p16INK4a is a Useful Marker of Human Papillomavirus Integration Allowing Risk Stratification for Cervical Malignancies

  • The present study was conducted to assess utility of $p16^{INK4a}$ immunopositivity as a surrogate marker for genomic integration of high-risk human papillomavirus infection (hrHPV). A total of 29 formalin-fixed, paraffin-embedded cervical low-grade squamous intraepithelial lesions (LSILs), 27 high-grade squamous intraepithelial lesions (HSILs) and 53 invasive squamous cell carcinomas (SCCs), histologically-diagnosed between 1st January 2006 to 31st December 2008 at the University of Malaya Medical Centre were stained for $p^{16INK4a}$ (CINtec Histology Kit (REF 9511, mtm laboratories AG, Heidelberg, Germany). Immunopositvity was defined as diffuse staining of the squamous cell cytoplasm and or nucleus (involving > 75% of the intraepithelial lesions or SCCs). Staining of basal and parabasal layers of intraepithelial lesions was pre-requisite. One (3.4%) LSIL, 24 (88.9%) HSIL and 46 (86.8%) SCC were $p^{16INK4a}$ immunopositive. All normal squamous epithelium did not express $p16^{INK4a}$. $p16^{INK4a}$ expression was significantly lower (p<0.05) in LSIL compared with HSIL and SCC with no difference in expression between HSIL and SCC. The increased $p16^{INK4a}$ immunopositivity in HSIL and SCC appears in line with the integrated existence of the hrHPV and may provide more insightful information on risk of malignant transformation of cervical squamous intraepithelial lesions than mere hrHPV detection.

Expression of DNA Methylation Marker of Paired-Like Homeodomain Transcription Factor 2 and Growth Receptors in Invasive Ductal Carcinoma of the Breast

  • Background: Paired-like homeodomain transcription factor 2 (PITX2) is another new marker in breast carcinoma since hypermethylation at P2 promoter of this gene was noted to be associated with poor prognosis. We investigated the expression of PITX2 protein using immunohistochemistry in invasive ductal carcinoma and its association with the established growth receptors such as estrogen receptor (ER), progesterone receptor (PR) and human epidermal growth receptor 2 (HER2). Methods: We conducted a cross sectional study using 100 samples of archived formalin-fixed paraffin embedded tissue blocks of invasive ductal carcinoma and stained them with immunohistochemistry for PITX2, ER, PR and HER2. All HER2 with scoring of 2+ were confirmed with chromogenic in-situ hybridization (CISH). Results: PITX2 protein was expressed in 53% of invasive ductal carcinoma and lack of PITX2 expression in 47%. Univariate analysis revealed a significant association between PITX2 expression with PR (p=0.001), ER (p=0.006), gland formation (p=0.044) and marginal association with molecular subtypes of breast carcinoma (p=0.051). Combined ER and PR expression with PITX2 was also significantly associated (p=0.003) especially in double positive cases. Multivariate analysis showed the most significant association between PITX2 and PR (RR 4.105, 95% CI 1.765-9.547, p=0.001). Conclusion: PITX2 is another potential prognostic marker in breast carcinoma adding significant information to established prognostic factors of ER and PR. The expression of PITX2 together with PR may carry a very good prognosis.

Integration Sites and Genotype Distributions of Human Papillomavirus in Cervical Intraepithelial Neoplasia

  • Objectives: To analyse HPV integration prevalence and genotype distributions in cervical intraepithelial neoplasia (CIN) in east part of China, furthermore to assess preferential sites for common HPV integrations and provide baseline information for cervical abnormality screening and prevention. Methods: Integration of HPV in 113 paraffin-embedded cervical intraepithelial neoplasia samples was assessed using Gencap technology in Key Laboratory of Biotechnologies in BGI-Shenzhen. Results: 64 samples were HPV-integrated and as the cervical lesions increased, the integration rate became higher significantly (P=0.002). Fifteen different HPV genotypes were detected, 14 high-risk (16, 18, 31, 33, 51, 52, 56, 58, 66, 68) and 1 low-risk (11). The most common genotypes were HPV-16, 58, 33, 52, 66, and 56. Thirteen patients had co-integration involving mainly HPV-16 and 58. The frequency of HPV gene disruption was higher in L1 and E1 genes than in other regions of the viral genomes. Conclusion: Some 56.6% of CIN lesions in Qingdao had HPV integrations, and 67.2% of HPV-integrated patients were HPV-16 and 58, more prone to be integrated in younger patients below 45 years old. There exist preferential sites for HPV-16 and HPV-58 integration, and they are more likely to be disrupted in the L1 and E1 loci.

Subjective Symptoms of Musculoskeletal Disorders among Excavator Drivers (굴삭기 운전자의 근골격계 자각증상에 관한 연구)

  • Objectives: The purpose of this study was to assess excavator drivers in order to determine the proportion showing subjective symptoms of musculoskeletal disorders and to evaluate the association between the symptoms and characteristics of the subjects. Methods: A questionnaire survey was conducted to collect the information on participants' age, average working hours per day, duration of career (years), work-related musculoskeletal symptoms, and more. The final dataset included 141 excavator drivers. Frequency and percentage were summarized and then the associations between the characteristics of the subjects and symptoms were evaluated as odds ratios (ORs) and 95% confidence intervals (CIs) using multiple logistic regression analysis. Results: Seventy-six percent of the currently reported musculoskeletal symptoms. Those who have musculoskeletal symptoms attributed their symptoms to unstable posture at work, mental stress or vibrations. There was a significant association between symptoms of musculoskeletal disorders and career of excavator drivers (P for trend=0.04). Conclusions: The results of this study suggest that the prevalence of musculoskeletal disorders may be high among excavator drivers and warrants well-designed studies on work-related disorders among excavator drivers.

Interleukin-6-174 Promoter Polymorphism and Susceptibility to Hepatitis B Virus Infection as a Risk Factor for Hepatocellular Carcinoma in Iran

  • Background: Hepatitis B virus (HBV) is a major risk factor for hepatocellular carcinoma (HCC). Cytokines play an important role in the regulation of immune responses and defense against viral infections. Human interleukin 6 (IL6) is a multifunctional cytokine that participates in these processes. Objective: The aim of this study was to assess the IL6-174 gene polymorphism in patients with chronic hepatitis B virus (HBV) infection as compared with healthy controls in an Iranian population. Materials and Methods: Totals of 297 HBV patients and 368 control individuals were evaluated. Genomic DNA was extracted from peripheral blood and the SSP-PCR (sequence specific primer-polymerase chain reaction) method was applied for genotyping. Results: The frequencies of genotypes C/C, G/G and C/G in HBV cases were 4.7%, 34.3%, 60.9% and in controls were 12.8%, 39.7% and 47.6%, respectively. The frequencies of G and C allele in patients and controls were 78.1%, 21.9% and 67.4%, 32.6 % respectively. There was a significant difference in the frequencies of G/G genotype (CI=1.8-7.1, OR=3.47, P=0.00001) and G allele (CI=1.34-2.23, OR=1.72, P=0.0001) between HBV patients and the control group. Conclusions: These findings suggest that the IL6-174 C/G genotype and the G allele are strongly associated with susceptibility to HBV infection. Demographic information showed that most of the subjects were male (74.4%). According to high frequency of G/G genotype in male participants (63.1%) men probably are more susceptible to hepatitis than women.

Development of Excitation Light Source for Photodynamic Diagnosis of Cancer (광역학적 암진단을 위한 여기 광원장치의 개발)

  • In this paper, the development of excitation light source is proposed for excitation light of the photodynamic therapy of cancer. Since the selection of the wavelength band of excitation light has an interrelation with fluorescence generation according to the selection of a photosensitizer. This study aims at designing and evaluating light source that can stably generate light with various kinds of wavelengths in order to make possible photodynamic diagnosis using a photosensitizer and diagnosis using auto-fluorescence. The light source device was a Xenon lamp and filter wheel, composed of an optical output control through iris and filters with several wavelength bands. It also makes the inducement of auto-fluorescence possible because it is designed to generate a wavelength band of 380-420nm, 430-480nm, 480-560nm. The transmission part of the light source was developed to enhance the efficiency of light transmission. To evaluate this light source device by KFDA#s technical reference, the characteristics of the light output and wavelength band were found.

Health Risk Assessment and Evaluation of Asbestos Release from Asbestos-cement Slate Roofing Buildings in Busan (부산시내 석면슬레이트지붕 건축물로부터의 석면 노출 및 건강위해성 평가)

  • This study was performed to evaluate the asbestos exposure levels and to calculate excess lifetime cancer risk (ELCR) for the risk assessment of the asbestos fibers released from asbestos-cement slate roofing (ASR) building. Total number of ASR buildings was into 21,267 in Busan, and 82.03 percent of the buildings was residential houses, and 43.61 percent of the buildings was constructed in 1970s. For this study, ten buildings were selected randomly among the ASR buildings. The range of airborne asbestos concentration in the selected ten ASR buildings was from 0.0016 to 0.0067 f/mL, and the concentration around no-admitted ASR buildings was higher than that around admitted buildings. The ELCR based on US EPA IRIS (integrated risk information system) model is within 3.5E-05 ~ 1.5E-04 levels, and the ELCR of no-admitted ASR buildings was higher than 1.0E-04 (one person per million) level that is considered a more aggressive approach to mitigate risk. These results indicate that the cancer risk from ASR buildings is higher than other buildings, and systematic public management is required for control of no-admitted ASR buildings within near future.

The Current Trend of Research about Bojungikki-tang (보중익기탕의 최근 연구동향)

  • Objective : The purpose of this study was to provide the information about Bojungikki-tang by domestic papers and theses. Method : Domestic papers related to Bojungikki-tang were reviewed and analyzed. These papers were classified by year, research method, experimental method, and subject. Result : The following results were obtained in this study. 1. The study of Bojungikki-tang started from 1984 and continued steadily. 2. The domestic papers about Bojungikki-tang were 47 volumes. Among them, there were 26 volumes since 2000. 3. Classified by research method from the year 2000, 20 experimental studies, 4 clinical experiments, and 2 documentary researches. 4. Classified by experimental method from the year 2000, 10 studies used mouse in vivo experiment and 4 studies used rat in vivo experiment and 6 studies used each cell in vitro experiment. 5. Classified by subject from the year 2000, papers related to immune enhancing effect, nerve and mental system, radiation protection effect, anti-allergy, anti-inflammatory, anti-cancer, remedy of diabetes, skin protection and so on. Conclusion : Many papers of experimental studies, clinical experiments and documentary researches related Bojungikki-tang are included in domestic journals. Bojungikki-tang is being researched variously in immune enhancing effect, nerve and mental system, radiation protection effect, anti-allergy, anti-inflammatory, anti-cancer, remedy of diabetes, skin protection and so on. However, more clinical studies on Bojungikki-tang are needed.

Analysis of studies on the by-products of the Broussonetia kazinoki Siebold or Broussonetia papyrifera (L.) Vent. (닥나무 부산물에 대한 문헌 연구)

  • Objective : To provide information about the accessory product of Broussonetia kazinoki Siebold or Broussonetia papyrifera (L.) Vent by analyzing old books of oriental medicine, domestic/international papers and related patents Methods : Old books related to the accessory product in the field of oriental medicine were reviewed. Research papers regarding the pharmacological activity of the by-products were reviewed and analyzed. Patents about the residual products were examined and classified by year and subject Results : Seven kinds of by-products from Broussonetia kazinoki Siebold or Broussonetia papyrifera (L.) Vent has been used as medicines in oriental medicine. Recently, anti-oxidating, anti-cancer, anti-mutagenic and anti-inflammation activity of the residual product of these plants has been investigated through scientific research. There were 19 patents related with the accessory products of these plants, which were in the subjects of functional cosmetics, anti-inflammation, cleansing goods, hair restorers or improvement of learning ability. Further investigations about the activity of these plants are needed in bone metabolism, water balance and hemostasis in the future. Conclusion : Residual products from these plants is being used in various ways. However, more studies on the efficacy and mechanism, as well as safety, of these plants should be conducted precisely in the future.

Automated Cell Counting Method for HeLa Cells Image based on Cell Membrane Extraction and Back-tracking Algorithm (세포막 추출과 역추적 알고리즘 기반의 HeLa 세포 이미지 자동 셀 카운팅 기법)

  • Cell counting is extensively used to analyze cell growth in biomedical research, and as a result automated cell counting methods have been developed to provide a more convenient and means to analyze cell growth. However, there are still many challenges to improving the accuracy of the cell counting for cells that proliferate abnormally, divide rapidly, and cluster easily, such as cancer cells. In this paper, we present an automated cell counting method for HeLa cells, which are used as reference for cancer research. We recognize and classify the morphological conditions of the cells by using a cell segmentation algorithm based on cell membrane extraction, and we then apply a cell back-tracking algorithm to improve the cell counting accuracy in cell clusters that have indistinct cell boundary lines. The experimental results indicate that our proposed segmentation method can identify each of the cells more accurately when compared to existing methods and, consequently, can improve the cell counting accuracy.
