Background: Four-dimensional computed tomographic (4DCT) images are increasingly used in clinic with the growing need to account for the respiratory motion of the patient during radiation treatment. One of the reason s that makes the dose evaluation using 4DCT inaccurate is a change of the patient respiration during the treatment session, i.e., intrafractional uncertainty. Especially, when the amplitude of the patient respiration is greater than the respiration range during the 4DCT acquisition, such an organ motion from the larger respiration is difficult to be represented with the 4DCT. In this paper, the method to generate images expecting the organ motion from a respiration with extended amplitude was proposed and examined. Materials and Methods: We propose a method to generate extra-phase images from a given set of the 4DCT images using deformable image registration (DIR) and linear extrapolation. Deformation vector fields (DVF) are calculated from the given set of images, then extrapolated according to respiratory surrogate. The extra-phase images are generated by applying the extrapolated DVFs to the existing 4DCT images. The proposed method was tested with the 4DCT of a physical 4D phantom. Results and Discussion: The tumor position in the generated extra-phase image was in a good agreement with that in the gold-standard image which is separately acquired, using the same 4DCT machine, with a larger range of respiration. It was also found that we can generate the best quality extra-phase image by using the maximum inhalation phase (T0) and maximum exhalation phase (T50) images for extrapolation. Conclusion: In the present study, a method to construct extra-phase images that represent expanded respiratory motion of the patient has been proposed and tested. The movement of organs from a larger respiration amplitude can be predicted by the proposed method. We believe the method may be utilized for realistic simulation of radiation therapy.
Objective : We retrospectively evaluated the efficacy of Gamma Knife radiosurgery (GKS) for recurrent nasopharyngeal carcinoma (NPC) in patients who previously underwent radiotherapy, and analyzed the treatment outcomes over 14 years. Methods : Ten patients with recurrent NPC who had previously received radiotherapy underwent stereotactic radiosurgery using a Gamma Knife® (Elekta Inc, Atlanta, GA, USA) between 2005 and 2018. The median target volume was 8.2 ㎤ (range, 1.7-17.8), and the median radiation dose to the target was 18 Gy (range, 12-30). The median follow-up period was 18 months (range, 6-76 months). Overall and local failure-free survival rates were determined using the Kaplan-Meier method. Results : The NPCs recurred at the primary cancer site in seven patients (70%), as distant brain metastasis in two (20%), and as an extension into brain in one (10%). The recurrent tumors in seven of the 10 patients (70%) were found on the routine follow-up imaging studies. Two patients presented with headache and one with facial pain. Local failure after GKS occurred in five patients (50%) : two of whom died eight and 6 months after GKS, respectively. No adverse radiation effects were noted after GKS. The 1- and 3-year overall survival rates after GKS were 90% and 77%, respectively. The local failure-free survival rates at 6 months, 1 year, and 3 years after GKS were 80%, 48%, and 32%, respectively. The median interval from GKS to local failure was 8 months (range, 6-12). Univariate analysis revealed that using co-registration with positron emission tomography-computed tomography (PET-CT) and magnetic resonance imaging (MRI) was associated with a lower local failure rate of recurrent NPC (p=0.027). Conclusion : GKS is an acceptable salvage treatment option for patients with recurrent NPC who previously received radiation therapy. PET-CT and MRI co-registration for dose planning can help achieve local control of recurrent NPC.
본 연구에서는 보건의료정보관리사 양성을 위한 대학에서 EMR 교육시스템을 활용하여 교육할 수 있는 실습모델을 연구하였다. 현재 보건의료정보관리사의 직무역량 강화를 위해 도입된 보건의료정보관리교육 평가·인증 기준에 보건의료정보관리를 위한 실습과정이 제시되어 있지 않다. 이에 따라서 보건의료 정보관리교육 평가·인증 편람에서 교육환경으로 제시한 실습프로그램을 EMR 교육시스템에서 실습할 수 있도록 프로그램을 구성였다. 또한 프로그램별로 보건의료정보관리 현장실습지침서에 따라 수행할 수 있는 실습모델을 연구하였다. 보건의료정보관리 교육용 EMR시스템을 이용하여 마스터데이터관리, 환자 등록, 의사처방, 진료비 수납, 건강보험청구관리, 서식관리, 퇴원등록, 암등록, 미비기록관리, 보건의료데이터관리, 보건의료통계, 정보보호/보안관리에 대한 실습을 수행할 수 있도록 실습모델을 연구하였다. 대학에서 체계적이고 표준화된 보건의료정보관리 실습과정을 수행하여 보건의료정보관리 교육의 질적 수준을 높임으로써 보건의료정보관리 전문가로써 역할을 수행할 수 있을 것이다. 이에 따라서 보건의료정보관리사의 실습교육을 통하여 의료데이터 분석을 기반으로 의료서비스를 개발하고 관리하는 보건의료 정보관리 전문가를 양성할 수 있도록 해야 할 것이다.
PHR(Personal Health Records : 개인평생건강기록) 은 진료정보를 제공하는 것을 기본서비스로 하며, 환자와 서비스 제공자에게 유용한 서비스를 제공해야 한다. 본 연구는 진료정보의 제공 외 사용자에게 적합한 서비스를 발굴하는데 목적이 있다. 서비스 대상은 암환자로 한정하였고 PHR 서비스 발굴을 위하여 기존 사례 분석, 의료진 인터뷰, 환자 설문을 진행하였다. 그 결과로 발굴된 서비스는 3가지이다. 첫째, 환자에게 치료계획(patient's clinical pathway)을 제공하고 두 번째 온라인 질의응답기능 제공, 세번째 환자 본인의 상태를 입력하는 기능 제공이다. 발굴된 명지 PHR 서비스는 웹과 앱(안드로이드)으로 약 3개월간 파일롯 테스트를 하였고 유용성을 확인하였다. 향후 상업화된 PHR 서비스가 되기 위해서는 치료계획의 등록을 위한 표준화 및 사용자 편의성을 고려한 모델이 되어야 할 것이다.
To establish the fundament for EBM of Traditional Korean Medicine, the papers on Sagunja-tang(Sijunzi-tang) frequently used in medical institutions of Traditional Korean Medicine were analyzed through researching domestic and international papers. The papers were classified by the registration of domestic or international journals, the year of publishment, experimental fields and the kinds of studies on biological activities. Of total 228 papers on Sagunja-tang(Sijunzi-tang), 121 volumes were selected according to creteria. 47 volumes were published in domestic journals, 71 in Chinese journal, 2 in Japanese journal, and 1 in Taiwan journal. The papers on instrumental analyses were preceeded by HPLC, MS, GC with standard compounds of herbal medicine in Sagunja-tang(Sijunzi-tang). The papers on biological activities of Sagunja-tang(Sijunzi-tang) showed improvement of gastrointestinal activity and blood circulation, immunoactivity, anti-cancer, anti-oxidant, anti-fatigue, anti-stress, pharmacokinetics, hepatic protection, radioprotection, muscular activity, hematopoiesis and nontoxical effect. Further studies including gastrointestinal disorder, immune related disease, cancer, oxidative injury and pharmacokinetic study need to be preceeded to establish the fundament for EBM of Sagunja-tang(Sijunzi-tang).
This paper presents the development of specialized software for annotating volume-of-interest on 18F-FDG PET/CT images with the goal of facilitating the studies and diagnosis of head and neck cancer (HNC). To achieve an efficient annotation process, we employed the SE-Norm-Residual Layer-based U-Net model. This model exhibited outstanding proficiency to segment cancerous regions within 18F-FDG PET/CT scans of HNC cases. Manual annotation function was also integrated, allowing researchers and clinicians to validate and refine annotations based on dataset characteristics. Workspace has a display with fusion of both PET and CT images, providing enhance user convenience through simultaneous visualization. The performance of deeplearning model was validated using a Hecktor 2021 dataset, and subsequently developed semi-automatic annotation functionalities. We began by performing image preprocessing including resampling, normalization, and co-registration, followed by an evaluation of the deep learning model performance. This model was integrated into the software, serving as an initial automatic segmentation step. Users can manually refine pre-segmented regions to correct false positives and false negatives. Annotation images are subsequently saved along with their corresponding 18F-FDG PET/CT fusion images, enabling their application across various domains. In this study, we developed a semi-automatic annotation software designed for efficiently generating annotated lesion images, with applications in HNC research and diagnosis. The findings indicated that this software surpasses conventional tools, particularly in the context of HNC-specific annotation with 18F-FDG PET/CT data. Consequently, developed software offers a robust solution for producing annotated datasets, driving advances in the studies and diagnosis of HNC.
목적: 본 연구는 보건소에 등록된 재가 호스피스 완화돌봄 대상자의 특성과 증상을 분석하는 것이다. 방법: 부산광역시 소재 6개구 보건소에 등록된 호스피스 완화돌봄 대상자 144명의 초기 방문기록지(호스피스대상자 등록카드, 초기 통증평가지, 초기 통증 외 증상평가지)를 후향적으로 분석하였다. 결과: 대상자의 평균 연령은 67.7세이었으며, 혼자 사는 대상자가 46.2%, 교육정도는 중졸 이하가 65%였다. 종교는 불교가 36.3%로 가장 많았고, 47.5%가 의료급여 대상자였다. 진단명은 폐암, 위암, 간암의 순이었고, 기능 상태는 일상생활이 어려운 경우가 48.9%로 나타났다. 등록 당시 암 치료 중인 대상자는 39.6%이었고, 84.5%가 말기상태를 인지하고 있었다. 대상자들의 83.6%에서 통증을 호소하였으며, 그 중 36.5%에서 중간정도 이상의 통증을 호소하였다. 통증 외 증상에서 가장 많은 대상자가 호소하는 증상으로는 피로(84.7%)였으며, 피로를 호소하는 대상자의 49.3%가 중증의 증상을 호소하는 것으로 나타났다. 결론: 본 연구결과 재가 호스피스 완화돌봄 대상자들은 사회경제적 취약군으로, 중간정도 이상의 통증과 통증 외 증상을 호소하는 대상자가 많았다. 따라서 재가 호스피스 완화돌봄 대상자의 효율적인 관리를 위해서는 대상자 특성에 따른 차별화된 통합적 전략이 필요하다.
최근 의학계에서도 딥러닝 기술을 이용한 질병의 치료결과 예측 연구가 활발하다. 그러나, 소규모 환자 데이터와 특정한 딥러닝 알고리즘을 선택·활용, 연구를 진행하여 특정 조건 아래에서 의미 있는 결과를 보여주었다. 본 연구에서는 연구 결과의 일반화를 위하여 환자 대상을 좀 더 확대·세분화하여 80대·90대·100대 남녀 대상으로 폐암 진단 후 사망률 예측 연구 결과를 도출하였다. 건강보험심사평가원의 대규모 진료 정보와 다종의 딥러닝 알고리즘을 제공하는 AutoML을 이용, 80대·90대·100대 남녀의 폐암 진단 후 84개월간의 사망률 예측을 위해 Decision Tree, Random Forest, Gradient Boosting, XGBoost, Logistic Regression 등 5개 알고리즘별 모델을 생성하고 이를 통해 예측 성능을 비교하고 사망률에 영향을 미치는 요인에 대한 분석도 추진하였다. 연구 결과, 80대와 90대에서 남성이 여성보다 사망 예측률이 더 높았으며 100대에서는 여성의 사망 예측률이 남성보다 높게 나타났다. 그리고 사망률에 가장 큰 영향을 미치는 요인으로는 치료기간으로 분석되었다.
While the threat from traditional communicable diseases have been decreasing non communicable chronic diseases are increasing due to the aging of population and change in life pattern of the people such as over intake of cholesterol and lack of physical exercise etc. On the other hand, since 1980s, AIDS is spreading rapidly throughout the globe and environmental pollution, accidents, addictive diseases such as drug abuse and alcoholism are becoming serious factors to hinder the health promotion of the people. In order to improve general public health and promote individual health status, existing program for communicable disease control by the government such as tuberculosis, leprosy, STD and acute communicable diseases should be effectively continued. In principle, effort should be placed on eradication of source of infection, reduction of communicability of source in infection, treatment of source of infection as well as increase of individual registance to the diseases through immunization and improvement of physical status. Since the pattern of illness is being shifted from communicable diseases to non communicable chronic diseases such as cancer, cerebral hemorrhage, heart disease and hypertention etc., special emphasis should be placed on the prevention and control of those adult diseases. Early detection of the patients, registration and treatment of patients and health education should be systematically developed for effective control of the diseases. In addition, program activities on MCH, nutrition, dentistry, mental health, environmental health, accident prevention. medical delivery system, health insurance. and all other health issues should be improved in order to achieve our goal of health promotion.
To establish the fundament for EBM of Traditional Korean Medicine, the papers on Pyeongwi-san (Pingwei-san) frequently used in medical institutions of Traditional Korean Medicine were analyzed through researching domestic and international papers. Of total 518 papers on Pyeongwi-san (Pingwei-san), 32 volumes were selected according to selection creteria. 20 volumes were published in domestic journals, 11 in Chinese journal and 1 in Japanese journal. The papers on instrumental analyses reported the quantification of standard compounds of herbal medicines in Pyeongwi-san (Pingwei-san) using HPLC, GC-MS. The papers on biological activities of Pyeongwi-san (Pingwei-san) showed improvement of gastrointestinal activity and water-electrolyte metabolism, immune regulation, anti-oxidant, anti-inflammatory, analgesic, anti-convulsant activities, hypnotic duration, blood pressure regulation, hepatic protection, congestive heart failure, anti-cancer activities. Further studies including gastrointestinal activity need to be preceeded to establish the fundament for EBM of Pyeongwi-san (Pingwei-san).
