방사선과 의사들은 CT 및 MRI 스캐너로부터 얻어진 인체의 단면 영상을 연속적으로 보고 실제 3차원적으로 인체가 어떻게 구성되어 있는지를 상상하여 병변을 구별하는데, 의학영상을 이용한 인체 장기의 3차원 시각화는 2차원 형태의 인체 단면 영상들을 복잡한 알고리즘이나 고성능의 컴퓨팅 파워를 사용하여 실제 인체와 같이 3차원으로 재구성하여 보여준다. 단면 영상의 추적, 관심영역의 표시 및 추출등과 같은 2차원 영상분석은 시간이 많이 소모되고, 주관적일 수가 있으며, 수작업인 관계로 빈번한 에러가 발생하는 단점을 가지는데, 이와 같은 2차원 의료 영상 분석의 단점을 보완하기 위해 의학영상처리 기술과 접목한 3차원 의료 영상의 시각화는 필수적이라 할 수 있다. 명암값 임계치 방법, 영역확장(region growing) 방법, 윤곽선(contour) 추출 방법 및 변형모델(deformable model) 방법을 사용하여 인체의 각 장기를 분리하였으며, 텍스쳐분석(texture analysis)을 통하여 고안된 특징자를 이용하여 암 부분을 인식하는데 사용하였고, 원근투영(perspective projection) 및 볼륨 데이터의 표면을 렌더링하기 위해 마칭큐브(marching cube) 알고리즘을 사용하였다. 인체 및 분리된 장기에 대한 3차원 시각화는 방사선치료계획(radiation treatment planning), 외과 수술계획, 모의수술, 중재적(interventional)시술 및 영상유도수술(image guided surgery)에 효과적으로 사용될 수 있다.
흉부방선균증은 임상양상 및 방사선학적 소견이 폐의 기타 염증성 질환이나 악성암과 유사한 경우가 많아 감별진단에서 놓치기 쉬우나, 대부분 항생제 치료에 좋은 반응을 보이므로 조기진단을 위한 적극적 접근으로 심각한 합병증의 발생 및 불필요한 수술을 막아야 한다. 저자들은 폐암을 의심하여 시행한 검사도중 폐를 침범하는 방선균증을 경험하였기에 문헌고찰과 함께 보고하는 바이다.
Osteoclast-like giant cell tumor of the liver is an extremely rare malignancy with poor prognosis. To our knowledge, 5 cases have been reported in English literatures, but there was no report about fine needle aspiration cytologic(FNAC) features. We experienced a case of osteoclast-like giant cell tumor of the liver obtained by computed tomography(CT)-guided FNAC and needle biopsy. The cytologic findings mimicked slant cell tumor of the bone. A large hepatic mass of the left lobe with abdominal wall invasion was found by CT in a 46- year-old female complaining of epigastric pain. The FNAC showed moderately cellular smears consisting of osteoclast-like giant cells and mononuclear cells, which were individually scattered or intermingled in clusters. The osteoclast-like giant cells had abundant cytoplasms and multiple small round nuclei with fine chromatin and distinct nucleoli. The mononuclear cells had moderate amount of cytoplasm and relatively bland-looking oval nuclei with single small nucleoli. All of the cytologic features recapitulated the histologic findings of bland-looking osteoclast-like multinucleated giant cells evenly dispersed throughout the background of mononuclear cell. The immunohistochemical study showed positive reaction for CD68 and vimentin, but negative for cytokeratin in both osteoclast-like slant cells and mononuclear cells.
Primary pericardial mesothelioma is extremely rare and the incidence is low among the mesotheliomas that originate from other parts of the body. The prognosis of the tumor is unfavorable due to its late presentation, difficulties in early diagnosis and complete resection, and the limited treatment options. Herein, we report a case of pericardial mesothelioma. The patient is a 55-year-old woman who presented with chronic cough and dyspnea. During the examination, pericardial effusion was found and pericardial window formation was followed. She visited our hospital because of persistent dyspnea, with right shoulder and chest pain. Four discrete masses were discovered in the chest CT. CT guided-fine needle aspiration biopsy was negative for malignancy. Right exploratory thoracotomy and partial resection of 3${\times}$3 cm mass abutting pericardium was performed and was histologically diagnosed as malignant mesothelioma, biphasic type. Pericardial mesothelioma is rare, but it should be remembered as an important differential diagnosis in patients with persistent pericardial effusion and symptoms of dyspnea and constrictive pericarditis.
The use of intravenous catheters are occasionally complicated by intravascular fragments and swelling of the catheter fragments. We present a patient in whom an intravenous catheter fragments was retrieved from the dorsal metacarpal vein following its incidental CT examination detection. The case of demonstrates the utility of microscopy and multi-detector CT in localizing small of subtle intravenous catheter fragments as a human error. A case of IV catheter fragments in the metacarpal vein, in which reproducible and microscopy data allowed complete localization of a missing fragments and guided surgery with respect to the optimal incision site for fragments removal. These reproducible studies may help to determine the best course of action and treatment for the patient who presents with such a case.
Radiation therapy is an important part of cancer treatment in which cancer patients are treated using high-energy radiation such as x-rays, gamma rays, electrons, protons, and neutrons. Currently, about half of all cancer patients receive radiation treatment during their whole cancer care process. The goal of radiation therapy is to deliver the necessary radiation dose to cancer cells while minimizing dose to surrounding normal tissues. Success of radiation therapy highly relies on how accurately 1) identifies the target and 2) aim radiation beam to the target. Both tasks are strongly dependent of imaging technology and many imaging modalities have been applied for radiation therapy such as CT (Computed Tomography), MRI (Magnetic Resonant Image), and PET (Positron Emission Tomogaphy). Recently, many researchers have given significant amount of effort to develop and improve imaging techniques for radiation therapy to enhance the overall quality of patient care. For example, advances in medical imaging technology have initiated the development of the state of the art radiation therapy techniques such as intensity modulated radiation therapy (IMRT), gated radiation therapy, tomotherapy, and image guided radiation therapy (IGRT). Capability of determining the local tumor volume and location of the tumor has been significantly improved by applying single or multi-modality imaging fur static or dynamic target. The use of multi-modality imaging provides a more reliable tumor volume, eventually leading to a better definitive local control. Image registration technique is essential to fuse two different image modalities and has been In significant improvement. Imaging equipments and their common applications that are in active use and/or under development in radiation therapy are reviewed.
고립성 섬유종은 흉막에서 흔히 발생하는 양성 종양으로 알려져 있으나, 신체 어느 곳에서나 발생할 수 있고 10%-30%에서는 악성이다. 전형적으로, 고립성 섬유종은 단일성의 조영증강되는 종괴로 나타나지만, 척추 주위의 양측성 종괴로 나타나는 경우는 보고되어 있지 않다. 이 증례에서는, 등 통증과 만성 농흉의 병력이 있던 88세 남자 환자의 영상 검사에서 침습적인 양측성 척추 주위 종괴가 발견되었고, 만성 농흉과 연관된 흉부 악성 종양을 먼저 의심하였다. CT 유도하 이중구조 바늘 생검을 통해 진단된 양측성의 척추 주위 악성 고립성 섬유종의 증례를 보고한다.
목적: 온보드 영상장치(On-Board Imager, OBI) 및 콘빔CT(Cone Beam Computerized Tomography, CBCT)를 이용하면 치료실에 위치한 환자의 자세 및 위치와 모의치료 시점의 환자의 자세 및 위치를 비교할 수 있다. 온라인 영상유도방사선치료(on-line Image Guided Radiation Therapy, on-line IGRT)에서는 이러한 정보를 이용하여 방사선 치료 직전에 환자의 위치를 확인하고 보정한다. 이때 모의치료 시 획득한 영상과 치료실에서 실시간 얻은 kV X선 영상 또는 콘빔CT 영상을 이용하여 2차원/2차원 맞춤(2D/2D Match) 또는 3차원/3차원 맞춤(3D/3D Match)의 이미지 퓨젼 프로그램을 사용하여 그 편차를 산출한다. 이 과정에서 주어지는 편차가 환자 자세에 대한 오차를 정확히 반영하고 있는지에 대해 알아보고자 한다. 대상 및 방법: 신체 내부 구조가 모사된 팬톰(The $RANDO^{(R)}$ Phantom, Alderson Research Laboratories Inc., Stamford, CT, USA)을 사용하여 실제 방사선 치료와 동일한 과정을 따라 모의치료 및 치료계획을 시행한 후 치료 테이블 위에 팬톰을 셋업한다. 그리고 모의치료 시 표시된 팬톰의 표면 지점에 치료실의 레이저에 일치시킨다. 이때, CT 모의치료실과 가속기가 있는 치료실의 벽면 고정 레이저에 대한 정렬의 일치만 확인하면, 치료테이블에 놓여진 팬톰의 위치는 모의치료 시 위치와 정확히 일치한다. 실제로는 팬톰 표면에 나타나는 레이저 선의 두께 정도되는 오차를 무시한다면, 두 시점에서 팬톰의 위치가 정확히 같다고 말할 수 있다. 정확히 위치가 재현되었다고 가정되는 팬톰에 대해 평행이동 또는 회전이동의 변화를 만들어 준 후, 위치가 옮겨지고 틀어진 팬톰에 대해 온보드 영상장치로부터 kV X선 영상을 그리고 콘빔CT로부터 CT 영상을 얻는다. kV X선 영상과 모의치료 시 획득한 CT영상을 이용하여 OBI 프로그램에서 제공되는 2차원/2차원 맞춤의 결과를 얻는다. 그리고 콘빔CT 영상과 모의치료 시 획득한 CT영상을 가지고 이미지 퓨젼 과정을 거쳐 3차원/3차원 맞춤의 결과를 얻는다. 이렇게 얻은 2차원/2차원 맞춤 및 3차원/3차원 맞춤의 결과와 처음에 팬톰에 인위적으로 만들어준 위치 변화를 비교한다. 결과: 온보드 영상장치로 획득한 kV X선 영상과 모의치료 시 영상을 비교하는 2차원/2차원 맞춤에서는 팬톰의 위치에 회전이동만 존재한다고 가정했을 때에는 평균 $0.06^{\circ}$의 오차 내에서 모의치료 시 팬톰의 위치에 대한 편차를 찾을 수 있었다. 또한 평행이동만 존재한다고 가정했을 때에는 편차 벡터의 크기가 평균 1.8 mm였다. 그리고 회전이동과 평행이동이 동시에 존재하는 일반적인 경우에는 편차 벡터의 크기는 평균 2.1 mm, 테이블 회전 방향으로 평균 $0.3^{\circ}$의 오차 내에서 모의치료 시 팬톰의 위치를 찾을 수 있었다. 콘빔CT로 획득한 영상을 이용하는 3차원/3차원 맞춤의 과정에서 팬톰의 위치가 회전이동만 존재할 때에는 평균 $0.03^{\circ}$의 오차 내에서, 평행이동만 있는 경우는 편차 벡터의 크기의 평균이 0.16 mm 내에서, 틀어지고 이동된 팬톰의 위치를 찾을 수 있었다. 그리고 회전이동과 평행이동이 동시에 존재하는 일반적인 경우에는 편차 벡터의 크기는 1.5 mm, 테이블 회전 방향으로 평균 $0^{\circ}$의 오차 내에서, 모의치료 시 팬톰의 위치와 맞출 수 있었다. 결론: 온보드 영상장치와 콘빔CT를 이용한 영상유도방사선치료(on-line IGRT)에서 모의치료 시 팬톰의 위치는 가속기의 치료테이블 위에서 매우 정확히 재현되어졌다. 온보드 영상장치는 kV X선 영상을 이용하여 간단하게 위치의 검증과 보정을 할 수 있었고, 콘빔CT를 이용하는 경우에는 2차원적인 정면 또는 측면 영상이 아니라, 3차원 영상을 비교함으로서 더욱 정확한 위치보정이 가능하였다.
Objective: Liposarcoma (LPS) is the most common soft tissue sarcoma and accounts for approximately 20% of all mesenchymal malignancies, often occurring in deep soft tissue of retroperitoneal space. Accurate preoperative diagnosis is therefore necessary. We explored whether computed tomography (CT) could be used to differentiate between the various types of retroperitoneal liposarcoma (RPLS). Method: Forty-seven cases of RPLS, diagnosed surgically and histologically, were analyzed retrospectively. CT features were correlated with postoperative pathological appearance. Results: The study radiologist identified 29, 11, 2, 2 and 3 RPLS as atypical lipomatous tumor/well-differentiated liposarcoma (ALT/WDL), dedifferentiated liposarcoma (DDL), myxoid/round cell liposarcoma (ML/RCL), pleomorphic liposarcoma (PL) and mixed-type liposarcoma. Analysis of CT scans revealed the following typical findings of the different subtypes of RPLS: ALT/WDL was mainly visible as a well-delineated fatty hypodense tumor with uniform density and integrity margin; DDL was marked by the combination of focal nodular density and hypervascularity. ML/RCL, PL and mixed liposarcoma showed malignant biological behaviour and CT findings need further studies. Conclusions: CT scanning can reveal important details including internal components, margins and surrounding tissues. Based on CT findings, tumor type can be roughly evaluated and biopsy location and therapeutic scheme guided.

Objective: To evaluate the computed tomography (CT) features for diagnosing metastatic cervical lymph nodes (LNs) in patients with differentiated thyroid cancer (DTC) and validate the CT-based risk stratification system suggested by the Korean Thyroid Imaging Reporting and Data System (K-TIRADS) guidelines. Materials and Methods: A total of 463 LNs from 399 patients with DTC who underwent preoperative CT staging and ultrasound-guided fine-needle aspiration were included. The following CT features for each LN were evaluated: absence of hilum, cystic changes, calcification, strong enhancement, and heterogeneous enhancement. Multivariable logistic regression analysis was performed to identify independent CT features associated with metastatic LNs, and their diagnostic performances were evaluated. LNs were classified into probably benign, indeterminate, and suspicious categories according to the K-TIRADS and the modified LN classification proposed in our study. The diagnostic performance of both classification systems was compared using the exact McNemar and Kosinski tests. Results: The absence of hilum (odds ratio [OR], 4.859; 95% confidence interval [CI], 1.593-14.823; P = 0.005), strong enhancement (OR, 28.755; 95% CI, 12.719-65.007; P < 0.001), and cystic changes (OR, 46.157; 95% CI, 5.07-420.234; P = 0.001) were independently associated with metastatic LNs. All LNs showing calcification were diagnosed as metastases. Heterogeneous enhancement did not show a significant independent association with metastatic LNs. Strong enhancement, calcification, and cystic changes showed moderate to high specificity (70.1%-100%) and positive predictive value (PPV) (91.8%-100%). The absence of the hilum showed high sensitivity (97.8%) but low specificity (34.0%). The modified LN classification, which excluded heterogeneous enhancement from the K-TIRADS, demonstrated higher specificity (70.1% vs. 62.9%, P = 0.016) and PPV (92.5% vs. 90.9%, P = 0.011) than the K-TIRADS. Conclusion: Excluding heterogeneous enhancement as a suspicious feature resulted in a higher specificity and PPV for diagnosing metastatic LNs than the K-TIRADS. Our research results may provide a basis for revising the LN classification in future guidelines.
