둔형 폐동맥 폐쇄증 1 (Pulmonary Atresia with Intact Ventricular Septum)

  • Pulmonary atresia with intact ventricular septum is a extremely rare congenital cardiac anomaly. With the history of cyanosis and failure to thrive, this anomaly should not be excluded and emergency management is necessary. Our patient was 69 day-old male with pulmonary atresia which was confirmed by cardiac angiography. Prostaglandin E1 was used for maintenance of pulmonary blood flow preoperatively. Right ventricular outflow reconstruction with pericardial patch and concomitant pulmonary valvotomy were done on beating heart for palliation. With this method, growing of right ventricle and tricuspid annulus are highly expected.

원발성 심섬유종 -수술체험 1례 보고- (A Case Report of Primary Cardiac Fibroma)

  • We report a case of primary cardiac fibroma which is a rare cardiac neoplasm and arising most frequently from right ventricular free wall and interventricular septum. A 2-month-old male infant with cyanosis was admitted for evaluation and treatment. Echocar-diographic lEnding was a cardiac mass. Successful nearly total removal of the tumor was performed. The pathologic result was compatible with fibroma. The postoperative course was not eventful, and have been in good condition upto now.

  • Primary cardiac fibroma of the left ventricular myocardium is a rare tumor of the heart which is usually located within the anterior wall and /or septum of the left ventricle and is the second most common cardiac tumor in infant and children. Although the tumor is benign histologically, it may cause severe cardiac dysfunction and sudden death. A 30-day-old neonate with a huge intramural fibroma involving the posterolateral wall of the left ventricle underwent it`s partial resection under extracorporeal circulation on April 1992.

Cardiac type 총폐정맥 이상환류증 수술치험 3 (Surgical correction of Total anomalous pulmonary venous drainage of cardiac type. - 3 cases -)

  • Majority of patients with total anomalous pulmonary venous drainage [TAPVD] have severe symptoms within the first few months of life. And early in the experience with correction of TAPVD, hospital mortality was high, especially in infant. In Sejong General Hospital, we operated on 3 infants with TAPVD of cardiac type. Repair was performed under the conventional cardiopulmonary by pass in one case and by the total circulatory arrest in other 2 cases. Interatial septum between enlarged coronary sinus opening and interatrial septal defect was excised and the coronary sinus was radically unroofed to make wide opening between left atrium and common pulmonary venous channel. The defect in atrial septum was closed with redundant pericardial patch. Postoperative courses were uneventful except transient dysrrhythmia of A-V dissociation. They are doing well on follow up check.

심장관통자상의 응급수술 치험 3례 (Penetrating Wound of the Heart: A Report of Three Cases)

  • Three patients who sustained penetrating stab wound of the heart have been treated successfully by emergency thoracotomy in the Department of Thoracic Surgery, Chonnam University Hospital. There were two knife and one glass wound. The location of the injury were all on the right ventricle, but in one patient, it was penetrated to ventricular septum. All patients were in shock with a systolic pressure under 60 mmHg when admitted to the emergency room. In one of the three patients, blood pressure was not detectable and subsequently cardiac arrest. Two patients required immediate thoracotomy because of intrathoracic hemorrhage and increased pericardial tamponade and the other one required prompt thoracotomy because of sudden onset of cardiac arrest. There were no death postoperatively. Two patients are living without any complication in 4 years and 4 weeks after operation. One who had penetrating wound to ventricular septum, turned to cardiac decompensation, but he is living now in 4$\frac{1}{2}$ years after operation. Exploratory thoracotomy should be performed immediately in all the patients in whom a penetrating wound of the heart or pericardial tamponade following a penetrating wound of the chest wall is suspected.

심장점액종의 외과적 치료 (Clinical Experience of Atrial Myxoma)

  • Seven patients [six cases of left atrial myxoma, one case of right atrial myxoma] from 24 to 66 years of age [4 male and 3 female, mean age 48 years] underwent excision of atrial myxoma between 1982 and 1989 at Keimyung University Dongsan Medical Center. All patients presented with congestive heart failure, six with cardiac murmur, three with syncope, two with sinus tachycardia, one each with sinus arrhythmia, atrial fibrillation, pleural effusion, peripheral embolization. Symptoms were present from 1 month to 8 years before operation [mean 28 months], All tumors originated from atrial septum and pedunculated. The myxomas were successfully removed in all patients, either shaving them from atrial septum [n=3] or by excising a portion at normal atrial septum with tumor [n=4]. One case was replaced mitral valve with carbomedics-31mm due to severe mitral regurgitation. Follow up is current. No recurrent myxoma has been identified clinically or by echocardiography. In this series, excellent results were obtained by simple excision of the tumor, with or without a margin of normal atrial septum.

  • Penetrating chest trauma may result in significant intracardiac injury. A traumatic ventricular septal defect is a rare complication that requires surgical management, particularly if heart failure ensues. We report a case of delayed repair of an outlet-type ventricular septal defect and perforation of the aortic and pulmonary valve leaflets following a stab wound. This report highlights diagnostic and surgical considerations and also presents an opportunity to review the conotruncal anatomy, which may be relatively unfamiliar to many adult cardiac surgeons.

Right to Left Atrial Septum Defect Compounded with Patent Ductus Arteriosus in a Maltese Dog

  • A nine-month-old intact female Maltese dog (body weight: 1.5 kg) presented with frequent syncopal episodes (1-2/day), exercise intolerance, and dyspnea. Cardiomegaly with marked dilation of right and left atria along with ascites was indicated on radiography. Tricuspid regurgitant jets (4.36 m/s), pulmonary regurgitant jets (3.4 m/s), left-to-right shunting flow at the ductus arteriosus, and an atrial septal defect were observed on echocardiography. A bubble study with agitated saline found right-to-left shunting at the interatrial septum. Based on diagnostic studies, the dog was diagnosed with right-to-left atrial septum defect (ASD) compounded with patent ductus arteriosus (PDA). The treatment was directed to reduce the pulmonary hypertension, improve cardiac performance, and lower fluid retention. Despite medical treatment for 2 months, the dog died of sudden cardiac arrest. Herein, we describe a very rare case of right-to-left ASD compounded with PDA.

심장 아밀로이드증의 지연 조영증강 MR 영상소견 (Delayed Enhancement Magnetic Resonance Imaging Findings in Cardiac Amyloidosis)

  • 목적 : 심장 아밀로이드증 환자의 심장 자기공명영상의 지연 조영증강영상에서 좌심근의 조영증강 패턴과 다른 심장부위의 조영증강 유무를 평가하고자 하였다. 대상 및 방법 : 2009년 9월부터 2011년 6월 사이 심장 아밀로이드증이 의심되어 심장 자기공명영상을 촬영한 9명의 환자를 후향적으로 분석하였다. 좌심근의 조영증강 여부 및 패턴, 그리고 좌우심방 및 우심실의 조영증강 유무를 평가하였다. 또한 심방중격 두께, 좌심방 혈액신호 강도에 대한 심방중격과 심외막 지방의 상대적 신호강도 비를 계산하여 허혈성 심질환 대조군 16명과 Wilcoxon rank sum test를 통해 통계적으로 비교분석 하였다. 결과 : 총 9명중 8명에서 좌심근 조영증강소견을 보였으며, 8명중 4명의 환자에서 심내막하 원형성, 나머지 4명에서 미만성 패턴을 보였다. 심방중격 두께는 심장 아밀로이드증 환자에서 허혈성 심질환 환자에 비해서 증가되어 있었고 (p = 0.02), 심장 아밀로이드증 환자군에서 심방중격 대 혈액 신호 강도비 (p = 0.0002) 및 심외막 지방 대 혈액 신호 강도비 (p = 0.0006) 모두 의미있게 높았다. 결론 : 좌심실의 심내막하 원형성 혹은 미만성 조영증강 패턴은 심장 아밀로이드증 환자의 지연 조영 심장 자기공명영상의 특징적인 소견이며, 심방중격 및 심외막 지방 대 혈액 신호 강도비 신호강도가 상대적으로 증가된다.

좌심방 양심실 연결증 [DOLA] (Double Outlet Left Atrium: A rare form of the atrioventricular septal defect with malposition of the atrial septum)

  • The case of a patient with abnormal position of the atrial septum resulting in a left atrium with two atrioventricular valves and a disconnected right atrium is presented with review of related articles. Anatomic details showed atrial situs solitus and a left sided cardiac apex. The right atrium received both venae cavae and a coronary sinus. No AV valve was found in the right atrium, and the floor of this chamber was placed above the posterior wall of right ventricular chamber. The atrial septum with secundum ASD was displaced to right anteriorly at its lower portion and inserted to right of tricuspid annulus. The tricuspid and mitral valve configuration was that of so-called partial ECD, i.e. mitral cleft with large anterior mitral leaflets. The ventricular septum was intact and both ventricular chambers were equally well developed with normal relationships. Surgical repair of this anomaly was performed by resecting the abnormally positioned lower part of the atrial septum, repairing the cleft of the anterior mitral leaflet, and septating the atrium for diverting the systemic and pulmonary venous blood to RV and LV, respectively.
