Influence of a glide path on the dentinal crack formation of ProTaper Next system

  • Objectives: The aim was to evaluate dentinal crack formation after root canal preparation with ProTaper Next system (PTN) with and without a glide path. Materials and Methods: Forty-five mesial roots of mandibular first molars were selected. Fifteen teeth were left unprepared and served as controls. The experimental groups consist of mesiobuccal and mesiolingual root canals of remaining 30 teeth, which were divided into 2 groups (n = 15): Group PG/PTN, glide path was created with ProGlider (PG) and then canals were shaped with PTN system; Group PTN, glide path was not prepared and canals were shaped with PTN system only. All roots were sectioned perpendicular to the long axis at 1, 2, 3, 4, 6, and 8 mm from the apex, and the sections were observed under a stereomicroscope. The presence/absence of cracks was recorded. Data were analyzed with chi-square tests with Yates correction. Results: There were no significant differences in crack formation between the PTN with and without glide path preparation. The incidence of cracks observed in PG/PTN and PTN groups was 17.8% and 28.9%, respectively. Conclusions: The creation of a glide path with ProGlider before ProTaper Next rotary system did not influence dentinal crack formation in root canals.

Comparison of the shaping ability of novel thermally treated reciprocating instruments

  • Objectives: The present study aimed to evaluate the shaping ability of 2 thermally treated nickel-titanium reciprocating systems in simulated curved canals. Materials and Methods: Forty simulated canals were prepared to apical size 25 using Reciproc Blue R25 (VDW) and WaveOne Gold Primary (Dentsply Sirona) instruments. Standard pre- and post-preparation images were taken and superimposed. The removal of resin material was measured at 5 standard points: the canal orifice, halfway between the canal orifice and the beginning of the curve, the beginning of the curve, the apex of the curve, and the end-point of the simulated canal. The data were analysed using the independent sample t-test with a 5% significance threshold. Results: The canals in which Reciproc Blue R25 was used showed a significantly greater widening than those in which WaveOne Gold was used at 4 of the 5 measurement points (p < 0.05). The Reciproc Blue R25 instrument removed significantly more resin from the inner aspect of the curve at 2 of the 5 points and similar amounts at the remaining 3 points. At the 2 apical points, there was no significant difference between the Reciproc Blue R25 and WaveOne Gold Primary instruments. Conclusion: Both instruments respected the original canal anatomy; however, WaveOne Gold resulted in a more conservative shape with less transportation.

Cyclic fatigue life of Tango-Endo, WaveOne GOLD, and Reciproc NiTi instruments

  • Objectives: To compare the fatigue life of Tango-Endo, WaveOne GOLD, and Reciproc NiTi instruments under static model via artificial canals with different angles of curvature. Materials and Methods: Reciproc R25, WaveOne GOLD Primary, and Tango-Endo instruments were included in this study (n = 20). All the instruments were rotated in artificial canals which were made of stainless steel with an inner diameter of 1.5 mm, $45^{\circ}$, $60^{\circ}$, and $90^{\circ}$ angles of curvatures and a radius of curvature of 5 mm until fracture occurred, and the time to fracture was recorded in seconds using a digital chronometer. The data were analyzed using Kruskal-Wallis and post-hoc Dunn tests were used for the statistical analysis of data in SPSS 21.0 software. Results: Tango-Endo files were found to have significantly higher values than WaveOne GOLD and Reciproc files in terms of fatigue life (p < 0.05). However, there was no statistically significant difference between fatigue life of Reciproc and WaveOne GOLD files (p > 0.05). It was determined that increasing the angle of curvature of the stainless canals caused significant decreases in fatigue life of all of three files (p < 0.05). Conclusions: Within the limitations of the present study, the cyclic fatigue life of Tango-Endo in canals having different angles of curvature was statistically higher than Reciproc and WaveOne GOLD.

Influence of reciprocating and rotary instrumentation on microbial reduction: a systematic review and meta-analysis of in vitro studies

  • Objectives: The purpose of this study was to conduct a systematic review and meta-analysis of in vitro studies regarding the effectiveness of reciprocating and rotary instrumentation on microbial reduction in root canals. Materials and Methods: PubMed, Scopus, Web of Science, the Cochrane Library, and the gray literature were searched through December 2019. Studies comparing the influence of reciprocating and rotary instrumentation on the removal of microorganisms from root canals that quantified the antimicrobial effect were included. Data extraction was completed using a systematic form for data collection. The risk of bias of the studies was evaluated. Standardized mean differences (SMDs) and confidence intervals (CIs) were calculated using a random effects meta-analysis. Results: Seventeen in vitro studies were included in this systematic review, of which 7 provided adequate data for inclusion in the meta-analysis. Both reciprocating and rotary systems were similarly effective in reducing the microbial load in infected root canals (SMD [95% CI], 0.0481 [-0.271, 0.367]). Three studies showed a low risk of bias, whereas most of the studies (82%) presented a medium risk. Conclusions: Although both techniques decrease the microbial content (with reductions of 23.32%-88.47% and 23.33%-89.86% for reciprocating and rotary instrumentation, respectively), they are not able to provide complete disinfection of root canals.

실시간 관개수로 수위 모니터링을 활용한 논 관개용수 공급적정성 평가 (Evaluation of Water Supply Adequacy using Real-time Water Level Monitoring System in Paddy Irrigation Canals)

  • Appropriate amount of water supply to paddy fields in proper time is important to achieve efficient agricultural water management. The purpose of this study is to evaluate the irrigation water supply adequacy for paddy fields using water level data in irrigation canals. For the evaluation, the real-time water level data were collected from main canals in the Dongjin irrigation district for 2 years. Using the water level data, delivered irrigation water amounts at the distribution points of each canal were calculated. The water balance model for paddy field was designed considering intermittent irrigation and the irrigation water requirement was estimated. Irrigation water supply adequacy was analyzed from main canals to the irrigation blocks based on the comparison between estimated requirement and delivered irrigation water amounts. From the adequacy analysis, irrigation water supply showed poor management condition in 2012 with low efficiency except the Daepyong canal section, and the adequacy in 2013 was good or fair except the Yongsung canal section. When irrigation water for paddy fields was insufficient, water supply adequacy was affected by irrigation area, but when irrigation water was enough to supply, adequacy was affected by distance from main canal to distribution points. These results of the spatial and temporal dimensions of the irrigation adequacy could be utilized for efficient irrigation water management to improve the temporal uniformity and equity in the water distribution for paddy fields.

수종의 Engine driven NiTi file과 stainless steel K-file을 이용한 근관형성 후 근관의 조직형태학적 변화에 관한 연구 (A STUDY OF HISTOMORPHOLOGICAL CHANGE OF CURVED ROOT CANAL PREPARATION USING GT ROTARY FILE, PROFILE AND STAINLESS STEEL K-FILE)

  • The purpose of this study was to compare the histomorphological change of curved root canal preparation using GT rotary File, Profile .04 taper and stainless steel K-file. 45 mesial canals(over 20 degree) of extracted human mandibular first molars were mounted in resin using a modified Bramante muffle system and divided into three groups. The roots were cross-sectioned at 2.5mm 5mm and 8mm levels from apical foramen. Tracings of the canals were made from preinstrumentation pictures of the cross section. The canals were prepared using a step-back technique with stainless steel K file(group 1), Profile .04 taper rotary file(group 2) and GT rotary file(group 3). Tracings of the prepared canals were made from postinstrumentation picture. Canal centring ratio. amount of transportation, area of dentin removed and shape of canal were measured and statistically were evaluated with Student-Newman-Keuls test using Sigma Stat(Jandel Scientific Software, USA). The results were as followings : 1 Amount of transportation of group 2 was the lowest at apical part, but there was no statistical difference. The direction of transportation was the outside of curvature at apical part. 2. Centering ratio at the apical part of group 1 was the highest, and there was statistical differences between apical and middle part, apical and coronal part(p<0.05). Centering ratio at the middle part of group 3 was the lowest, and there was statistical difference between apical and middle part(p<0.05). Centering ratio of group 2 was the lowest at apical part, but there was no statistical difference. 3. Amount of dentin removed of group 1 was the highest at coronal, middle and apical part among three groups, and there was statistical difference(p<0.05). 4. The majority of the cross-sectioned canal shape after instrumentation were irregular at coronal, middle and apical part. But there are more number of round shaped canals at group 3 than other group.

근관충전용 실러의 물리적 성질에 관한 연구 (A STUDY ON THE PHYSICAL PROPERTIES OF ROOT CANAL SEALERS)

  • The purpose of this study was to compare and estimate the physical properties of five root canal sealers classified Calciobiotic root canals sealer as calcium hydroxide based sealer, Apatite root sealer type II as calcium phosphate based sealer, AH-26 as resin based sealer, Canals and Pulpdent root canals sealer as zinc oxide eugenol based sealer. The author investigated dimensional change and flow rate of canal sealers, diametral tensile strength and shear bond strength of sealers to dentin to evaluate the physical properties on affect of complete obturation of root canal and performed the total 100 specimens of each 25 sealers under the condition of root temperature according to manufacturer's instructions. All specimens were stored at $37{\pm}1^{\circ}C$ in 100 % relative humidity. A microscope for measurement of micro distance is used for the dimensional change test and evacuation methods using vaccum were used for the flow rate test. The result differed by the storage time measured on the tests of diametral tensile strength and shear bond strength to dentin. The following results were obtained ; 1 On the test of dimensional change, Canals and Pulpdent expanded slightly, AH-26 and Apatite showed the severe shrinkage after 48 hours. 2. AH-26 and Apatite were the excellent with each 24.59mm, 31.19mm after 3 minutes in the aspect of flow property. 3. On the diametral tensile strength, Calciobiotic root canals sealer showed the highest strength with 27.13kg/$cm^2$ after 48 hours, Apatite root sealer type II showed highest strength with 84.57kg/$cm^2$ after 120 hours. 4. On the shear bond strength to dentin, AH-26 was most excellent with 55.73kgf/$cm^2$ after 24 hours and with 134.71kgf/$cm^2$ after 120 hours.

Analysis of C-shaped root canal configuration in maxillary molars in a Korean population using cone-beam computed tomography

  • Objectives: The purpose of this study was to investigate the incidence of root fusion and C-shaped root canals in maxillary molars, and to classify the types of C-shaped canal by analyzing cone-beam computed tomography (CBCT) in a Korean population. Materials and Methods: Digitized CBCT images from 911 subjects were obtained in Chosun University Dental Hospital between February 2010 and July 2012 for orthodontic treatment. Among them, a total of selected 3,553 data of maxillary molars were analyzed retrospectively. Tomography sections in the axial, coronal, and sagittal planes were displayed by PiViewstar and Rapidia MPR software (Infinitt Co.). The incidence and types of root fusion and C-shaped root canals were evaluated and the incidence between the first and the second molar was compared using Chi-square test. Results: Root fusion was present in 3.2% of the first molars and 19.5% of the second molars, and fusion of mesiobuccal and palatal root was dominant. C-shaped root canals were present in 0.8% of the first molars and 2.7% of the second molars. The frequency of root fusion and C-shaped canal was significantly higher in the second molar than the first molar (p < 0.001). Conclusions: In a Korean population, maxillary molars showed total 11.3% of root fusion and 1.8% of C-shaped root canals. Furthermore, root fusion and C-shaped root canals were seen more frequently in the maxillary second molars.

  • ;A new root canal instrument and instrumentation technique: a preliminary report. Cleaning and shaping the root canal system has been and continues to be a challenge for even the most experienced endodontist. Curved. narrow canals. in particular. cause difficulties for the beginner as well as the specialist. A new instrument designed to incorporate new concepts was developed to ameliorate the problems in cleaning and shaping root canal systems. This new instrument has been given the name of SW (Senia and Wildey). The new SW instrument uses controlled right and left rotational forces. This motion was used to clean and shape simulated root canals in plastic blocks and root canals in extracted teeth. Instrumentation appeared to be easier. faster. and more precise than with conventional instruments. especially in curved canals where there was remarkable reduction of canal transportation. A mechanical version of the SW instrument was also developed. It was used to flare the coronal portion of the root canal system. Wildey WL. Various factors must be considered in an analysis of instrumentation of the root canal system: the dentin that is cut: the technique used to cut it: the design of the instruments: the material and manufacturing process used to make the instruments: the irrigant used during the procedure: and the anatomic configuration of the root canal system. An analysis of these factors clearly indicates that existing root canal instruments and techniques are less than ideal and. in fact. do not accomplish what is expected of them. Root canals must be properly. but. at the same time. destructive and unnecessary removal of dentin should be kept to a minimum. The Flex-R and Canal Master instruments were developed to address some of the shortcomings of existing instruments and techniques. 근관감염세균의 분포에 관한 연구 (DISTRIBUTION OF ORAL PATHOGENS IN INFECTIONS OF ENDODONTIC ORIGIN)

  • It has been documented that periodontopathic bacteria are also implicated in endodontic infections. 168 rDNA gene-directed PCR was to examine the prevalence of periodontopathic bacteria including Actinobacillus actinomycetemcomitans (Aa), Prevotella intermedia (Pi), Prevotella nigrescens (Pn), Porphyromonas gingivalis (Pg), Porphyromonas endodontalis (Pe), and Treponema denticola (Td) in the root canals of 36 endodontically infected teeth having apical lesions with or without clinical symptoms like pain, swelling, and fistula. 1. In 36 infected root canals, most frequently detected bacterial species was Pg (61.1%), followed by Td (52.8%) and Pe (38.9%). 2. Of 36 infected root canals, Aa was detected in 6 canals (16.7%) of the teeth, all of which showed clinical symptoms. 3. Of 36 infected root canals, Pi and Pn were found in 4 03.9%) and 5 (33.3%), respectively. Notably, prevalence of Pn in the symptomatic teeth was 50.0%. 4. One of black-pigmented anaerobic bacteria (BPB) including Pi, Pn, Pe, and Pg was detected in all of the teeth that showed pain or especially swelling but not fistula. It was, however, found that prevalence of BPB in the asymptomatic teeth or the teeth with fistula was only 40%. 5. Pe and Pg were detected in the teeth regardless of the presence or absence of symptoms. 6. Td was detected in the teeth regardless of the presence or absence of symptoms. High prevalence of BPB in the symptomatic teeth but low in the asymptomatic teeth suggests that BPB may play an important role in the pathogenesis of periapical lesions.
