동적조형회전조사 시 표적종양의 위치변위와 조사반경의 변화에 따른 선량전달 오류분석 (Analysis of Dose Delivery Error in Conformal Arc Therapy Depending on Target Positions and Arc Trajectories)

  • 본 연구의 목적은 회전조사 시 표적종양의 위치변위와 갠트리의 조사반경에 의한 치료깊이 변화에 따른 모의치료계획 결과와 선량전달 결과상의 오차를 분석하고자 하였다. 깊이 변위가 가장 이상적인 경우, 즉 팬텀의 중심에 표적이 위치한 경우와 한쪽으로 2.5 cm, 5 cm씩 치우친 경우로 나누어 모의실험하였다. 표적의 위치 변화에 따른 모의치료계획을 실시하기 위하여 IMRT Body 팬톰(I'mRT Phantom, Wellhofer Dosimetry, Germany)를 이용하여 전산화단층촬영장치(Computed Tomography, Light speed 16, GE, USA)로 데이터를 획득하였다. 획득된 영상을 이용하여 치료계획장치(Treatment Planning System, Eclipse, ver. 6.5, VMS, Palo Alto, USA)를 이용하여 정중앙, 2.5 cm, 5 cm에 가상의 치료표적을 만들어 모의치료계획을 수립하였다. 선형가속기(CL21EX, VMS, Palo Alto, USA)의 6 MV 광자선과 최근 개발된 Gafchromic 필름(EBT2, ISP, Wayne, USA)을 이용하여 선량분포를 측정하였고, 선량분석프로그램(OmniPro-IMRT, ver. 1.4, Wellhofer Dosimetry, Germany)을 이용하여 모의치료계획 데이터와 측정 데이터를 정량적으로 분석하였다. 분석프로그램으로 횡축방향 선량분포 프로파일(Cross-plane profile)과 선량분포를 정량적으로 분석하기 위하여 감마인덱스(DD: 3%, DTA: 2 mm) 히스토그람을 이용하였다. 표적과 표적주변의 선량분포는 Conformity index(CI), Homogeneity index(HI)를 이용하여 정량적으로 분석하였다. 치료표적 전체체적에 대한 100% 선량분포에 포함되는 체적을 비교하여 분석하였다. 표적의 위치가 5 cm 에 있는 경우 다문동적회전조사(Multiple Conformal Arc Therapy, MCAT)는 23.8%, 단일동적회전조사(Single Conformal Arc Therapy, SCAT)는 35.6%, 고정조사는 37%였고, 표적이 2.5 cm에 있는 경우 MCAT 61%, SCAT 21.5%, 고정조사 14.2%로 분석되었다. 표적의 위치가 중앙에 있는 경우 MCAT 70.5%, SCAT 14.1%, 고정조사 36.3%로 나타났다. 표적의 위치가 5 cm 치우쳐 있는 경우를 제외하고 MCAT의 100% 선량분포에 포함되는 체적이 가장 크게 나타났다. 감마인덱스 히스토그램 분석결과, SCAT의 경우 37.1, 27.3, 29.2로 MCAT의 경우 9.2, 8.4, 10.3에 비해 최소 2.8배, 최대 4배 오차가 크게 나타났다. 결론적으로, 동적조형회전조사 시 표적종양의 위치변이와 조사반경의 변화에 따라 선량전달오류의 가능성을 알 수 있었으며 치료표적의 위치가 정중앙이 아닐 경우, 깊이와 회전반경을 최적화함으로써 정확한 선량 전달을 할 수 있다고 생각한다.

  • 배 경 : 폐기종의 중증도 판정에 있어서 병리학적 기준은 기강(air space)의 확장 정도에 있으며, 이는 고해상도 전산화 촬영 (high resolution computed tomography, HRCT)에 의해 결정되는 폐기종 점수(emphysema scoring)와 좋은 상관성을 보인다는 것은 주지의 사실이다. 한편 폐기종의 주증상은 운동성 호흡곤란이므로 폐기종에 대한 임상적 평가는 운동능력의 감소를 측정하는 것이 타당하다고 알려져 있다. 그러나 대개의 경우 폐기종의 중증도 판정은 안정시 폐기능 검사에 의존하는 경우가 흔하며, 병리학적 중증도 판정과 상관성이 높다고 알려져 있는 고해상도 전산화 촬영에 의한 폐기종 점수와 운동부하 폐기능검사 지표들과의 상관성 여부에 대한 연구는 드문 실정이다. 이에 본 연구에서는 폐기종의 중증도 판정에 있어서 HRCT와 안정시 폐기능 검사 및 운동부하 폐기능 검사간에 상호 관련성을 확인하여 보았다. 방 법 : 평균 연령 $60.6{\pm}10.3$세인 14명의 폐기종 환자들을 대상으로 HRCT, 안정시 폐기능 검사(forced expiratory flow volume curve, lung volumes by He dilution method, DLco, ABGA), 그리고 점진적운동부하폐 기능검사(incremental cycle ergometer)를 시행하였으며 HRCT는 GE highlight를 이용하여 조영증강 없이 최대흡기시에 1.5mm collimation, 10mm 간격으로 폐전체를 스캔하였고, 환자마다 모든 스캔에서 density mask를 이용하여 -400 HU를 기준으로 한 총폐면적과 -900 HU를 기준으로 한 폐기종 면적을 각각 구하여 백분율로 환산하여 폐기종 점수를 구하였다. 결 과 : 평균 폐기종 점수는 $37.4{\pm}14.9%$ 이었다. 폐기종 점수와 안정시 폐기능 검사의 DLco(r=-0.75)와 $PaO_2$(r=-0.66) 사이에서만 유의한(p<0.05) 상관성이 관찰되었다. 반면 폐기종 점수와 운동부하폐기능 지표들간의 상관성은 최대 산소섭취량(r=-0.68), 혐기성 역치(V-slope method, r=-0.690), 최대운동부하(r=-0.74), 최대운동시 $O_2$ pulse(r=-0.73), 최대운동시의 생리적 사강비율 (r=0.80) 등과 높은 유의성 (p<0.01)을 나타내었다. 그러나 호흡예비율과 심박수 예비율간에서는 유의한 상관성이 없었고, pulse oxymeter로 측정한 산소포화도와의 상관성도 유의하지 않았다. 결 론 : 이상의 결과에서 폐기종의 병리학적 중증도를 잘반영한다고 알려진 HRCT 폐기종 접수는 폐기종에 의한 생리학적 장애를 잘 반영하는 운동부하 폐기능 검사의 주요 지표들과 유의한 상관성이 있음을 확인할 수 있었다.

유제한성 병기의 소세포 폐암에서 3주 간격으로 시행된 irinotecan과 cisplatin을 이용한 과다분할 방사선 동시 요법 (Phase II Study of Induction Irinotecan + Cisplatin Chemotherapy Followed by Concurrent Irinotecan + Cisplatin Plus Twice-Daily Thoracic Radiotherapy)

  • 배경: Irinotecan hydrochloride는 topoisomerase I inhibitor로서 소세포 폐암에 효과적인 약제로 알려져 있다. Irinotecan은 cisplatin과 더불어 방사선감작물질로 작용하기도 한다. 본 연구는 이전에 치료받은 경험이 없는 제한성 병기의 소세포 폐암 환자에서 irinotecan과 cisplatin(IP)의 방사선 동시화학요법의 효과를 평가하기 위하여 시행되었다. 방법: 2002년 12월부터 2004년 11월까지 충남대학교 병원에서 새로이 제한성 병기의 소세포 폐암으로 진단된 24명의 환자들을 대상으로 하였다. Irinotecan $60mg/m^2$을 제 1일과 제 8일째 투여하였고 cisplatin $60mg/m^2$을 제 1일째 투여하였으며 매 3주 간격으로 시행되었다. 제 3차 항암화학요법을 시작하는 날과 동시에 과다 분할방사선 치료(twice-daily thoracic irradiation; 45 Gy total)을 시작하였다. 예방적 전 뇌 방사선 조사(Prophylactic cranial irradiation)가 방사선 동시화학요법이 끝난 후 완전반응(complete response)을 나타낸 환자에서 시행되었다. 제 2차 항암요법과 제 6차 항암요법이 끝난 후에 흉부 전산화 단층촬영과 기관지경 등을 통한 병기의 재평가가 이루어졌다. 결과: 병기의 재평가는 19명의 환자에게 이루어졌다. 중앙 추적관찰기간은 12.5개월이고 전체 99회의 항암치료가 시행되었다. 평균 한 환자당 5.2회의 항암치료가 시행되었다. 실제 용량강도는 cisplatin $19.6mg/m^2$/week과 irinotecan $38.2mg/m^2$/week이었다. 9명의 환자가 완전반응을 보였고 10명의 환자가 부분반응(partial response)을 보여서 전체 반응률은 95%였다. 3에서 4도의 혈액학적 독성은 백혈구 감소증(35% of cycles), 빈혈(7% of cycles), 혈소판 감소증(7% of cycles) 등으로 나타났다. 3에서 4도의 비 혈액학적 독성은 설사(5% of cycles)였다. 3에서 4도의 방사선 식도염(10% of patients)을 제외하고는 과다 분할 방사선 치료를 이용한 방사선 동시 화학요법의 기존의 방법과 독성 면에서는 큰 차이가 없었다. 치료와 관련된 사망은 관찰되지 않았다. 평가가 가능한 환자들에서 1년 생존율과 2년 생존율은 각각 89% (16/18)와 47% (9/18)였다. 결론: 3주 간격으로 시행된 irinotecan과 cisplatin을 이용한 과다분할 방사선 동시 요법은 제한성 병기의 소세포 폐암 환자에서 부작용은 높지 않으면서 효과적인 치료법으로 고려될 수 있을 것이다.

생식의 섭취가 납중독 흰쥐의 피해 경감에 미치는 영향에 관한 연구 (Protective Effect of Saengshik Supplementation on Lead Induced Toxicity in Rats)

  • 생식 제품이 납중독 흰쥐의 피해경감에 미치는 영향을 규명하기 위하여 12mg의 납을 매일 1회 쥐에게 경구 투여하면서 생식제품이 $12.5\%,\;25\%,\;50\%$ 함유된 식이로 6주간 사육 후 체중 증가율 및 사료 효율, 혈액학적 검사, 간 조직의 과산화지질함량, SOD활성, GSH 함량과 간 및 신장 조직내 납 함량을 측정하였다. 체중 증가율과 사료 효율은 납 투여시 대조군과 비교하여 유의적으로 감소되는 것으로 나타났고(p<0.05), 생식의 급여는 체중과 식이효율을 다소 증가시키는 경향을 보였으며, 특히 $12.5\%$ 생식 섭취군에서는 식이 효율이 납 단독 투여군과 비교하여 유의적으로 증가되는 것으로 나타났다(p<0.05). 납에 의한 혈액학적 독성 증상으로 납 단독 투여군에서는 RBC를 제외한 HGB, HCT, MCV, MCH, RDW에서 유의적인 손상이 나타났으며(p<0.05), HCT의 경우 $50\%$ 생식 섭취군에서 유의적으로 증가되는 것으로 나타났고(p<0.05), MCV, MCH, RDW의 경우 $25\%$$50\%$ 생식 급여 시 납에 의한 손상이 완화되는 것으로 나타났다(p<0.05). 그러나 HGB의 경우 생식 농도에 따른 수치 증가는 관찰되었지만 유의 차는 나타나지 않았다. 또한 납 투여로 간조직 의 과산화지 질 생성이 다소 증가되는 것이 관찰되었으며, $12.5\%$$50\%$ 생식 섭취군에서 과산화지질 생성이 유의적으로 감소되는 것으로 타나났다(p<0.05). 그러나 납 투여에 따른 SOD활성 변화는 관찰되지 않았으나 생식은 SOD활성을 다소 증가시키는 경향을 보였다 뿐만 아니라 납 단독 투여는 항산화 및 해독 작용 관련 물질인 GSH 수준을 유의적(p<0.05)으로 증가시켜주는 것에 반해 생식의 섭취는 증가된 GSH의 함량을 농도 의존적으로 낮추는 것으로 나타났다(P<0.05). 또한 생식은 간과 신장 조직에서의 납 축적을 억제시켜 간 조직의 경우 $25\%$$50\%$ 생식군에서 유의적인 감소 효과가 있었고, 신장 조직에서는 $25\%$ 생식군에서 유의적인 감소 효과가 있었다(p<0.05). 이상의 연구 결과에 의하면 생식의 섭취는 납중독으로 발생되는 혈액학적 및 산화적 손상을 완화시켜 주고, 조직에서의 납축적을 억제시켜 주는 것으로 나타나 생식은 납중독에 의한 피해를 경감시켜줄 수 있는 해독 기능을 갖는 것으로 사료된다. 그러나 본 연구는 생식의 유효성을 평가하기 위한 기초 자료 일환으로 동물실험 수준에서 평가된 연구로서 인체에 적용하기는 아직 무리가 있으며, 향후 용량 설정에 대한 연구 및 임상단계에서 효능을 평가하기 위한 다각적인 연구가 더 필요할 것으로 여겨지고, 이와 함께 후속 연구를 통한 생식의 납 해독 기전이 규명되어야 할 것이다.

선형가속기와 토모치료기를 이용한 전림프계의 방사선 치료시 선량분포에 관한 연구 (The Study of Dose Distribution according to the Using Linac and Tomotherapy on Total Lymphnode Irradiation)

  • 전림프계의 방사선치료시 선형가속기와 토모테라피를 이용하여 치료계획 수립 시 유용성을 확인하였다. 실험에 동의한 실험대상자 15명(남: 7명, 여: 8명)의 모의치료영상을 Somatom Sansation Open 16 channel으로 획득하여 이를 각각 의료용 선형가속기 치료계획 장치와 토모테라피 치료계획 장치로 전송하고 종양체적과 정상조직(전체 폐, 척수, 우측신장, 좌측신장)을 구분하여 종양조직에 750 cGy를 설정하여 종양조직의 선량 적합성, 정상조직의 선량흡수정도, 선량분포양상 그리고 선량체적곡선을 비교하여 평가하였으며 SPSS Ver. 18.0을 이용하여 대응표본검정을 실시하였다. 종양의 흡수선량 측정결과 토모테라피의 경우 $751.0{\pm}4.7cGy$, 선형가속기는 $746.9{\pm}14.1cGy$의 선량을 보였으며 이는 통계적으로 유의하지 않았다(p>0.05). 정상조직의 경우 전체 폐, 척수, 우측신장, 좌측신장에 입사되는 평균 방사선량은 토모테라피가 선형가속기보다 다소 낮은 방사선 흡수량을 보였다. 선량체적곡선에서 종양조직 및 정상조직 모두 적합한 양상을 보였다. 즉, 종양 및 정상조직의 선량흡수정도, 선량분포양상, 선량체적 곡선을 살펴본 결과 모두 적합한 치료효과비를 보인 것으로 판단되며 토모테라피 치료가 다소 높은 치료효율을 보였다. 토모테라피를 이용한 치료는 폐쇄형 치료공간과 긴 치료시간의 단점이 있기 때문에 오랜시간 자세재현이 불가능한 환자의 경우, 제한적으로 선형가속기 치료를 실시하여도 무방할 것으로 판단된다.

폐암의 SABR(Stereotactic Ablative Radiotherapy)시 복부압박(Abdominal compression)과 CPAP(Continuous Positive Air Pressure)를 이용한 치료계획의 비교 및 평가 (Comparison and evaluation of treatment plans using Abdominal compression and Continuous Positive Air Pressure for lung cancer SABR)

  • 목 적 : 폐암의 정위 절제 방사선 치료(Stereotactic Ablative Radiation Therapy, SABR)시 복부압박을 이용한 치료계획과 양압지속유지기(The Continuous Positive Air Pressure, CPAP)를 이용한 치료계획을 비교, 분석하여 방사선 치료 효과 향상에 기여하고자 한다. 대상 및 방법 : 본원의 폐의 SABR 환자 중 2명을 대상으로 복부 압박 고정 장치(the Body Pro-Lok, BPL)와 CPAP를 이용한 치료 계획을 수립하여, RTOG 0813에서 제안한 parameter들과 균질도(Homogeneity Index, HI), 일치도(Conformity Index, CI)를 통해 치료계획을 분석하였다. 또한 모든 4D CT에서 각 Phase별 계획용 표적 체적(Planning Target Volume, PTV) 중심의 X, Y, Z축 움직임을 분석하고, PTV와 손상위험장기(Organ At Risk, OAR)의 체적과 평균 선량을 구하여 비교하였다. 그리고 4개의 원뿔형빔 전산화단층촬영(Cone Beam Computed Tomography, CBCT)을 이용하여, PTV 중심점과 0°, 90°, 180°, 270° 중 세 방향에서의 흉강내 접점까지의 직선거리를 측정하고, 각 방향에서 평균거리 값과의 차이를 비교하였다. 결 과 : BPL과 CPAP를 사용하여 얻은 치료계획은 모두 RTOG의 권고값을 따랐으며, 균질도와 일치도에서도 큰 차이가 없었다. PTV 중심의 X축, Y축, Z축 움직임은 A 환자의 경우 BPL 사용시 0.49 cm, 0.37 cm, 1.66 cm, CPAP 사용시 0.16 cm, 0.12 cm, 0.19 cm을 보였고, B 환자의 경우 BPL 사용시 0.22 cm, 0.18 cm, 1.03 cm, CPAP 사용시 0.14 cm, 0.11 cm, 0.4 cm을 보였다. CPAP 사용시 BPL 사용시보다 A 환자의 경우 ITV가 46.27% 감소하였고 좌측 폐의 체적이 41.94% 증가하였으며, 평균선량은 심장에서 52.81% 감소하였다. B 환자의 경우 좌측 폐 106.89%, 우측 폐 87.32% 체적이 증가하였고, 평균선량은 위에서 44.30% 감소하였다. 각 방향의 직선거리 값과 평균거리 값과의 최대 차이는 A 환자의 경우 a방향에서 0.05 cm, b방향에서 0.05 cm, c방향에서 0.41 cm 났고, B 환자의 경우 d방향에서 0.19 cm, e방향에서 0.49 cm, f방향에서 0.06 cm 차이가 났다. 결 론 : CPAP 사용시 폐용적의 증가를 통해 표적 근처 OAR의 선량을 BPL 사용시보다 더 효과적으로 감소시킬 수 있으며, 호흡에 따른 종양의 움직임 제한에도 더 효과적으로 기여할 수 있다는 것을 확인하였다. 추후 CPAP의 다양한 부위 적용과 다른 치료기와의 결합을 통해 방사선 치료 효과를 개선시킬 수 있을 것으로 사료된다.

쇄골 상부 림프절을 포함하는 왼쪽 유방암의 VMAT 치료계획시 X-Jaw split을 이용한 선량비교에 관한 연구 (Study on dose comparison using X-Jaw split in VMAT treatment planning for left breast cancer including supraclavicular lymph nodes.)

  • 목 적 : VMAT을 이용한 쇄골 상부 림프절을 포함하는 왼쪽 유방암 치료 시 X-Jaw를 나눈 치료계획과 기존 Full field VMAT 치료계획과의 선량분포를 비교 분석하여 X-Jaw split VMAT의 유용성을 평가하였다. 대상 및 방법 : 쇄골 상부 림프절을 포함하는 왼쪽 유방암 환자 10명을 대상으로 Vacuum cushion을 이용하여 CT simulation 하고 2 Arc Full field VMAT과 4 Arc X-Jaw split VMAT을 각각 치료계획 하였다. 각 치료계획은 처방 선량(Prescription dose)이 치료계획표적용적(Planning Target Volume, PTV)의 95% 이상 포함이 되고 주변 정상장기(Organ at risk, OAR)에 최소한으로 조사되도록 설계하였다. 이후 선량체적용적(Dose volume histogram, DVH)을 통해 PTV과 OAR의 대한 선량분석을 하였다. 결 과 : 10명의 환자의 Full field VMAT치료계획과 X-Jaw split VMAT치료계획의 데이터를 평균값으로 나타내어 비교하였다. 처방선량지수(conformity index, CI) 1.05±0.04, 1.04±0.03, 선량균질지수(homogeneity index, HI) 1.07±0.008, 1.07±0.009 로 두 치료계획 간 차이는 크지 않았다. OAR의 경우 왼쪽 폐의 V5 56.1±6.50%, 50.4±6.30%, V20 20.0±4.15%, 13.52±3.61% 로 Full field VMAT대비 X-Jaw split VMAT에서 V5이 10.0% V20은 32.6% 감소하였다. 심장의 V30은 3.68±1.85%, 2.23±1.52%, 평균 선량은 8.93±1.65 Gy, 7.67±1.52 Gy로 X-Jaw split VMAT에서 V30은 39.3%, 선량은 14.1% 감소하였다. 정상조직인 왼쪽 폐와 심장은 통계적 유의 수치인 p-value 0.05미만으로 나타났다. 결 론 : PTV의 체적이 크고 X Jaw의 길이가 15 cm 이상이 되는 쇄골 상부 림프절을 포함하는 왼쪽 유방암치료의 경우 X-Jaw split VMAT 치료계획은 기존의 치료계획과 동등한 HI와 CI로 유사한 PTV coverage를 유지 하면서 개선된 선량분포를 나타내 OAR인 폐와 심장 등의 방사선량을 줄일 수 있어 부작용을 줄이는데 효과적이라고 생각된다.

  • 목 적: 본 연구는 6MV-FFF(Flattening Filter Free) 에너지를 사용하는 Tomotherapy, Halcyon, True-Beam 장비를 이용하여 가상의 표적 용적에 대한 표면 선량 값을 측정, 비교평가하고자 한다. 대상 및 방법: IMRT 팬텀(IBA, Germany)을 이용하였으며 표면 선량 측정의 정확도를 높이기 위해 without bolus, 0.5 cm bolus, 1 cm bolus 총 세 가지 조건으로 전산화 단층촬영을 시행하였다. 가상의 표적 깊이에 치료 계획 용적(Planning Target Volume, PTV)을 설정하고 1회, 200 cGy로 치료 계획을 각각 수립하였다. 표면 선량 측정은 Gafchromic EBT3 필름을 전산화 치료 계획과 동일한 구간에 위치시켜 10회 반복 측정한 뒤 분석하였다. 결 과: 각 장비 별 실험 결과 without bolus, 0.5 cm bolus, 1 cm bolus 순서로 표면 선량 측정값의 평균은 Tomotherapy에서 115.2±2.0 cGy, 194.4±3.3 cGy, 200.7±2.9 cGy로 확인되었고, Halcyon에서 104.7±3.0 cGy, 180.1±10.8 cGy, 187.0±10.1 cGy로 확인되었으며, TrueBeam에서 92.4±3.2 cGy, 148.6±5.7 cGy, 155.8±6.1 cGy로 확인되었다. 세 가지 조건에서 모두 치료 계획 시스템과 동일하게 Tomotherapy, Halcyon, TrueBeam 순으로 높게 측정되었다. 결 론: Tomotherapy와 Halcyon에서 TrueBeam 장비에 비해 표면 선량이 높게 측정되었다. 치료 부위와 치료 목적에 맞춰 장비 별 특성이 고려된다면 치료 효율성의 증가뿐 아니라 환자의 치료 만족도도 높아질 것으로 사료된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Fo and explosion depth (from the surface to the point of maximun temperature) are increased when Nk and Bi are increased.To find out the minimum laser power for explosive evaporative removal process, steady state analysis has been also made. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm.
