Airway Improvement with Medium-Term Compression Duration after Right Pulmonary Artery Anterior Translocation

  • Left main bronchus compression occasionally occurs in patients with cardiac disease. A 19-month-old female patient weighing 6.7 kg was admitted for recurrent pneumonia and desaturation. S he had an atrial septal defect (AS D) with a right aortic arch. Her left main bronchus had been compressed between the enlarged right pulmonary artery (RPA) and the descending thoracic aorta for 14 months. We conducted ASD closure and RPA anterior translocation via sternotomy. The left main bronchus compression was relieved despite the medium-term duration of compression.

군병원에서의 심혈관계 수술 114례에 대한 임상적 고찰 (Clinical Experience of Cardiovascular Surgery in Military Hospital - 114 Cases -)

  • One hundred and fourteen patients were underwent cardiovascular operation from the 11th June 1985 to 12th July 1988 in the department of cardiovascular surgery of Seoul District Hospital. There were one hundred and one open heart surgery patients and thirteen non-open heart surgery patients. Of open heart surgery cases, seventy-four were acyanotic, twenty four were cyanotic congenital heart disease and three were acquired valvular heart disease. VSD was most common[55 cases] among acyanotic group and TOF[21 cases] was most common among cyanotic group. Of non-open heart patients, there were eleven cases of PDA and two cases of shunt[modified Blalock-Taussig shunt using PTFE graft] for TOF. There were one operative death[0.9%] due to inadequate relief of right ventricular outflow tract obstruction and peripheral pulmonary artery stenosis for TOF patient. No Patients were dead during follow-up.

Chylous Manifestations and Management of Gorham-Stout Syndrome

  • Gorham-Stout disease (GSD) was first described by Gorham and colleagues in 1954, but its precise mechanism and cause remain to be elucidated. In this condition, voluminous and potentially fatal chylous effusions into the thorax can occur. Herein, we describe a case of GSD in which the patient presented with massive pleural effusions and mottled osteolytic bone lesions. We performed multiple operations, including thoracic duct ligation using video-assisted thoracoscopic surgery and thoracotomic decortication, but these procedures did not succeed in preventing recurrent pleural effusion and chest wall lymphedema. After administering sirolimus ($0.8mg/m^2$, twice a day) and propranolol (40 mg, twice a day), the process of GSD in this patient has been controlled for more than 2 years.

  • Dementia is a clinical syndrome characterized by progressive impairment of cognitive and functional abilities. As currently applied treatments for dementia can only delay the progression of dementia and cannot fundamentally cure it, much attention is being paid to reducing its incidence by preventing the associated risk factors. Cardiovascular and metabolic diseases are well-known risk factors for dementia, and many studies have attempted to prevent dementia by treating these risk factors. Growing evidence suggests that sex-based factors may play an important role in the pathogenesis of dementia. Therefore, a deeper understanding of the differences in the effects of drugs based on sex may help improve their effectiveness. In this study, we reviewed sex differences in the impact of therapeutics targeting risk factors for dementia, such as cardiovascular and metabolic diseases, to prevent the incidence and/or progression of dementia.

Pragmatic Clinical Studies: An Emerging Clinical Research Discipline for Improving Evidence-Based Practice of Cardiovascular Diseases in Asia

  • Pragmatic clinical studies, an emerging clinical research discipline, include a wide range of studies that are largely embedded with routine clinical practice and aim to evaluate the comparative effectiveness and safety of different clinical intervention strategies. Increased availability and quality of electronic medical/health records drives the development of pragmatic clinical studies. In this review, we describe evolution of the conceptual framework of pragmatic clinical studies and share perspectives on the importance of pragmatic clinical studies in evidence-based practice for cardiovascular diseases, as a complement to conventional randomized controlled trials. We also highlight specific needs of pragmatic clinical studies in improving evidence-based practice for cardiovascular disease in Asian countries. The main challenges of pragmatic clinical studies are discussed briefly in this review.

  • When we prescribed Shiryungtang to a 65 years-old male patient with ARF after stroke, we could observe that his symptoms, I/O balance and laboratory findings were all improved. Therefore, we suggest that shiryungtang might be useful for ARF in the patient with stroke.

소아심장판막치환술 (Pediatric Valve Replacement)

  • Between 1985 and 1993, 29 children from 1 to 15 years of age have undergone cardiac valve replacements at Buchon Sejong Hospital. The patients were composed of 20 males and 9 females and 17 patient had congenital heart disease and 12 patients had acquired heart disease. Two of these patients have had second valve replacements due to paravalvular leakage and valve thrombosis. Single valve replacements were 29 and double valve replacements were 2. All the patients had received prosthetic valves except one. Among the 25 patients who had definite post-operative records, the overall mortality was 12%[4% was early mortality and 8% was late mortality].25 patients were followed up with coumadin anticoagulation for total 633 patient-months[minimum 2 months to maximum 93 months, mean 25.3 months] and actuarial survival rate was 88.5 $\pm$ 6.3% at 7 years and event free rate was 70.3 $\pm$ 11.7% at 7 years. These results suggest that pediatric valve replacements can now be performed at a low operative risk although various problems are still remained and the choice of valve is prosthetic valve mainly due to its durability at the present time.

내과적 치료를 받는 관상동맥질환자의 간호표준 개발 (The Development of Nursing Standards for Coronary Artery Disease Patients Who Received Medical Treatment)

  • Purpose: This study was conducted to develop nursing standards for medical patients with Coronary Artery Disease (CAD) patients. Methods: This study was a methodological study. The content of nursing standards for medical patients with CAD developed in this study was validated. The nursing standards of cardiovascular nursing developed by the American Nurses Association (2008), the nursing standards developed by the Korea Nurses Association (2003) and a clinical manual of patients with CAD developed by K teaching hospital were reviewed. Literature regarding CAD nursing standards was also reviewed. The basic contents of nursing standards for medical patients with CAD were selected by an expert group including two nursing faculties, a cardiovascular unit manager, and two cardiologists. A pilot study was conducted then in real clinical settings, in which includes cardiovascular outpatient clinic, cardiovascular inpatient units, and cardiac intensive care units to evaluate clinical suitability of the nursing standards. Results: The final version of the nursing standards for medical patients with CAD included 12 standards, 24 criteria, 38 indicators and 92 nursing activities. Conclusion: The nursing standards developed in this study can be used in evaluating quality of nursing service and in educating nurses who are involved in patients with CAD.

The role of lipids in the pathogenesis and treatment of type 2 diabetes and associated co-morbidities

  • In the past decade, the incidence of type 2 diabetes (T2D) has rapidly increased, along with the associated cardiovascular complications. Therefore, understanding the pathophysiology underlying T2D, the associated complications and the impact of therapeutics on the T2D development has critical importance for current and future therapeutics. The prevailing feature of T2D is hyperglycemia due to excessive hepatic glucose production, insulin resistance, and insufficient secretion of insulin by the pancreas. These contribute to increased fatty acid influx into the liver and muscle causing accumulation of lipid metabolites. These lipid metabolites cause dyslipidemia and non-alcoholic fatty liver disease, which ultimately contributes to the increased cardiovascular risk in T2D. Therefore, understanding the mechanisms of hepatic insulin resistance and the specific role of liver lipids is critical in selecting and designing the most effective therapeutics for T2D and the associated co-morbidities, including dyslipidemia and cardiovascular disease. Herein, we review the effects and molecular mechanisms of conventional anti-hyperglycemic and lipid-lowering drugs on glucose and lipid metabolism.

  • This paper presents some simulations of fluxes and pressures in the coronary network, in the case of very severe coronary disease (several stenoses on the left branches and total occlusion of the right coronary artery). In that case, coronary artery bypass graft surgery is the commonly performed procedure. However, the success of the intervention depends on many factors. Modeling of the coronary circulation is thus important since it can help to understand the influence of all these factors on the coronary haemodynamics. We previously developed an analog electrical model that includes the eventual presence of collateral flows, and can describe the different revascularization strategies (two grafts, three grafts, ...). The aim of the present work is to introduce in our simulations the time-dependence of the coronary microvascular resistances, in order to better represent the effect of the systolic ventricular contraction (which induces an elevation of the resistances because the vessels are squeezed).
