조영법을 이용한 뇌수막종에서 3차원 CT영상의 유용성 (Usefulness of Three-Dimensional CT Image in Meningioma Using Contrast Method)

  • 본 연구에서는 뇌수막종이 늦게 조영증강 된다는 점을 착안하여 뇌수막종을 최대 조영 증강시킨 검사를 시작하였다, 검사 후에 두개내의 수막종과 혈관과의 관계를 알아보고 전산화 단층 촬영 조영술의 3차원 CT 영상 기법(이하, 3차원 CT 영상 기법)과 고식적 혈관 촬영 조영술의 영상을 비교하였다. 연구대상은 3차원 CT 영상 기법과 고식적 혈관 촬영 조영술을 병행 시행하였던 6명의 환자를 대상으로 하였으며. 그 중 추체접형골동사대부 5예, 방시상동에 생긴 1예였다. 검사방법으로는 조영제를 초당 3/120 ml를 주입하여, GE Medical System Program(smart prep)을 이용하여 CT Number 값이 100에 도달한 후, 검사를 시작하였다. 스캔파라미터는 조영제가 모두 주입된 직후에 1.25 mm / 3.75 mm, HQ-Mode로 scan한 후 1 mm interval로 재구성하였다. 검사한 영상을 3D-Med Software Program(Rapidia)을 이용하여 3차원 CT 영상 기법으로 영상을 재구성한 다음 고식적 혈관 촬영 조영술과 비교하였다. 결과를 살펴보면 뇌수막종의 최대조영 시간은 조영제을 주입한 후 약120-180초에 혈관과 종양과의 관계를 가장 잘 구별할 수 있었다. 3차원 CT 영상 기법으로 재구성한 6예는 모두 종양과 혈관과의 관계를 잘 구별할 수 있었다. 또한 종양과 동반된 동맥류도 1예에서 잘 보여주었다. 이를 종합하여 보면 두 개 내 뇌수막종의 환자에서 조영제를 주입하여 3차원 CT 영상을 시행하였던 영상은 종양과 혈관과의 관계를 입체적으로 구분할 수 있어 수술에 많은 도움이 되었다.

고속 3차원 CT/CTA 영상 정합 기법 및 DS-CTA 응용 (Fast 3D CT/CTA Image Registration and its Application to DS-CTA)

  • 이 논문에서는 3 차원 CT/CTA 영상 데이터에 대하여 고속 자동 정합 기법을 제안한다. 제안하는 기법은 다해상도 (multi-resolution) 구조의 정규 상호 정보량(normalized mutual information) 을 최대화하는 정합 방식에서, 정합 유사도를 계산하는 볼륨 영역을 효율적으로 줄여 정합 속도를 증가시키는 방법이다. 제안된 정합방식을 CT/CTA (CT angiography) 팬텀 데이터와 7 세트의 실제 CT/CTA 임상 데이터에 적용하여 테스트하였다. 이로부터 제안하는 방식이, 정합 정확도를 유지하는 동시에 정합 속도를 10 ∼ 60% 로 감소시킴을 확인 할 수 있었다. 또한 제안된 정합 방식을 DS-CTA (digital subtraction CT angiography) 에 적용하여, CT/CTA 영상으로부터 혈관 영상을 성공적으로 추출하였다.

  • Coronary computed tomography angiography (CCTA) is routinely used for anatomical assessment of coronary artery disease (CAD). However, invasive measurement of fractional flow reserve (FFR) is the current gold standard for the diagnosis of hemodynamically significant CAD. CT-derived FFRCT and CT perfusion are two emerging techniques that can provide a functional assessment of CAD for risk stratification and clinical decision making. Several clinical studies have shown that the diagnostic performance of concomitant CCTA and functional CT assessment for detecting hemodynamically significant CAD is at least non-inferior to that of other routinely used imaging modalities. This article aims to review the current clinical evidence and recent developments in functional CT techniques.

  • Pseudoaneurysm arising from the superficial temporal artery (STA) is a rare and potentially critical cause of palpable mass. Most pseudoaneurysms form as a result of blunt trauma and present as painless, pulsatile mass that may be associated with pathologic finding and enlarged size. We report a rare case of pseudoaneurysm arising from STA caused by blunt injury and diagnosed by 3-dimensional computed tomography (CT) angiography.

  • The aim of this work is to develop 3-D stereotactic localization system in order to determine the precise shape, size and location of the lesion in the brain in the field of Stereotactic Radiosurgery(SRS) and neurosurgery using CT/MRI/angiography and multi-purpose QA phantom. The algorithms to obtain a 3-D stereotactic coordinates of the target have been developed, and targets on each CT image were superimposed each other on MR/angiography images without distortion corretion. This system was implented in Visual C++ as a PC-based application program.

Congenital portal vein aplasia with portocaval shunting in two dogs

  • Two dogs presented with vomiting and head pressing. In both dogs, a large vessel was revealed in computed tomography (CT) angiography, which was found to leave the portal vein (PV) cranial to the splenomesenteric confluence and enter the pre-hepatic caudal vena cava cranial to the right renal vein. The flow of portal blood to the liver was not identified. Based on CT angiography, the dogs were suspected to have congenital PV aplasia with portocaval shunting. Diagnostic imaging of potential malformations for PV continuation should be conducted before attempting shunt closure.

  • A woman who had a spontaneous subarachnoid hemorrhage [SAH] and temporal intracerebral hemorrhge [ICH] without any causative lesions on computed tomography [CT] and digital angiography at the day of the stroke. She was considered to have an angiographically negative SAH and scheduled for a repeated angiography. While she was waiting for the next study, she developed a second hemorrhage. CT angiography showed an aneurysmal shadow in the course of the posterior cerebral artery. After the operation, the aneurysm proved to be a pseudoaneurysm.

Distal Middle Cerebral Artery M4 Aneurysm Surgery Using Navigation-CT Angiography

  • Unruptured non-traumatic dissecting aneurysm in the M4 segment of the middle cerebral artery (MCA) accompanied by complete occlusion of the ipsilateral internal cerebral artery (ICA) has never been reported. A 41-year-old man presented with an infarction manifesting as left-sided weakness and dysarthria. Magnetic resonance angiography revealed a subacute stage infarction in the right MCA territory and complete occlusion of the right ICA. Angiography demonstrated aneurysmal dilatation of the M4 segment of the right MCA. Surgery was performed to prevent hemorrhage from the aneurysm. The aneurysm was proximally clipped guided by Navigation-CT angiography and flow to the distal MCA was restored by superficial temporal artery-middle cerebral artery (STA-MCA) anastornosis. We report this rare case with literature review.

  • In clinical practice, CT Angiography is a powerful technique for the visualziation of blood flow in arterial vessels throughout the body. However CT Angiography images of blood vessels anywhere in the body may be fuzzy if the patient moves during the exam. In this paper, we propose a novel technique for removing global motion artifacts in the 3D space. The proposed methods are based on the two key ideas as follows. First, the method involves the extraction of a set of feature points by using a 3D edge detection technique based on image gradient of the mask volume where enhanced vessels cannot be expected to appear, Second, the corresponding set of feature points in the contrast volume are determined by correlation-based registration. The proposed method has been successfully applied to pre- and post-contrast CTA brain dataset. Since the registration for motion correction estimates correlation between feature points extracted from skull area in mask and contrast volume, it offers an accelerated technique to accurately visualize blood vessels of the brain.
