토끼에서 나린진이 니모디핀의 생체이용률에 미치는 영향 (Effects of Naringin on the Bioavailability of Nimodipine in Rabbits)

  • The purpose of this study was to investigate the effect of naringin, one of flavonoids, on the pharmacokinetics and bioavailability of nimodipine in rabbits. Pharmacokinetic parameters of nimodipine were determined in rabbits after oral administration of nimodipine (16 mg/kg) with or without naringin (1, 5 or 15 mg/kg). Nimodipine was analyzed by high performance liquid chromatography using Hypersil ODS column. Naringin significantly (p<0.05) increased the area under the plasma concentration-time curve (AUC) and the peak concentration ($C_{max}$) of nimodipine at 5 and 15 mg/kg. The absolute bioavailability (AB%) of nimodipine by prescence of naringin (5 or 15 mg/kg) increased from 32.2-36.9% (p<0.05) compared to the control (22.0%). However, presence of naringin had no significant effect on the elimination rate constant ($K_{el}$) of nimodipine. There were no apparent changes of the time of peak concentration ($T_{max}$) of nimodipine by coadministration. These results suggest that the increased bioavailability and the significant changes of these pharmacokinetic parameters of nimodipine by naringin may be attributed to the potential of narigin to inhibit cytochrome P450 (CYP) 3A4 and P-glycoprotein efflux pump in the liver and intestinal mucosa.

Effect of Naringin Pretreatment on Bioavailability of Verapamil in Rabbits

  • The aim of present study is to investigate the effect of naringin on the pharmacokinetics of verapamil and its major metabolite, norverapamil in rabbits. The pharmacokinetic parameters of verapamil and norverapamil were determined after administering verapamil (9 mg/kg) orally to rabbits in the pretreated with naringin (1.5, 7.5, and 15 mg/kg). Naringin pretreatment significantly altered the pharmacokinetic parameters of verapamil. Compared with the control group (given verapamil alone), the $K_a,\;C_{max}$ and AUC of verapamil were significantly (p<0.05 or p<0.01) increased in the pretreatment of naringin, However there were no significant change in $T_{max}\;and\;t_{1/2}$ of verapamil. Consequently, pretreatment of naringin significantly (p<0.05, p<0.01) increased the AB% of verapamil significantly in a dose dependent manner (p<0.05 or p<0.01 ), and elevated the RB% of verapamil by 1.26- to 1.69-fold. the MR of verapamil were significantly (p<0.05) increased in the pretreatment of naringin, implying that pretreatment of naringin may effectively inhibit the CYP3A4-mediated metabolism of verapamil. In conclusion, pretreatment of naringin enhanced the oral bioavailability of verapamil. Based on these results, the verapamil dosage should be adjusted when given with naringin or a naringin-containing dietary supplement.

흰쥐에서 나린진이 로살탄의 생체이용율에 미치는 영향 (Effect of Naringin on the Bioavailability of Losartan in Rats)

  • The present study was to investigate the effect of naringin, a flavonoid, on the pharmacokinetics of losartan in rats. Pharmacokinetic parameters of losartan in rats were determined after an oral administration of losartan (9 mg/kg) in the presence or absence of naringin (0.5, 2.5 and 10 mg/kg). The pharmacokinetic parameters of losartan were significantly altered by the presence of naringin compared with the control group (given losartan alone). Presence of naringin significantly (p<0.05, 2.5 mg/kg; p<0.01, 10 mg/kg) increased the area under the plasma concentration?time curve (AUC) of losartan by 43.7~63.0% and peak plasma concentration ($C_{max}$) of losartan by 31.7~45.5%. Consequently, the absolute bioavailability (AB) of losartan in the presence of naringin was 43.8~62.9%, which was enhanced significantly (p<0.05, p<0.01) compared to that in the oral control group (22.4%). The relative bioavailability (R.B.) of losartan increased by 1.44- to 1.63-fold in the presence of naringin. However, there was no significant change in the peak plasma concentration ($T_{max}$) and terminal half-life ($t_{1/2}$) of losartan in the presence of naringin. In conclusion, the presence of naringin significantly enhanced the oral bioavailability of losartan, implying that presence of naringin might be mainly effective to inhibit the cytochrome P450 (CYP)3A-mediated metabolism, resulting in reducing gastrointestinal and hepatic first-pass metabilism and Pglycoprotein (P-gp)-mediated efflux of losartan in small intestine. Concurrent use of naringin or naringin-containing dietary supplement with losartan should require close monitoring for potential drug interactions.

  • Phenoxy compounds, 2,4-Dichlorophenol acetoxy acid (2,4-D) and 2,4-dichlorophenol (DCP), are widely used as a hormonal herbicide and intermediate for pesticide manufacturing, respectively. In order to assess the potential of these compounds as endocrine disruptors, we studied the androgenicity of them wing in vivo and in vitro androgenicity assay system. Administration of 2,4-D (50 mg/kg/day, p.o.) or DCP (100 mg/kg/day, p.o.) to rats caused an increase in the tissue weight of ventral prostate, Cowpers gland and glands penis. These increase of androgen-dependent tissues were additively potentiated when rats were simultaneously treated with low dose of testosterone (1 g/kg, s.c.). 2,4-D increased about 350% of the luciferase activity in the PC cells transiently cotransfected phAR and pMMTV-Luc at concentration of $10^{-9}$ M. In 2,4-D or DCP-treated castrated rats, testosterone 6$\beta$-hydroxylase activity was not significantly modulated even when rats were co-treated with testosterone. In vitro incubation of 2,4-D and DCP with microsomes at 50 $\mu$M inhibited testosterone 6$\beta$-hydroxylase activity about 27% and 66% in rat liver microsomes, about 44% and 54% in human liver microsomes and about 50% and 45% in recombinant CYP3A4 system, respectively. The amounts of total testosterone metabolites were reduced about 33% and 75% in rat liver microsomes, 69% and 73% in human liver microsomes and 54% and 64% in recombinant CYP3A4 by 2,4-D or DCP, respectively. Therefore, the additive androgenic effect of 2,4-D or DCP by the co-administration of the low dose of testosterone may be due to the increased plasma level of testosterone by inhibiting the cytochrome P450-mediated metabolism of testosterone. These results collectively suggested that 2,4-D and DCP may act as androgenic endocrine disrupter by binding to the androgen receptor as well as by inhibiting the metabolism of testosterone.

17β-estradiol이 기수산 물벼룩의 Ecdysteroid 경로에 미치는 영향 (Effect of 17β-estradiol on Ecdysteroid Pathway Related Genes in the Brackish Water Flea Diaphanosoma celebensis)

  • 17β-estradiol (E2)는 난소로부터 방출되는 호르몬으로 가정 및 축산 오폐수에 포함되어 환경으로 지속적으로 유출된다. E2는 높은 에스트로겐 활성을 가지고 있어 갑각류의 발달과 생식에 영향을 미치는 내분비계교란물질로 알려져 있다. 갑각류의 발달은 탈피호르몬(ecdysteroid)의 신호 전달 과정에 의해 이루어지지만 E2가 소형 갑각류의 탈피호르몬 경로 유전자를 어떻게 조절하는지에 대한 연구는 매우 미흡하다. 본 연구에서는 기수산 물벼룩 Diaphanosoma celebensis에서 E2에 대한 급성 독성 시험을 통해 24-h LCx 값을 도출하였고, E2 노출에 따른 탈피호르몬 경로에 관여하는 7개의 유전자(CYP314a1, EcRA, EcRB, USP, ERR, Vtg, VtgR)의 시간별 발현 변화를 quantitative real time polymerase chain reaction (qRT-PCR)을 이용하여 분석하였다. D. celebensis의 24-h LC50 값은 9.581 mg/l (95% C.I.: 7.697~11.927 mg/l), 24 h-LC10 값은 4.842 mg/l(95% C.I.: 3.683~6.366 mg/l)로 나타났다. CYP314a1, EcRA, USP, VtgR 유전자의 발현이 12시간 또는 24시간에 유의하게 증가하는 양상을 보였다. 이러한 결과는 E2가 D. celebensis의 탈피호르몬 경로에 관련하는 유전자의 발현을 조절함으로써 탈피와 생식에 영향을 미칠 수 있을 것임을 시사한다. 본 연구는 소형 갑각류에서 내분비계교란물질이 탈피 경로에 미치는 영향에 대한 분자 기전을 이해하는데 도움이 될 것이다.

토끼에서 나린진이 틸티아젬과 그대사체, 디아세틸딜티아젬의 생체이용율에 미치는 영향 (Effect of Pretreatment of Naringin on the Bioavailability of Diltiazem and Deacetyldiltiazem in Rabbits)

  • The purpose of this study was to investigate the effect of naringin pretreatment on the bioavailability and phar-macokinetics of diltiazem and one of its metabolites, deacetyldiltiazem, in rabbits. Pharmacokinetic parameters of diltiazem and deacetyldiltiazem were determined after oral administration of diltiazem (15 mg/kg) pretreated with naringin (1.5, 7.5 and 15 mg/kg). Absorption rate constant ($k_a$) of diltiazem after oral administration of diltiazem pretreated with naringin was significantly (p<0.05 or p<0.0l) increased compared to the control group. Area under the plasma concentration-time curve (AUC) and peak concentration ($C_{max}$) of the diltiazem were significantly (p<0.05 or p<0.01) higher than those of the control. Absolute bioavailability ($AB\%$) of diltiazem pretreated with naringin ranged from $13.5\%$ to $18.6\%$, being enhanced compared to that of the control, $7.2\%$. Relative bioavailability ($RB\%$) of diltiazem was $1.9\~2.6$ times higher than that of the control group. There was no significant change in terminal half-life ($t_{1/2}$) and $T_{max}$ of diltiazem in the presence of naringin. AUC of deacetyldiltiazem pretreated with naringin was significantly (p<0.05) higher than (p<0.05) that of the control. But the metabolite ratios (MR) were significantly decreased (p<0.05), implying that pretreatment of naringin could be effective to inhibit the CYP 3A4-mediated metabolism of diltiazem. In this study, pretreatment of naringin significantly enhanced the oral bioavailability of diltiazem. These results suggested that the diltiazem dosage should be adjusted when it is administered with naringin or a naringin-containing dietary supplement in the clinical setting.

와파린-리팜핀 병용 시 용량 조절 (Dosage Adjustment before and after Warfarin - Rifampin Combination Therapy)

  • 배경: 와파린은 항응고제로 쓰이는 약물로서 주로 간 대사에 의해 배설되는 약물이다. 리팜핀은 결핵 혹은 심내막염 등에 쓰이는 항생제로 2C9과 3A4를 포함한 CYP계열 효소 유도를 일으키는 대표적인 약물이다. 따라서 두 약물을 병용할 경우 리팜핀의 효소 유도에 의한 와파린 대사율 증가로 와파린의 항응고 효과는 감소한다. 이에 따라 와파린의 적절한 용량 조절이 요구되나 정확한 증량과 감량 정도는 제시되지 못하고 있는 실정이다. 이에 본 연구에서는 와파린 복용 환자 중 리팜핀을 병용하게 된 환자를 대상으로 두 약물의 병용 전후, 상호작용의 정도를 시간 경과에 따라 평가하고, 상호작용에 영향을 미치는 요인을 분석하고 또한 이를 토대로 두 약물의 병용 전후, 임상에서 활용할 수 있는 와파린 용량 결정 방법을 설정하고자 하였다. 대상 및 방법: OO병원 항응고 치료 상담 팀의 상담기록지를 1998년 1월부터 2006년 9월까지 후향적으로 검토하여 리팜핀을 병용하게 된 환자를 대상으로 하였다(n=15). 결과: 리팜핀 병용 전 전체 환자의 평균 INR은 $2.25{\pm}0.52$이며 병용 초기 100일간의 평균 INR은 $1.98{\pm}0.28$이었다. 이 경우 병용 전과 병용 초기의 평균 INR은 유의한 차이가 없었다(paired t-test, p>0.05). 리팜핀 병용 중단 직전 2회 측정한 INR의 평균은 $2.19{\pm}0.34$이고 병용 중단 이후 INR의 평균은 $2.49{\pm}0.43$으로 병용 중단 전과 후의 INR 평균은 유의한 차이를 보였으나(paired t-test, p<0.05)모두 치료유효역 범위 내에 있었다. 결론: 항응고 치료 상담 팀의 용량 조절이 적절하다고 판단하여 항응고 치료 상담 팀의 조절을 근거로 병용 시작 시와 병용 중단시의 와파린 용량조절 수식을 도출해냈다

  • 이비인후과 영역에 있어서 위산 역류에 의한 질환의 진단은 쉽지 않고 분명하지 않은 점이 많지만, 병변의 영향은 광범위하며, 실제로 역류에 의한 증상을 가지고 내원하는 환자도 증가하는 추세이다. 인후두역류질환의 치료는 크게 생활습관의 변경, 약물복용, 항역류수술로 나눌 수 있으며, 사용약제는 크게 두 부류로 나누는데, 제산제, H2 수용체 차단제, PPI(Proton Pump Inhibitor) 제제와 같은 산억제 약물군과 Prokinetic 약물군이다. Rabeprazole sodium(Pariet(R))은 PPI 제제에 해당하는 약제로 기존의 omeprazole, lansoprazole, pantoprazole과는 달리 대사 과정 중 CYP2C19에 대한 의존도가 낮아, 개체 간 차이가 적고 빠르고 일정하게 산분비 억제 효과를 나타내는 것으로 알려져 있은 약물이다. 2001년 5월부터 2002년 4월까지 32개 병원에서 Pariet(R) 를 복용한 2166명의 환자를 대상으로 분석하였다. 복용기간에 따라 4군(1군;1-14일, 2군;15-28일, 3군;29-56일, 4군:57일 이상)으로 나누었으며, 8가지 증상(Heart burn, Regurgitation, Chronic cough, Hoarseness, Globus sensation, Chronic throat clearing, Sore throat, Dysphagia)에 대한 호전 여부 및 후두내시경상 개선 정도, 의사에 의한 유용도 평가, 부작용 발생 여부에 대해 연구하였다. 증상개선율 50%이상을 치료 반응군으로 했을때 전체 2166명중 1627명(75.1%)에서 증상의 호전을 보았으며, 이는 복용기간이 길수록 증가하였다. 후두 내시경상 개선 정도는 현저한 개선이 32.9%, 중등도 개선이 38.7%로 대부분 환자에서 객관적인 병변의 호전을 보였으며, 유용도 평가에서는 매우 유용이 37.6%, 유용이 50.3%로 치료효과에 대한 만족도도 높은 것을 알수 있었다. PPI 제제의 부작용으로 보고되고 있는 두통, 오심, 설사, 복통. 변비, 어지럼증. 피곤 중. 소수의 환자가 두통을 호소하였으나, 그다지 심각한 정도는 아니었다. 인후두역류증 치료제로서 Pariet(R) 는 비교적 안전하고 효과가 높은 약물임이 임상 연구 결과 밝혀졌기에 보고하는 바이다.

DC23, a Triazolothione Resorcinol Analogue, Is Extensively Metabolized to Glucuronide Conjugates in Human Liver Microsomes

  • DC23, a triazolothione resorcinol analogue, is known to inhibit heat shock protein 90 and pyruvate dehydrogenase kinase which are up-regulated in cancer and diabetes, respectively. This study was performed to elucidate the metabolism of DC23 in human liver microsomes (HLMs). HLMs incubated with DC23 in the presence of uridine 5'-diphosphoglucuronic acid (UDPGA) and/or ${\beta}$-nicotinamide adenine dinucleotide phosphate (NADPH) resulted in the formation of four metabolites, M1-M4. M1 was identified as DC23-N-Oxide, on the basis of LC-MS/MS analysis. DC23 was further metabolized to its glucuronide conjugates (M2, M3, and M4). In vitro metabolic stability studies conducted with DC23 in HLMs revealed significant glucuronide conjugation with a $t_{1/2}$ value of 1.3 min. The inhibitory potency of DC23 on five human cytochrome P450s was also investigated in HLMs. In these experiments, DC23 inhibited CYP2C9-mediated tolbutamide hydroxylase activity with an $IC_{50}$ value of $8.7{\mu}M$, which could have implications for drug interactions.

인후두역류질환 (Laryngopharyngeal Reflux Disease, LPRD)에서 Rabeprazole Sodium($Pariet^{\circledR}$)의 임상효과 (The clinical effects of rabeprazole sodium($Pariet^{\circledR}$) in the treatment of Layngopharyngeal Reflux)

  • Although there is a wide range of diseases caused by gastric acid reflux and the number of cases is on the rise, it is difficult for the laryngologist to make the correct diagnosis. The treatment for laryngopharyngeal reflux can be grouped into 3 categories - changes in lifestyle, medication, and surgery. The medication used to treat laryngopharyngeal reflux are prokinetic agents and acid supressive agents such as antacids, H2 blockers, and PPIs(Proton pump inhibitor). Rabeprazole sodium($Pariet^{\circledR}$) is a newly developed agent belonging to the PPI group, but in contrast with the existing drugs such as omeprazole, lansoprazole, pantoprazole, has a low dependency on CYP2C19 during the metabolic cycle. Thus, it is known to have a quick but fixed antiacid effect and less individual differences. We analyzed 2166 patients from 32 hospitals who were prescribed $Pariet^{\circledR}$ from May, 2001 to April, 2002. The patients were divided into 4 groups according to the duration of treatment - Group 1: 1-14 days, Group 2: 15-28 days, group 3: 29-56 days, Group 4: more than 56 days. The cases were then analyzed for improvement of 8 symptoms(heart bum, regurgitation, chronic cough, hoarseness, globus sensation, chronic throat clearing, sore throat, and dysphagia), improvement on laryngoscope, usefulness to the doctor, and complication development. Of the total of 2116 patients, 1627(75.1%) cases showed at least 50% improvement of symptoms and the amount of improvement increased according to the duration of medical treatment. Most of the patients showed objective improvement on the laryngoscope, with 32.9% showing significant improvement and 38.7% showing moderate improvement. 37.6% of the doctors questioned replied that $Pariet^{\circledR}$ was very useful and 50.3% said it was useful, showing that most were satisfied with the treatment results. The complications known to develop after taking PPI are headache, nausea, diarrhea, abdominal pain, constipation, dizziness, fatigue, and of these, only a small percentage of the patients complained of mild headache. $Pariet^{\circledR}$ has shown to be a relatively safe and effective drug for the treatment of laryngopharyngeal reflux.
