Pharmacological action of extracts of Scutellaria baicalensis on Cardiovascular System (황금(黃芩)의 심장(心臟)에 대한 약리작용(藥理作用))

  • The adrenergic blocking activity and refractory period of cardiac muscle on isolated rabbit atria were measured after administration of Scutellaria. In rabbits and cats the antiarrhythmic action of Scutellaria on atrial and ventricular arrhythmias produced by epinephrine or ouabain was examined and also compared with that of propranolol and quinidine. The alcoholic extract of Scutellaria produced a marked decrease in heart rate and contractile amplitude of the isolated rabbit atria. Pretreatment with Scutellaria rendered the atria to fail to respond to epinephrine, indicating that this crude drug possesses an adrenergic blocking activity. The extract produced a marked prolongation of the refractory period of atrial muscle. The extract effectively abolished the spontaneous arrhythmia occurring in the isolated rabbit atria. As propranolol and quinidine it also suppressed the atrial arrhythmia induced by ouabain. The extract prevented, as propranolol and quinidine, the induction of ventricular arrhythmia arising from excessive dose of epinephrine in anesthetized rabbits and cats. With regard to the ventricular arrhythmia induced by a continuous infusion of ouabain, the alcoholic extract of Scutellaria exerted some suppressive effect in anesthetized rabbits but no effect on cats. From the above results, it may be concluded that Scutellaria is effective against atrial and ventricular arrhythmias. The antiarrhythmic effects of this drug may be the result of adrenergic beta receptor blocking and cardiac depressive activities including prolongation of the refractory period of cardiac muscle.

  • Recently the convergence of healthcare technology is used for daily life healthcare monitoring. Cardiac arrhythmia is presented by the state of the heart irregularity. Abnormal heart's electrical signal pathway or heart's tissue disorder could be the cause of cardiac arrhythmia. Fatal arrhythmia could put patient's life at risk. Therefore arrhythmia detection is very important. Previous studies on the detection of arrhythmia in various ECG analysis and classification methods had been carried out. In this paper, an ECG signal processing techniques to detect abnormal ECG based on DTW minimum accumulation distance through the template matching for normalized data and variable threshold method for ECG R-peak detection. Signal processing techniques able to determine the occurrence of normal ECG and abnormal ECG. Abnormal ECG detection algorithm using DTW minimum accumulation distance method is performed using MITBIH database for performance evaluation. Experiment result shows the average percentage accuracy of using the propose method for Rpeak detection is 99.63 % and abnormal detection is 99.60 %.

The Effect of Moxibustion Treatment for Cardiac Arrhythmia : A Systematic Review (부정맥의 뜸 치료 효과에 대한 체계적 문헌고찰)

  • Objectives: The purpose of this study was to assess the effects of moxibustion treatment for Cardiac arrhythmia through a systematic review. Methods: Electronic database including Cochrane library, Pubmed, China National Knowledge Infrastructure, Embase, DBpia, and Korean studies Information Service System were searched by combining the keywords such as "arrhythmias", "cardiac arrhythmias", "心律失常", "moxibustion" and "艾灸". Through the searching, 5 randomized controlled trials(RCTs) were included except animal testing and cellular experiment etc. The quality of each RCTs was assessed using the Cochrane risk of bias tool. Results: We included 5RCTs involving 468 participants. Two RCTs compared moxibustion versus conventional treatment, another RCT compared moxibustion plus acupuncture versus conventional treatment, another RCT compared moxibustion plus herbal medicine verus conventional treatment, the other RCT compared moxibustion plus herbal medicine and ointment versus conventional treatment. These studies were evaluated by the efficacy rate of treatments. Studies measured outcomes in efficacy, heart rate, electrocardiogram and symptoms. Each of 5 trials statistically showed a significant differences in efficacy rate. One study reported about side effects and another study reported about complications. The overall risk of bias was unclear in 5 studies. Conclusions: The evidence suggests that moxibustion treatment may help to reduce cardiac arrhythmia, however, it is limited, low-certainty and we cannot rule out benefits or risks with this treatment. High-quality studies that report adverse effects are needed.

  • Life-threatening cardiac arrhythmia is a frequent complication of open heart surgery. There are many causes of postoperative cardiac arrhythmias. Electrolyte imbalance such as hypokalemia and acidemia are major causes of ventricular arrhythmias. Infrequently, however, antiarrhythmic agents and /or hypomagnesemia induce[s] a ventricular arrhythmia such as "torsade de pointes" by increasing the repolarization time of myocardium, Recently, we have experienced two cases of "Torsade de pointes" associated with hyp-omagnesemia after replacement of mitral valve and one of whom after use of procainamide. Intravenous infusion of magnesium immediately and successfully abolished the torsade de pointes in both cases.intes in both cases.

Successful Surgical Treatment of Cardiac Complication of Graves Disease

  • Cardiac complications such as arrhythmia and heart failure are common in Graves disease. Early detection and proper treatment of hyperthyroidism are important because cardiac complications are reported to be reversible if the thyroid function is normalized by medical treatment. We report here a case of cardiac complication of Graves disease that was too late to reverse with medical treatment and required surgical treatment.

  • A fluid extract of Zizyphi Semen was employed in this experiment. The cardiac effects of Zizyphi Semen were examined on isolated rabbits atria and heart in situ of anesthetized cats and rabbits. The adrenergic blocking activity and refractory period of cardiac muscle were measured after administration of this drug. In rabbits and cats the antiarrhythmic action of Zizyphi Semen on atrial and ventricular arrhythmias produced by epinephrine or ouabain was examined. The results were following: 1. Zizyphi Semen produced a decrease in rate and contractile amplitude of the isolated rabbit atria and had a week blocking effect on epinephrine acceleration of atrial movement. 2. Zizyphi Semen effectively abolished the spontaneous arrhythmia occurring in the isolated rabbit atria ana the atrial arrhythmia induced by ouabain. 3. Zizyphi Semen produced a marked prolongation of the refractory period in isolated atrial muscle of rabbit. 4. Zizyphi Semen prevented the induction of ventricular arrhythmia arising from excessive dose of epinephrine in anesthetized rabbits and cats. 5. With regard to the ventricular arrhythmia induced by a continuous infusion of ouabain, Zizyphi Semen exerted suppressive effect and produced a marked prolongation of cardiac arrest time in anesthetized rabbits and cats. From the above results, it may be concluded that Zizyphi Semen is effective against atrial and ventricular arrhythmias. The antiarrhythmic effect of this drug may be the result of direct myocardial depressive and partially adrenergic beta receptor blocking activities including prolongation of the refractory period of cardiac muscle.

  • In this study, we have developed 64 channel computerized cardiac mapping system using micro-computer for basic research of electrophysiology and electrical propagation in cardiac arrhythmias. The significant problems of this study are the simultaneous acquisition of large amount data at 64 sites, the need of accurate and rapid analysis, and the effective display of the analyzed data. To solve these problems, we made 64 channel signal pre-processing board in order to amplify and filter the raw signals. And the software for cardiac isochronous mapping which were presented immediately via computer-generated graphics has been developed. This system is expected to enable us to study pathophysiology of cardiac arrhythmia and to improve the results of diagnosis and surgical treatment for cardiac arrhythmia.

Pharmacological Effect of Cholates of Cardiac Function (심운동(心運動)에 대한 담즙산의 영향)

  • The effects of sodium taurocholate(STC) and sodium deoxycholate(SDC) on cardiac function were examined by using isolated atria of rabbit and guinea pig and heart of anesthetized frog. Also the antiarrythmic action of STC and SDC on atrial arrhythmias induced by epinephrine or ouabain was studied. The results were following. The cholates exhibited a slight decrease in rate and contractile amplitude of the isolated rabbit atria. The cholates abolished partially the spontaneous arrhythmic occurring in isolated rabbit and guinea pig atria but no effect on the atrial arrhythmia induced by ouabain and epinephrine was observed. Concomitant administration of cholates with ouabain produced a marked prolongation of atrial arrhythmia in comparison to that of ouabain alone in both isolated rabbit and guinea pig atria. The cholates exhibited a marked prolongation in ventricular arrhythmia and cardiac arrest time in comparison to that of ouabain treatment. However, the combined treatment with cholates and ouabain produced a slight prolongation in comparison to that of ouabain alone in the heart of anesthetized frog. The above results suggest that cholates have a slight antiarrythmic effect on the heart but this effectiveness is different from those of propranolol that is non-selective antiarrhythmic drug.

Characteristics of Patients Undergoing Telemetry in the Cardiology Ward (심장내과 병동의 텔레메트리 적용 환자 특성)

  • Purpose: The purpose of this study was to provide basic information to develop appropriate nursing guidelines for cardiac telemetry. Methods: This retrospective research was conducted to identify the current usage of cardiac telemetry and considered 1,000 patients hospitalized for telemetry. The collected data were analyzed using IBM (SPSS Statistics for Windows 21.0). Results: Four-hundred and ninety-two patients (49.2%) were diagnosed with arrhythmia and 209 (20.9%) with heart failure. Electrocardiogram (ECG) rhythm changes were detected via telemetry in 464 cases. Major arrhythmias were ventricular tachycardia (183, 39.4%) and bradycardia (99, 21.3%). Interventions after detecting arrhythmia were cardioversion (16, 3.4%), defibrillation (1, 0.2%), and cardiopulmonary cerebral resuscitation (5, 1.1%); other patients were treated conservatively with close observation (381, 82.1%). Conclusion: There was appropriate intervention in 46.4% of the considered cases with the detection of ECG changes before patient notification, which implied that cardiac telemetry could be considered for application to patients with hemodynamic instability. Clear standards and guidelines are required to determine who requires telemetry and when to end the telemetry monitoring.

  • Although antiarrhythmic medication has been the main treatment modality for arrhythmia in children, in recent decades technological development and computerization have made great advances in nonpharmacological therapy. This article reviews the transcatheter radiofrequency ablation for tachycardia in children, recent advances of device therapy for bradycardia, antitachycardia pacing, implantable cardioverter defibrillator. As a new field of device therapy, cardiac resynchronization therapy for congestive heart failure is also mentioned.
