Prevalence of antimicrobial resistance and integrons in extended-spectrum β-lactamases producing Escherichia coli isolated from Nakdong and Gumho river (낙동강과 금호강에서 분리된 광범위 베타 락탐 분해효소 생성 Escherichia coli 내 항균제 내성 및 integrons의 분포)

  • This study was conducted to investigate the antimicrobial resistance, presence of ${\beta}$-lactamase genes and integrons in 83 ESBL-producing Escherichia coli isolated from Nakdong river and Geumho river in Daegu. Among the ${\beta}$-lactam antimicrobials, all isolates were resistant to ampicillin, cephalothin, cefamandole and cefotaxime, followed by piperacillin (98.8%), ampicillin/sulbactam (86.7%), aztreonam (60.2%) and cefepime (59.0%), whereas resistance to piperacillin/tazobacram, ticarcillin/clavulanic acid and cefoxitin was less than 30%. Many of the ESBL-producing Escherichia coli were also resistant to non-${\beta}$-lactams antimicrobials such as nalidixic acid (83.1%), sulfonamides (72.3%), ciprofloxacin (62.7%) and gentamicin (38.6%). All isolates showed resistance to seven or more antimicrobial agents. The most frequently detected gene was $bla_{TEM+CTX-M}$ (49.4%), followed by $bla_{CTX-M}$ (27.7%), $bla_{TEM}$ (6.0%) and $bla_{OXA}$ (1.2%). But $bla_{SHV}$ was not found. Class 1 integrons were found in 61.4% (51 isolates) of isolates, however, class 2 and 3 integrons were not detected. The results showed water from Nakdong river and Geumho river is contaminated with ESBL-producing E. coli isolates. These results suggest the need for further investigation of antibiotic resistant bacteria to prevent public health impacts in the water environment.

Fecal Carriage of Antimicrobial-Resistant Enterobacteriaceae in Healthy Korean Adults

  • The spread of antibiotic-resistant Enterobacteriaceae in the community is one of the main challenges for antibiotic treatment of community-onset infections. We evaluated the microbiological and molecular characteristics of stool samples from adults with comprehensive health examinations. Of 109 fecal samples, bacterial growth was observed in 86 samples and 61 gram-negative bacterial isolates were identified, of which 45 were Escherichia coli isolates (73.8%). Two isolates of Raoultella showed imipenem resistance, and both E. coli and Citrobacter freundii showed intermediate resistance to imipenem. Colistin resistance was identified in isolates of Klebsiella variicola and Salmonella subterranean, but no isolates carried mcr-1. As for E. coli genotypes, 35 sequence types were identified. $bla_{TEM-1}$, $bla_{TEM-30}$, and $bla_{CTX-M}$ were identified in 15, 1, and 4 E. coli isolates, respectively. In addition, all four Klebsiella pneumoniae isolates carried $bla_{SHV}$. Many genotypes that have been identified in isolates causing human infections were found in isolates in this study. There is a need to control the rise and spread of antibiotic-resistant pathogens by fecal carriage.

Antimicrobial-resistant Escherichia coli isolated from dogs and cats at animal hospitals in Daegu (대구지역 동물병원에서 입원중인 개와 고양이로부터 분리된 항생제 내성 대장균)

  • This study was carried out to investigate the antimicrobial resistance profiles and resistance genes in 62 Escherichia coli isolated from dogs and cats hospitalized at animal hospitals in Daegu. E. coli isolates showed high resistance to nalidixic acid (46.8%) and ampicillin (45.2%). Resistance to the other antimicrobial agents was less than 30%, and no resistant isolates were detected for imipenem and amikacin. Of the 28 ampicillin-resistant isolates, TEM and CTX-M genes were detected in 16 (57.1%) and 11 (39.3%), respectively. The aadA gene was found in 4 (26.7%) of 15 gentamicin-resistant isolates, and strA-strB gene was found in 10 (66.7%) isolates. The sul I and sul II genes were detected in 11 (61.1%) and 14 (77.8%) of 18 trimethoprim/sulfamethoxazole-resistant isolates, and tetB gene in 9 (81.8%) of 11 minocycline-resistant isolates, and cmlA gene in 2 (22.2%) of 8 chloramphenicol-resistant isolates. The qnrB and qnrS genes were found in 3 (10.3%) and 1 (3.4%) of 28 nalidixic acid-resistant isolates, respectively. Whereas, none of the SHV, CMY-2, tetA, dfr Ia and dfr VII, and qnrA genes were found. Our results show a wide variety of resistance genes in E. coli isolates from dogs and cats. This study also represents the first report of qnrB and qnrS gene producing E. coli isolates from dogs in republic of Korea.

Characterization of Extended-Spectrum-$\beta$-Lactamase Genotype TEM, SHV and CTX-M from Clinical Isolates of Klebsiella pneumoniae and Comparison with Antibiotic Susceptibility Test

  • Resent studies have reported increased isolation of extended-spectrum $\beta-lactamase$ (ESBL) producing strains at several hospital in Korea. We studied to investigate the isolation rates of ESBL strains from clinical isolates of Klebsiella pneumoniae and to characterize differences in types using analyses of genotyping and antibiotic susceptibility test. Antibiotic susceptibility test with confirmation of ESBL by double disk synergy test was performed on the 54 ESBL strains of Klebsiella pneumoniae from a hospital in Busan. Transfer of resistant gene in ESBL strains resistant to 3rd generated antibiotics was confirmed by transconjugation test using E. coli $RG176^{nal(r)}$. blaTEM, blaSHV, blaCTX-M genes were detected by PCR. ESBL producing strains had 100% of resistant rate to ampicillin, azteronam, cefazolin, cefepime and ceftriaxone ($\beta-lactam$ antibiotics). Forty strains of bla TEM$(74\%)$, 41 strains of bla SHV $(76\%)$, 23 strains of bla CTX-M $(43\%)$ were found, respectively. The strains had one or more genes. They had high resistant rates to $\beta-lactam$ antibiotics including cephalosporin. The resistant rates of strains with multiple resistant genes were higher than those of strains with single resistant gene.

가금병원성대장균의 항생제 내성 및 분자유전학적 역학분석

  • This study was conducted to investigate O group serotyping, antimicrobial drug resistance and distribution of extended spectrum ${\beta}$-lactamase of 203 Escherichia coli(E. coli) isolated from poultry in Korea during the period from April in 2003 to December 2005. The serogroup of 69.4% of isolates was determinated ; O 78(32.5%), O88(7.9%). O15(6.9%) and O141(6.4%) were the most common. These E. coli isolates showed resistance to nalidixic acid(92.6%), streptomycin(81.8%), ampicillin(77.3%), ciprofloxacin(70.9%), sulfisoxazole(66.5%) and trimethoprim(58.1%), respectively. The bla CTX$_{-M-3\;like}$(2 strains) and bla$\;_{CMY-2}$(2 strains) genes producing extended spectrum ${\beta}$ - lactamase(ESBL) were detected in four wild strains resistant to the third generation cephalosporin, respectively. The presence of the ESBL genes was confirmed in all transconjugants by PCR analysis with primers encoding CTX-M-3 like types or CMY-2.

Survey of extended-spectrum β-Lactamase (ESBL) in pathogenic Escherichia coli isolated from poultry in Korea (국내 가금유래 병원성 대장균의 extended-spectrum β-lactamase(ESBL) 특성 조사)

  • This study was conducted to investigate incidence of extended-spectrum ${\beta}$-lactamase (ESBL) producing strains and characteristics of ESBL gene in pathogenic Escherichia coli isolated from poultry during the period from April 2003 to December 2005 in Korea. Among 203 isolates, 4 isolates (3 from broilers and 1 from layer) were confirmed as ESBL producing strains by double disk synergy test, polymerase chain reaction and sequencing for ${\beta}$-lactamase genes. $bla_{CTX-M-15}$ and $bla_{CMY-2}$ were detected in these 4 isolates and were transferred to recipient by conjugation, respectively. Also, these ESBL producing strains were associated with multiple drug resistance. In conclusion, these results exhibit incidence of CTX-M and CMY-2 ${\beta}$-lactamase in pathogenic E coli from poultry in Korea, and clinically important meaning in human. And they also suggest the needs for rapid and broad surveillance to monitor ESBL genes and R plasmid transferring resistant gene in poultry.

The prevalence of Extended Spectrum β-Lactamase type produced by Clinical Isolates of Escherichia coli from ChungCheong Area (충청 지역에서 분리된 대장균이 생성하는 Extended Spectrum β-Lactamase 유형의 검출 빈도)

  • The study aims primarily to evaluate the resistance of antibiotics and the prevalence of these enzymes among Escherichia coli the most frequent isolate of Enterobacteriaceae producing Extended-Spectrum ${\beta}$-Lactamase(ESBLs), to differentiate the types of enzymes in these isolates. Total 74(26.2%) Strains of producing ESBLs among the 282 E. coli isolates were isolated from hospitals of Chungcheong area (Daejeon, Chungnam, and Chungbuk) during a 6 month-period from February to July, 2013. 282 E. coli isolates including ESBL shown resistance rates of aztreonam 30.8%, Cefotaxim 30.9%, and Ceftazidime 32.2%, 74 isolates producing ESBLs in E. coli were resistant rates to Aztreonam 58.1%, Cefotaxim 100%, and Ceftazidime 63.5% of ${\beta}$-lactam antibiotics. CTX-M-2 (48 isolates) was the most prevalent type of ESBLs identified. Followed the order of frequency by PER-1 (28 isolates), VEB-1 (26 isolates) and CTX-M-8 (20 isolates), of the 74 isolates, 2 isolates only showed GES-1 in Chungnam province. Accurate identification type of ESBLs would aid in hospital infection control. This would give aid to the physician to prescribe more appropriate antibiotics.

Prevalence of Extended Spectrum $\beta-Lactamase-Producing$ Clinical Isolates of Escher­ichia coli in a University Hospital, Korea (국내 대학병원에서 분리된 Eschepichia coli의 Extended-spectrum $\beta-Lactamase$ (ESBL) 현황)

  • Recently, the rapid increase and global spread of extended-spectrum $\beta-lactamase$ producing clinical isolates has become a serious problem. The incidence of extended-spectrum $\beta-lactamase$ producing clinical isolates of Escherichia coli in Korea and susceptibility to antimicrobial agents were investigated. Total 233 isolates of E. coli were obtained from urine from hospitalized patients in Guro hospital, Korea University in 2001. One hun­dred and eighty four isolates $(78.9\%)$ were resistant to ampicillin, 80 isolates $(34.3\%)$ were resistant to ceph­alothin, 93 isolates $(39.9\%)$ were resistant to gentamicin, and 64 isolates $(27.5\%)$ were resistant to norfloxacin. Among 233 isolates, 17 isolates $(7.3\%)$ were positive as determined by the double disk synergy test. When min­imal inhibitory concentrations were assayed with additional 6 antimicrobial agents, 13 isolates $(76.5\%)$ were multi-drug resistant to at least four different class antimicrobial agents. Extended-spectrum $\beta-lactamase$ were characterized with isoelectric focusing gel electrophoresis and DNA sequencing. They were TEM-1 in 5 iso­lates, TEM-15 in 1 isolate, TEM-20 in 1 isolate, TEM-52 in 4 isolates, TEM-1 and AmpC in 2 isolates, TEM-1 and OXA-30 in 1 isolate, TEM-1 and OXA-33 in 1 isolate, TEM-1, CTX-M-3, and AmpC in 1 isolate, but SHV was not detected. Antimicrobial resistance genes were transferred to animal isolate of E. coli (CCARM No. 1203) by the filter mating method. Extended spectrum $\beta-lactamase$ producers studied in the current study have low correlation to each other as determined by random amplified polymorphic DNA and pulsed field gel elec­trophoresis. This is a contradictory result from the general hypothesis that extended-spectrum $\beta-lactamase$ pro­ducers in one hospital is a result from a clonal spread.
