A Study on CV12 Moxibustion of King Sukjong(肅宗) of Joseon Dynasty -Focusing on Seungjeongwon Ilki(承政院日記) Yakbang(藥房) Records - (숙종(肅宗)의 중완혈(中脘穴) 수구사(受灸事) 연구 - 『승정원일기』 약방기록을 중심으로 -)

  • Objectives : To investigate the progress, characteristics, and reasons of CV12 moxibustion that King Sukjong of Joseon Dynasty had experienced 31 years since 1684. Methods : This study mainly extracted the records and related records about CV12 moxibustion of King Sukjong and relevant records from Seungjeongwon Ilgi, and analyzed them. Aside from that, medical books wrote at that time and relevant records were additionally investigated. Results : King Sukjong cauterized CV12 or other CV12 related acupoints with moxa 21 years out of his 31 years from 1684 to 1714. The cautery period was usually between late January and mid February. He had 100 pieces of moxa for cautery. On the last day of his moxibustion, he cauterized ST36 with moxa. The moxibustion process consisted of the following steps: suggestion or requirement of moxibustion, decision of date and time, and moxa cautery. In 1684, the reason why King Sukjong cauterized CV12 with moxa was to treat his joint pain and nauseous symptoms. The effect can be found in Donguibogam, many other Korean medical books, and other general books at that time. In 1685, he began to cauterize CV12 with moxa to prevent diseases. Since then, most moxa cautery on CV12 had been performed in the same context. Conclusions : For 21 years, King Sukjong repeatedly cauterized CV12 with moxa. The moxa therapy was conducted to prevent diseases mostly. And King Sukjong had a lot of interest in the therapy. For the reasons, the therapy had repeatedly been performed so long.

EEG 16 channel variations between the non-stimulation and the moxibustion stimulated subjects for CV4, CV8, and CV12 acupuncture points (중완, 신궐 및 관원 경혈의 뜸 자극과 무 자극 대상군의 16채널 뇌파 변화에 관한 연구)

  • The moxibustion therapy of orient medical is well known. Moxibustion method is used to burning moxibustion. The variation factors of the moxibustion stimulation for body can be applied to the electronic moxibustion method. This study is to analyze and examine the electroencephalogram(EEG) variations for moxibustion stimulation. In experiment, we divided six healthy male subjects into two same sized groups : with and without stimuli. The given stimuli are the moxibustion for CV4(Kwan-Won), CV8(Shin-Guel), and CV12(Jung-wan) acupuncture points. We have analyzed the power spectrum for ${\alpha}$, ${\beta}$, ${\delta}$, ${\Theta}$ waves and the average EEG variation level and each channels variation level of EEG. The result was that the moxibustion stimulation decreased the EEG levels of parietal lobe(ch 2, 3, 6, 7) according to the somatosensory system. And the apparent different points are that mild-heat($42{\sim}44^{\circ}C$) stimulation generating ${\alpha}$-wave is increased and ${\beta}$-wave is decreased, also slight-hot($45{\sim}48^{\circ}C$) stimulation made ${\alpha}$-wave decrease and ${\beta}$-wave increase for occipital lobe(ch 4,8) on the simulation group.

The Effects of Herbal Acupuncture(Fructus Hordei Germiniatus) at Acupoint Jung-wan($CV_{12}$) on the Obese Rats Induced by High Fat Diet (중완(中脘)에 시술(施術)한 맥아약침(麥芽藥鍼)이 고지방성(高脂肪性) 식이(食餌)로 유발된 비만(肥滿) 백서(白鼠)에 미치는 영향(影響))

  • Objectives : This research was performed to investigate the effects of Fructus Hordei Germiniatus-herbal acupuncture(HA; herbal acupuncture) at acupoint Jung-wan($CV_{12}$) on weight gain, food consumption, food efficiency, serum of lipid concentrations and liver function of rats fed high fat diet for 7weeks. Methods : Experimental groups were divided into high fat diet group(Control), high fat diet and saline injection at acupoint $CV_{12}$ treated group(Saline), high fat diet and Fructus Hordei Germiniatus-HA at acupoint $CV_{12}$ treated groups. According to HA concentration, HA groups were classified into 4 groups ; HA-1($0.108mg/g/m{\ell}$), HA-2($0.054mg/g/m{\ell}$), HA-3($0.022mg/g/m{\ell}$), HA-4($0.011mg/g/m{\ell}$). HA was treated at $20{\mu}{\ell}$ per 4days, total 12times in 49days. Results : 1. Body weight per week was significantly decreased in HA-1, HA-2, HA-3 and HA-4 And body weight per day was significantly decreased in HA-3. 2. Food consumption per week was significantly decreased in HA-1, HA-2, HA-3 and HA-4. 3. Food efficiency per week was significantly decreased in HA-2, HA-3 and HA-4 And food efficiency per day was significantly decreased in HA-3. 4. The level of serum total cholesterol was significantly decreased in HA-1 and HA-3 And the level of serum HDL-cholesterol was significantly decreased in HA-1. 5. The level of serum triglyceride was significantly decreased in HA-1, HA-2, HA-3 and HA-4. And the level of serum total bilirubin was significantly decreased in HA-1and HA-2. 6. The level of serum AST was significantly decreased in HA-2 and HA-3 And the level of serum ALT, r-GTP were significantly decreased in HA-2. Conclusions : According to the above results, Fructus Hordei Germiniatus-HA at acupoint $CV_{12}$ can control body weight, food consumption, food efficiency, serum lipid concentrations and liver function. Asthe same time this kind of Fructus Hordei Germiniatus-HA does not seem to be injurious on liver function. To use Fructus Hordei Germiniatus-HA for obesity, further studies about their underlying mechanism may be needed.

Normalization Effect of Both ST36 and CV11, 12, 13 Meridian Points on the Abnormal Gastric Myoelectrical Activity in Two Cases of Functional Dyspeptic Patients (족삼리와 상완, 중완, 하완혈의 침 자극으로 기능성 소화불량증 환자의 위 평활근 전기적 활성 장애를 정상화시켰던 증례 2례)

  • Objectives: The aim of this study was to introduce the normalization effects of stimulation of both ST36 and CV11, 12, 13 meridian points on the abnormal gastric myoelectrical activity in two cases of functional dyspeptic patients with gastric dysmotility. Methods: Gastric myoelectrical activity was recorded by electrogastrography every two weeks until their gastric myoelectrical activity finally reached the normality. Dominant frequency of gastric slow waves in the fasting and postprandial periods and a dominant power ratio were obtained on each occasion. Patients were treated three times each day: 9 am, 1 pm, and 7 pm. The first treatment consisted of manual and immersion stimulation on all used meridian points for 20 min. In the second and third treatments, electrical stimulation of both ST36 was added. It was conducted for 20 min at a strength intensity of 1.2 times the pain threshold at a frequency of 3 Hz. Results: Stimulation of the above meridian points normalized abnormal gastric myoelectrical activity. The time taken to return from abnormal gastric myoelectrical activity to normal was 16-19 weeks. Conclusion: Stimulation of both ST36 and CV11, 12, 13 meridian points normalized the abnormal gastric myoelectrical activity in the functional dyspepsia of gastric dysmotility.

The Effects of Moxibustion at CV12 on Cerebral Blood Flow and Peripheral Blood Flow (중완(中脘)에 신기구 뜸시술이 뇌혈류 및 말초혈류 변화에 미치는 영향)

  • Objectives : This study was performed to evaluate the effects of moxibustion on cerebral blood flow, peripheral blood flow and blood pressure. Methods : We designed a randomized, cross-over study that included 20 healthy, male volunteers who were randomly assigned to one of two groups (the moxibustion group or the control group). The initial trial was followed by a washout period of one week before the groups were switched and the trial repeated. The cerebral blood flow and peripheral blood flow of the volunteers in both groups were measured by mean velocity (Vm), peak systolic velocity (Vs), pulsatility index (PI), perfusion units (PU) and mean blood pressure (BP) before the intervention (baseline), 30 minutes after the intervention, and again 60 minutes after the intervention. Results : 1. The Vm increased significantly in the moxibustion group. The Vs also constantly increased in moxibustion group, but there was no significant difference. 2. The PI decreased significantly in the control group at 0-30 minutes and in the moxibustion group at 30-60 minutes. 3. The PU constantly decreased in moxibustion group. In control group, the PU decreased at 30 minutes and increased slightly at 60 minutes. 4. The mean blood pressure did not show any significant difference between the two groups. Conclusions : This study shows that moxibustion at CV12 increased the cerebral blood flow. Based on this result, we can suggest that moxibustion at CV12 might be helpful in improving the functional recovery of stroke patients. However, further research needs to be conducted on a larger numbers of subjects in order to confirm these effects.

  • Objectives : To learn the effects of $CV_{12}\;{\cdot}\;CV_9$ and $LI_4\;{\cdot}\;LI_{11}$ electroacupuncture stimulation on high fat diet induced-obese mouse. Methods : The control group was provided with high fat diet without electroacupuncture stimulation, while the experimental group $CV_{12}\;{\cdot}\;CV_9$ and $LI_4\;{\cdot}\;LI_{11}$ with three frequencies(10Hz, 100Hz, 200Hz) for 5 times a week during five week periods with the high fat diet. We investigated the body weight, weight gain, and food intake. Results : After the electroacupuncture stimulation, a significant loss of increasement in weight was found in the experimental group compared with the high fat diet group. But there was no significant difference made by the change of acupoint combinations and frequencies. After the electroacupuncture stimulation, no significant change was found in the food intakes, but the weight gain a week was decreased compared with high fat diet group. Conclusions : These results suggest that electroacupuncture stimulation on $CV_{12}\;{\cdot}\;CV_9$ and $LI_4\;{\cdot}\;LI_{11}$ might be effective in treatments of obesity to high fat diet.

An Analysis of the Domestic Study Trends on Interventions of Korean Medicine for Abnormal Uterine Bleeding (비정상 자궁 출혈의 한의학적 치료에 대한 국내 연구 동향 분석)

  • Objectives: This study was performed to analyze the domestic study trends on abnormal uterine bleeding treated with Korean medicine. Methods: We searched the studies of abnormal uterine bleeding treated with Korean medicine via searching four Korean web databases. After that, we analyzed 15 studies which were selected according to the selection and exclusion criteria. Results: All 15 selected studies were case reports, and the total number of patients included was 33. Herbal medicine was used in all patients, and Igwiseungyang-tang-gami and Jeonsaenghwalhyol-tang-gami were the most commonly used. Other Korean medicine treatment such as acupuncture and moxibustion were performed. Acupuncture point most frequently used in acupuncture treatment were 三陰交 (SP6), 太衝 (LR3), 合谷 (LI4), and in moxibustion treatment were 關元 (CV4), 中脘 (CV12), 氣海 (CV6). In all studies, uterine bleeding was reduced after treatment. Conclusions: This study shows that Korean medicine could be helpful in treating abnormal uterine bleeding. In order to increase the basis for clinical use in the future, high quality of additional studies should be conducted.

The Effect of CV12, ST25, ST36 Acupuncture in General Diet and High Fat Diet Rat (고지방식이 및 일반식이 백서에 대한 중완·천추·족삼리 침자의 유효성 검증)

  • Objectives : It was conducted to experimentally analyze the effects of acupuncture treatment at CV12, ST25, and ST36 on weight, FBCS, fat metabolism, microbiome, and metabolome changes in the general diet rat and the high-fat diet rat. Methods : It was classified into four groups: general diet & non-treatment group (ND), general diet & acupuncture treatment group (ND+AT), high-fat diet & non-treatment group (HFD), and high-fat diet & acupuncture treatment group (HFD-AT). After acupuncture treatment was performed on CV12, ST25, and ST36, changes in body weight, FBCS, fat metabolism, microbiome, and metabolome were analyzed. Results : Compared to the ND group, acupuncture treatment performed on CV12, ST25, and ST36 in the ND+AT group had no significant effect. Compared to the HFD group, CV12, ST25, and ST36 acupuncture in the HFD+AT group reduced weight, fat weight, inflammatory cytokine IL-6 expression, and lipid droplet accumulation in liver tissue. Acupuncture can promote fat metabolism and relieve inflammatory conditions. Differences in diversity between ND and HFD groups were clear in changes in microbiome, fecal metabolites, and serum metabolites. As a result of some microbiome and metabolites involved in fat decomposition, intestinal lipid absorption, and blood lipid concentration control, such as Intestinimonas, Ruminococcus 1, pyroglutamic acid, tryptophan, and inositol, it was observed that the acupuncture treatment effect was evident in the disease-induced imbalance. Conclusions : Acupuncture treatment performed on CV12, ST25, ST36 clearly observed various regulatory actions on obesity induced by high-fat diet, confirming that the action of acupuncture treatment mainly plays a role in controlling an unbalanced state.

A Clinical Study on the Diagnosis and Observation of Functional Dyspepsia - Focused on Algometer

  • Objectives: This study aimed to determine whether comparing the pressure pain threshold (PPT) with an algometer before and after treatment for functional dyspepsia is of diagnostic value and investigate a possible correlation between PPT measured using an algometer and symptom improvement before and after treatment. Methods: A total of 99 patients with functional dyspepsia symptoms admitted to the OO Korean Medical Hospital from April 14, 2020 to January 21, 2021 were investigated. On the 1st and 14th days of hospitalization, the pressure of the first pain complaint at acupuncture points Juque (巨厥, CV14), Shangwan (上脘, CV13), Zhongwan (中脘, CV12), Xiawan (下脘, CV10), Guanuan (關元, CV4), Tianshu (天樞, ST25), and Daju (大巨, ST27) was measured using the algometer, and the visual analog scale (VAS) scores for patient's symptoms were evaluated. The algometer PPT and patient-symptom VAS scores were compared by repeated measures corresponding to the sample t-test to analyze the changes after treatment. A correlation analysis was performed to identify the correlation between patient-symptom VAS scores and algometer PPT. Results: The PPT measured using the algometer significantly increased after treatment in the 99 patients. The patient-symptom VAS score decreased significantly in most cases as treatment progressed. Analysis of the correlation between algometer PPT and patient-symptom VAS scores revealed some notable negative correlations. Conclusion: The algometer can help to set the diagnostic and treatment baselines for patients with functional dyspepsia.

The Diagnostic Significances of D.I.T.I. on the Patients of Cold-limbs (수족냉증(手足冷症) 환자(患者)에 대(對)한 컴퓨터 적외선(赤外線) 체열촬영(體熱撮影)의 의의(意義))

  • To make a objective diagnosis of the syndrome of cold-limbs, We investigated the 20 patients with cold limbs and GI trouble and 20 normal people as a control group. And we compared the thermal difference between Chon-jung(CV17) and Chung-wan(CV12 中脘), Chon-jung(CV17) and Ki-hae(氣海 CV6) and we compared the thermal differences of No-gung(PE8 勞宮) and Yong-chon(湧泉 KI1), too. The results were as follows. 1. All 20 patients had GI trouble and cold limbs. They had the symptom-Indigestion(16 cases-80%) with heart burn, tympanites, abdominal distention, hiccup, belching. Beside that symptom they also had constipation(6 cases-30%), diarrhea(3 cases-15%), headache & dizziness(6 cases-30%). And some had the menstrual syndrome, chronic fatigue, palpitation, insomnia, edema, arthralgia. 2. The thermal difference of the palms between the patients group and the control group were $25.70^{\circ}C,\;25.82^{\circ}C$, but they were not significant. 3. The thermal difference of the soles between the patients group and the control group were $23.58^{\circ}C,\;24.42^{\circ}C$ and the significancy was P=0.020 so it was significant(P<0.05). 4. The thermal difference of the palms and Chon-jung(CV17) between the patients group and the control group were $1.08^{\circ}C,\;0.76^{\circ}C$, but they were not significant. 5. The thermal difference of the sales and Chon-jung(CV17) between the patients group and the control group were $3.01^{\circ}C,\;1.90^{\circ}C$ and the significancy was P=0.003 so it was significant(P<0.05). 6. The thermal difference of Chon-jung(CV17) and Chung-wan(CV12 中脘) between the patients group and the control group was $0.30^{\circ}C,\;0.62^{\circ}C$ and the significancy was P=0.793 so it was significant(P<0.05). 7. The thermal difference of Chon-jung(CV17) and Ki-hae(CV6 氣海) between the patients group and the control group was $0.53^{\circ}C,\;0.68^{\circ}C$, but they were not significant. From the above, I could find the significance of D.I.T.I to diagnose the cold-feet not the cold-hands and the thermal difference between the trunk and limbs was more significant on cold-feet than on cold-hand, either.
