect, patent ductus arteriosus, and a single sinus origin of the coronary arteries. The single coronary artery originated from the left sinus (sinus 2), had a proximal left circumflex arterial branch, and passed anteriorly to the right side of the aorta, further branching into the right coronary and left anterior descending arteries. We successfully performed an arterial switch operation and coronary transfer by tube graft reconstruction with autologous aortic tissue to treat the dextro-transposition of the great arteries and atrial septal defect with a single-sinus origin of the coronary arteries.

  • A 50-year-old female was admitted to Pusan National University Hospital with complaints of fatigue and sweating. Echocardiography showed a small patent ductus arteriosus (PDA) and highly mobile vegetations on the aortic valve. Emergency operation was performed due to the high risk of embolization and severe aortic regurgitation. When the pulmonary artery opened, we found unexpected fresh vegetation. The tissue of the PDA was fragile and infected. We successfully removed the infected tissue, closed the PDA with a patch, and replaced the aortic valve with a mechanical prosthesis.

선천성심질환 청소년의 우울과 극복력의 관계분석 (Relationship between depression and resilience in adolescents with congenital heart disease)

  • 목 적 : 선천성심질환 청소년의 우울정도와 극복력의 상관관계를 파악하고 우울과 관련된 변수를 확인하고자 하였다. 방 법 : 선천성심질환으로 수술을 받고 2개 대학병원 및 심장전문병원에 등록되어 추적관찰 중인 만 13-18세 청소년 231명을 대상으로 하였다. 연구 도구는 만성질환아를 대상으로 개발한 극복력 도구와 Beck의 우울척도를 8-18세에 맞게 변형시킨 Kovasc의 CDI를 사용하였다. 결 과 : 선천성심질환 청소년의 우울정도는 0-54점 가능 점수에서 평균 16.74점이었고, 극복력은 70-160점 가능 점수에서 115.84점이었다. 극복력에 따른 우울정도는 극복력과 우울은 유의한 역상관관계(r=-0.59, P<0.01)를 나타났다. 또한, 극복력의 하부 개념인 개인내적요인(r=-0.69, P<0.01), 대처양상(r=-0.39, P<0.05), 대인관계(r=-0.59, P<0.01)도 우울정도와 통계적으로 유의한 역상관관계를 보였다. 다중회귀분석 결과 선천성심질환 청소년의 우울과 관련된 유의한 변수는 부모의 양육태도(${\beta}=-0.48$, P<0.01) 및 극복력(${\beta}=-0.62$, P<0.01)으로 나타났다. 결 론 : 선천성심질환 청소년에서 청소년 자신의 극복력이 높을수록 부모가 긍정적인 양육태도를 가질수록 우울정도가 낮음을 알 수 있었다. 따라서 환자 자신의 극복력을 파악하고 증진할 수 시킬 수 있는 심층 연구와 선천성심질환 청소년과 같이 만성질환을 가진 자녀의 부모에게 양육태도의 중요성을 가질 수 있도록 돕는 중재개발이 필요하다.

D형 인격 척도의 표준화 예비연구 : 내적일치도 및 구성타당도 (Preliminary Study on the Standardization of Korean Version of Type D Personality Scale 14 : Internal Consistency and Construct Validity)

  • 연구목적 : D형 인격은 국내에서는 아직 생소하며, 심혈관 질환을 비롯한 다양한 질환과의 관련성, 그리고 그 질환에 대한 예측인자로서의 가능성 등에 대해 아직까지 연구된 바가 거의 없다. 그러나, 기존 외국의 연구 결과들은 D형 인격이 심혈관 질환의 발병이나 경과에 중요한 예측인자가 될 수 있음을 보여 주었으며, 현재에도 관련 연구결과가 지속적으로 보고되고 있어 국내에서도 관련 연구가 요구되는 상황이다. 이에 본 저자들은 예비연구를 통해 D형 인격의 검사 도구인 DS14를 번안하고 이에 대한 신뢰도와 타당도를 검증함으로써, D형 인격 평가 척도의 표준화된 기준을 제시하고자 본 연구를 진행하였다. 방법 : 1차 조사에서는 총 17문항으로 구성된 DS14의 예비 척도를 국문으로 번역하여, 서울 및 경기도 안산지역에서 무작위 표집한 심혈관 질환이 없는 정상인 372명에게 작성하도록 하였다. 예비척도에서 척도화가 가능한 문항을 추출하여 14문항의 DS14를 완성하고, 그 신뢰도를 알아보았다. 또한 2차조사에서는 1차조사의 대상 중 40세 이상을 대조군으로 심혈관 질환 환자들과 DS14에 의한 D형 인격의 분포를 비교하였다. 결과 : 1차 조사에서 예비 척도의 분석결과 '사회적 억제' 하위척도의 10문항 중 7개의 문항이 결정되어 14문항의 DS14척도가 완성되었다. 부정적 정서와 사회적 억제 각 하위척도의 내적일치도를 나타내는 신뢰도계수는 각각 0.817, 0.797이었다. 2차조사에서는 관상동맥질환 군에서 일반인구군에 비해 D형 인격의 빈도가 유의하게 높음을 확인할 수 있었다. 결론 : 한국어판 DS14척도는 신뢰도와 타당도를 고려할 때 부정적 정서와 사회적 억제의 양상을 보이는 D형 인격의 평가에 유용한 척도로 생각된다. 또한 본 연구에서는 한국어판 DS14척도를 사용하여, 관상동맥질환의 예측인자로서 D형 인격의 가능성을 확인하였다.

개심술 180례에 대한 임상적 고찰 (Open Heart Surgery:Clinical Analysis of 180 Cases)

  • Between Feb. 1990 and Aug. 1993, 180 cases of the open heart surgery were performed under cardiopulmonary bypass in the Department of Thoracic & Cardiovascular surgery, Gil General Hospital. There were 83 cases with congenital heart diseases [CHD] and 97 cases with acquired heart diseases [AHD]. The CHD consisted of 78 acyanotic[mortality: 3.8 %] and 5 cyanotic cases with heart anomaly[mortality:l case]. The AHD were 97 cases, which contained 53 valvular, 27 ischemic heart diseases, 10 aortic diseases, 5 cases with myxoma, 1 case with post-infarct VSD, and 1 case with removal of infected pacing wire in right ventricle. In the 53 valvular heart diseases, there were 45 cases with valve replacement[MVR 27, AVR 9,MVR + AVR 9] and 8 cases with valvuloplasty. The number of the implanted prosthetic valves were 53. In MVR, 25 St. Jude, 6 Sorin, 3 Carpentier-Edward and 2 Intact medical valves were used. In aortic position, 13 St. Jude, 3 Sorin and 1 Intact medical valves were applied. The operative mortality was 5.6 % [3/53]. The annuloplasty applying artificial ring was performed in 17 patients[4 cases associated with MVR] and the number of the implanted ring was 19, which included 14 Duran ring[10 mitral, 4 tricuspid] and 5 Carpentier ring [3 mitral, 22 tricuspid]. In the 27 ischemic heart diseases, there were 9 cases with left main coronary artery lesions, 7 one vessel, 5 two vessels, and 6 three vessels. Average number of anastomosis was 2.8 per patient. The operative mortality was 14.3 % [4/27]. Among the 10 patients with aortic diseases, 7 cases were aortic dissection[type A: 5, type B: 2] and 3 cases were descending thoracic aortic aneurysm. The operative morality occurred in 3 cases. The overall mortality and the operative mortality of congenital and acquired heart disease was 7.8 %, 4.8% and 10.4%, respectively.

Surgery for Partial Anomalous Pulmonary Venous Connections: Modification of the Warden Procedure with a Right Atrial Appendage Flap

  • Background: Surgical repair of a partial anomalous pulmonary venous connection (PAPVC) to the superior vena cava (SVC) may be complicated by sinus node dysfunction or SVC obstruction. We modified the Warden procedure by using a right atrial auricular flap to decrease the occurrence of these complications. Methods: Between February 2005 and July 2012, 10 consecutive patients underwent a modified Warden procedure to correct PAPVC. The median patient age was 5.7 years. Eight patients (80%) had an atrial septal defect. To surgically correct the PAPVC, we made a U-shaped incision on the right atrial appendage and sutured the flap to the posterior wall of the SVC. The anterior wall was reconstructed with various patch materials. Results: No early or late deaths occurred, nor did any patient require early or late reoperation for SVC or pulmonary venous obstruction. No new arrhythmias appeared during follow-up, which was complete in all patients (mean, 29.5 months). Conclusion: Our modification of the Warden procedure seems to be effective and safe. This technique may lower the risk of SVC obstruction, pulmonary venous obstruction, and sinus dysfunction.

The Clinical Outcomes of Damus-Kaye-Stansel Procedure According to Surgical Technique

  • Background: The Damus-Kaye-Stansel (DKS) procedure is a method for mitigating the risk of systemic ventricular outflow tract obstruction (SVOTO). However, there have been few reports on which surgical technique shows a better outcome. The objective of this study was to compare the outcome of the DKS procedure according to the surgical technique used. Methods: We retrospectively reviewed 12 consecutive patients who underwent the DKS procedure from March 2004 to April 2013. When the relationship of the great arteries was anterior-posterior, the double-barrel technique (group A) was performed. If the relationship was side-by-side, the ascending aortic flap technique (group B) was performed. Results: There was no early mortality and 1 late mortality in group B. There was no statistically significant difference in the median peak pressure gradient of preoperative subaortic stenosis in both groups: 14 mmHg (range, 4 to 53 mmHg) in group A and 15 mmHg (range, 0 to 30 mmHg) in group B (p=0.526). Further, a significant postoperative pressure gradient was not observed in either group A or group B. More than moderate postoperative neoaortic regurgitation was observed in 1 patient of group B; this patient underwent neoaortic valve replacement 66 months after the DKS procedure. No one had a recurrent SVOTO during follow-up. Conclusion: The DKS procedure is an effective way to minimize the risk of SVOTO, and there is little difference in the outcomes of the DKS procedure according to the surgical technique used.

Effects of Insamsansa-eum (Renshenshanzha-yin) on Hypercholesterolemia and Analysis of Its Effects according to the Pattern Identification

  • Backgrounds : Hyperlipidemia is a major cause of cardiovascular disease (CVD). Lowering serum cholesterol levels could reduce the risk of CVD. Insamsansa-eum (Renshenshanzha-yin, ISE), composed of Ginseng Radix and Crataegii Fructus, is a new medicine developed to treat hyperlipidemia and CVD. Objectives : In this study, we intended to explore the clinical effects of ISE on patients with hypercholesterolemia, and moreover we also compared its effects according to the pattern identification. Methods : Subjects were administered ISE with the dose of 600 mg three times a day for 4 weeks. Patterns of subjects were identified with diagnostic scoring system for Yin-Yang and the condition of Excess-Deficiency before treatment. Serum lipids were measured at baseline and after 4 weeks of medication. Results : ISE lowered total cholesterol(TC), triglyceride(TG), total lipid(TL), phospholipid(PL) and low density lipoprotein cholesterol(LDL) significantly. Compared with the data of our previous study, it was less effective than Atorvastatin but showed equal lipids-lowering effect to Chunghyul-dan (Qingxue-dan, CHD). In Yang pattern group, ISE was less effective in lowering TG and LDL than it was in not-Yang-not-Yinpattern group. On safety assessment, there was no adverse effect, hepatic or renal toxicity. Conclusions : We suggest that ISE is a safe and useful herbal medicine for hypercholesterolemia, and moreover it could be more useful when it is used for patients with not Yang pattern.
