Mini-pig 하악골에서 저강도진동초음파가 골형성에 미치는 영향 (The Effect of Low-intensity Pulsed Ultrasound on Osteogenesis in Mini-pig Mandibles)

  • Purpose: The objective of this study was to examine the affected period and the amount of bone formation during osteogenesis of intramembranous bone using low-intensity pulsed ultrasound (LPUS) $in$ $vivo$. Methods: Xeno-bone (Bio-oss) and autogenous bone were grafted bilaterally into mini-pig mandibles. The left mandible served as the control and the other mandible was treated with 3 MHz, 160 mW (output, 0.8 mW) ultrasound stimulation for 7 days 15 minutes per day. The mini-pigs were sacrificed at 1, 2, 4, and 8 weeks, and micro computed tomography (${\mu}CT$), a microscopic examination, and a statistical analysis were performed on the specimens. Results: Based on a computerized image analysis of the ${\mu}CT$ scans, the experimental group had an average 150% more new bone formation than that in the control group. The effect of LPUS continued during the post operative 2 weeks. The histomorphological microscopic examination showed similar results. Conclusion: Our results suggest the LPUS had an effect on early intramembranous bone formation in vivo.

3D CACT-assisted Radiofrequency Ablation Following Transarterial Chemoembolization for Hepatocellular Carcinoma: Early Experience

  • Background: To explored the value of 3D C-arm CT (CACT) guidance system in performing radiofrequency ablation (RFA) following transarterial chemoembolizationon (TACE) for hepatocellular carcinomas. Materials and Methods: RFA of hepatocellular carcinomas (HCC) were performed on 15 patients (21 lesions) with the assistance of CACT guidance system. Technical success, procedure time, complications and patient radiation exposure were investigated. The puncture performance level was evaluated on a five-point scale (5-1: excellent-poor). Complete ablation rate was evaluated after two months follow-up using enhanced CT scans. Results: The technical success rate of RFA procedure under CACT navigation system was 100 %. Mean total procedure time was $24.24{\pm}6.53min$, resulting in a mean effective exposure dose of $15.4{\pm}5.1mSv$. The mean puncture performance level rated for CACT guided RFA procedure was $4.87{\pm}0.35$. Complete ablation (CA) was achieved in 20 (95.2%) of the treated 21 tumors after the first RFA session. None of patients developed intra-procedural complications. Conclusions: 3D CACT guidance system enables reliable and efficient needle positioning by providing real-time intraoperative guidance for performing RFA on HCCs.

응급의료서비스를 통해 내원한 뇌출혈 환자의 주취 유무에 따른 KTAS Level과 검사시간의 차이 (Emergency department triage and medical process according to alcohol intoxication in brain hemorrhage)

  • Purpose: Alcohol intoxication is frequently observed in patients with brain hemorrhage. The purpose of this study was to determine whether intoxication affects the Korean Triage and Acuity Stage (KTAS) level and the emergency medical process in emergency departments. Methods: This study was a retrospective observational study enrolled 253 brain hemorrhage patients (47 of those intoxicated) who visited the emergency medical center on public EMS ambulance from January. 1, 2017 to April, 30, 2019. Data were collected through the electronic medical record (EMR). KTAS level and time to computerized tomography (CT) were compared to evaluate whether inebriation affects care and examination processes. All data were analyzed using SPSS program. Results: Of the 47 patients intoxicated patients, 85.1% were male, and 74.5% accompanied by trauma. Initial KTAS level showed significant differences (77.2%; p=.000) when the level 3,4 was not drunk. The average time taken from triage to CT scans showed a significant difference of 24.81±23.72 (min) when the drunken state was not 58.38±56.54 (min)(p=.000). Conclusion: In patients with brain hemorrhage admitted to ED from public EMS, undertriage and delay after initial assessment were detected in inebriated patients. Careful initial evaluation and prompt medical response should be considered for patients transported by EMS.

Ultrasonographic and Computed Tomographic Appearance of Spontaneous Cutaneous Fistula Resulted from Retained Surgical Gauze in a Dog

  • A 6-year-old, spayed female Maltese was presented with the condition of a chronic recurrent abscess formation in the left flank region. Despite the antibiotics and drainage therapy given to the dog, the lesion formed a continued serosanguineous to the point that a purulent discharge was evident. In the meantime, an abdominal ultrasound revealed the presence of a well-defined mass with a hypoechoic outer margin, and a hyperechoic inner rim in the cranial of the kidney. A fistula was noted as being present with a connection between the subcutaneous lesion of the left flank and the abdominal mass. It is emphasized that CT scans revealed the existence of a soft tissue dense mass with low attenuation area, as seen in some internal areas and also a peripheral contrast enhancement was noted within a nonenhancing central region. There was additional nonenhancing fluid found dorsal to the inflammatory tract passing under the epaxial muscles and at the peritoneum. Likewise, the tract exited the skin surface in the left flank. A tentative diagnosis of an abdominal abscess with spontaneous cutaneous fistula was made based on the ultrasonographic and CT appearances. A foreign body such as surgical gauze should always be considered a potential cause of draining tract in small animals, as was considered to be the problem in this case.

Pulmonary Metastases of Alveolar Soft-Part Sarcoma: CT Findings in Three Patients

  • Alveolar soft-part sarcoma is a rare soft tissue sarcoma of young adults with unknown histogenesis, and the organ most frequently involved in metastasis is the lung. We report the CT findings of three patients of pulmonary metastases of alveolar soft-part sarcoma, which manifested as clearly enhanced pulmonary nodules or masses. On enhanced scans, some of the masses were seen to contain dilated and tortuous intratumoral vessels.

Enhanced CT-image for Covid-19 classification using ResNet 50

  • Disease caused by the coronavirus (COVID-19) is sweeping the globe. There are numerous methods for identifying this disease using a chest imaging. Computerized Tomography (CT) chest scans are used in this study to detect COVID-19 disease using a pretrain Convolutional Neural Network (CNN) ResNet50. This model is based on image dataset taken from two hospitals and used to identify Covid-19 illnesses. The pre-train CNN (ResNet50) architecture was used for feature extraction, and then fully connected layers were used for classification, yielding 97%, 96%, 96%, 96% for accuracy, precision, recall, and F1-score, respectively. When combining the feature extraction techniques with the Back Propagation Neural Network (BPNN), it produced accuracy, precision, recall, and F1-scores of 92.5%, 83%, 92%, and 87.3%. In our suggested approach, we use a preprocessing phase to improve accuracy. The image was enhanced using the Contrast Limited Adaptive Histogram Equalization (CLAHE) algorithm, which was followed by cropping the image before feature extraction with ResNet50. Finally, a fully connected layer was added for classification, with results of 99.1%, 98.7%, 99%, 98.8% in terms of accuracy, precision, recall, and F1-score.

Significantly Low Effective Dose from 18FDG PET/CT Scans Using Dose Reducing Strategies: "Lesser is Better"

  • Background: Fluorodeoxyglucose ($^{18}FDG$) PET/CT imaging has become an important component of the management paradigm in oncology. However, the significant imparted radiation exposure is a matter of growing concern especially in younger populations who have better odds of survival. The aim of this study was to estimate the effective dose received by patients having whole body $^{18}F$-FDG PET/CT scanning as per recent dose reducing guidelines at a tertiary care hospital. Materials and Methods: This prospective study covered 63 patients with different cancers who were referred for PET/CT study for various indications. Patients were prepared as per departmental protocol and 18FDG was injected at 3 MBq/Kg and a low dose, non-enhanced CT protocol (LD-NECT) was used. Diagnostic CT studies of specific regions were subsequently performed if required. Effective dose imparted by 18FDG (internal exposure) was calculated by using multiplying injected dose in MBq with coefficient $1.9{\times}10^{-2}mSv/MBq$ according to ICRP publication 106. Effective dose imparted by CT was calculated by multiplying DLP (mGy.cm) with ICRP conversion coefficient "k" 0.015 [mSv / (mG. cm)]. Results: Mean age of patients was $49{\pm}18$ years with a male to female ratio of 35:28 (56%:44%). Median dose of 18FDG given was 194 MBq (range: 139-293). Median CTDIvol was 3.25 (2.4-6.2) and median DLP was 334.95 (246.70 - 576.70). Estimated median effective dose imparted by $^{18}FDG$ was 3.69 mSv (range: 2.85-5.57). Similarly the estimated median effective dose by low dose (non-diagnostic) CT examination was 4.93 mSv (range: 2.14 -10.49). Median total effective dose by whole body 18FDG PET plus low dose non-diagnostic CT study was 8.85 mSv (range: 5.56-13.00). Conclusions: We conclude that the median effective dose from a whole body 18FDG PET/CT in our patients was significantly low. We suggest adhering to recently published dose reducing strategies, use of ToF scanner with CT dose reducing option to achieve the lower if not the lowest effective dose. This would certainly reduce the risk of second primary malignancy in younger patients with higher odds of cure from first primary cancer.

PET/CT에서 방사성 의약품 주입량이 방사선 피폭에 미치는 영향분석: 초기연구 (Analysis of Relationship Between Injection Dose and Exposure Dose in PET/CT Scan: Initial Study)

  • PET/CT 검사에서 $^{18}F$-FDG가 가장 널리 이용되며, 장비의 물리적 특성에 따라 환자 주입 $^{18}F$-FDF량이 다르게 권고되고 있다. 또한 검사 특성상 방사선종사자와 환자의 접촉으로 인하여 방사선의 피폭이 불가피하기에 본 연구에서는 각기 다른 PET/CT 장비를 대상으로 환자에게 주입되는 $^{18}F$-FDG가 방사선종사자에게 미치는 피폭선량과의 관계를 분석하였다. 총 3대의 각각 다른 PET/CT(Scanner1(S1): 0.15 mCi/kg, Scanner2(S2): 0.17 mCi/kg, Scanner3(S3): 0.12 mCi/kg)를 대상으로 각 장비에 숙련도를 고려하여 총 6명의 방사선종사자를 5개월간 순환근무하였고, 하루에 검사하는 환자수를 일정하게 유지하였다. 또한 검사진행 방법을 유사하게 유지하고, 방사선종사자의 개인피폭선량계인 열형광유리선량계(TLD)를 매월 판독하여 분석하였다. 개인의 월별 평균 피폭선량은 방비에 따라 S1은 0.76 mSv, S2는 0.93 mSv, S3는 0.47 mSv였다. 피폭선량은 개인 최대 1.12 mSv, 최저 0.42 mSv로 숙련도와 경험에 따라 유의한 차이를 보였고, 또한 각 주입량에 따른 PET/CT의 종류에 따라 피폭선량은 유의한 상관관계를 나타냈다. 본 연구를 통하여 주입 $^{18}F$-FDG가 적을수록 방사선종사자의 피폭선량이 낮았다. 또한 개인 숙련도에 따라 피폭선량이 감소하였으나, 장비의 특성에 따라 적은 방사선의약품 주입량의 영향이 방사선종사자의 피폭선량을 현저하게 감소할 수 있기에 이에 대한 연구가 보다 활성화 되어야 할 것이다.

Cardiac dose reduction with breathing adapted radiotherapy using self respiration monitoring system for left-sided breast cancer

  • Purpose: To quantify the cardiac dose reduction during breathing adapted radiotherapy using Real-time Position Management (RPM) system in the treatment of left-sided breast cancer. Materials and Methods: Twenty-two patients with left-sided breast cancer underwent CT scans during breathing maneuvers including free breathing (FB), deep inspiration breath-hold (DIBH), and end inspiration breath-hold (EIBH). The RPM system was used to monitor respiratory motion, and the in-house self respiration monitoring (SRM) system was used for visual feedback. For each scan, treatment plans were generated and dosimetric parameters from DIBH and EIBH plans were compared to those of FB plans. Results: All patients completed CT scans with different breathing maneuvers. When compared with FB plans, DIBH plans demonstrated significant reductions in irradiated heart volume and the heart $V_{25}$, with the relative reduction of 71% and 70%, respectively (p < 0.001). EIBH plans also resulted in significantly smaller irradiated heart volume and lower heart $V_{25}$ than FB plans, with the relative reduction of 39% and 37%, respectively (p = 0.002). Despite of significant expansion of lung volume using inspiration breath-hold, there were no significant differences in left lung $V_{25}$ among the three plans. Conclusion: In comparison with FB, both DIBH and EIBH plans demonstrated a significant reduction of radiation dose to the heart. In the training course, SRM system was useful and effective in terms of positional reproducibility and patient compliance.

A Case of Delayed Diagnosis of Pulmonary Paragonimiasis due to Improvement after Anti-tuberculosis Therapy

  • Here, we report a case of pulmonary paragonimiasis that was improved with initial anti-tuberculosis (TB) therapy but confused with reactivated pulmonary TB. A 53-year-old Chinese female presented with a persistent productive cough with foul smelling phlegm and blood streaked sputum. Radiologic findings showed subpleural cavitary consolidation in the right upper lobe (RUL). Bronchoscopic and cytological examination showed no remarkable medical feature. She was diagnosed with smear-negative TB, and her radiologic findings improved after receiving a 6-month anti-TB therapy. The chest CT scans, however, obtained at 4 months after completion of anti-TB therapy showed a newly developed subpleural consolidation in the RUL. She refused pathologic confirmation and was re-treated with anti-TB medication. Nevertheless, her chest CT scans revealed newly developed cavitary nodules at 5 months after re-treatment. She underwent thoracoscopic wedge resection; the pathological examination reported that granuloma caused by Paragonimus westermani. Paragonimiasis should also be considered in patients assessed with smear-negative pulmonary TB.
