Umbilical Cord Blood Transplantation

  • The number of umbilical cord blood transplantation is increasing worldwide as it has expanded the ability of the transplantaion community to meet the growing needs of their patients. Clinical data over the last decade show promising results in transplantation using both related as well as unrelated cord bloods. Cord blood banks are essential for the clinical use for transplantation and are now established around the world with the major efforts to standardize banking in collection, processing and distribution of cord blood for providing the highest quality stem cells for the patients. In Korea, Medipost, Histostem and some regional cord blood banks were established some years ago and collected thousands of cord blood for public but it had some limitations and was not expanded as the cord blood transplantation was not covered by medical insurance. Recently with the change in the policy of medical insurance to cover the cord blood transplantation, several venture companies are showing great interests in cord blood banking and trying to establish private cord blood banks in Korea. This review article discusses the current status of cord blood transplantaion and also the clincial use of stem cells from cord blood.

Unilateral vocal cord paralysis after open heart surgery -A report of 2 cases- (개심술후 발생한 일측성 성대마비 -2례 보고-)

  • We have experienced 2 cases vocal cord paralysis after open heart surgery. One was a postoperatively developed right unilateral vocal cord paralysis after prosthetic mitral valve replacement with tricuspid valve annuloplasty. The other was a postoperative left unilateral vocal cord paralysis after prosthetic aortic and mitral valve replacement with tricuspid annuloplasty. They were intubated for forty-eight and seventy-two hours but after extubation complained of hoarseness, aphonia, anxiety, and ineffective coughing Indirect laryngoscopy performed at about postoperative one week, revealed partial paralysis and decreased mobility of the vocal cord. After active phonation therapy, symptoms were improved gradually and in the follow up indirect laryngoscopy, the vocal cord paralysis was improved. The symptoms were recovered completely at about postoperative one month in both. The cause of vocal cord paralysis after open heart surgery may be any retraction or stretching injury to the recurrent laryngeal nerve, especially right side, during median sternotomy retraction and open heart operation procedures. As a result, avoid of excessive spread of median sternotomy retractor and excessive manipulation and retraction of the heart during open heart procedures will reduce the occurrence of the vocal cord paralysis.

Comparison of Tension According to the Type of Sling Cord during the Bridging Exercise with Sling in Stroke Patients (뇌졸중 환자의 교각 운동 시 슬링 줄의 종류에 따른 장력 비교)

  • Objectives The purpose of this study was to examine comparison of tension according to the type of sling cord during the bridging exercise with sling in stroke patients. Methods Twenty six stroke patients were recruited for this study. Subjects were randomly performed sling bridging exercise with three types of sling cords such as nonelastic cord with affected side-nonelastic cord with non affected side (NC-NC), nonelastic cord with affected side-elastic cord with non affected side (NC-EC), and elastic cord with affected side-nonelastic cord with non affected side (EC-NC). They were measured tension with a tensiometer of sling cord during the bridging exercise with sling. Results The tension of sling cord was significantly different in affected side, non affected side, and tension ratio of affected side/non affected side. There were significant differences tension in NC-EC from NC-NC and EC-NC and the NC-EC method was increased tension of affected side and decreased tension of non affected side. Conclusions These results showed that the NC-EC method was improved the symmetry of affected side and non affected side. Sling exercise with appropriate type of sling cord should be increased activities of affected side and improved recovery in stroke patients.

Estimation of Incidence Rate and Factors Related to Occurrence of Traumatic Spinal Cord Injury in Seoul (서울지역 외상성 척수손상환자의 발생률 추정 및 발생관련요인)

  • For the purpose of disclosing estimated annual incidence rate and causative factors of traumatic spinal cord injury and obtaining basic data for the establishment of effective measures to prevent this injury, the medical record of 204 cases of Seoul residents with spinal cord injury which admitted in 46 general hospitals and National rehabilitation center located in Seoul, including Sam Yook Rehabilitation Center and Jung Ang Hospital in Kyungi do and occurred in 1995 were analyzed. The results were as follows: 1) The estimated annual incidence rate of traumatic spinal cord injury in 1995 of Seoul residents was 20.5 per million population. This incidence rate was the highest in the 40th years (34.1), followed by in the 50th years (26.5) and in the 20th years (25.6) in descending order. 2) The incidence rate ratio between male and female was 3.5:1. 3) The leading cause of injury was the highest in traffic accidents (42.2%), followed by falls from elevation (29.4%) and falls on the same level (9.8%). 4) Traffic accident mainly occured from 20th years to 40th years and falls from elevation in 50th years. 5) The level of spinal cord injury was the highest in cervical cord (63.2%), followed by the thoracic cord (21.1%) and lumbosacral cord (15.7%). Traffic accidents mainly caused in cervical cord injury whereas thoracic cord injury and lumbosacral cord injury were caused from falls from elevation. Above theconsiderations in mind, it suggested that in order to prevent traumatic spinal cord injuries, safety education of falls from elevation should be focused on the 20th years and 50th years male, moreover safety education of traffic accidents will be needed in the 30th years and 40th years male.

Role of Dehydrocorybulbine in Neuropathic Pain After Spinal Cord Injury Mediated by P2X4 Receptor

  • Chronic neuropathic pain is one of the primary causes of disability subsequent to spinal cord injury. Patients experiencing neuropathic pain after spinal cord injury suffer from poor quality of life, so complementary therapy is seriously needed. Dehydrocorybulbine is an alkaloid extracted from Corydalis yanhusuo. It effectively alleviates neuropathic pain. In the present study, we explored the effect of dehydrocorybulbine on neuropathic pain after spinal cord injury and delineated its possible mechanism. Experiments were performed in rats to evaluate the contribution of dehydrocorybulbine to P2X4 signaling in the modulation of pain-related behaviors and the levels of pronociceptive interleukins and proteins after spinal cord injury. In a rat contusion injury model, we confirmed that chronic neuropathic pain is present on day 7 after spinal cord injury and P2X4R expression is exacerbated after spinal cord injury. We also found that administration of dehydrocorybulbine by tail vein injection relieved pain behaviors in rat contusion injury models without affecting motor functions. The elevation in the levels of pronociceptive interleukins ($IL-1{\beta}$, IL-18, MMP-9) after spinal cord injury was mitigated by dehydrocorybulbine. Dehydrocorybulbine significantly mitigated the upregulation of P2X4 receptor and reduced ATP-evoked intracellular $Ca^{2+}$ concentration. Both P2XR and dopamine receptor2 agonists antagonized dehydrocorybulbine's antinociceptive effects. In conclusion, we propose that dehydrocorybulbine produces antinociceptive effects in spinal cord injury models by inhibiting P2X4R.

Knowledge and Attitude Regard to Cord Blood of Early Postpartum Women after Donating Cord Blood or Storing Cord Blood (제대혈 기증.보관한 산부의 제대혈 관련 지식 및 태도)

  • Purpose: There is increasing necessity of using cord blood cell worldwide. In order to improve the effectiveness of cord blood's usage in Korea, the study of knowledge and attitude regarding cord blood is needed. Method: As descriptive correlation study, data was collected from 212 early postpartum women who stored or donated cord blood from 2007 April to July. Data was analyzed using $X^2$-test, t-test, ANOVA, and logistic regression. Result: Total score of knowledge and attitude of early postpartum women who donated cord blood was higher than who stored. According to the source of education and the source of influence toward decision of using cord blood, early postpartum women who educated or influenced by mass media showed difference in attitude. Job status and income, and attitude of cord blood were the predictor of cord blood donation. Conclusion: Attitude regard to cord blood affect to usage of cord blood, and the attitude and knowledge of cord blood in early postpartum women who donated it higher than who stored.

  • Purpose : Tethered cord syndrome is characterized by progressive motor and sensory disturbances in lower extremities, foot deformities caused by a pathologic fixation of spinal cord resulting in excessive stretching of the spinal cord. It is also frequently associated with urological symptoms include urinary frequency, incontinence, enuresis, urgency and recurrent urinary tract infection. Because there is few report in the literature about urological manifestations of theterd cord syndrome, we conducted a retrospective study on the patients diagnosed as tethered cord syndrome to delineate the characteristics of urologic manifestations in tethered cord syndrome and to establish the policy to evaluate patients who is suspected of tethered cord syndrome. Method : A retrospective study was conducted by reviewing the medical records of nine patients who was diagnosed as tethered cord syndrome from November 1991 to July 1996 in Yeungnam University Hospital. Result : 1) The age distribution of nine patients was as follows; 5 patients were under 2 years, 1 case from 2 to 6 years and 3 cases from 6 to 10 years. 2) Of 9 patients 6 had voiding frequency, urinary incontinence, enuresis, urgency and loss of micturation sense. Radiologic urodynamic studies revealed neurogenic bladder in 5 patients, hydronephrosis and hydroureter in 3, vesicoureteral reflux in 3. 3) Of 6 patients with urological abnormal manifestations 4 underwent spinal cord surgery (detethering). In spite of surgical intervention, the urological manifestations improved in only one patient. Conclusion : Urological abnormalities were common in tethered cord syndrome. Abnormal laboratory findings including urodynamic study were found even before the onset of urological symptoms. We have to concern tethered cord syndrome as one of common causes of voiding frequency and enuresis and to evaluate urological abnormalities as thetered cord syndrome is diagnosed.

The Clinical Study On 1 Case for The sensation of patient with Spinal Cord Injury whose is improved by using sweet BV (Sweet BV 병행 치료 후 척수 손상 환자의 감각 분절 호전 1례)

  • Objective : Patients with spinal cord injury are increasing in numbers. However, there is no reliable treatment guide in both conventional & complementary medicine. Also, there are not much clinical case of patients with spina cord injury in oriental medical field. We investigated effect of sweet BV on subacute stage patient with spinal cord injury. Method : 31-year old female patient with spinal cord injury was treated with herb medicine(TID), electro arcupunture (BID), sweet BV injection(QOD), Physical treatment(QD), and conventional-medicine. Result : We had a satisfactory result with using sweet BV injection. The patient's ASIA grade improved from 34 to 52. And Frankle classification of the patient shifted from A to B. Conclusion : We reach a conclusion Using Sweet BV improve the sensation of patient with spinal cord injury. And more study about this disease is needed.

Isokinetic Evaluation Comparison of Shoulder Girdle Muscles in the Spinal Cord Injury Group and Normal Young Adults Group (척수 손상 환자와 정상 성인과의 견관절 주위근에 대한 등속성 근력 평가 비교)

  • The purpose of this studied was the comparison with shoulder girdle muscles of isokinetic evaluation in the spinal cord injury group, and adults normal group. Data was collected from 20 cases spinal cord injury from January 10. 1994 to February 10, 1995 in a Chungnam University hospital and adults normal group to 20 cases. The results were as follows; 1. Low speeds peak torque was higher spinal cord group than normal group shoulder girdle muscles of flexor, extensor, external rotator showed statistically significant in spinal cord injury group(p<0.05). Against normal groups shoulder girdle muscles of internal rotator showed statistically significant in normal group(p<0.05). 2. High speeds peak torque was higher spinal cord group than normal group shoulder girdle muscles of flexor, extensor, showed statistically significant in spinal cord injury group(p<0.05). 3. High speeds total work was higher spinal cord group than normal group shoulder girdle muscles of flexor, extensor, external rotator showed statistically significant in spinal cord injury group(p<0.05).(p<0.01). Against normal group shoulder girdle muscles of internal rotator showed statistically significant in normal group(p<0.05).

A Study of the Evaluation of the Spinal Cord Injuries (척수 손상 환자에 관한 실태 조사)

  • The incidence of spinal cord injury increase due to traffic accident, industrial accident and leisure sports. Spinal cord injury damages motor and sensory function below the injury level, also affects autonomic functions associated with voiding and defecation. Sexual dysfunction and psychosocial, vocational maladaptations are also some of the unwanted consequences of injury. The purpose of this study is look for means to prevent and to manage complications in spinal cord injury through investigation and analysis. The subjects of this study in spinal cord injured patients were admitted to the department of physical therapy, Kwangju christian Hospital, Nam Kwang Hospital, Chun Nam university Hospital and Cho Sun university Hospital, from April, 1, 1995 to March, 31, 1996. The results are as follows: 1. The subjects comprised 96 cases of spinal cord injury, ranging from 17 to 85(mean-40.8 yrs) and included 72 males and 24 females. Among these patients, 58 were cervical injury, 20 were thoracic injury and 18 were lumbar injury. 2. As for a major causative of spinal cord injury were traffic accident(59.4%), fall down (27.1 %), and motocycle(4.2%).. 3. The bladder control were taken by indwelling cathetar(41.7%), Crede maneuver(37.5%) and self voiding(16.7%). The bowel control were taken by all aid(61.5%), assitance(32.3%) and self defecation(6.2%). 4. Possible of sexual function were 35 cases (47.9%). 5. The device of transfer used wheel chair(69.8%) and bed(16.7%). 6. The patients with higher cord lesion got more serious pain than lower cord lesion. Also the patients with higher cord lesion got a serious spasticity. 7. The incidence of decubitus ulcer among 96 patients were in case 46(47.9%). The largest group of the pressure sore sites were sacral portion(82.0%), less than 1 month of onset occured a large numbers(50%). Incidence of pressure sore by spasticity occured many patients in case of mild or moderate. Incidence of pressure sore by pain occured many patients in case of severe pain.
