Nine cases presented with a calcifying odontogenic cyst to the Dental Hospital of the College of Dentistry, Yonsei University. from January 1987 to September June, 1996, Clinical or histopathological findings were observed according to each radiologic criteria. The results obtained are as follows : 1. The male to female ratio was 2 : 1 with a mean age of 25 years. 2. The radiographic appearences of nine cases were well-defined radiolucent lesions in which eight cases were shown unilocular lesions and only one lesion was seen multilocular lesion. 3. Radiologically. three of nine cases were pure radiolucent lesions and others, six cases were radiolucent contained a variable amount of radiopaque material. 4. Histologically, two cases were classified simple cyst, five were cyst associated odonoma, two were neoplastic type. 5. Histological findings according to the radiological classification, two of three radiolucenct lesions were simple cysts; another was a cyst associated with odontoma. While, four of six radiologic mixed lesions turned out to be a cyst associated with odontoma, two was the neoplastic type.
The calcifying odontogenic cyst (COC) is a rare disorder of the jaws and shows various radiographic features. The purpose of this study is to describe the different radiographic appearances of 2 cases of COC. Case 1 was located in the posterior maxilla extending into maxillary sinus, showing unilocular radiolucency with a well-defined margin. Cortical bone expansion and thinning were prominent. Root resorption of adjacent teeth was apparent. Case 2 showed unilocular radiolucency with a calcified material. Calcification was supposed to be dystrophic dental hard structures, detected at the periphery of the lesion. Ghost cell and proliferation of ameloblastoma-like tissues were common features for these two lesions on histopathological findings. This reports presented common and atypical radiographic features of the COC.
A case of calcifying odontogenic cyst associated with an unerupted tooth which appeared in the left maxillary sinus of a 16-year-old woman, was reported. Clinical examination revealed painless swelling on the left maxillary posterior region and the cheek. Radiographically, this lesion showed a monolocular radiolucent shadow with calcified materials and unerupted maxillary second premolar. Histological examination revealed cystic structure with presence of the ghost cells, calcified tissue and unerupted tooth. This lesion was classified as Type IB of COC according to classification of the Praetorius. After surgical removal of the cyst, the wound healed uneventfully and the lesion is now free of symptoms.

The calcifying odontogenic cyst (COC) predominantly affected Maxillary anterior segment and it is developmental cyst. But COC showed diverse terminology or classification, clinicopathologic features as well as its biologic behavior COC usually presents as slowly enlarging but otherwise symptomless swelling. Association with impacted teeth and odontoma is described in $24{\sim}30%$. The epithelial lining of COC(ghost cell) appears to have ability to induce the formation of dental tissues in the asjcents connective tissue wall. This case is a COC associated with a odontoma involving an impacted left maxillary canine in 14-year-old female child. Radiographic examination revealed a well-demarcated radiolucent lesion partially occupied by a radiopaque mass, involving the left canine. The histologic sections showed cystic cavity lined with ameloblastic epithelium containing ghost cell masses with regular and irregular shape odontoma. The final pathologic diagnosis was calcifying odontogenic cyst with odontoma(Type IB by Preatorius). Enucleation and elimination of the included tooth were performed. Now endodontic treatment was preformed on the 1st premolar of the upper left jaw, which had a lesion. And the patient and their parents want to have the orthodontic treatment performed and would like to keep the space maintainer.

The neoplastic variant of calcifying odontogenic cyst has various designation, and its malignant counterpart has been reported as aggressive epithelial ghost cell tumor or odontogenic ghost cell carcinoma. Odontogenic ghost cell carcinoma(OGCC) is a rare carcinoma first documented in 1985. It is composed of varying sized islands of anucleated cells with homogenous, pale eosinophilic cytoplasm, so called ghost cells, were admixed with nucleated cells. We report a case of maxillary OGCC developed from odontogenic epithelial tumor in a 25-year-old man with literature review.
Common radiographic appearances of ossifying fibroma (OF) are well demarcated margin, radiolucent or mixed lesion. Lesions for the radiographic differential diagnosis with OF include fibrous dysplasia, focal cemento-osseous dysplasia. Other confusing lesions might be the mixed lesions such as calcifying odontogenic cyst, adenomatoid odontogenic tumor, calcifying epithelial odontogenic tumor, and benign cementoblastoma. We reported three cases of OF in posterior mandible. These cases showed a little distinguished radiographic features of OF and diagnosed from a combination of clinical, radiographic, and histopathologic information. We need to further refine radiographic and histopathological features of OF and other confusing lesions with literatures review because some cases of these lesions are not easily differentiated radiographically and histopathologically.

Calcifying odontogenic cyst(COC) is a rare developmental odontogenic cyst, which shows diverse classification and terminology. Cystic epithelial lining of COC is composed of basal cell layer of columnar cells and overlying layer of stellate reticulum. In the epithelium, ghost cells that might induce adjacent mesenchymal tissue to develop dental organ are shown characteristically. In spite of low rate of recurrence, we have to get a histopathological examination so that odontogenic lesions may recur without fully curettage of lining epithelium. 7-year-old male child came pediatric dentistry in wonkwang university dental hospital in order to check the delayed eruption of left maxillary central incisor. Radiographic examination revealed a well-defined radiopaque mass, overlapping impacted left central and lateral incisor crown. Enucleated mass was tooth-like features and also had epithelium lining. Results of histopathologic procedure, we saw the lots of ghost cell and proliferating hard dental tissues. Also we saw the cystic epithelium cells. It revealed diagnosis of the COC associated complex odontoma. For this reason one should consider of COC when patients present odontoma-like lesion with impacted tooth.

A 31 years old female had been suffered from a bony swelling in right premolar region of the mandible for 12 years, recently grown rapidly. A fistula tract developed on the right anterior mandibular border, but the lesion was relatively asymptomatic. In the radiological examination, the tumor mass was irregularly mixed with radiolucent and radiopaque areas, forming multiple cystic spaces. Under the diagnosis of calcifying odontogenic cyst, the mandibular mass was resected and examined pathologically. After decalcification, the dissected tumor mass showed multiple small cystic spaces and calcifying fibrous tissue, mimicking calcifying odontogenic cyst or ameloblastoma. Histological observation showed many calcifying cementoid materials and ossifying trabeculae. The cystic spaces were turned out to be dilated vascular channels lined by endothelial cells, containing plasma fluid. However, the main lesion was diagnosed as cemento-ossifying fibroma (COF), and the atypical vascular channels were greatly dilated and gradually expanded the whole tumor mass. The present COF was examined through immunohistochemical (IHC) array, and investigated for tumor cell characteristics, exhibiting abnormal ossification and aneurysmal cystic changes. IHC array disclosed that the tumor cells grew progressively in the lack of apoptosis, and that they showed lower expression of RUNX2 than BMP-2, RANKL, and OPG, and increases of protein expression in $HIF-1{\alpha}$, VEGF-A, and CMG2. These data suggested that the reduced expression of RUNX2, osteoblast differentiation factor, be relevant to abnormal ossification of COF, and that the consistent expressions of angiogenesis factors be relevant to de novo angiogenesis in COF, subsequently resulted in aneurysmal cystic changes.

The osteonectin is a sort of glycoprotein which is secreted in human tissues. The osteonectin is generally detected in number of normal or neoplastic human tissues in vivo, but hasn't been studied the role of osteonectin in developing human teeth and odontogenic tumors. We evaluated degree of the expression of osteonectin immunohistochemically in 20 cases of developing tooth germ which growth from fetus 5 to 38 weeks, and total 51 odontogenic tumors whitch has taken from routine biopsy, such as 10 ameloblastomas, 5 cases of adenomatoid odontogenic tumors and odontomas and odontogenic fibromas, 4 cases of cementomas and calcifying epithelial odontogenic cyst and odontogenic keratocyst and dentigerous cysts and periapical cysts, and 3 cases of ameloblastic fibromas and myxomas. The results were as follows: 1. The osteonectin on the bud stage of tooth germ was strongly expressed in the epithelial dental lamina and in the outer dental epithelium on the early bell stage, and also strongly expressed in the inner dental epithelium on the late bell stage of tooth germs. 2. In ameloblastoma, the osteonectin was strongly expressed in the epithelial tumor component and especially in the acanthomatous types. 3. In both of calcifying epithelial odontogenic tumor and adenomatoid odontogenic tumors, the osteonectin was moderately expressed on the duct like spindle cells and epithelial tumor cells around calcification areas. 4. In odontogenic tumors originated from epithelial-mesenchymal tissues, the osteonectin was moderately expressed on the epithelial tumor components and in odontogenic cysts, it was expressed in ghost cells and calcification areas only. These were summaried the osteonectin may be strongly related to the developing tooth germ and odontogenic tumors and could be regulated hard tissue of human tooth in morphogenesis and involved with calcification mechanism in development odontogenic tumors.

The calcifying odontogenic cyst(COC) showed diverse terminology or classification, clinicopathologic features as well as in its biologic behavior, although it was recognized as a distinct clinicopathologic entity. The epithelial lining of a COC appears to have the ability to induce the formation of dental tissues in the adjacent connective tissue wall, and that other odontogenic tumors may sometimes be associated with it. This case is a COC associated with a complex odontoma involving an impacted left maxillary lateral incisor in a 5-year-old female child. Radiographic examination revealed a well-demarcated radiolucent lesion partially occupied by a radiopaque mass, involving the left lateral incisor crown. The histologic sections showed a cystic cavity lined with ameloblastic epithelium containing ghost cell masses with admixed with complex odontoma components. The presence of mixed radiolucent-radiopaque lesion in children as observed in this case, the possibility of COC must be considered. In this case, there was no recurrence 1 year after enucleation and the space control is ongoing now.
