Development of an Educational Program to Prevent Cervical Cancer among Immigrants in Korea

  • Background: This study developed and measured the effects of a cervical cancer prevention program for married women immigrants. Materials and Methods: A nonequivalent control group pre-test/post-test design was used with a group of married women immigrants registered at the multi-cultural center in the city of "J." Data on for 30 participants in the intervention group and 27 participants in the control group (N=57) were used for analysis. The intervention group attended a 4-session cervical cancer prevention program. Results: The knowledge of the intervention and control groups about cervical cancer post-intervention was significantly different (F=12.55, p<0.001). The perceived susceptibility score before and after the experiment, for the intervention group, and 29.4 and 28 for the control group, was significantly different (t=2.063, p=0.043). After the program, cancer prevention behavior was significantly different in the intervention group (t=2.646, p=0.010). Conclusions: The results obtained in this study indicate that the cervical cancer preventive program was effective in increasing cervical cancer knowledge, perceived susceptibility, and cancer prevention behavior.

  • Purpose: The purpose of this study was to investigate the effect of cancer-overcome BeHaS exercise program on pain, flexibility, grip strength and stress in patient with breast cancer following surgery. Method: Study was designed as a non-synchronized research method with a nonequivalent control group. Data collection was performed from September 2007 to January 1, 2008. Participants were 34 patients (experimental group: 19, control group: 15) undergoing surgery for breast cancer in a hospital in D city. Women in the experimental group were participated in the cancer-overcome BeHaS (Be Happy and Strong) exercise program for 60 minutes per session once a week for 8 weeks and it was recommended that they do the exercises once more each week at home. The control group received one education session on breast cancer management. Results: There were no significant differences between the two groups for pain, flexibility or grip strength. But stress in experimental group was significantly decreased compared to that of the control group. Conclusion: This result suggests that cancer-overcome BeHaS exercise program is helpful to reduce stress in patients after breast cancer surgery. Further research is needed for multi-dimensional evaluation on psycho-social effects of cancer-overcome BeHaS exercise program.

  • Purpose: The purpose of this study was to analysis the effects of a forgiveness nursing intervention program on hope and quality of life in woman with cancer. Methods: The used design was a nonequivalent control group design with pretest and post-test. Thirty patients diagnosed with uterine cervical cancer, breast cancer, ovarian cancer were recruited from a University Hospital in Busan, Korea. An experimental group (n=15) was given the forgiveness nursing intervention program, and a control group (n=15) was done a common nursing intervention. The period of data collection was from June 10 to September 10, 2004. The obtained data were analyzed using ${\chi}^2-test$, Fisher's exact test, t-test, and ANCOVA. Results: The first hypothesis, "The experimental group would have higher hope state than control group" was supported(F =16.967, p = .000). The second hypothesis, "The experimental group would have higher quality of life state than control group" was supported (F =4.850, p =.036). Conclusions: The findings showed that the forgiveness nursing intervention program was effective to increase hope and quality of life in women with cancer. Therefore, this program may be used for a nursing practice for cancer patients who are suffering from emotional distress.

The National "Smoking Cessation Clinics" Program in the Republic of Korea: Socioeconomic Status and Age Matter

  • Background: Between 1998-2009 South Korea experienced significant progress in reducing the male smoking rate from 66.3% to 46.9%. As part of a significant government effort in the area of smoking cessation intervention, the Korean government implemented the national "Smoking Cessation Clinics (SCC)" program in 2004. Materials and Methods: Data covered 804,334 adult male smokers participating in SCC program at 253 public health centers between 2006-2009. We examined participant cessation rates with the SCC program, their characteristics and program intervention components using health insurance status as a socioeconomic status (SES) indicator. Multivariate logistic regression analyses were performed correcting for intra-class correlations within public health centers. Results: The overall 6-month quit rate was high (46.8%). Higher odds of smoking cessation were positively associated with higher levels of behavioral counseling sessions, but not nicotine replacement therapy (NRT). Cessation rates were lower for Medicaid participants than for regular health insurance participants. Disadvantaged younger smokers were less likely to participate in the program. Older smokers were more likely to quit regardless of SES. Stress was cited as major reason for failure. Conclusions: SES inequalities across different age groups exist in smoking cessation among Korean adult male smokers. There is a need for intervention programs specifically targeting sub-populations of SES by different age groups.

Factors Associated with Attending the National Cancer Screening Program for Liver Cancer in Korea

  • Background and Aims: The National Cancer Screening Program (NCSP) for liver cancer was initiated in 2003 in Korea. The objective of this study was to evaluate the participation rate of the program and to provide preliminary information on its results based on data collected by the NCSP in 2009. Methods: The target population of the NCSP for liver cancer in 2009 was comprised of 373,590 adults aged ${\geq}40$ years at high risk for liver cancer. Participation rates and positivity rates were assessed in this population. Multivariate logistic regression analysis was performed to determine the factors associated with participation in the NCSP for liver cancer. Results: The overall participation rate was 37.9% and 1,126 participants were positive at screening. The highest participation rates were observed in women, those in their 60s, National Health Insurance beneficiaries, and individuals positive for hepatitis B surface antigen. Positivity rates for men, those in their 70s, Medical Aid Program recipients and individuals with liver cirrhosis were the highest in the respective categories of gender, age, health insurance type, and risk factor for liver cancer. Conclusions: The participation rates of the NCSP for liver cancer are still low, despite the fact that the program targets a high-risk group much smaller than the general population. Efforts to facilitate participation and to reduce disparities in liver cancer screening among Korean men and women are needed. These results provide essential data for evidence-based strategies for liver cancer control in Korea.

암환자 의료비 지원사업 대상자의 만족도 관련 요인 (Factors Affecting Beneficiary Satisfaction on Financial Aid Program for Cancer Patients in Korea)

  • Limited research has investigated the satisfaction of patients with cancer. This study was performed to explore patients' satisfaction and the related factors. The data were collected by telephone survey for the participants who were beneficiaries on the national financial aid program for cancer patients between January and October in 2009. Student's t-tests and analysis of variance were performed first to determine if the mean satisfaction score differed by the characteristics of study objects, followed by stepwise multiple regression analyses to examine the factors affecting satisfaction. When comparing the relating factors with patients' satisfaction according to the sociodemographic characteristics, the male, old-aged, higher educated, lower cancer stage, lung cancer group showed a significantly higher level of recognition for satisfaction. A public health center, better public relations, recognized more helpful, don't have expectation, put a person to expense, and don't feel pressured for medical cost groups were showed a higher level of recognition for satisfaction. The result of the multiple regression analysis, short waiting time, well known program policy, recognized helping of policy and had the financial burden had significantly influence on the satisfaction of patients with cancer.

일본의 지역사회 암 조기 검진사업에 관한 접근 방안 (The Approach Method of Community-based Cancer Screening Program in Japan)

  • The Community based cancer screening program passed in 1960 was a milestone for initiating a national and local health program in Japan. And since then local governments and Cancer Society have been developing and providing cancer screening programs of Stomach, Cervix, Breast and Colorectum for population. To apply the effectiveness of community based cancer screening program, it is important to understand the key issue related to cancer screening participation of population and technology of cancer detection. The purpose of this study was to understand the community based cancer screening program in Japan, and to apply the information for establishment of community based cancer screening program in Korea. The characteristics of community based cancer screening program in Japan were as follows. The first, community based cancer screening program was implemented by the National Health and Medical Services Law for the Aged since 1983. The second, Cancer Society and Cancer Detection Center were core for cancer screening program. The third, the budget for cancer screening program was established by the National Health and Hygiene. The fourth, the continuous quality control for medical staff was provided by Cancer Society and Cancer Detection Center The fifth, the efforts for the promotion of cancer screening rate.

Barriers to Cancer Screening among Medical Aid Program Recipients in the Republic of Korea: A Qualitative Study

  • Few studies have examined reasons why those with low socioeconomic status do not adequately receive cancer screening. We therefore conducted a qualitative study to assess the barriers to cancer screening in people with low socioeconomic status, and to examine the reasons why Medical Aid Program (MAP) recipients do not participate in the National Cancer Screening Program (NCSP). A focus group methodology was used. Participants included MAP recipients (men aged 45-79 years, women aged 35-79 years) who had been invited to cancer screening at least twice based on the NCSP protocol, but had not been screened for any cancer from 2009-2012. We recruited participants living in the cities of Goyang and Paju. A total of 23 MAP recipients participated in four focus group discussions, including 12 men and 11 women. In this qualitative study, we identified six barriers to screening: lack of trust in the NCSP and cancer screening units; fear of being diagnosed with cancer; discomfort or pain from the screening procedure; lack of time, lack of knowledge about cancer screening or lack of awareness of the existence of the NCSP; physical disability or underlying disease; and logistic barriers. Interventions such as individualized counseling, letters and reminders, or other individually-targeted strategies, especially for those with lower socioeconomic status are required to increase participation and reduce disparities in cancer screening.

재활프로그램이 유방암절제술 환자의 신체건강, 생리지수 및 삶의 질에 미치는 효과 (The Effects of a Rehabilitation Program on Physical Health, Physiological Indicator and Quality of Life in Breast Cancer Mastectomy Patients)

  • Purpose: The purpose of this study was to investigate the effects of a rehabilitation program on physical health, physiological indicators and quality of life in breast cancer mastectomy patients. Methods: The subjects included thirty-one patients with breast cancer(17 in the experimental group and 14 in the control group). The subjects in the experimental group participated in a rehabilitation program for 10 weeks, which was composed of an exercise program, teaching, counseling and support for 2 sessions per week. Results: There was a significant increase in flexion, internal rotation and external rotation but no significant increase in extension in the experimental group compared to the control group. The total cholesterol, triglyceride, HDL, LDL, and CD56 in the experimental group compared to the control group was not significantly decreased after the rehabilitation program. Compared to the control group, quality of life in the experimental group was significantly improved and fatigue in that group was significantly decreased after the rehabilitation program. Conclusion: The 10-week rehabilitation program showed a large affirmative effect on physical health, physiological indicators and quality of life in breast cancer mastectomy patients.

  • Purpose: The purpose of this study was to identify the effects of a home based exercise program for patients with stomach cancer who were undergoing oral chemotherapy. Methods: The home-based exercise program was developed from the study findings of Winningham (1990) and data from the Korea Athletic Promotion Association (2007). The home-based exercise program consisted of 8 weeks of individual exercise education and exercise adherence strategy. Participants were 24 patients with stomach cancer who were undergoing oral chemotherapy following surgery in 2007 or 2008 at a university hospital in Seoul. Patients were randomly assigned to either the experimental group (11) or control group (13). The effects of the homebased exercise program were measured by level of cancer related fatigue, NK cell ratio, anxiety, and quality of life. Data were analyzed using SPSS/WIN 13.0 version. Results: The degree of cancer related fatigue and anxiety in the experimental group decreased compared to the control group. The NK cell ratio and the degree of quality of life of experimental group increased while that of the control group decreased. Conclusion: This study result indicate the importance of exercise and provide empirical evidence for continuation of safe exercise for patients with cancer during their chemotherapy.
