Lung cancer is the leading cause of cancer death for men and women in the industrialized world. It is desirable to detect disease at a stage when it is not causing symptoms and when control or cure is possible. If the screening test detects patients with the disease at an early stage, they can be examined to confirm the diagnosis and intervention can alter the natural history of the disease. The results of screening programs designed to detect early lung cancer using either conventional chest radiograph or sputum cytology are disappointing for a diagnostic screening test. Because of advances in helical CT imaging techniques, screening for lung cancer has been suggested as a possible method of improving outcome. Findings in recent publications suggest that substantial dose reduction is possible in chest CT. The advantages of low-dose CT are more sensitive than chest radiograph for detecting small pulmonary nodules that may be lung cancers, shorter scanning time than conventional chest CT scan without intravenous contrast injection, cheaper cost than standard CT, low radiation dose. However, the true clinical significance of the small tumors found by screening is still unknown, and their effect on mortality awaits future investigation. Furthermore, in addition to detecting an increased number of lung cancers, low-dose CT found at least one indeterminate nodule in many of all screened patients. The majority should be benign but evaluation of all these indeterminate nodules is not a trivial problem in routine practice. In conclusion, lung cancer screening with low-dose CT is a complex subject. The true effectiveness of lung cancer screening (a reduction in mortality from lung cancer) with low-dose CT can be determined through well-designed randomized control trials with enrolment of appropriate subjects.
본 연구는 파킨슨질환의 진행 정도를 평가할 수 있는 선조체를 모사한 brain 팬텀을 직접 제작하여, PET-CT검사 시 최적의 영상의 질을 유지하면서 CT 스캔에 의한 brain 선량 저감을 위한 가능성을 평가하였다. 팬텀의 hot sphere와 background (radi oacti vi ty rati o 3:1)에 $^{18}F$ FP-CIT를 주입하고, 관전압(100, 120 kVp)과 관전류(80, 140, 200 mAs)의 조건을 변화시키며 CT 스캔을 실험하였다(기준조건; 120 kVp, 140 mAs). 각 조건에 따라 예상유효선량을 conversion factor를 적용해 계산하였고, SNR과 CRChot의 영상평가 인자를 설정하여 영상의 질을 평가하였다. 실험결과 CT 촬영 기준조건 이하에서의 예상유효선량은 최소 10%에서 최고 60% 정도 감소하였고, 기준조건 이상에서의 예상유효선량은 40% 증가하였다. 또한 PET 영상의 SNR과 CRChot의 유의한 차이는 없었으므로, 관전압과 관전류의 감소에 따라 brain 선량이 감소함을 확인하였다. 이와 동시에 스캔 조건의 변화에도 불구하고 SNR과 CRChot 측면에서의 영상의 질에는 유의한 변화가 없었다. 이러한 사실은 낮은 선량 조건으로도 기존의 선량 조건으로 획득한 영상의 질 수준을 얻을 수 있었으므로, 추후 brain PET-CT 스캔의 선량감소와 동시에 영상의 질 향상에 관한 연구의 기초자료로 활용이 가능할 것으로 사료된다.
두부 CT 검사에서 눈의 수정체는 높은 방사선 감수성으로 보호되어야 할 장기 중에 하나이다. 두부 CT 검사에서 눈의 피폭선량감소 목적으로 사용되는 비스무스 차폐체는 현저한 선량 감소 효과가 있지만 화질저하의 문제점이 있다. 본 연구는 인체팬텀을 이용한 두부 CT 검사에서 피폭 선량을 줄이기 위해 사용되는 관전류변조기법인 new organ based-tube current modulation, longitudinal-TCM, angular-TCM과 차폐기법인 비스무스, 납 안경을 사용하여 눈의 선량과 화질의 변화 정도를 평가하였다. 연구결과, new OB-TCM에서 눈의 선량은 25.88% 감소되었으며 비스무스를 사용한 검사와 비교하여 CT number, 노이즈, SNR의 변화가 작은 것을 확인하였다. 따라서 new OB-TCM을 임상의 두부 CT 검사에 적용할 경우, 눈의 피폭선량을 감소시키면서 영상의 화질 저하를 방지할 수 있을 것으로 기대된다.
The effective dose and the organ absorbed dose, which are given to a breast in the cases of using and not using the bismuth breast protection shield for the protection of a breast with the coronary artery CT angiography, have been measured and compared for the manual exposure control (MEC)and the automatic exposure control (AEC). In the cases of using and not using the bismuth breast protection shield, it has been found that the measured dose shows the reduction of about 23 to 26% for the MEC and about 22 to 25% for the AEC when the shield is used compared to the case of not using it. By comparing the shield and non-shield cases for the AEC and the MEC, it can be said that the value measured by carrying out the scanning process with the AEC mode has decreased by about 24 to 30% compared to the case of applying the MEC mode. Such a result shows that it is recommended to use the AEC mode for the reduction of the patient's exposure dose during the CT examination.
CT 검사는 의료 방사선 검사 중 높은 피폭 선량으로 발암의 위험성을 높일 수 있다고 보고되고 있으며, 특히 여성의 흉부 CT 검사 시 방사선에 민감한 유방에 대한 방사선 노출이 불가피하다. 이에 본 연구에서는 여성의 흉부 CT 검사 시 압박밴드 착용에 따른 선량 감소효과에 대해 평가하고, 검사 시 노출되는 유효선량에 기인한 생애 암 발생 위험도를 추정하였다. 그 결과 압박밴드 사용 시 흉부 외부 둘레가 작아짐에 따른 실효 관전류량이 감소되었으며, 이로 인해 CT 선량지수와 유효선량 또한 감소된 것으로 분석되었다. 또한 흉부 CT 검사에 의한 생애 암 발생 위험도는 압박밴드 착용 시 30대 여성 기준으로 모든 암에서 10만명 당 3.2명, 고형암에서 10만명 당 0.2명, 유방암에서 10만명 당 0.8명 정도의 암 발생 위험도를 감소시키는 결과를 나타내었다. 추후 CT 검사 시 적절한 검사 조건 사용과 더불어 압박밴드와 같은 선량 최적화 방안을 통해 암 발생 위험을 줄일 수 있을 것으로 판단된다.
Purpose: Maxillofacial trauma predominantly affects young adults between 20 and 40 years of age. Although radioprotection is a legal requirement, the significant potential of dose reduction in computed tomography (CT) is still underused in the clinical routine. The objective of this study was to evaluate whether maxillofacial fractures can be reliably detected and classified using ultra-low-dose CT. Materials and Methods: CT images of 123 clinical cases with maxillofacial fractures were classified by two readers using the AOCOIAC software and compared with the corresponding results from post-treatment images. In group 1, consisting of 97 patients with isolated facial trauma, pre-treatment CT images at different dose levels (volumetric computed tomography dose index: ultra-low dose, 2.6 mGy; low dose, <10 mGy; and regular dose, <20 mGy) were compared with post-treatment cone-beam computed tomography (CBCT). In group 2, consisting of 31 patients with complex midface fractures, pre-treatment shock room CT images were compared with post-treatment CT at different dose levels or CBCT. All images were presented in random order and classified by 2 readers blinded to the clinical results. All cases with an unequal classification were re-evaluated. Results: In both groups, ultra-low-dose CT had no clinically relevant effect on fracture classification. Fourteen cases in group 2 showed minor differences in the classification code, which were no longer obvious after comparing the images directly to each other. Conclusion: Ultra-low-dose CT images allowed the correct diagnosis and classification of maxillofacial fractures. These results might lead to a substantial reconsideration of current reference dose levels.
최근의 나선형 CT와 MDCT는 기존의 고식적 CT보다 X-선 조사시의 겹침 현상과 영상 재구성에 있어서의 보간삽입처리로 인해 보다 높은 선량을 환자에게 주게 되었다. MDCT와 나선형 CT장치가 보다 많은 의학적 정보를 제공하는 것에도 불구하고 환자가 받는 방사선 노출은 기존의 고식적 CT검사에 비해 $2{\sim}4$배 정도로 증가되고 있는 실정이므로, 그 잠재적 위험성을 아무리 강조해도 지나침이 없다. CT장치에서의 보다 많은 X-선에 관련된 자료들, 특히 선량효율적 디자인이나 X-선 조절 소프트웨어에 대한 자료들이 필요하다. 왜냐하면 CT장치의 디자인 요소는 임상적 진단에 있어서 환자선량을 성공적으로 줄일 수 있는 중요한 요소이기 때문이다. 이에 본 연구에서는 최근 급격히 확산되어 사용되고 있는 여러 단계의 MDCT의 z-축 선량효율을 측정하여 SDCT와 비교하였다. 그리고 MDCT에서 스캔 시 채택하는 focal spot size와 beam collimation, 검출기 조합 등을 비롯한 파라메터들의 변화에 따른 z-축 선량효율을 파악하여 다음과 같은 결과를 얻었다. 1. SDCT가 z-축 기하학적 선량효율이 가장 높았고, 4 슬라이스 MDCT가 가장 낮았다. MDCT 중에서는 small beam collimation 적용 시 64 MDCT가 기하학적 선량효율이 가장 높았고 16, 8, 4 슬라이스 MDCT 순이었으며, large beam collimation 적용 시에는 small focal spot에서는 8 MDCT가, large focal spot에서는 16 MDCT가 가장 높았다. 2. MDCT의 경우 large focal spot에 비해 small focal spot의 z-축 기하학적 선량효율이 최저 0.67%에서 최대 13.62%의 범위에서 높았다. 3. MDCT의 경우 small beam collimation에 비해 large beam collimation의 z-축 기하학적 선량효율이 $3.13{\sim}51.52%$의 범위에서 높았다. 4. 동일한 focal spot size와 beam collimation을 채택한 상태에서 detector combination 차이에 따른 z-축 기하학적 선량효율은 4 슬라이스 MDCT의 모든 경우와 8 슬라이스 MDCT의 large beam collimation에서 일정하였다. 하지만 8 슬라이스 MDCT의 small beam collimation과 16 슬라이스 MDCT에서는 z-축 기하학적 선량효율이 차이를 보였으며 변화의 일률성은 없었다. 5. 동일한 스캔 파라메터를 적용시 나선형 스캔과 고식적 스캔 모드의 z-축 기하학적 선량효율은 동일하였으며, pitch를 변화시키거나 영상재구성 시 슬라이스 두께와 간격을 변화시켜도 z-축 기하학적 선량효율은 변화가 없었다. 결론적으로, CT검사 시 환자가 받는 X-선 피폭선량을 줄이기 위해 연구자는 CT장치의 선량효율에 대해 각별히 주의하여야 하며, Z-축 선량효율성을 높이는 동시에 최적의 임상적 정보를 보존할 수 있는 스캔 파라메터를 선택하여야 한다.
두부 CT 검사 시 일반적으로 검사 시 적용하는 스캔조건 및 알고리즘을 기준으로 선정 후 알고리즘 변경 적용을 통한 선량감소효과를 실험을 통해 알아보고자 하였다. 그 결과 선량감소효과는 관전압 보다 관전류 변화 시 의미가 있었으며, 인체 팬텀을 이용한 화질 평가는 Smooth 알고리즘적용 후 선량을 감소 시 Bone 알고리즘적용 후 선량을 감소 시 보다 상대적으로 화질 감소가 적었으며, 특히 Bone 알고리즘적용 시에는 평균 CT Number 또는 Pixel value의 편차가 상대적으로 크게 측정되었다. 즉 선량은 감소되고 화질은 유지하여 환자의 피폭 선량을 감소시키고 동일한 화질의 영상을 얻을 수 있는 조건은 Smooth 알고리즘 적용과 120 kVp, 160 mA라는 결과 값을 얻을 수 있었다. 이 때 선량은 약 28% 감소되며 평균 CT Number 또는 Pixel value도 오차가 상대적으로 적게 측정 되었다. 향 후 스캔조건 및 다양한 알고리즘 적용 후 그 결과를 검진 또는 추적검사 등으로 내원한 환자에게 적용하여 검사 한다면 환자 피폭선량 감소에 있어 매우 유용할 것으로 사료된다.
PET/CT 검사 시 피폭 선량 감소를 위한 방법들이 지속적으로 개발되고 있다. 본 논문에서는 사용자에 의해 parameter 변경이 가능한 3가지 방법인 automatic exposure control(AEC), automated dose-optimized selection of X-ray tube voltage(CAREkV), sinogram affirmed iterative reconstruction(SAFIRE) 적용 시 각 방법의 적용 시와 3가지 방법의 조합에 따른 피폭선량 감소효과와 영상의 질 그리고 SUV 변화 유무를 평가하였다. Bograph mCT64 (Siemens, Germany)장비를 사용하여 anthropomorphic head, chest, pelvis phantom을 torso 모형으로 접합하여 스캔하였다. 120 kV, 40 mAs 조건으로 AEC의 적용 유무와 120 kV, 40 mAs AEC 조건으로 CAREkV의 적용 유무에 따른 피폭 선량 감소 효과를 평가 하였다. 120 kV, 25 mAs SAFIRE 조건에서 영상을 획득하여 120 kV, 40 mAs SAFIRE 미적용 시 대비 노이즈와 피폭선량 감소효과가 평가되었다. 120 kV, 40 mAs AEC 적용, 120 kV, 25 mAs 3가지 방법의 조합 조건으로 AAPM perfomance, anthropomorphic, IEC body phantom을 스캔하여 노이즈, 공간 분해능, 피폭선량 감소효과 그리고 PET SUV의 변화 유무를 평가하였다. AEC 적용 시, 미적용 대비 CTDIvol 50.52%, DLP 50.62% 감소하였다. CAREkV 적용 시 100 kV가 적용됨에 따라 mAs가 61.5% 증가하였으나 CTDI 6.2%, DLP 5.5% 감소하였다. Reference mAs를 낮게 지정할수록, strength값을 높게 지정할수록 피폭선량 감소 효과는 증가하였다. SAFIRE의 경우 40 mAs에서 25 mAs로 tube current를 37.5% 감소시켰음에도 불구하고 mean SD 2.2%, DLP 38% 감소하였다. AAPM phantom에서는 3가지 방법의 조합 시 AEC 대비 SD는 5.17% 감소하였으며 공간 분해능의 경우 유의한 차이가 없었다. Torso phantom의 경우 3가지 조합에서 AEC 대비 mean SD 6.7% 증가, DLP 36.9% 감소하였으며 IEC phantom 실험에서 PET SUV는 통계적으로 유의한 차이가 없었다(P>0.05). 본 논문에서 CT선량 감소를 위한 각 방법들 모두 피폭 선량 감소 효과를 보였으며 3가지 방법의 조합을 통하여 화질 저하와 PET SUV 변화 없이 AEC만 적용 시 대비 36.9%의 선량감소 효과가 있는 것으로 나타났다. 선량 감소 방법들의 최적화를 통하여 환자 피폭선량 저감화를 위한 지속적인 노력이 필요하며 특히 방사선 감수성이 높은 소아 환자에 적극적으로 적용되어야 할 것으로 사료된다.
소아 CT 촬영시 피폭선량을 저감시킬 수 있는 조건을 찾기 위하여 pediatric phantom을 이용하여 소아 방사선량을 비교 분석하였다. 관전류변동제어(tube current modulation:TCM) 방식과 low dose CT protocol(LDCTP)에 따라 소아 검사를 시행하였으며, 검사는 brain, chest, abdomen-pelvis 부위를 전산화 단층촬영 장치를 이용한 3개의 검사 프로토콜을 사용하였다. 거의 모든 MDCT 장치는 volume CT dose index(CTDIvol), dose length product(DLP)와 유효선량 평가 결과를 기록, 보관하고 있으며, 이러한 정보들은 환자들의 방사선 노출 및 위험도를 평가하는 데 필수적이다. 방사선 노출 위험도를 감소시키기 위해서 적정화 및 정당화 원칙을 반드시 준수해야 한다. 관전압(kV)에 따른 TCM 방식을 사용하였을 경우 kV가 낮아질수록 유효선량이 낮아지는 경향을 나타내었으며, low dose CT protocol을 사용한 경우가 유효선량이 상대적으로 더 적은 것을 알 수 있었다. 또한 평균 유효선량은 brain, chest, abdomen-pelvis CT의 결과가 독일의 reference dose 보다 각각 47%, 13.8%, 25.7%로 더 낮게 나타났고. 영국의 reference dose와 비교해도 55.7%, 10.2%, 43.6%에 불과했다. 결론적으로 선량 감소를 위해 주의할 것은 body-weight based tube current adaption을 포함한 TCM, 그리고 LDCTP 등 모든 선량 감소 방안을 이용하여야 한다.

회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
