Studies on the Biological Behaviors of Taxol Derivatives (Taxol 유도체들의 생물학적 거동에 관한 연구)

  • This study was designed to prospect the $^{111}In$-labelled paclitaxel as tumor imaging agent. In order to provide a taxol molecule with a functional group which is able to chelate In-111, taxol-DTPA conjugate and 2'-hemisuccinyltaxol were synthesized by esterification of taxol at C-2'on C-13 carbon with DTPA anhydride and succinic anhydride, respectively. Synthesis yield of the taxol derivatives was 34% for taxol-DTPA and 80% for 2'-hemisuccinyltaxol. Cytotoxicity of the taxol derivatives were measured by MTT method toward cell lines HT29, B16, P388, and CT26. The cytotoxic activities of the taxol derivatives were maintained, although less active than taxol. Radiolabelling of the taxol derivatives were proceeded directly with $^{111}InCl_3$ or indirectly with $^{111}In$-citrate(ligand-exchange method). The ligand-exchange method was not suitable because some precipitates appeared during the reaction. On the contrary, by direct radiolabelling method, we were able to obtain taxol-DTPA-$^{111}In$ in 100% radiochemical yield. However, 2'-hemisuccinyltaxol was not labelled by both methods. Yield and radiochemical purity of the radiolabelled com-pound were determined by HPLC, paper chromatography and instant thin layer chromatography. Taxol-DTPA-$^{111}In$ was characterized to be hydrophilic by lipophilicity test, and nearly non-adhesive to HT29, B16, P388, and CT26 by cell binding affinity test. Binding affinity of the taxol-DTPA-$^{111}In$ complex to serum proteins was also examined by protein precipitation with 30% trichloroacetic acid. The results showed that 30% of the taxol-DTPA-$^{111}In$ complex binds with serum proteins.

Ossification of the Transverse Ligament of the Atlas on CT: Frequency and Associated Findings (CT를 이용한 환추횡인대 골화증의 빈도와 연관 소견)

  • Purpose To determine the frequency of ossification of the transverse ligament of the atlas (OTLA) and to investigate the associated findings on cervical spine CT and plain radiography. Materials and Methods We reviewed 5201 CT scans of the cervical spine of 3975 consecutive patients over an 11-year period for the presence of OTLA and compared them with those of age- and sex-matched controls. The frequency and associated findings of OTLA were investigated and statistically correlated. Results The overall frequency of OTLA was 1.1% (45 of 3975 patients) and increased with age (p < 0.005). The frequency of OTLA in patients over 80 years was 12%. The space available for the spinal cord (SAC) was smaller in patients with OTLA (p < 0.005). Mineralization of the complex of the anterior atlantooccipital membrane and Barkow ligament, ossification of the ligamentum flavum, and kyphosis of the cervical spine positively correlated to the presence of OTLA (p < 0.005). Conclusion OTLA was associated with age, SAC narrowing, cervical kyphosis, and ossification of other cervical ligaments and may be associated with degenerative spondylosis, systemic hyperostotic status, or mechanical stress or instability.

  • Objectives: This study was conducted to better understand the relationship between health effects and exposure to dusts from a briquette fuel complex in Ansim, Daegu, Korea. Methods: The subjects of this study consisted of 2,980 persons over 40 years old who had lived 20 years or more around a briquette fuel complex in Daegu. We conducted a questionnaire survey, and chest radiography was performed. In addition, chest computed tomography(CT) (335 cases) and pulmonary function test(PFT) (658 cases) were done. Pneumoconiosis was diagnosed if one of three radiologists determined(or suspected) pneumoconiosis. We also conducted in-depth interviews for pneumoconiosis cases. We defined the exposed group as subjects residing within a 500 meter radius from the walls of the briquette fuel complex, and the others were defined as the control group. Results: Subjects in the exposed and control groups are respectively 715(24%) and 2,265 cases(76%). Major respiratory symptoms in the exposed group such as sputum, dyspnea, chest tightness and wheezing were significantly higher than in the control group. By chest radiography, 173 cases of pneumoconiosis or suspicious pneumoconiosis were detected. By PFT, 62 cases(29.5%) of chronic obstructive pulmonary disease(COPD) among 210 asymptomatic subjects were detected. Finally, by chest CT we concluded 28 cases to be pneumoconiosis, and eight cases among them proved to be pneumoconiosis by environmental exposure. Conclusions: Through this study, we concluded that health outcomes such as respiratory symptoms, pneumoconiosis, and COPD were caused by continuous exposure to dusts from the briquette fuel complex. Policies to reduce environmental exposure are needed, and cases of environmental disease should be intensively followed up by the government.

  • Ameloblastic fibro-odontoma is a rare benign mixed odontogenic tumor with histologic characteristics of ameloblastic fibroma and complex odontoma. As with ameloblastic fibroma, it may be asymptomatic or found because of painless swelling and delayed eruption of associated tooth. This report presents a case of ameloblastic fibro-odontoma in the posterior mandible of a 14-year-old girl and analyses its clinical features and radiographic features on plain X-ray film and CT images.   • A low power(9 mW) highly-digitized 2.4 GHz receiver for sensor network applications(IEEE 802.15.4 LR-WPAN) is realized by a $0.18{\mu}m$ CMOS process. We adopted a novel receiver architecture adding an intermediate frequency (IF) level detection scheme to a low-power complex fifth-order continuous-time(CT) bandpass L:tl modulator in order to digitalize the receiver. By the continuous-time bandpass architecture, the proposed $\Sigma\Delta$ modulator requires no additional anti-aliasing filter in front of the modulator. Using the IF detector, the achieved dynamic range(DR) of the over-all system is 95 dB at a sampling rate of 64 MHz. This modulator has a bandwidth of 2 MHz centered at 2 MHz. The power consumption of this receiver is 9.0 mW with a 1.8 V power supply.

  • Objective : This study was case report to show the good effects of herbal medicine (Daekumeum-ja) and acupuncture complex therapy for central pontine myelinolysis (CPM). Method : A 44-year-old man suffering from CPM was chronic alcoholic without electrolyte disturbance. He showed symptoms of dysarthria, dysphagia, dizziness, weakness of lower extremities and gait disturbance. CPM was diagnosed at his former hospital by MR imaging. He had no positive response to western treatments. We treated the patient with herbal extract (Daekumeum-ja) and acupuncture. Results : After our treatments, there was no change of Brain CT. But overall symptoms of CPM were improved. Conclusion : This study suggests that herbal medicine (Daekumeum-ja) and acupuncture complex therapy is probably effective in the treatment of CPM.

  • Knowledge of the complex biomechanical behavior of the human mandible is of great importance in various clinical situations. Various approaches can be used to evaluate the physical behavior of bone. In this study, we developed the voxel mesh program(Bionix) and generated FE models of mandible using Bionix and using handmade work and compared them with free vibration results derived from finite element analysis(FEA). The data of FE models based on DICOM File exported from Computed tomography(CT). Comparing the two models, we found a good correlation about mode type and natural frequency. The voxel based finite element mesh is a valid and accurate method to predict parameters of the complex biomechanical behavior of human mandibles.

Hevey Metal Levels in Urine of Residents in Industrial Area (일부 공단지역 주민의 요 중 중금속 농도에 관한 연구)

  • This study analyzed the concentration of the heavy metals(Cd, Hg, iAs) of urine(n=576) from May, 2007 to Oct 2007. The subject was residents in G, Y, H industrial area, Jeollanam-do, in which exposure due to the adjacency of the industrial complex. As to the heavy metal concentration in the urine of the residents in the whole exposed region and the comparing region, the content of cadmium, mercury, and inorganic arsenic in the exposed region group were 1.23, 1.85, and 8.80 ${\mu}g$/g_ct respectively, and those of the comparing region group were 1.87, 2.00, and 8.93 ${\mu}g$/g_ct respectively, which indicates that the concentration of the comparing group was higher than that of the exposed group. The heavy metal concentration for each age group increased in proportion to age except those under 10 for some substances(p<0.01). As to geometric mean concentration cadmium and inorganic arsenic in urine according to the smoking history of the subject, the concentration of the smoking group and the non-smoking group were 1.65 ${\mu}g$/g_ct and 9.13 ${\mu}g$/g_ct respectively, while those of the non-smoking group were 1.47 ${\mu}g$/g_ct and 8.91 ${\mu}g$/g_ct respectively, which indicates that the former is higher than the latter. As to the inorganic arsenic concentration in urine according to the food preference, in order of vegetable, fish, and meat showed high concentration (p<0.01). To clarify the factors affecting the heavy metal concentration in urine among the subjects, the multiple regression analysis was conducted. As a result, it turned out that as to cadmium content in urine, gender, age, drinking, and smoking have influence on the subjects, with explanatory adequacy of 37.5 %.

Comparative Analysis of Surgical Outcomes of C1-2 Fusion Spine Surgery between Intraoperative Computed Tomography Image Based Navigation-Guided Operation and Fluoroscopy-Guided Operation

  • Objective : Fixation of the C1-2 segment is challenging because of the complex anatomy in the region and the need for a high degree of accuracy to avoid complications. Preoperative 3D-computed tomography (CT) scans can help reduce the risk of complications in the vertebral artery, spinal cord, and nerve roots. However, the patient may be susceptible to injury if the patient's anatomy does not match the preoperative CT scans. The intraoperative 3D image-based navigation systems have reduced complications in instrument-assisted techniques due to greater accuracy. This study aimed to compare the radiologic outcomes of C1-2 fusion surgery between intraoperative CT image-guided operation and fluoroscopy-guided operation. Methods : We retrospectively reviewed the radiologic images of 34 patients who underwent C1-2 fusion spine surgery from January 2009 to November 2018 at our hospital. We assessed 17 cases each of degenerative cervical disease and trauma in a study population of 18 males and 16 females. The mean age was 54.8 years. A total of 139 screws were used and the surgical procedures included 68 screws in the C1 lateral mass, 58 screws in C2 pedicle, nine screws in C2 lamina and C2 pars screws, four lateral mass screws in sub-axial level. Of the 34 patients, 19 patients underwent screw insertion using intraoperative mobile CT. Other patients underwent atlantoaxial fusion with a standard fluoroscopy-guided device. Results : A total of 139 screws were correctly positioned. We analyzed the positions of 135 screws except for the four screws that performed the lateral mass screws in C3 vertebra. Minor screw penetration was observed in seven cases (5.2%), and major pedicle screw penetration was observed in three cases (2.2%). In one case, the malposition of a C2 pedicle screw was confirmed, which was subsequently corrected. There were no complications regarding vertebral artery injury or onset of new neurologic deficits. The screw malposition rate was lower (5.3%) in patients who underwent intraoperative CT-based navigation than that for fluoroscopy-guided cases (10.2%). And we confirmed that the operation time can be significantly reduced by surgery using intraoperative O-arm device. Conclusion : Spinal navigation using intraoperative cone-beam CT scans is reliable for posterior fixation in unstable C1-2 pathologies and can be reduced the operative time.
