Perception and Practices on Screening and Vaccination for Carcinoma Cervix among Female Healthcare Professional in Tertiary Care Hospitals in Bangalore, India

  • Background:Cervical cancer is potentially the most preventable and treatable cancer. Despite the known efficacy of cervical screening, a significant number of women do not avail themselves of the procedure due to lack of awareness. Objectives: This study was conducted to elicit information on the knowledge, attitude and practice (KAP) regarding screening (Pap test) and vaccination for carcinoma cervix among female doctors and nurses in a tertiary care hospital in Bangalore and to assess barriers to acceptance of the Pap test. Materials and Methods: A cross-sectional, descriptive study was conducted with semi-structured, self-administered questionnaire among female health professionals. The study subjects were interviewed for KAP regarding risk factors for cancer cervix, Pap test and HPV vaccination for protection against carcinoma cervix. Results: Higher proportion of doctors 45 (78.9%) had very good knowledge as compared to only 13 (13.3%) of the nurses, about risk factors for cancer cervix and Pap test (p=0.001). As many as 138(89.6%) of the study subjects had favorable attitude towards Pap test and vaccination, but 114 (73.6%) of the study subjects never had a Pap test and the most common reason 35 (31%) for not practicing was absence of disease symptoms. Conclusions: In spite of good knowledge and attitudes towards cancer cervix and Pap test being good, practice remained low among the study subjects and most common reasons for not undergoing Pap test was absence of disease symptoms. The independent predictors of ever having a Pap test done was found to be the occupation and duration of married life above 9yrs. Hence there is a strong need to improve uptake of Pap test by health professionals by demystifying the barriers.

Does Microinvasive Adenocarcinoma of Cervix Have Poorer Treatment Outcomes than Microinvasive Squamous Cell Carcinoma?

  • Background: To compare the pathological findings and oncologic outcomes of stage IA cervical carcinoma patients, between adenocarcinoma and squamous cell carcinoma cases. Materials and Methods: A total of 151 medical records of stage IA cervical carcinoma patients undergoing primary surgical treatment during 2006-2013 were reviewed. Information from pathological diagnosis and recurrence rates were compared with descriptive statistical analysis. The Kaplan-Meier method and Cox proportional hazards model were used for survival analysis. Results: The median age was 48.9 years. There was no significant difference in rates of lymph node, parametrium, uterine, vaginal, or ovarian metastasis, when comparing adenocarcinoma with squamous cell carcinoma. Overall recurrence rates of adenocarcinoma (5.7%) and squamous cell carcinoma (2.6%) were not statistically significant different, even when stratified by stage. When comparing progression free survival with squamous cell carcinoma, adenocarcinoma had an HR of 0.448 (0.073-2.746), p=0.386. Conclusions: Microinvasive adenocarcinoma of cervix has similar rate of extracervical involvement and oncologic outcomes to squamous cell carcinoma.

자궁부암의 조기발견을 위한 집단 적격 검사 계획의 경제학적 고려 (Economic Consideration of Mass Screening Program for Early Detection of Carcinoma of the Uterine Cervix)

  • Authors reviewed 2,362 cases of consecutive vaginal and cervical smears submitted to the Department of Pathology, Pusan Gospel Hospital during one year period from Jan. 1, 1974 to Dec. 31, 1974. Prevalence of dysplasia, carcinoma in situ and invasive carcinoma of the uterine cervix was analyzed, and cost per a lesion was calculated. The followings are conclusions: 1. Prevalence of dysplasia, carcinoma in situ and invasive carcinoma was 2.88%, 0.34% and 2.58% restectively. 2. Cost per a lesion for dysplasia was calculated as 34,735 Won, for carcinoma in situ, as 295,250 Won and for invasive carcinoma as 38,721 Won. Cost per a lesion for dysplasia and carcinoma in situ was calculated as 31,079 Won and for dysplasia and for all the lesions as 17,248 Won. 3. The results obtained suggested that mass cytologic screeiding for detection of dysplasia, carcinoma in situ and invasive carcinoma was reasonable in the present status of economy.

An Audit of 204 Histopathology Reports Over Three Years of Carcinoma of Cervix: Experience from a Tertiary Referral Centre

  • Background: The aim was to see compliance to minimum data set information in carcinoma cervix histopathology reports from a team of 13 pathologists; and also to analyse the distribution of parameters like tumor size, grade, depth of cervical stromal invasion, lymph node yield and pTNM stage. Materials and Methods: All pathology reports of radical hysterectomy for carcinoma cervix operated in house within a three year duration (2010-2012), (n=204) were retrieved from medical records and analyzed for the above parameters. Results: In 2010- 59 cases, in 2011- 67 cases and in 2012- 78 cases of carcinoma cervix underwent operations in our hospital. The median age was 50.5 years and the maximum T diameter was 2.8 cms in the reports of three years. Squamous carcinoma was the commonest subtype amongst all the tumors. It was noted that 60.8% of cases had cervical stromal involvement more than half the thickness of the cervical stroma. Parametrial involvement was seen in 4.82% of cases. pTNM Staging was not mentioned in 65.06% of the cases. The mean bilateral pelvic lymph node yield count in our study was 16.6 inclusive of all the three years. Conclusions: Compliance with provision of a minimum dataset in our team of 13 pathologists was generally good. Lymph node yield in our hands is reasonable, but constant striving for greater numbers should be made. pTNM staging should be more meticulously documented. Use of proformas /checklists is recommended.

TAO Applicator를 이용한 자궁경암 강내조사시의 선양계산에 관한 고찰 (A Study on Dose Calculation in Intracavitary Radiotherapy of the Carcinoma of the Uterine Cervix with TAO Applicator)

  • Various methods are available for determination of exposure time in intracavitary radiotherapy of the carcinoma of the uterine cervix. To determine the accuracy of dose calculation with isodose curve for TAO applicator, comparison with results calculated by computer for radiotherapy treatment Planning was done in 24 procedures done in 12 consecutive patients with the carcinoma of the uterine cervix from May to December, 1983. The results are as follows: 1. The average dose rate Per hour of Point A was 87.70 rad, being 89.91 rad ana 85.49 rad in left and right, respectively. 2. The average percentage of dose rate of point A calculated by isodose curve method over that by computer was $101.28\%$ and the difference was less than $5\%$ in 17 Procedures and over $10\%$ in only 3 procedures. 3. The average percentage in case of point B was $108.67\%$. In conclusion, in most cases the difference was less than 200 rad for point A and less than 100 rad for point B during 2 courses of intracavitary radiotherapy. And so the dose rate calculation with isodose curve for TAO applicator is comparatively accurate.

자궁경부암 환자에서의 Lymphoscintigraphy 소견 (Iliopelvic Lymphoscintigraphic Findings in Carcinoma of Uterine Cervix)

  • Iliopelvic lymphoscintigraphy was performed to visualize the changes of the lymphatic system using Technetium-99m antimony sulfide colloid in carcinoma of uterine cervix. There were no differences between various indices and clinical stages, but comparison with computed tomographic and postoperative findings, there was meaningful difference in index of decrease and increase, respectively. And lymphoscintigraphy showed by pass way of lymphatic channel in 42 cases (16%) and this suggested the presence of normal shunts of lymphatic channels. Iliopelvic lymphoscintigraphy may be useful for evaluation of lymphatic system in carcinoma of uterine cervix, expecially for screening method.

자궁경부암에서 대동맥 임파절 전이의 치료 (Radiotherapy of Para-Aortic Node Metastases in Carcinoma of the Uterine Cervix)

  • Forty one patients with para-aortic node metastases from carcinoma of the uterine cervix treated with radiotherapy at Department of Radiation Oncology, Yonsei University, College of Medicine from January 1982 to December 1987 were retrospectively analyzed. Eleven patients were diagnosed at the time of diagnosis of carcinoma of the cervix (early diagnosis) and 30 patients were diagnosed during follow up period after definitive radiotherapy of primary site (late diagnosis). The most important factors affecting the survival in this study were time of diagnosis and dose of irradiation. Overall 5 year actuarial survival rate of 41 patients was $25.7\%$. Five year survival rate for early diagnosis was $60.3\%$, but late diagnosis was $16.9\%$. And survival rate for high dose (over 4000 cGy) radiation group and low dose radiation group were $42.2\%\;and\;8.9\%$ respectively The most leading cause of death was para-aortic node failure, so early diagnosis and maximum palliation with full dose radiotherapy (over 4000 cGy) is necessary to improve the survival and the quality of life.

초기 자궁경부 유리세포암의 치료 (Treatment of Early Glassy Cell Carcinoma of Uterine Cervix)

  • 목적: 7예의 자궁경부의 유리세포암 초기 병기 환자들을 통하여 임상적 특징, 병리조직학적 소견, 치료방법 및 예후를 알아보고자 하였다. 대상 및 방법: 1993년 1월부터 2005년 12월까지 계명의대 동산의료원에서 병리조직학적으로 자궁경부암기 유리세포암으로 확인된 FIGO 병기 IB 6예와 IIA환자 1예, 모두 7예의 환자를 대상으로 하였으며 이들 중 1예의 IB 환자에서는 자궁 내 임신 22주 상태였다. 환자들의 의무기록을 통하여 임상적 특징, 병리조직학적 소견, 치료방법 및 예후를 분석하였다. 진단은 병리조직학적으로 유리세포암의 특징적 소견을 보이는 세포들로 50% 이상 구성되어있을 때 유리세포암으로 진단하였다. FIGO 임상병기 IB인 6명의 환자에서 근치적 자궁절제술 및 양측 골반 림프절 절제술을 시행하였으며 이들 중 2명은 수술 후 동시 항암화학방사선요법을 받았다. 1예의 IIA 환자는 동시 항암화학방사선요법으로 근치적 방사선치료를 시행하였다. 결과: 조직학적으로 자궁경부암으로 진단 받은 전체 환자 3,745명 중 유리세포암 환자는 7명이었다(0.2%). 7예의 자궁경부 유리세포암 환자의 평균 연령은 44세였으며, 연령 분포는 35세에서 53세까지였다. 가장 흔한 증상은 질 출혈로 7예 중 5예에서 나타났다(86%). 치료 전 시행한 펀치 생검 소견으로 전체 7예에서 2예에서만 유리세포암으로 진단 가능하였으며, 나머지 5예는 선편평상피세포암 1예, 미분화 선암 2예, 편평상피세포암 2예로 진단되었으나, 수술 후 병리조직학적 검사에서 유리세포암으로 확진되었다. 추적관찰기간은 13개월에서 150개월이며, 평균 추적기간은 73개월로 모두 무병상태로 생존하고 있다. 결론: 자궁경부의 유리세포암은 비록 공격적이고 악성도가 높은 종양이지만, 초기 암 환자는 정확한 병기평가와 근치적 수술 후 동시 항암화학방사선치료와 같은 적극적인 보조치료를 시행하면 보다 향상된 생존율을 얻을 수 있을 것으로 예상된다.

Predicting Factors for Positive Vaginal Surgical Margin Following Radical Hysterectomy for Stage IB1 Carcinoma of the Cervix

  • Background: To examine the incidence of positive vaginal surgical margins and determine the predicting factors following radical hysterectomy for stage IB1 carcinoma of the cervix. Materials and Methods: The clinical and histological data of 656 FIGO stage IB1 cervical cancer patients who had radical hysterectomy with bilateral pelvic lymphadenectomy (RHPL) from January 2003 to December 2012 were retrospectively reviewed and were analyzed for their association with a positive vaginal surgical margin. A p-value of < 0.05 was considered significant. Results: Thirty-five patients (5.3%) had positive vaginal surgical margins following RHPL; 24 (3.7%) for intraepithelial lesions and 11 (1.7%) for carcinoma. On multivariate analysis, microscopic vaginal involvement by high-grade squamous intraepithelial lesion and/or carcinoma (adjusted odd ratio (OR) 186.8; 95% confidence interval (CI) 48.5-718.5) and squamous histology (OR 8.7; 95% CI 1.7-44.0), were significantly associated with positive vaginal surgical margin. Conclusions: Microscopic vaginal involvement by HSIL and/or carcinoma are strong predictors for positive vaginal surgical margins for stage IB1 cervical cancer patients undergoing radical hysterectomy. Preoperative 'mapping' colposcopy or other strategies should be considered to ensure optimal vaginal resection.
