Aspergillus is a fungus of Ascomycetes class that occurs world wide and found in many of the molds on foods, frutis, grains and plants. Its ubiquitous spores, present in dust, enter the respiratory system of man, where, under proper condition, they become pathogenic. We present a case of primary maxillary sinus aspergillosis developed from a 52-year old male patient when has oroantral fistula after extraction of maxillary Lt. 1st molar tooth. The treatment composed with Caldwell-Luc's operation, Oro-antral fistula closure and associated anit-fungal treatment with Amphoatrisin B.
The postoperative maxillary cyst develops as a delayed complication after surgical intervention or Caldwell-Luc operation in the maxillary sinus and was also reported that it could occur after Le Fort I osteotomy. This is also called as surgical cliated cyst because of its lining epithelium is usually lined by a pseudo-stratfied ciliated columnar epithelium. This report represents a case of postoperative maxillary cyst which developed within the anterior of maxilla and in association with nasal mucosa 6 years after a Le Fort I osteotomy. In 1989, 26-year-old male complained of his mandibular prognathism and underwent orthogmathic surgery, Le Fort I osteotomy, bilateral sagittal split ramus osteotomy, partial glossectomy.
Cholesterol granuloma is a disease in which cholesterol crystals act as a foreign substance in the surrounding tissues and cause granulomatous reaction and fibrosis within the chamber. It is found in various locations of the body, but the most common location is the temporal bone associated with middle ear disease. Because the disease is associated with breathing disturbance, it may also occur in the paranasal sinus. However, it has been rarely reported since its first report by Graham and Michaels in 1978. Recently, we experienced a case of cholesterol granuloma of the right maxillary sinus of a 63-year-old female patient without any nasal symptoms. We successfully managed this case with Caldwell-Luc operation. Also, we summarized the cases of cholesterol granuloma of the sinonasal region reported in Korea.

A 60-year-old male complained of headache, nasal discharge, and diplopia for over one month with a history of left upper molar extraction, and he had recently experienced severe discharge of purulent exudate from his left antrum. Under the diagnosis of maxillary sinusitis, the Caldwell-Luc operation was performed, and several fragments of amorphous white mucoroid materials were removed. In the histological observation, sinus mucosa was relatively well preserved, but showed diffuse infiltration with eosinophilic polymorphonuclears. Huge molds of mucormycosis were associated with the surface of mucosa. He was treated with amphotericin-B deoxycholate, resulting in the uneventful healing of the antral lesion. The current case of antral mucormycosis was very rare but effectively treated by surgical removal of antral mucosa and the following antibiotic therapy for the strong inhabitants of fungal molds. We also presumed that the patient was superinfected with commensal fungus of mucormycosis during broad spectrum antibiotic therapy for the previous dental infection.
Purpose: Surgical intervention into the maxillary posterior area can lead to inadvertent communication with the maxillary sinus. Spontaneous healing of 1 to 2 mm openings can occur. However, in patients with larger oro-antral communications and those with a history of sinus disease, surgical closure is often indicated. Materials and Methods: In this case, a modified free gingival graft technique was used. The oro-antral sinus track after extraction was about $2{\times}4\;mm$. But the patient had a history of Caldwell-Luc operation. Immediately after extraction, a modified free gingival graft was used for the closure of oro-antral sinus track. Result: A modified free gingival graft technique was successfully used for the closure of oro-antral sinus track. With this technique, esthetic results including similar colors, textures were achieved. Conclusion: It is thought that this technique is very useful in the closure of oro-antral sinus track.
Purpose: The purpose of this study is to analyze the clinical features of postoperative maxillary cyst (POMC) according to the patient's medical history of previous sinus operation and symptoms with radiological characteristics. Materials and Methods: The subjects of this study were 41 patients who had been diagnosed with POMC via clinical and histological examination from 2007 to 2016. Result: Thirty-five patients had medical histories of Caldwell-Luc procedures and four patients had maxillary sinus surgery, such as cyst enucleation and open reduction for maxillary bony fractures. From the computed tomography images, 25.6% (11/43) showed multilocular cysts and 74.4% (32/43) showed unilocular lesions. As for the treatment methods, cyst enucleation was conducted on 34 patients, and among them, three were treated previously with marsupialization. Their symptoms included diverse locations of pain and swelling. Conclusion: The clinical features of POMC varied from unilocular to multilocular and the symptoms included pain and swelling.
Patient with complaints of swelling, pain in the maxillaly region and discomfort visited Seoul National University Dental Hospital in August last year. Clinical examination and diagnostic imagings implied he was suffered from fungal hyphal infection but no causative fungus was found by the histopathologic and microbiologic investigation. Therefore he was diagnosed with nonspecific inflammation. But as yet, we do think this case is very similar to some kinds of mucormycosis. So we presented this case for more thorough discussion.Followings are founded in the examination. 1. Patient had suffered from Diabetes mellitus and complained of stuffness, headache, swelling in buccal cheeks and paresthesia And we found more maxillary bony destruction and ulcer with elevated margin in the palate by clinical examination. 2. In the first visit, Plain films revealed general bony destruction of the maxilla, radiopaqueness in the sinonasal cavities. or and MRI showed soft tissue mass filled in the paranasal sinus except frontal sinus and bony destruction in involved bones. 3. No causative bacteria and fungus was found in the biopsy and microbiologic cultures. 4. Caldwell-Luc operation and curettage were carried and antJbiotics were taken for 4 months. But now he was worse than in the past 5. In the second visit, involvement of orbit, parapharyngeal sinus, clivus, cavernous sinus and middle cranial fossa were seen clearly in the or and MRI.

Clinic에 내원하는 환자중 nasal symptoms을 호소하는 환자의 높은 비율에서 알레르기 질환이 있는 것을 느끼게 되며 특히 nasal cavity내에 오는 질환의 원인 중 알레르기가 놀라울 정도로 많은 것을 생각하면 이비인후과 영역에서의 알레르기에 대한 관심도 높다고 하겠읍니다. 일반적으로 비용증, 만성비염. 만성부비동염의 치료 방법으로는 약물요법과 수술요법을 병행하여 시행하는데 치료후 증상이 호전되다가 다시 계속적인 치료전의 증상이 재발되는 경우를 흔히 경험하게 된다. 특히 비용증 환자의 치료에서는 높은율의 재발을 경험하게 되는데 nasal symptoms을 일으키는 질환의 많은 원인이 allergy 라는데 착안하여 allegy symptoms은 호소하지 않았으나 비용증을 동반한 만성부비동염 환자에 수술 및 일반치료를 시행하였으나 얼마후 증상이 재현되어 Rinkels technique의 allergy test를 시행 하였다. 특정한 allergen에 양성을 보이지는 않았으나 일반적으로 nasal allergy의 많은 원인이 되는 House dust와 Mold groups의 allergen을 이용 (#2 ∼ #3 Solution)하여 계속적인 Desensitization을 시행함으로서 근치에 가까운 효과를 얻었기에 문헌고찰과 함께 보고하는 바이며 이러한 환자들에게 계속적인 allergy test상 양성반응을 보이지 않더라도 위의 방법을 적용하여 치료한 결과를 추후 발표드릴 것을 약속드립니다.
The neuorogenic tumor is known to be originated from neural crest, and the involved cells are Schwann cell, ganglion cell, and paraganglion cell. The Schwannoma, neurofibroma, and malignant schwannoma arise from the schwann cell, ganglioneuroma is from ganglion cell, and carotid body tumor and glomus tumor are originated from paraganglion cell. Authors reviewed thirty-eight patients of the neurogenic tumors in the head and neck, excluding intracranial tumor and Von-Recklinghausen disease, surgically treated at the Department of Surgery, Pusan Paik Hospital from January 1981 to May 1996. Of the 38 cases, 28 cases were schwannoma, 6 cases neurofibroma, 2 cases malignant schwannoma, and 2 cases paraganglioma. These tumors occurred at any age, but the majority of patients occurred in the fourth decade of life. There was female preponderance (M : F=1 : 1. 53) in sex ratio. The lateral cervical region was the most common distribution. 12 cases arose from the anterior triangle of neck, and 12 cases from the posterior triangle of neck. The major nerve origin of tumor could be identified in 30 cases (80%). 11 cases were treated by simple excision, and partial excision was 3 cases. Excision with parotidectomy 1 case, enucleation 11 cases, enucleation with parotidectomy 7 cases, radical neck dissection 1 cases, upper neck dissection 2 cases, suprahyoid dissection 1 case, CaldwellLuc operation 1 case. The postoperative complications were hoarseness (2 cases), facial palsy (1 case), Homer syndrome (1 case), and hypoesthesia of tongue (1 case).

Nine cases of maxillary sinus aspergillosis during a period from February of 1992 to June of 2000 were investigated to analyze the clinical, radiologic and pathologic features. Maxillary sinus aspergillosis is rare disease, but it was increasing tendency with overuse antibiotics, steroid hormones, and anticancer agents. Aspergillosis of the maxillary sinus may occur as a chronic disease in an otherwise healthy person. The clinical features of maxillary sinus aspergillosis were similar to the non-fungal, chronic sinusitis. Intrasinus calcification is known to be a characteristic feature of maxillary sinus aspergillosis. It is suggested that excess root filling materials containing zinc oxide in the maxillary sinus could favour the formation of a local, non-invasive maxillary sinus aspergillosis. And this "dental" model of pathogenensis of maxillary sinus aspergillosis is an alternative to the widely accepted concept of spore inhalation and "aero-genic" pathogenensis of maxillary sinus aspergillosis. The radical surgery such as Caldwell-Luc operation was one of the most effective treatment modalities. Our results of this study indicate that maxillary sinus aspergillosis might occur mainly in healthy individuals rather than debilitating patients. It could efficiently treated with radical surgery alone without the antifungal agents. 4 cases were suspected to be related with teeth extraction and endodontic treatment. There were no recurrence in all cases.
