The serum level of 25-hydroxyvitamin D for maximal suppression of parathyroid hormone in children: the relationship between 25-hydroxyvitamin D and parathyroid hormone

  • Purpose: Serum level of 25-hydroxyvitamin D (25-OHD) is considered as the most appropriate marker of vitamin D status. However, only a few studies have investigated the relationship between 25-OHD and parathyroid hormone (PTH) in children. To this end, this study was aimed at evaluating the lowest 25-OHD level that suppresses the production of parathyroid hormone in children. Methods: A retrospective record review was performed for children aged 0.2 to 18 years (n=193; 106 boys and 87 girls) who underwent simultaneous measurements of serum 25-OHD and PTH levels between January 2010 and June 2014. Results: The inflection point of serum 25-OHD level for maximal suppression of PTH was at 18.0 ng/mL (95% confidence interval, 14.3-21.7 ng/mL). The median PTH level of the children with 25-OHD levels of <18.0 ng/mL was higher than that of children with 25-OHD levels ${\geq}$ 18.0 ng/mL (P<0.0001). The median calcium level of children with 25-OHD levels<18.0 ng/mL was lower than that of children with 25-OHD levels${\geq}18.0ng/mL$ (P=0.0001). The frequency of hyperparathyroidism was higher in the children with 25-OHD levels<18.0 ng/mL than in the children with 25-OHD levels${\geq}18.0ng/mL$ (P<0.0001). Hypocalcemia was more prevalent in the children with 25-OHD levels<18.0 ng/mL than in the children with 25-OHD levels${\geq}18.0ng/mL$ (P<0.0001). Conclusion: These data suggest that a vitamin D level of 18.0 ng/mL could be the criterion for 25-OHD deficiency in children at the inflection point of the maximal suppression of PTH.

  • 구리를 운반하는 P형 ATPase 단백을 합성하는 ATP7B 유전자의 돌연변이로 인하여 간, 뇌, 각막, 신장 및 적혈구 등 여러 조직에 구리가 축적되어 발병하는 상염색체 열성 유전 질환인 윌슨병은 간 증상과 신경증상이 주요 증상이지만 이외에도 용혈성 빈혈, 심장, 신장 및 내분비 이상 증상을 초래할 수 있다. 저자들은 윌슨병으로 진단되어 치료 받던 중 부갑상샘의 구리 침착에 의한 것으로 추측되는 부갑상샘저하증, 여러 복합요인에 의한 비타민 D 결핍 그리고 원인이 불명확한 저마그네슘혈증으로 인하여 발생한 테타니를 칼슘, 마그네슘 및 비타민 D 투여로 치료하였던 1예를 경험하였기에 보고하는 바이다.

중 고등학생 대상 영양교육 목표 설정을 위한 영양 관련 문제점 분석 (Setting Instructional Goals for Nutritional Education Program Through an Analysis of Problems Identified in Junior/senior High School Students)

  • For the purpose of developing 'Web-Based Nutritional Education Program', this study analyzed nutritional problems of junior/senior high school students and set appropriate instructional goals for nutritional education program. Survey questionnaires were distributed from October 21,2002 to October 26, 2002 to a total of 564 students at three junior high schools and five senior high schools located in Seoul. The total number of questionnaires collected and used in our study was 479, which is 84.9% of the total questionnaires distributed. According to the survey results, three student groups (male and female students at junior high schools and female students at senior high schools) had lower than an ideal body weight. In case of their Basal Metabolic Indices (BMI), the female students and junior high school students showed a tendency to be underweight, while the other three student groups were within the normal range. The result of the survey showed that all four groups of students showed a deficiency in nutrient intake of calories, calcium and thiamin when compared with Korean RDA. Particularly, in the case of the female students at senior high schools, the deficiency of iron intake was a problem. The survey about food attitudes and food habits of the students showed that the senior high school students had more problems when compared with their junior high school counterparts (p < .001). Therefore, the instructional goals of nutritional education program were set as follows: 1) To improve the level of understanding and the perception of junior and senior high school students about the importance of a dietary lifestyle. 2) To change food attitudes and nutritional knowledge related to individual dietary 1ifestyle, so as to maintain good health through the enhancement of the students' physical activities. We found out that it is critically important to factor in the existing problems of food attitudes and food habits, as well as nutrient intake among junior and senor high school students when setting objectives for nutritional education programs. 임신 중기의 아연 영양상태가 임신결과에 미치는 영향 (Zinc Status of South Korean Women in the Second Trimester and the Effect of Those on Pregnancy Outcome)

  • Maternal zinc deficiency is relatively common worldwide, but its consequences for pregnancy outcome are not established. The purpose of this study was to examine the effect of zinc status in the second trimester on pregnancy outcome. Subjects were 248 pregnant Korean women (25-28 wk gestation). Cord blood was collected from subgroup of 69 women and pregnancy outcome data were obtained from 185 babies. Anthropometry measurement, dietary intake, and biochemical characteristics of pregnancy and cord serum substances were measured. The subjects were divided into quartiles on the basis of maternal serum Zn concentration; $ZnQ_1$ (< 25 percentile), $ZnQ_2$ ($26\~50$ percentile), $ZnQ_3$ ($51\~75$ percentile), and $ZnQ_4$ ($76\~100$ percentile). Zn groups were compared in terms of various maternal factors, concentrations of cord serum substances, and pregnancy outcome. Maternal serum Zn concentration was $118.4\;{\pm}\;35.5\;{\mu}g/dl$ as mean. Intake of Zn was lower than Korean RDA. The rate of Zn deficiency among all subjects was $8.5\%$. Maternal serum Zn levels belonged to normal range. Cord serum Zn level was about $154.7\%$ of maternal serum level. Intakes of energy, calcium, iron, folic acid, and riboflavin did not meet the Korean RDA for pregnant women by gestational age. The mean birth weight of neonates is 3083 $\pm$ 697 g, of whom $9.1\%$ were of low birth weight (< 2,500 g). Maternal serum Zn level was positively correlated with pre-pregnancy weight, pre-pregnancy body mass index, and vitamin C intake (p < 0.05). $ZnQ_1$ group had significantly lower maternal serum iron concentration and higher cord serum cholesterol than those in other groups. Maternal serum Zn level, cord serum Zn level, and dietary Zn intake were no related to the pregnancy outcome. The birth weight had a correlation with the maternal hemoglobin and albumin concentration. In conclusion, at this study, we could not find the association with maternal Zn status in 2nd trimester and pregnancy outcome.

Isoflavones Extracted from Sophorae fructus Upregulate IGF-1 and TGF-$\beta$ and Inhibit Osteoclastogenesis in Rat Born Marrow Cells

  • Isoflavones have been a central subject in research on the natural phytoestrogens found in Leguminosae. Their effects on bone formation and remodeling are important in that they can act like estrogen by binding on estrogen receptors on the target cell surface. We, therefore, believed that isoflavones may help in the treatment of patients with estrogen deficiency disease such as estrogen replacement therapy (ERT) for osteoporosis. As commonly known, osteoporosis is one of the hormonal deficiency diseases, especially in menopausal women. When estrogen is no longer produced in the body a remarkable bone remodeling process occurs, and the associated events are regulated by growth factors in the osteoblast lineage. In the present study, we investigated whether isoflavones (Isocal) extracted from Sophorae fructus affect the growth factors IGF-I and TGF-$\beta$ that have been known to be related with bone formation. In the study, we found that the active control (PIII) effectively enhanced the level of nitric oxide (NO) and growth factors, and thereby inhibited osteoclastogenesis. The most efficient concentration was $10^{-8}$% within five days, whereas the comparative control (soybean isoflavone) was not as effective even at a lower concentration. In conclusion, the products which contain enriched glucosidic isoflavone and nutrient supplements such as shark cartilage and calcium can be used for osteoporosis therapy by enhancing the production of IGF-I and TGF-$\beta$. Furthermore, the NO produced through endothelial constitutive NO synthase (ecNOS) may playa role in inhibiting bone reabsorption.

Vitamin D intake and bone mineral density in Korean adults: analysis of the 2009-2011 Korea National Health and Nutrition Examination Survey

  • BACKGROUND/OBJECTIVES: The prevalence of vitamin D deficiency in Koreans is quite high; however, until recently, Korean National Health and Nutrition Survey (KNHANES) had not analyzed the vitamin D intake among Koreans. Additionally, the Korean Dietary Reference Intake for vitamin D was established based on insufficient evidence. Therefore, we investigated vitamin D intake and its relationship with bone mineral density (BMD) in Korean adults using the combined data from the 2009-2011 KNHANES. MATERIALS AND METHODS: This study was conducted in 11,949 healthy adults. Vitamin D intake was assessed using a 24-h recall method, and the BMD was measured using dual-energy X-ray absorptiometry. RESULTS: The prevalence of vitamin D deficiency (< 20 ng/mL) was 64% in men and 77% in women. In women aged ≥ 50 yrs and men aged < 50 yrs, there was a significant positive correlation between vitamin D intake and serum 25-hydroxyvitamin D level after sun exposure adjustment. The BMD of postmenopausal women aged ≥ 50 yrs with a vitamin D intake of 5 ㎍/day or more was significantly higher than that of women with intake less than 5 ㎍/day. After adjusting for age, energy, and calcium intake, the vitamin D intake of the osteoporotic group was significantly lower than that of the osteopenia group in women. CONCLUSIONS: Since the relationship between vitamin D intake and BMD was observed in women aged ≥ 50 yrs, further research is needed to clarify these findings using cohort or randomized controlled trials.

여천산업단지(麗川産業團地)의 만성(慢性) 대기오염(大氣汚染)에 의한 해송(海松) 엽중(葉中) 무기양료(無機養料)의 변화(變化) (Foliar Nutrients Status of Pinus thunbergii Influenced by Chronic Air Pollution in Yochon Industrial Complex in Korea)

  • 만성적인 대기오염의 영향을 받고 있는 여천산업단지 지역에서 15개 해송임분을 선정하여 당년생 엽과, 1년생 엽중 수용성 유황의 축적과 질소, 인, 칼륨, 칼슘, 마그네슘 등 무기양료의 함량변화를 파악하였다. 수용성 유황 함량은 당년생 엽이 0.11% - 0.35%. 1년생 엽이 0.13% - 0.32%로서 전 지역에서 대조지역보다 높았다. 질소의 강우 당년생 엽은 0.60% - 1.42%로써 7개 지점에서, 1년생 엽은 0.58% - 0.88%로써 전 지역에서 대조구에 비해 그 함량이 낮아 1년생 엽에서의 질소 결핍이 뚜렷하였다. 인과 칼륨의 경우에는 일부 지역에서 대조구에 비하여 함량이 낮은 수준을 보였다. 칼슘은 당년생 엽에서는 0.12% - 0.35%로 8개 지역에서, 1년생 엽에서는 0.20% 0.37%로써 9개 지역에서 대조구보다 낮았다. 마그네슘은 당년생 엽에서 0.08% - 0.15%로 2개 지역에서, 1년생 엽에서는 0.06% - 1.11%로써 12개 지역에서 결핍증세를 보였다. 대조구를 제외한 15개 지점에 대해서 각 분석 항목이 상관관계를 살펴본 결과 당년생 엽의 경우에 칼슘과 마그네슘은 유황이 축척될수록 유의적으로 감소하였다. 1년생 엽의 경우에는 질소, 칼륨, 칼슘, 마그네슘의 함량이 수용성 유황함량과 유의적인 부의 상관을 나타내 여천산업단지에서 배출되는 황화합물의 퇴적이 해송의 양료상태를 교란시켜 해송림의 쇠퇴에 영향을 미치는 것으로 파악되었다.

Tobit 모형을 이용한 중·고등학교 우유급식이 우유소비에 미치는 효과 분석 (The effect of School Milk Program for Junior & Senior High School Students on Milk Consumption)

  • 우리나라 청소년층인 중${\cdot}$고등학생($13{\sim}19$세)의 칼슘 섭취량은 권장량 대비 55.4%로 심각하게 부족한 것으로 조사되었다. 이러한 측면에서 학교우유급식은 청소년층의 칼슘섭취량을 증가시킬 수 있는 유력한 수단으로 활용되고 있으나 일부에서는 학교우유급식이 전체 우유섭취량 증대보다는 가정소비 우유의 대체로 전체 청소년층의 우유소비 증가에는 효과가 없다는 견해가 있다. 이 연구의 목적은 학교우유급식이 중·고등학생들의 우유소비를 증가시키기 위한 효과적인 수단인지 여부를 밝혀내는데 있다. 이를 위해서 우유급식을 실시하는 중${\cdot}$고등학교에 재학하는 학생 1,079명을 대상으로 2006년 6월 15일부터 7월 15일 까지 1개월간 설문조사를 실시하였으며, 통계분석에는 Tobit 모형을 이용하였다. 분석결과 학교우유급식이 학생들의 우유음용에 대한 건전한 습관을 형성하고 우유급식을 하지 않는 학생보다 우유소비량이 더 많으며 그 차이 또한 학교에서 급식으로 섭취하는 양 이상인 것으로 나타났다. 따라서 중${\cdot}$고등학교 우유급식의 확대는 청소년층의 칼슘섭취량을 증가시킬 수 있는 효과적인 수단임을 알 수 있었다.

배나무 엽 황화증상 발생 과원의 토양 및 엽 특성 분석 (Analysis of Soil and Leaf Characteristics of Pear Orchards with Lime-Induced Chlorosis Leaves)

  • 본 연구에서는 황화증상이 발생한 전국 4개 지역의 배 과원에서 토양 특성과 잎의 특성을 분석하여 황화증상의 원인을 구명하고자 하였다. 황화증상이 발생한 잎의 색은 육안 또는 SPAD와 Hunter 값을 통해 진단할 수 있었다. 황화증상이 발생한 과원의 토양은 정상 과원에 비해 토양의 pH가 유의하게 높았다. 유효토심은 황화증상과 큰 관계가 없었으나, 토양에 석회 등이 과다 시비되는 조건과 결합되어 식물체의 철 흡수를 저해할 가능성이 있다. 황화증상이 발생한 잎에서는 정상엽에 비해 철과 칼슘 함량이 유의하게 낮았고, 마그네슘 함량이 유의하게 높았다. 따라서 고온다습한 6-7월경 2차 신초 발달이 일어나는 시기에 집중적으로 황화증상이 발생하는 것은 철과 칼슘의 흡수 저해와 밀접한 관계가 있는 것으로 판단된다. 이를 해결하기 위해서는 석회의 과다 시비를 피하고, 철 성분을 함유한 비료를 엽면시비하거나 수관부 토양에 관주하는 방식이 적합하다.

고등학교 급식 조리종사원의 영양지식과 영양교육 경험과의 관계 연구 (Influence of School Food Service Employees' Nutrition Education on Nutrition Knowledge)

  • This study was designed to investigate the relationships between nutrition education, nutrition awareness and nutrition knowledge of school food service employees. We analyzed 288 self-administered questionnaires. Most of the employees (89.8%) were with contract-managed food services, 45.1% were $41{\sim}50$ years old, and 45.4% had chef certification. Two-thirds of the employees received nutrition education on 'nutritionally balanced diet' and 'sources of calcium', whereas less than 50% received education on 'problems with carbohydrates', 'functions of iron', 'deficiency and sources of iron'. The degree of nutrition awareness was generally higher than the level of nutrition education experience. The mean nutrition knowledge score was 11.7 out of 20 possible points, The majority of employees correctly identified 'sources of calcium (91.8%)', 'functions of carbohydrates (91.4%)', 'sources of cholesterol (91.0%)', and 'problems with lipids (90.4%)'; less than 50% correctly answered 'sources of proteins (18.9%)', 'functions of iron (27.9%)', 'recommended dietary allowances (32.0%)', 'functions of lipids (40.2%)', and 'sources of vitamins (44.3%)'. A chi-square analysis revealed that the employees' actual knowledge did not differ significantly by nutrition education - with the exception of three topics; 'recommended dietary allowances', 'changes in vitamins during cooking', 'functions of water' - or by their nutrition awareness. Nutrition knowledge scores did not differ by sex, decreased with age, increased with academic background, and increased in careers of >10 years or that were contract managed. There were significant correlations between nutrition education scores and nutrition awareness scores (p<0.001); nutrition knowledge scores and nutrition education scores (p<0.001); nutrition knowledge scores and nutrition awareness scores (p<0.001).
