Assessment of Relapsing Urolithiasis from K43 with Erosive Gastritis (미란성 위염 환자 K43에서 재발성 요로 결석에 관한 연구)

  • Nephrolithiasis is the most common disorder of the urinary tract in hospitalized patients, more frequently increased in 30~50 years of age, more common in males than in females, prior right stone to left side, and than upper ureteral stone is found in cultural country, while lower ureteral stone is increased in uncultural country. Stone components are classified as calcium oxalate, calcium phosphate, magnesium ammonium phosphate, uric acid, cystine, and their mixed stone, respectively. According to the pathophysiology of urinary stones, supersaturation/crystalization of inorganic salt concentration in urine, organic matrix, inhibitor deficiency, and epitaxy theory could be based on the stone formation. Not only hypercalciuria, hyperparathyroidism, hyperoxaluria, hyperuricosuria, and cystinuria, but also renal tubular acidosis, hypervitaminosis D, and peptic ulcer, are significantly associated with nephrolithiasis. In this study upper ureteral stone component were analyzed with chemical analysis, infrared spectrum, and image analyzer from K43 patient wit erosive gastritis. As the results, mixed stone of calcium oxalate dihydrate and calcium phosphate apatite was identified, the values of clinical test in blood and urine maintained normal revels. The relapsing urinary stone from K43 have no correlation between factors for stone formation reported early, also have no evidence for risk from erosive gastritis.

A Study on Correlation between Premenstrual Syndrome and Nutrient Intake, Exercise Habit of Women

  • This study was designed to identify how the incidence and severity of premenstrual. syndrome (PMS) correlate with the nutrient intakes and exercise habit of women. The subjects of this study were 299 women residing in Busan metropolitan city. Each subject was asked to complete a menstrual discomfort questionnaire (MDQ) for PMS and nutrient intakes. PMS symptom scores of women in their twenties ranked in order of severity were: behavioral change (2.45), followed by pain (2.36) and water retention (2.28), negative effects (2.20), autonomic reaction (1.91), arousal (1.87), decreased concentration (1.76) and decreased control (1.74). For Women in their thirties, the symptom of pain was the most dominant (2.93) followed by autonomic reaction (2.69) and behavioral change (2.54), and for those in their forties, negative effect (3.06) was highest, followed by pain (2.97) and autonomic reaction (2.86). The overall symptoms of PMS significantly increased with age (20': 2.07 points, 30': 2.34 points, 40': 2.47 points). There was no correlation of the BMI of the subjects with the symptoms of PMS, but there was a significant negative correlation between the symptoms of PMS and exercise frequency for women in their thirties and forties. Subjects in their twenties exhibited a significant negative correlation for PMS symptoms with the intake of carbohydrate (p < 0.05), calcium (p < 0.05) and vitamin E (p < 0.05). For subjects in their thirties, PMS symptoms were negatively correlated with the intake of calcium (p < 0.05) and vitamin C (p < 0.05); and in women in their forties, calcium (p < 0.01) and carbohydrate (p < 0.05) intakes were negatively correlated with PMS symptoms. This suggests that PMS represents the clinical manifestation of a nutrient deficiency state, especially calcium. Therefore, we concluded that nutrient supplementation and exercise management are likely to be of benefit in relieving PMS symptoms.

Cisd2 deficiency impairs neutrophil function by regulating calcium homeostasis via Calnexin and SERCA

  • In the context of aging, the susceptibility to infectious diseases increases, leading to heightened morbidity and mortality. This phenomenon, termed immunosenescence, is characterized by dysregulation in the aging immune system, including abnormal alterations in lymphocyte composition, elevated basal inflammation, and the accumulation of senescent T cells. Such changes contribute to increased autoimmune diseases, enhanced infection severity, and reduced responsiveness to vaccines. Utilizing aging animal models becomes imperative for a comprehensive understanding of immunosenescence, given the complexity of aging as a physiological process in living organisms. Our investigation focuses on Cisd2, a causative gene for Wolfram syndrome, to elucidate on immunosenescence. Cisd2 knockout (KO) mice, serving as a model for premature aging, exhibit a shortened lifespan with early onset of aging-related features, such as decreased bone density, hair loss, depigmentation, and optic nerve degeneration. Intriguingly, we found that the Cisd2 KO mice present a higher number of neutrophils in the blood; however, isolated neutrophils from these mice display functional defects. Through mass spectrometry analysis, we identified an interaction between Cisd2 and Calnexin, a protein known for its role in protein quality control. Beyond this function, Calnexin also regulates calcium homeostasis through interaction with sarcoendoplasmic reticulum calcium transport ATPase (SERCA). Our study proposes that Cisd2 modulates calcium homeostasis via its interaction with Calnexin and SERCA, consequently influencing neutrophil functions.

Characterization of Symptom and Determination of Tissue Critical Concentration for Diagnostic Criteria in 'Maehyang' Strawberry (Fragaria $\times$ ananassa Duch.) as Influenced by Calcium Concentrations in the Fertigation Solution ('매향' 딸기의 칼슘 영양진단을 위한 결핍증상 및 식물체 내 한계농도)

  • Objective of this research was to investigate the effect of calcium concentrations in the fertilizer solution on growth and development of Ca deficiency in 'Maehyang' strawberry (Fragaria $\times$ ananassa Duch.). The margins of the youngest leaves were scorched and they developed a cupped shape as they were expanded in Ca deficient plants. The vein area of the youngest leaves became brown when the deficiency became severe. The quadratic responses were observed in dry weight production to elevated Ca concentrations in fertilizer solutions with the highest growth in 4.5 mM treatment. The regression equation was y=2.4026+1.0209x-$0.098x^2$ $(R^2=0.3546^{***})$. However, tissue Ca contents increased lineally as the Ca concentrations in fertilizer solutions were elevated (y=1.2108+0.1333x, $R^2=0.9189^{***}$). In changes of the fresh weight and Ca concentrations in petiole sap, fresh weight production showed quadratic responses to elevated Ca concentrations in fertilizer solution, but Ca concentration increased lineally. The equations in changes of fresh weight and Ca concentrations were y=9.273+4.882x-$0.4245x^2$ $(R^2=0.4935^{***})$ and y=52.311+3.2917x $(R^2=0.6918^{***})$, respectively. When the concentration of calcium at which plant growth was retarded by 10% is regarded as critical concentration level, the calcium contents based on dry weight of above ground plant tissue and in petiole sap should be in the range between 1.6 to 2.25% and 63 to $79mg{\cdot}kg^{-1}$, respectively.

  • Vitamin D is present in two forms, ergocalciferol (vitamin $D_2$) produced by plants and cholecalciferol (vitamin $D_3$) produced by animal tissues or by the action of ultraviolet light on 7-dehydrocholesterol in human skin. Both forms of vitamin D are biologically inactive pro-hormones that must undergo sequential hydroxylations in the liver and the kidney before they can bind to and activate the vitamin D receptor. The hormonally active form of vitamin D, 1,25-dihydroxyvitamin D3 $[1,25(OH)_2D]$, plays an essential role in calcium and phosphate metabolism, bone growth, and cellular differentiation. Renal synthesis of $1,25(OH)_2D$ from its endogenous precursor, 25-hydroxyvitamin D (25OHD), is the rate-limiting and is catalyzed by the $1{\alpha}$-hydroxylase. Vitamin D dependent rickets type I (VDDR-I), also referred to as vitamin D $1{\alpha}$-hydroxylase deficiency or pseudovitamin D deficiency rickets, is an autosomal recessive disorder characterized clinically by hypotonia, muscle weakness, growth failure, hypocalcemic seizures in early infancy, and radiographic findings of rickets. Characteristic laboratory features are hypocalcemia, increased serum concentrations of parathyroid hormone (PTH), and low or undetectable serum concentrations of $1,25(OH)_2D$ despite normal or increased concentrations of 25OHD. Recent advances have showed in the cloning of the human $1{\alpha}$-hydroxylase and revealed mutations in its gene that cause VDDR-I. This review presents the biology of vitamin D, and $1{\alpha}$-hydroxylase mutations with clinical findings.

Food Insecurity and Related Risk Factors in the Elderly: Korea National Health and Nutrition Examination Survey 2013 Data (우리나라 노인의 식품불안정성 및 관련 요인: 2013년 국민건강영양조사자료)

  • The purpose of this study was to assess risk factors associated with food insecurity in the Korean elderly aged over 65 years using data from the Korea National Health & Nutrition Examination Survey 2013 (KNHANES VI). A total of 1,200 subjects were analyzed among the participants of the KNHANES 2013 by using SPSS statistics complex samples (ver. 21.0). Food insecurity was measured by using the modified US Household Food Security/Hunger Survey Module. Thirteen percent of subjects lived in food insecure households. There were differences in the prevalence of food insecurity according to sex, educational level, income level, and household structure. Mean age of the food insecurity group was significantly higher than that of the food security group. The results of logistic regression analysis showed that food insecurity was significantly associated with alcohol intake (OR: 1.82), prevalence of melancholy (OR: 2.07) and suicidal thoughts (OR: 2.67), and intake deficiency of energy (OR: 1.60), calcium (OR: 1.97), iron (OR: 1.97), potassium (OR: 1.96), riboflavin (OR: 1.76), and niacin (OR: 1.64), while not with smoking, physical activity, chronic diseases including obesity, anemia, diabetes, and osteoarthritis, and deficiency of protein, vitamin A, thiamin and vitamin C. These findings suggest that food insecurity is strongly related to mental health and certain nutrient intakes. Prospective research is needed to establish the effects of food insecurity on chronic diseases.

Prevalence of vitamin D deficiency in Korea: Results from KNHANES 2010 to 2011 (한국인의 비타민 D 부족 유병률에 관한 연구: 국민건강영양조사 2010~2011 분석결과)

  • Vitamin D deficiency (VDD) is becoming an epidemic and thereby a global health problem. Further, VDD adversely affects calcium metabolism and skeletal health, and is associated with increased risk of several diseases, e.g., autoimmune diseases, several types of cancers, type 2 diabetes mellitus, cardiovascular diseases, infectious diseases, asthma, psoriatic arthritis, and etc. To evaluate the prevalence of VDD in Korea, and then to evaluate the association of several factors with serum 25(OH)D level, the author analyzed the data of 14,456 individuals who were 10 years of age and over from the Fifth Korea National Health and Nutrition Examination Survey 1 & 2 (KNHANES V-1 & 2) conducted by the Korean Centers for Disease Control & Prevention. As a result, among Koreans (age $${\geq_-}$$ 10years), 65.9% of males and 77.7% of females were below optimum blood serum 25(OH)D (20 ng/mL). VDD is more severe in female than in male at all age groups. In addition, the younger generations had less 25(OH)D level than older generations in Korea. The analysis by complex sample general linear model (CSGLM) suggested that blood 25(OH)D concentration was related with gender (p < .001), residence (p = .030), occupation (p < .001), anemia (p < .001) and physical activity (p < .001). In conclusion, VDD is pandemic and it is more severe in younger generations in Korea. Further, from the results by CSGLM, serum 25(OH)D status is closely related with the life style of Koreans.

The Nutrition Survey, Nutrition Surveillance and Nutrition Status in China

  • With the prosperous economy, the status of diet and nutrition among the urban and rural Chinese population has been improved significantly. This paper will focuses on three main surveys in China. (1)'Food and Nutrition Surveillance System in China': Phase I focused on analysis of previous data. Phase II was a pilot survey initiated in Beijing, Hebei, Heilongjiang, Ningxia, Zhejiang, Guangzhou, and Sichuan in 1990. (2) 'The China Health and Nutrition Survey' is an ongoing longitudinal project covered 8(9) provinces in 1991,1993,1997,2000. This paper will analysis the adults aged 18 45 of 1991, 1993, 1997, and 2000. (3) Survey on the status of nutrition and health of the Chinese (2002) is an across sectional study covered 31 provinces, autonomous region and the municipalities. The data was used to analyze the status of food consumption and dietary nutrients intake of Chinese population. In the past ten years, the prevalence of malnutrition and nutrition deficiency has been continuously decreased. The consumptions of animal products, milk, and edible oil have been increased, while the dairy and legume products consumption decreased. The unbalance was showed with a downward trend in cereals, vegetables and fruits consumption and a significant increase in animal products and oil intake among urban people. Dietary pattern of some subjects went away to 'high energy density' diet. The deficiency of calcium, retinal and ascorbic acid etc, especially in the rural areas still exist. China is undergoing a remarkable, but undesirable, rapid transition towards a stage of the nutrition transition characterized by high rates of DR NCDs in a very short time. China is facing the dual challenges of nutrition deficiency and nutrition imbalance. The results can help to understand the dietary structure and the status of nutrition among Chinese people over the past years. It will provide the basis for formulating relevant state policies.

Comparison of Nutrient Intake, Dietary Behavior, Perception of Body Image and Iron Nutritional Status among Female High School Students of Urban and Rural Areas in Kyunggi-do (경기도 일부 도시 및 농촌지역 여고생의 영양섭취, 식행동, 체형인식 및 철분영양상태 비교연구)

  • This study was undertaken to investigate the nutrient intake, dietary behavior, perception of body image and iron deficiency of high school adolescent girls. The subjects were 463 high school girls in Inchon. This cross-sectional survey was conducted by questionnaires and data were analysed by SAS program. Nutrient intakes collected from 3 day-retails were analyzed by the Computer Aided Nutritional Analysis Program. The following anthropometric measurements were made on all participants height, weight, body fat (%), skinfold thirkness, subscapular skinfold thickness, mid-upper arm circumference and waist-hip circumference. Blood samples were obtained and analyzed for iron nutritional status. The mean obesity index of rural students were highest among students. However, over 18% of the urban and rural students belonged to the low-weight group. Proportions of skipping meals were most high in urban vocational students and the main reason was lack of time for meal. Most nutrient intakes of the students were below the RDA, in particular calcium, iron, vitamin A and vitamin B$_2$. Most of the students were concerned with body image and weight control. Also they were not satisfied with their body image. The mean fell hemoglobin and transferrin saturation of rural students were lowest among students. The total binding capacity of urban vocational student\ulcorner were Invest among students. Prevalence of iron deficiency was most high in urban general students when judged by Hb, MCV and serum ferritin. In conclusion, this study indicates that nutrient intake in the female high school students was considerably lower than RDA. They should also be notified that severe weight control is very harmful for health. And they had lower self -satisfaction for their body image and undesirable flood habits. Therefore, prefer nutrition education is required to maintain desirable flood habits and improve their iron status in female high school students.

Role of Vitamin D Deficiency and Lack of Sun Exposure in the Incidence of Premenopausal Breast Cancer: a Case Control Study in Sabzevar, Iran

  • Background: Vitamin D has been suggested as one of the critical factors for female reproductive health with protective activities against different cancers but there are conflicting facts regarding its role on breast cancer without any clear data on premenopausal cases. This study aimed to evaluate the role of vitamin D from dietary sources and sunlight exposure on the incidence of premenopausal breast cancer. Materials and Methods: We conducted a case control study on 60 newly diagnosed premenopausal breast cancer patients and 116 normal women who lived in Sabzevar and surrounding villages in Razavi, Khorasan, a rural and conservative area of Iran. Results: The mean concentrations of 25-OH vitamin D in cases and controls were $15.2{\pm}8.15$ vs $15.5{\pm}7/45ng/ml$, both well below normal values elsewhere. In fact 50% of analyzed individuals showed very severe or severe vitamin D deficiency and the rest (25%) were detected in suboptimal levels. Although the lack of vitamin D and calcium supplementation increased slightly the risk of premenopausal breast cancer (p=0.009, OR=1.115, CI 95%=1.049-1.187), higher prevalence of weekly egg consumption (86.66% vs 96.55%, p=0.023, OR=0.232, CI 95% 0.065-0.806) showed a slight protective role. The last but the most important risk factor was lack of sunlight exposure because the breast cancer patients had total body coverage from sun (p=0.007, OR=10.131, CI 98% 0.314-78.102). Conclusion: This study pointed out the role of vitamin D and other possible risk factors on the development and growth of breast tumors in this special geographical region. Although this study has revealed the interactions between hormonal and environmental factors in this province of Iran, understanding the deficiency pattern and its contribution to other lifestyle factors elsewhere is also necessary.
