Effectiveness of Nutrition Counseling on Dietary Behavior and Serum Levels of Lipids and Antioxidants in Patients with Cardiovascular Disease (영양상담에 의한 심혈관질환자의 식행동, 혈청 지질 및 항산화영양소 상태 변화)

  • The purpose of this study was to investigate whether nutrition counseling could lead to a beneficial outcomes in patients with cardiovascular disease(CVD). Fifty CVD patients(23 men and 27 women) participated in a dietary counseling program which was based on serum lipid management. Various markers of disease risk including lifestyles, anthropometric indices, eating behaviors, serum lipids, antioxidants, lipid peroxides were measured before and after the program. The program lasted 3-11 months depending on individuals. Waist/hip ratio decreased after nutrition counseling only in women. Smoking and alcohol drinking status did not change significantly after counseling. Food habit score increased significantly in both men and women(p<0.05), and patients consumed mixed cereal rice, fruits, seaweeds, legumes more frequently and snacks less frequently after nutrition counseling (p<0.05). Although intakes of energy, carbohydrate and vitamin B1 were reduced significantly after nutrition counseling, nutrient densities of calcium and iron of diets have increased significantly after nutrition counseling, and those of other micronutrients were not different Serum total cholesterol decreased significantly in men and LDL cholesterol decreased significantly in women. In women lipid peroxide level decreased and $\alpha$-tocopherol level increased significantly, while there was no significantly change in men. We conclude that well-planned nutrition counseling would reduce risks of cardiovascular disease through improving dietary behavior, lipid profile and antioxidant status.

Analysis of Cardiovascular Medication Use in Dementia Patients (치매환자에서의 심혈관계 약물사용 분석)

  • Background: Dementia is one of important social and economic healthcare issues in the aging age. Therefore, it signifies to analyze the relationship between chronic disease or cardiovascular drug use and the incidence of dementia to establish a basis for increasing or preventing the risk of dementia. The purpose of this study was to investigate the correlation between the prevalence of chronic diseases and the use of cardiovascular drugs in patients diagnosed with dementia. Methods: In this study, we used data from sample of elderly patients from the Health Insurance Review and Assessment Service. We analyzed by logistic regression analysis with age, gender, and medication as covariates. KCD-7 was used to diagnosis of the disease, and drugs were analyzed using ATC codes and Korean standardized drug classification codes. Results: A total of 1,276,331 patients were analyzed in the sample of the elderly population, of which 532,075 (41.7%) were male and 744,256 (58.3%) were female. The patients have the higher risk of dementia in the older, women, and lower socioeconomically status. Cerebral infarction and ischemic heart disease increases risk of dementia. Patients taking statins, angiotensin converting enzyme inhibitor (ACEI) or angiotensin II receptor antagonists (ARB) showed low incidence of dementia. Conclusion: This study has been shown that ACEI, ARB, and statin drugs may associate with lower incidence of Alzheimer's and other dementia except vascular dementia.

A Study on the Comparison of Predictive Models of Cardiovascular Disease Incidence Based on Machine Learning

  • In this paper, a study was conducted to compare the prediction model of cardiovascular disease occurrence. It is the No.1 disease that accounts for 1/3 of the world's causes of death, and it is also the No. 2 cause of death in Korea. Primary prevention is the most important factor in preventing cardiovascular diseases before they occur. Early diagnosis and treatment are also more important, as they play a role in reducing mortality and morbidity. The Results of an experiment using Azure ML, Logistic Regression showed 88.6% accuracy, Decision Tree showed 86.4% accuracy, and Support Vector Machine (SVM) showed 83.7% accuracy. In addition to the accuracy of the ROC curve, AUC is 94.5%, 93%, and 92.4%, indicating that the performance of the machine learning algorithm model is suitable, and among them, the results of applying the logistic regression algorithm model are the most accurate. Through this paper, visualization by comparing the algorithms can serve as an objective assistant for diagnosis and guide the direction of diagnosis made by doctors in the actual medical field.

Short-term Effects of a Lifestyle Intervention Program on Eating Behaviors, Physical Activity and Cardiovascular Risks in Korean Adults (생활습관개선 프로그램이 성인의 식이행동과 신체활동 및 심혈관위험요인에 미치는 효과: 중재 3개월 결과를 중심으로)

  • Objectives: The aim of this study was to evaluate the effects of a lifestyle modification program for Korean adults with cardiovascular disease risk factors on their health behaviors and health status. Methods: A total of 448 adults with abdominal obesity and additional cardiovascular disease risk factors(high blood pressure, low HDL-cholesterol, high triglyceride or high blood glucose) were randomly assigned to either an intensive intervention group (IIG, n=216) or a minimal intervention group(MIG, n=232). Participants in the IIG received lifestyle modification program which consisted of health counseling with nutrition assessment, health booklet and health diary, while those in MIG received minimal information. Results: The participants in the IIG significantly improved dietary habits(p<.05), retrained eating(p<.001), external eating(p<.01) behaviors, leisure time physical activity(p<.05), dietary self-efficacy(p<.01), exercise self-efficacy(p<.01) and MetS score(p<.001) after 3 months. In addition, the participants in the IIG showed more improvement in dietary habits(p<.05) compared with those in the MIG. Conclusion: The lifestyle modification program was effective in improving some health behaviors, behavioral determinants and cardiovascular risk factors for a short term.

  • Saccular aneurysm of the external jugular vein presenting as a neck mass is very rare. We report the surgical treatment of an external jugular venous aneurysm in a 48-year-old female patient due to the cosmetic problem of neck engorgement, concomitant with thyroidectomy for cancer.

Hereditary protein S deficiency presenting acute pulmonary embolism

  • Protein S deficiency is one of the several risk factors for thrombophilia and can cause blood clotting disorders such as deep vein thrombosis and pulmonary embolism. A 54-year-old man was admitted with the complaint of dyspnea and was diagnosed with pulmonary embolism. The patient had very low level of free protein S, total protein S antigen, and protein S activity (type I protein S deficiency). In history taking, we found that his mother, 78 year old, had a history of same disease 10 years ago, and confirmed the pronounced low level of protein S. The patient's son also had very low level of protein S, however there had not been any history of pulmonary embolism yet. This case study suggests that asymptomatic persons with a family history of protein S deficiency and pulmonary embolism should be checked regularly for early detection of the disease, as protein S deficiency can be suspected.

Primary Pulmonary Chordoid Meningioma

  • Primary pulmonary meningioma is a rare disease, and chordoid meningioma is an uncommon variant of meningioma in the central nervous system (CNS) with a high recurrence rate. We report a case of primary pulmonary chordoid meningioma that presented as a solitary pulmonary nodule (SPN). The SPN was resected by thoracoscopic wedge resection and was revealed to have characteristics of chordoid meningioma. After confirming the absence of a meningioma in the CNS by brain imaging, the nodule was diagnosed as a primary pulmonary chordoid meningioma. The patient remained disease-free after 26 months postoperatively. To our knowledge, this is the third case of primary pulmonary chordoid meningioma to be reported.

Clinical Outcomes After Drug-Coated Balloon Treatment in Popliteal Artery Disease: K-POP Registry 12-Month Results

  • Background and Objectives: The popliteal artery is generally regarded as a "no-stent zone." Limited data are available on the outcomes of drug-coated balloons (DCBs) for popliteal artery disease. This study aimed to evaluate the 12-month clinical outcomes among patients who received DCB treatment for atherosclerotic popliteal artery disease. Methods: This prospective, multicenter registry study enrolled 100 patients from 7 Korean endovascular centers who underwent endovascular therapy using IN.PACT DCB (Medtronic) for symptomatic atherosclerotic popliteal artery disease. The primary endpoint was 12-month clinical primary patency and the secondary endpoint was clinically driven target lesion revascularization (TLR)-free rate. Results: The mean age of the study cohort was 65.7±10.8 years, and 77% of enrolled patients were men. The mean lesion length was 93.7±53.7 mm, and total occlusions were present in 45% of patients. Technical success was achieved in all patients. Combined atherectomy was performed in 17% and provisional stenting was required in 11%. Out of the enrolled patients, 91 patients completed the 12-month follow-up. Clinical primary patency and TLR-free survival rates at 12 months were 76.0% and 87.2%, respectively. A multivariate Cox regression analysis identified female and longer lesion length as the significant independent predictors of loss of patency. Conclusions: DCB treatment yielded favorable 12-month clinical primary patency and TLR-free survival outcomes in patients with popliteal artery disease.

  • Coronary arterial involvement in Takayasu arteritis (TA) is not uncommon. Herein, we describe a case of TA with celiac trunk and superior mesenteric artery occlusion combined with coronary artery disease. Bilateral huge internal thoracic arteries (ITAs) and the inferior mesenteric artery provided the major visceral collateral circulation. After percutaneous intervention to the right coronary artery, off-pump coronary artery bypass grafting for the left coronary territory was done using a right ITA graft and its large side branch because of its relatively minor contribution to the visceral collateral circulation.

  • Cerebrovascular and cardiovascular diseases (CVDs) are a major cause of deaths among workers as well as in general population in Korea. Occupational stress like long working hours or overwork can cause sudden unexpected death in a worker who had been suffering from an aggravated preexisting condition such as hypertensive or arteriosclerotic diseases, The Korean government has compensated the cases of cerebrovascular disease and cardiovascular disease due to overwork, but neither a resonable definition of overwork nor detailed evaluation guideline has been officially provided to verify overwork, so that the prevention of, as well as compensation for workers' occupational stress-related cardiovascular attacks, cannot be carried out efficiently. A thorough understanding of the etiopathology of WR-CVDs can be very helpful in developing a prevention strategy.
