소아 두부외상 환자에서의 반복적인 두부 CT 검사의 유용성 (Value of Repeat Brain Computed Tomography in Children with Traumatic Brain Injury)

  • Purpose: Traumatic brain injury (TBI) is the most common cause of pediatric trauma patients came to the emergency department. Without guidelines, many of these children underwent repeat brain computed tomography (CT). The purpose of this study was to evaluate the value of repeat brain CT in children with TBI. Methods: We conducted a retrospective study of TBI in children younger than 19 years of age who visited the emergency department (ED) from January 2011 to December 2012. According to the Glasgow Coma Scale (GCS) and Pediatric Glasgow Coma Scale score of the patients, study population divided in three groups. Clinical data collected included age, mechanism of injury, type of TBI, and outcome. Results: A Total 83 children with TBI received repeat brain CT. There were no need for neurosurgical intervention in mild TBI (GCS score 13-15) group who underwent routine repeat CT. 4 patients of mild TBI group, received repeat brain CT due to neurological deterioration, and one patient underwent neurosurgical intervention. Routine repeat CT identified 12 patients with radiographic progression. One patient underwent neurosurgical intervention based on the second brain CT finding, who belonged to the moderate TBI (GCS score 9-12) group. Conclusion: Our study showed that children with mild TBI can be observed without repeat brain CT when there is no evidence of neurologic deterioration. Further study is needed for establish indication for repetition of CT scan in order to avoid unnecessary radiation exposure of children.

임상에서 흔히 관찰되는 영상판독 오류 (Image interpretation errors often observed in a dental clinic)

  • These days, the clinical course of dental imaging sector has done a lot of implant-related imaging courses, including cone beam CT. In contrast, the general image reading course is not given a lot of opportunities to learn. Therefore, it is imperative that we talk about the general image interpretation that can be read easily applied in a dental clinic. When we see a strange radiographic finding of our patient in the dental clinic, we should first check whether the radiographic finding is a normal finding or a morbidity. If the finding is diagnosed as a morbidity, you should make plans for the appropriate therapy. The most important step is classification between normal state and morbidity. Some lesions may occur without any clinical symptoms. Therefore, we should read all the parts of radiographs, even the patient does not have clinical symptoms.

Particle Swarm Optimization을 이용한 PET/CT와 CT영상의 정합 (Image Registration for PET/CT and CT Images with Particle Swarm Optimization)

  • 영상정합 기술은 두 개 이상의 영상을 서로 맞추어, 각각의 영상이 가지고 있는 단점을 보완하여, 새로운 정보를 획득하게 하는 기술이다. 본 논문은 의료 영상간의 2D 영상 정합을 통해 환자의 점진적 병세파악에 도움을 주는 것을 목적으로 하고 있다. 서로 다른 시점과 장비로부터 얻어진 CT와 PET/CT영상을 정합하기 위하여 정확한 해부학적 정보를 제공하는 CT영상간의 정합을 먼저 수행하고 이를 통하여 얻어진 기하학적 정합파라미터들을 PET 영상에 적용하여, 독립 CT영상 위에 PET영상을 중첩하였다. 정합작업을 위해 먼저 각각의 CT영상에 대해 전처리 작업을 실시하였고, 영상의 변형은 affine 좌표변환을 이용하였다. 정합할 영상간의 유사도 평가를 위해 mutual information을 이용하였으며, 빠르고 정확한 정합을 위하여 최적화 알고 리듬인 particle swarm optimization 방법을 이용하였다. 이를 통해 실제 환자의 독립 CT와 PET/CT영상을 이용하여 실험하였고, PET/CT의 영상에서 확인할 수 있었던 병소에 대한 해부학적 위치 정보가 영상정합 과정을 통해 독립 CT 영상에서도 동일한 위치에 표시됨을 확인하였다. 제안된 알고리듬은 PET/CT 뿐만 아니라 향후 도입될 SPECT/CT, MRI/PET 등 다중영상기기와 기존의 독립 CT 영상기기와의 정합에도 폭넓게 사용될 것으로 기대된다.

Pediatric Orbital Medial Wall Trapdoor Fracture with Normal Computed Tomography Findings

  • With advances in diagnostic technology, radiologic diagnostic methods have been used more frequently, and physical examination may be neglected. The authors report a case of pediatric medial orbital trapdoor fracture in which the surgery was delayed because computed tomography (CT) findings did not indicate bone displacement, incarceration of rectus muscle, or soft tissue herniation. A healthy 6-year-old boy was admitted to the emergency room for right eyebrow laceration. We could not check eyeball movement or diplopia, because the patient was irritable. Thus, we performed facial CT under sedation, but there was normal CT finding. Seven days later, the patient visited our hospital due to persistent nausea and dizziness. We were able to perform a physical examination this time. Lateral gaze of right eye was limited. CT still did not show any findings suggestive of fracture, but we decided to perform exploratory surgery. We performed exploration, and found no bone displacement, but discovered entrapped soft tissue. We returned the soft tissue to its original position. The patient fully recovered six weeks later. To enable early detection and treatment, thorough physical examination and CT reading are especially needed when the patient shows poor compliance, and frequent follow-up observations are also necessary.

정중 흉골절개술을 통한 동시적 양측 폐기포 절제술 (Median Sternotomy for Simultaneous Bilateral Bullectomy)

  • To prevent recurrence of spontaneous pneumothorax, 23 patients were operated through median sternotomy for simultaneous resection of bilateral bullae, And 27 patients with spontaneous pneumothorax were treated with unilateral thoracotomy, We studied the number, duration and sites of recurrence including findings of CT scan, as well as comparing the both operated group. The incidence of spontaneous pneumothorax was 88% in patients with the ages between 16 to 35 Forty one patients[82%] were operated with the indication of recurrent pne-umpthorax. The number of pneumothorax attack was 2.34 per patient with recurrent pneumothorax. The 87.8% of recurrence was occured within 6 months from last attack. Ips-ilateral recurrnet pneumothorax was 56.1% and contallateral involve was 43.9%. The bilaterality of visible bullae was 90.9% in the findings of chest CT scan and 91.3% in the operative finding. The sensitivity and accuracy for bulla detection with chest CT were 92.6%, respectively. Exclude one case of complicated median sternotomy infection, the postoperative hospital stay was shorter in median sternotomy approached group[P<0.05]. In conclusion, the bullous lesions of the lung have tendency of bilaterality so that median sternotomy for simultaneous resection of bilateral bullae should be considered in patients with contralateral visible bullae with chest CT.

SNOMED CT 용어체계에서 형제 노드의 유사도 분석 기법 (Similarity Analysis of Sibling Nodes in SNOMED CT Terminology System)

  • 본 논문에서는 SNOMED CT 용어체계가 가지는 불완전성을 논의하고 이를 유지하는 방법으로 형제 노드 간 유사성을 평가하는 지표를 제안한다. SNOMED CT는 방대한 양의 의학용어를 포함하고 있으나 계층구조 내 컨셉의 누락 등 온톨로지의 불완전성 문제가 존재한다. 누락된 컨셉 발견을 위해 다수의 노드로 구성된 형제 노드 그룹 내에서의 노드 간 유사도 평가를 위한 지표를 제안하고 유사도가 낮은 그룹을 도출하였다. 2023년 3월 SNOMED CT 국제 배포판에 적용하여 형제 노드 그룹들의 유사도를 분석한 결과 임상적 발견 컨셉의 하위 컨셉들 중 2개 이상의 형제 노드를 가진 29,199개의 형제 노드 그룹의 평균 유사도는 0.81로 나타났다. 반면, 유사도가 가장 낮은 그룹은 중독 컨셉의 자식 컨셉으로 그 유사도는 0.0036으로 확인되었다.

  • Pseudoaneurysm arising from the superficial temporal artery (STA) is a rare and potentially critical cause of palpable mass. Most pseudoaneurysms form as a result of blunt trauma and present as painless, pulsatile mass that may be associated with pathologic finding and enlarged size. We report a rare case of pseudoaneurysm arising from STA caused by blunt injury and diagnosed by 3-dimensional computed tomography (CT) angiography.

  • In this paper, we propose a new scheme for automatic segmentation of the liver in CT images. The proposed scheme is carried out on region of interest(ROI) blocks that include regions of the liver with high probabilities. The ROI approach saves unnecessary computational loss in finding the accurate boundary of the liver. The proposed method utilizes the composition of multi-size morphological filters with a prior knowledge, such as the general location or the approximate intensity of the liver to detect the initial boundary of the liver. Then, we make the gradient image with the weight of the initial liver boundary and segment the liver legion by using an immersion-based waters hed algorithm in the gradient image. finally, the refining process is carried out to acquire a more accurate liver region.

부비동 유두종에서의 FDG 섭취 증가를 보인 예 (A Case of Paranasal Sinus Papilloma with Increased FDG Uptake)

  • The false-positive FDG uptakes on head and neck areas are common due to benign lesion, iatrogenic and physiologic changes. The Schneiderian papilloma is uncommon benign tumor arising from the mucosa of the paranasal sinuses and nasal cavity. The findings of paranasal papilloma on conventional modality such as CT and MRI are non-specific and they could be confused with inflammatory polyp or retention cyst. Despite of benign tumor, the papilloma usually shows locally aggressive growth with malignant potential, therefore the FDG can be actively accumulated in this lesion. We describe the case of 18F-FDG PET/CT finding in a 77-year-old woman who demonstrates oncocytic papilloma in maxillary sinus.

Small Solitary Ground-Glass Nodule on CT as an Initial Manifestation of Coronavirus Disease 2019 (COVID-19) Pneumonia

  • The 2019 novel coronavirus (2019-nCoV) outbreak in Wuhan, Hubei Province, China in 2019 led to large numbers of people being infected and developing atypical pneumonia (coronavirus disease 2019, COVID-19). Typical imaging manifestations of patients infected with 2019-nCoV has been reported, but we encountered an atypical radiological manifestation on baseline computed tomography (CT) images in three patients from Wuhan, China infected with the 2019-nCoV. Surprisingly, the only similar CT finding was a solitary sub-centimeter ground-glass nodule adjacent to bronchovascular bundles, which could be easily overlooked. In addition, the follow-up images in these patients showed how COVID-19 pneumonia evolved from these small nodules. The radiologic manifestation of the three cases will expand contemporary understanding of COVID-19.
