머신러닝 기반의 자동차보험 사고 환자의 진료 기간 예측 기술 (Machine Learning-Based Prediction Technology for Medical Treatment Period of Automobile Insurance Accident Patients)

  • 자동차보험 사고 환자의 진료비 감소를 위한 대책 마련에 도움을 주기 위해 본 연구에서는 자동차보험 사고 40대~50대 경상 환자들의 진료비에 가장 핵심 요소인 진료 기간을 예측하고 진료 기간에 영향을 미치는 요인을 분석하였다. 이를 위해 Decision Tree 등 5개 알고리즘을 활용한 머신러닝 모델을 생성하고 모델간에 그 성능을 비교·분석하였다. 진료 기간 예측에 정밀도, 재현율, FI 점수 등 3가지 평가 지표에서 좋은 성능을 나타낸 알고리즘은 Decision Tree, Gradient Boosting 및 XGBoost 등 3가지였다. 그리고 진료 기간 예측에 영향을 미치는 요인 분석 결과, 병원의 종류, 진료 지역, 나이, 성별 등으로 나타났다. 본 연구를 통해 AutoML을 활용한 손쉬운 연구 방법을 제시하였으며, 본 연구 결과가 자동차보험 사고 진료비 경감을 위한 정책에 도움이 되기를 기대한다.

Fuzzy-AHP를 활용한 한·중 카페리 피견인 트레일러 상호주행 시 장애요인에 관한 연구 (A Study on Obstacle Factors at the Mutual Access of Towed Trailer of Car-Ferry between Korea and China using Fuzzy-AHP)

  • 본 연구는 Fuzzy-AHP를 이용하여 한 중 카페리 피견인 트레일러 상호주행 시 장애요인의 가중치 분석을 연구의 목적으로 하였다. 이를 위해 본 연구에서는 문헌연구를 통해 1차적으로 요인을 선정한 후, 현 카페리업체에 종사 중인 화물 전문가집단과의 Indepth-Interview를 수행하여 요인을 최종 선정하였다. 선정된 상위 장애 요인은 불안정적 서비스 제공, 제한적 정책 지원, 추가 비용 발생이며, 이중 불안정적 서비스 제공이 가장 큰 장애요인으로 분석되었다. 선정된 측정변수 11가지 항목은 화물 장치장 공급 부족, 트레일러 보험 및 관리 문제, 특수장비 부족, 전문화된 인력부족, 투입 서비스 항로의 제한, 마케팅 홍보 전략의 부족, 수속 비용 증가 등이며, 이중 불균형 화물 집화에 따른 장비 회송 비용 증가가 가장 큰 장애 요인으로 분석되었으며, 서비스 항로의 제한, 특수장비 부족, 수속의 지연 및 추가 절차 발생이 그 뒤를 이었다.

보험사기범죄에 대한 분석 고의 교통사고 유도 - 합의금 요구 사건을 중심으로 (An Analysis of Insurance Crimes: The Case of Blackmail in Automobile Accidents)

  • 이 논문은 자동차보험분야에서 고의사고를 통한 보험사기에 대한 게임이론적 분석을 통하여 개선안을 제시한다. 보험사기범이 고의로 교통사고를 유발하여 보험금과 합의금을 받아내는 상황을 게임모형으로 분석한다. 현행제도 하에서는 최초 교통법규를 위반하여 사고를 낸 운전자(즉 궁극적인 보험사기의 피해자)의 교통법규 위반행위에 대한 처벌수준이 너무 높아서 최초법규위반자와 보험사기범과의 협상게임에서 협상력이 과도하게 약화된다. 보험사기범은 이를 악용하여 과도한 합의금을 (협박하여) 요구하게 되는 것이다. 즉, 최초 법규위반 운전자에 대한 과다한 처벌이, 본래 목적이 교통질서를 유지하려는 것임에도 불구하고, 오히려 보험사기범죄를 유발하는 역효과를 발생시키고 있는 것이다. 이러한 보험사기에 대한 대책으로 보통 제시되는 방안은 범죄에 대한 적발확률을 높이고 적발시 처벌강도를 높여서 보험사기범에 대한 기대처벌비용을 높이자는 것이다. 이러한 제안에 추가하여 이 논문에서 보이려는 것은, 보험사기범과 사기피해자(최초 법규위반/사고원인 제공자)의 게임상황에서 사기 피해자의 협상력을 낮추지 않는(즉 올리는) 것이 보험사기를 억제하는 데에 보다 더 중요하다는 점이다. 보험사기를 억제하기 위해서는 보험사기범에 대한 공권력 등을 사용한 적발 처벌도 중요하지만, 그와 병행하여 사기 피해자의 협상력을 높이는 것이 중요하다. 중요 교통법규위반 중 선량한 운전자가 어쩔 수 없이 위반하게 되는 법규위반 행위에 대한 처벌을 적절한 수준으로 낮추는 것이 고의사고를 유발하는 보험사기 범죄를 줄이는 방안이 될 것이다.

Perforator Based Tibialis Anterior Segmental Muscle Island Flap in Lower Extremity Reconstruction

  • Reconstruction of the lower extremities is difficult due to a lack of skin laxity and muscular tissues. Here, we present a case of lower extremity reconstruction via the anterior tibial artery perforator based segmental muscle island flap. Our patient was a 75-year-old male with a chronic ulcerative wound on the right lower leg from an old car accident. A $5.0{\times}0.5cm$ size ulcerative wound with tibial bone exposure was noted. We planned to reconstruct the lower extremity defect with a free flap, but the vessel status was severely compromised intraoperatively. Thus, we found the anterior tibial artery perforator using Doppler ultrasound, elevated the tibialis anterior muscle segment flap, and transposed it to cover the defect successfully. The flap presented with a nice contour and the skin graft covering the flap survived completely. There were no complications of the surgical site at three months follow-up and no gait morbidity. This is a meaningful case applying the concept of segmental muscle flap based on a perforator that had advantages including proper bulkiness, vascularization, and preservation of function, which were well applied, leading to great success.

국내 하지손상의 발생현황에 대한 분석 (Incidences of Lower Extremity Injuries in Korea)

  • Purpose: We conducted this retrospective epidemiological study to assess the incidence and severity of lower extremity injuries in Korea Methods: For this study, we retrospectively reviewed nationwide lower-extremity injury data compiled from 2001 to 2003 based on the National Injury Database, what included National Health Insurance Corporation (NHIC), Car Insurance, and Industry Insurance data. Data were standardized in terms of demographic characteristics, region, and socioeconomic status by using NHIC data. To assess the degree of the injuries, we used the Modified Abbreviated Injury Scale (MoAIS), what has been changed from the International Classification of Disease-10 (ICD-10) code. By using the Excess Mortality Ratio-adjusted Injury Severity Score (EMR-ISS), we classified the degree of severity into four categories: mild, moderate, severe and critical. Results: From 2001 to 2003, lower extremity injuries increased slightly, with a yearly average of 2,437,335. Insurance data should that lower-extremity injuries were the most common, followed by upper-extremity injuries. Significant difference were seen in the numbers of lower extremity injuries based on gender and age. As for provinces, Seoul and Gyeongi provinces had the highest numbers of cases. Junlabukdo had the highest rate of 55,282 cases per 1 million people for standardized gender and population. The annual incidence of the insured patients with lower extrimity injuries was higher than the employer's medical insurance contributions to the medical insurance program. Daily cases occur most often in May and June, with the lowest occurrences being in January and February. Conclusion: The result of this study shows that lower extremity injuries comprised common cause of all injuries. In addition, differences associated with gender, location and socioeconomic status were observed. Further studies are needed to find reasons and then this knowledge will allow strategies to prevent the lower extremity injuries.

자동차가 환경적으로 지속가능한 도시교통에 미치는 영향 (The Environmental Impact of Automobiles on Sustainable Urban Transportation)

  • As of July 1997, the registered number of automobiles exceeded 10 million and Korean auto industry produced 2,850,000 cars. Many environmentalists warn that the passenger cars are not sustainable urban transportation system in large cities. The cars produce about 80% of air pollutants, and consume 30% of petroleum imported. For the past 30 years the administration increased the road system, but the length of road per car is decreased from 813m in 1965 to 9m in 1995. The cost of traffic congestion was estimated to be \14.7 trillion in 1996, and is feared to increase without changing the present transportation system. The undesirable impact of cars include the casualties from traffic accidents, insurance loss, and separation of human relatioships. To construct sustainable urban transportation system, three principles should be followed. The first principle is to make the urban streets walkable for the people. The second principle is to encourage bicycling. Roads and traffic systems should be designed to let bicylces travel safely. The third principle is to supply more buses and construct integrated transportation system based on buses. The subway system is too expensive to construct and without the support of a well-organized bus system it may not work efficiently. The Brazilian city of Curitiba has constructed a very efficient bus system only with 1/80 of the estimated cost of subway system. The car-oriented transportation system does not seem to be an envrironmentally sustainable transportation system in most of the Korean cities.

저속 후면 추돌 시 목부상해 예방을 위한 연구 (Stildy on the Methodology to Prevent Neck Injury at tow Speed Rear-End Impact)

  • 141,841 car-to-car collision had occurred in 2003, and among the accidents 51,796 were rear-end impact. According to insurance company for loss or damage, more than $60\%$ of rear-end impact victims suffer neck injury. This means at least 31,000 neck injury victims have happened in 2003. More than $97\%$ of the neck injury victims have low severity injury than A.I.S 2. Head restraint, which is designed to limit rearward head movement and equipped on seat, can considerably protect neck from rear-end impact. In this paper we evaluated head restraint geometry and drivers' sitting position according to RCAR standard and carried out low speed volunteer crash test. The crash speed is 4km/h and N.I.C value is used to determine injury probability. Through these research results we can introduce the method to prevent neck injury at rear-end impact.

국가별 좌석 안전성 평가 방법에 따른 머리지지대 최적화 설계 (Optimized Design of the Head restraint according the regional seat safety assessment)

  • The whiplash Injuries due to rear collision occur frequently. As result, in many countries, seat performance is being assessed and developed to improve head whiplash injury in rear collision of passenger car. This study compares whiplash assessment methods in each country. Using the DFSS(Design for Six Sigma) method, the correlation between influence parameters of head restraints and whiplash injury criteria is analyzed. Four control factors are used in this study. And total 11 whiplash injury criteria from NCAP(New Car Assessment Program) of Korea, Europe, China and IIHS(Insurance Institute for Highway Safety) of USA are used for output response. By the experimental design, L9 orthogonal coordinate system is configured and is tested by sled test equipment, twice. By using average assay value and ANOVA, the correlation between control factors and injury criteria has been comprehended. Optimization design of head restraint according the regional seat safety assessment was derived through the correlation.

고속도로 사고잦은 지점 분석방법 연구 (An Analysis of the Hazardous Highway Segments Using Continuous Risk Profile Method)

  • We have to develop more correct and systematic way to choose Hazardous Highway Segments. In this research, we applied CRP(Continuous Risk Profile) technique which developed by UC Berkeley Traffic Safety Center in year of 2007, and can analyze yearly dangerous level tendency of continuity in the route of main road that is under California Department of Transportation(Caltrans). We changed standard of CRP to suit in Korean circumstance with consideration in radius of curve and traffic volume. For the verification by actual accident data, we embodiment the CRP by using the data from total of 587 case of accident in latest 10 years in Gyeong-Bu Highways, the amount of 56km. Finally, the effectiveness of technique in this research has been verified by obtained same result with current method for Hazardous Highway Segments. In addition, when calculating the Hazardous Highway Segments with technique that presented in this research we obtained following statements. First, identified dangerous level of continuity in the route by using CRP. Second, Accurate of Actual Hazardous Highway Segments selection has been developed by using last 10 year's data and profile making which provide simplicity analyze of Tendency. Third, after reforming the way of selection, effective range has been wider than former selection and it gives advantage for the policy side.

주유소 내 연료전지설비에 대한 사고피해예측 연구 (A Study on Damage Assessment for Fuel Cell Facilities in Gas Stations)

  • 연료전지는 저탄소 발전원으로 주유소 내 연료전지를 설치 시 분산에너지와 전기차 충전인프라를 확충할 수 있다. 주유소 내 연료전지 설치 시 안전성 확보를 위하여 국‧내외 주유소 및 연료전지의 사고데이터를 기반으로 사고시나리오를 도출하고 사고피해예측을 위한 정량적 위험성평가를 실시하였다. 최악의 사고시나리오가 아닌 현실적으로 발생 가능한 화재 및 폭발사고의 피해범위를 산출하고, 피해영향을 분석한 결과 주유기로부터 9.0 m, 주유 중 차량으로부터 15.5 m, 통기관으로부터 4.1 m, 연료전지의 정압기 등 가스조정장치로부터 1.1 m 이상 이격 시 사고로 인한 심각한 피해 위험을 낮출 수 있는 것으로 나타났다. 이러한 연구 결과는 주유소 내 연료전지 배치 및 사고피해를 경감할 수 있는 안전대책 수립에 활용할 수 있을 것으로 기대된다.
