Short-acting β2-agonist prescriptions in patients with asthma: findings from the South Korean cohort of SABINA III

  • Background/Aims: Despite short-acting β2-agonist (SABA) overuse being associated with poor asthma outcomes, data on SABA use in South Korea is scarce. Herein, we describe prescription patterns of SABA and other asthma medications in patients from the South Korean cohort of the SABA use IN Asthma (SABINA) III study. Methods: This study included patients with asthma aged ≥ 12 years, who had ≥ 3 consultations with the same healthcare provider, and medical records containing data for ≥ 12 months prior to the study visit. Patients were classified by investigator-defined asthma severity (per 2017 Global Initiative for Asthma recommendations) and practice type (primary or specialist care). Data on disease characteristics, asthma treatments, and clinical outcomes in the 12 months before the study visit were collected using electronic case report forms. Results: Data from 476 patients (mean age, 55.4 years; female, 63.0%) were analyzed. Most patients were treated by specialists (83.7%) and had moderate-to-severe asthma (91.0%). Overall, 7.6% of patients were prescribed ≥ 3 SABA canisters (defined as over-prescription). In patients prescribed SABA in addition to maintenance therapy, 47.4% were over-prescribed SABA. Most patients (95.4%) were prescribed a fixed-dose combination of an inhaled corticosteroid and a long-acting β2-agonist as maintenance therapy. Although asthma was well-controlled/partly-controlled in 91.6% of patients, 29.6% experienced ≥ 1 severe asthma exacerbation. Conclusions: SABA over-prescription was reported in nearly 50% of patients prescribed SABA in addition to maintenance therapy, underscoring the need to align clinical practices with the latest evidence-based recommendations and educate physicians and patients on appropriate SABA use.

Development of a duplex stainless steel for dry storage canister with improved chloride-induced stress corrosion cracking resistance

  • The chloride-induced stress corrosion cracking (CISCC) is one of the major integrity concerns in dry storage canisters made of austenitic stainless steels (ASSs). In this study, an advanced duplex stainless steel (DSS) with a composition of Fe-19Cr-4Ni-2.5Mo-4.5Mn (ADCS) was developed and its performance was compared with that of commercial ASS and DSS alloys. The chemical composition of ADCS was determined to obtain greater pitting and CISCC resistance as well as a proper combination of strength and ductility. Then, the thermomechanical processing (TMP) condition was applied, which resulted in higher strength than ASSs (304L SS and 316L SS) and better ductility than DSSs (2101 LDSS and 2205 DSS). The potentiodynamic polarization and electrochemical impedance spectra (EIS) results represented the better pitting corrosion resistance of ADCS compared to 304L SS and 316L SS by forming a better passive layer. The CISCC tests using four-point loaded specimens showed that cracks were initiated at 24 h for 304L SS and 144 h for 316L SS, while crack was not found until 1008 h for ADCS. Overall, the developed alloy, ADCS, showed better combination of CISCC resistance and mechanical properties as dry storage canister materials than commercial alloys.

A Preliminary Study on the Source Fingerprints of Volatile Organic Compounds (휘발성유기화합물의 배출원 구성물질 성분비에 관한 기초 연구)

  • The purpose of this study was to develop the preliminary source fingerprints of volatile organic compounds (VOC). The source categories studied were vehicles, gasoline vapor, gasoline storage tank, coating, dry cleaning and road covering. The source samples were collected using 6L electro-polished stainless steel canisters for about 20 seconds. From this study, the main component emitted from VOC sources in Korea was toluene. The toluene proportion for road covering, vehicles, coating and gasoline vapor were 35, 18, 16 and 5%, respec- tively. The C$_2$-C$\sub$5/ alkane and alkene compounds were mainly emitted from vehicles, gasoline vapor and gasoline storage tank. The main compounds of coating were m/p-xylene(34%), toluene(16%), 1,2,4-TMB(10%) and o-xylene(9%), which are aromatic hydrocarbons. In the case of dry cleaning, nonane(41%), 1,2,4-TMB (22%) and 1,3,5-TMB(13%) were mainly emitted.

Development of Real-time and Simultaneous Quantification of Volatile Organic Compounds in Ambient with SIFT-MS (Selected Ion Flow Tube-Mass Spectrometry) (선택적다중이온질량분석기를 이용한 대기 중 휘발성유기화합물 실시간 동시분석법 개발 및 적용)

  • Volatile organic compounds (VOCs) are representative air pollutants due to their detrimental effects on human health and their role in formation of secondary organic aerosols. Assessments and monitoring programs of VOCs using periodic grab sampling like Tedlar bags, canisters, and sorbent traps provide limited information, often with delay times of days or weeks. Selected ion flow tube mass spectrometry (SIFT-MS) is an emerging analytical technique for the real-time quantification of VOCs in air. It relies on chemical ionization of the VOCs molecules in air introduced into helium carrier gas using $H_3O^+$, $NO^+$, and $O_2{^+}$ precursor ions. Real-time monitoring method of 60 VOCs in the ambient air was developed using TO-15 standard gas mixture. Calibration curves, method detection limit, and quantitation reproducibility of the target compounds were tested. Dynamic dilution system was used to dilute standard gas from 0.174 ppbv to 100 ppbv, where calibration curves showed good linearity with $r^2$> 0.95 in all target analytes. Limit of detection (LOD) all compounds were sub ppbv, and some halogenated compounds showed pptv levels. Seven consecutive analyses of target compounds showed good repeatability with relative standard deviation of less than 10%. One day monitoring of VOCs in ambient air was conducted in Geoje. Average concentration of target VOCs in Geoje were relatively lower than other regions, among which formaldehyde showed the highest concentration ($15.4{\pm}5.78ppbv$). SIFT-MS provided good temporal resolution data (1 data per 3.2 minute), which can be used for identifying ephemeral short-term event. It is expected that SIFT-MS will be a versatile monitoring platform for VOCs in ambient air.

Evaluation of the Safty for the Disposal of High-level Nuclear Waste in the Granite (화강암지역에 고준위 원자력 폐기물 처리에 대한 안정성 평가)

  • All the radionuclides in high-level nuclear waste will decay to harmless levels eventually but for some radionuclides decay is so slow that their radiation remains dangerous for times on the order of tens or hundreds of thousands of years. At the present time, the most favorite disposal plan for high-level radioactive waste is a mined geological disposal in which canister enclosing stable solid form of radioactive waste is placed in mined cavities locating hundred meters below the surface. The chief hazard in such disposal is dissolution of radionuclides from the waste in the groundwater that will eventually carry the dissolved radionuclides to surface environments. The hazard from possible escape of the radionuclides through groundwater can be delayed by engineered and geologic barriers. The engineered barriers can become useless by unexpected geologic catastrophe such as volcanism, earthquake, and tectonic movement and by fraudulent work such as careless construction, improperly welded canisters within the first few decades or centuries. As a result, dangerously radioactive waste which is still intensively radioactive is directly exposed to attack by moving groundwater. All the more, it is almost impossible to control repositories for times more than 10,000 years. Therefore, naturally controlled geologic, barriers whose properties will not be changed within 10,000 years are important to guarantee the safety of repositories of high-level radioactive waste. In Sweden and France, the suitability of granite for the mined geological disposal of high-level waste has been studied intensively. According to the research in Sweden and France, granites has the following physio-chemical characteristics which can delay the transportation of radionuclide by groundwater. First, the permeabilities of granites decreases as the depth increases and is $10^{-8}{\sim}10^{-12}m/s$ at depth below 300 m. Second, groundwater at depth below 300 m has pH=7-9 and reducing condition (Eh=-0.1~0.4). This geochemical condition is desirable to prevent both canister and solid waste from corrosion. Third most radionuclides are not transported by low solubilities and some radionuclide with high solubility such as Cs and Sr are retarded by absorption of geologic media through which ground water flows. Therefore, if high-level waste is disposed at depth below 300 m in the granite body which has a low permeability and is geologically stable more than 10,000 years, the safety of repositories from the hazard due to radionuclide escape can guaranteed for more than 10,000 years.

A Prediction of Saturated Hydraulic Conductivity for Compacted Bentonite Buffer in a High-level Radioactive Waste Disposal System (고준위방사성폐기물 처분시스템의 압축 벤토나이트 완충재의 포화 수리전도도 추정)

  • A geological repository comprises a natural barrier and an engineered barrier system. Its design components consist of canisters, buffers, backfill, and near-field rock. Among the engineered barrier system components, bentonite buffers minimize the groundwater flow from near-field rock and prevent the release of nuclide. Investigation of the hydraulic conductivity of the buffer to groundwater flow is an important factor in the performance evaluation of the stability and integrity of the engineered barrier of the repository. In this study, saturated hydraulic conductivity tests were performed using Gyeongju bentonite at various dry densities and temperatures, and a hydraulic conductivity prediction model was developed through multiple regression analysis using the 120 result sets of hydraulic conductivity. The test results showed that the hydraulic conductivity tends to decrease as the dry density increases. In addition, the hydraulic conductivity increased with increasing temperature. The multiple regression analysis results showed that the coefficient of determination (R2) of the hydraulic conductivity prediction equation was as high as 0.93. The hydraulic conductivity prediction equation presented in this study could be used for the design of engineered barrier systems.

Development of CANDU Spent Fuel Disposal Concepts for the Improvement of Disposal Efficiency (처분효율 향상을 위한 CANDU 사용후핵연료 처분개념 도출)

  • There are two types of spent fuels generated from nuclear power plants, CANDU type and PWR type. PWR spent fuels which include a lot of reusable material can be considered to be recycled. CANDU spent fuels are considered to directly disposed in deep geological formation, since they have little reusable material. In this study, based on the Korean Reference spent fuel disposal System(KRS) which is to dispose both PWR and CANDU spent fuels, the more effective CANDU spent fuel disposal systems have been developed. To do this, the disposal canister has been modified to hold the storage basket which can load 60 spent fuel bundles. From these modified disposal canisters, the disposal systems to meet the thermal requirement for which the temperature of the buffer materials should not be over $100^{\circ}C$ have been proposed. These new disposals have made it possible to introduce the concept of long tenn storage and retrievabililty and that of the two-layered disposal canister emplacement in one disposal hole. These disposal concepts have been compared and analyzed with the KRS CANDU spent fuel disposal system in terms of disposal effectiveness. New CANDU spent fuel disposal concepts obtained in this study seem to improve thermal effectiveness, U-density, disposal area, excavation volume, and closure material volume up to 30 - 40 %.

Preliminary Design of the Forced Gas Drying System for Spent Nuclear Fuel Dry Storage (사용후핵연료 건식저장을 위한 기체강제순환 건조장치 예비설계)

  • For dry storage of the spent nuclear fuel (SNF) stored in the storage pool of a nuclear power plant, essentially all moisture must be removed to prevent corrosion of the assembly and canister internals and/or degradation of fuel cladding integrity after SNF canister loading operation. R&D work is now in progress on a forced gas drying system that can be used to remove residual water in canisters. In this work, preliminary design is performed to manufacture the forced gas drying system. This process includes a case study of dry methods for canister moisture removal, relative codes and standards, confirmation of adequate dryness, needs analysis at plant sites, and characteristics of SNF stored in pools. Through this preliminary design work, we obtained a conceptual flow diagram and preliminary P&ID of the forced gas drying system. The results of this study can be used to determine details of the design to manufacture the forced gas drying system.

A Structural Analysis of the SNF(Spent Nuclear Fuel) Disposal Canister with the SNF Basket Section Shape Change for the Pressurized Water Reactor(PWR) (고준위폐기물다발의 단면형상 변화에 따른 가압경수로(PWR)용 고준위폐기물 처분용기의 구조해석)

  • A structural model of the SNF(spent nuclear fuel) disposal canister for the PWR(pressurized water reactor) for about 10,000 years long term deposition at a 500m deep granitic bedrock repository has been developed through various structural safety evaluations. The SNF disposal baskets of this canister model have the array type whose four square cross section baskets stand parallel to each other and symmetrically with respect to the center of the canister section. However, whether this developed structural model of the SNF disposal canister is optimal is not determinable yet. Especially, there is still a problem in weight-reduction of the canister. The cross section shape of the SNF basket should be changed to solve this problem. There are two ways in changing the cross section shape of the SNF basket; the one is to rotate the cross section itself and the other is to change the cross section shape as other shape different from the square cross section. The previous study shows that the canister with $30{\sim}35^{\circ}$ rotated basket array is structurally more stable than the canister with un-rotated parallel basket array. However, whether this canister with rotated basket array is optimal is not either determinable as yet, because it is not revealed that the canister with other cross section different from the square cross section is structurally more stable than other canisters. Therefore, the structural analysis of the SNF disposal canister with other cross section shape which is also symmetric with respect to the canister center planes is very necessary. The structural analysis of the canister with various cross section shape basket array in which each basket is arrayed symmetrically with respect to the center planes is carried out in this paper. The structural analysis result shows that the SNF disposal canister with circular cross section shape baskets located symmetrically with respect to the center of the canister section is structurally more stable than the previously developed SNF disposal canister with the parallel basket array.
