Analysis Software based on Center of Pressure to Improve Body Balance using Smart Insole

  • Body balance necessary for ordinary daily activities can be undermined by diverse causes. In this study, as a way to control such a problem, we have produced smart insole as a wearable device in the form of insole and developed analysis software evaluating body balance, which measures ground reaction force applied to each area of sole and Center of Pressure (COP). The software visualized changes in COP positions while a user was moving and average COP positions, and it is also capable of measuring the COP values in the Anterior-Posterior (AP) and Medial-Lateral (ML) areas of feet. Through gait analysis, it can analyze the time of walking, strides, speed, COP trajectory while walking, etc. In addition, we have developed training contents for body balance improvement designed in consideration of Y-Balance Test and Timed Up and Go (TUG) Test. They were established in virtual reality similar to daily living environment so that people can expect more effective training results regardless of places.

요통 환자의 요추 주변 근육 단면적과 신체안정성(Stability), 체중심(Center of Pressure)과의 관련성 : 동적평형검사(Tetrax)를 중심으로 (The Association between Cross-section of Lumbar Regional Muscle analyzed by MRI and Stability, Center of Pressure assessed by Tetrax)

  • Objectives : To identify the association between cross-sectional area of lumbar regional muscle and stability(ST), center of pressure(COP) assessed by tetrax. Methods : Patients(n=55) who had taken L-SPINE MRI and Tetrax(Sunlight, Israel) were analyzed retrospectively. To evaluate the cross-sectional area of lumbar regional muscle, L-SPINE MRI was used. Data of ST, COP were accumulated by using dynamic equilibrium analysis by Tetrax. Of the lumbar regional muscles, the Psoas muscles and the Paraspinalis muscles were examined. Using Pearson correlation, we analyzed COP, ST and the difference between the cross-sectional area of lumbar regional muscles. Results : The variance of cross-sectional area of both sides of Psoas muscle and COP had moderate positive correlation(r=0.621). Between variance of cross-sectional area of both sides of Paraspinalis and lumbar regional muscle and COP there was low positive correlation(r=0.287, r=0.329) ST also had low correlation with variance of cross-sectional area of both sides of Psoas muscle. Conclusion : The variance of cross-sectional area between both sides of Psoas muscle had moderate correlation with COP.

족압검사를 이용한 만성 발목 불안정성과 압력중심의 연관성 (Correlation between Chronic Ankle Instability and Center of Pressure Using Pedobrarograph)

  • Purpose: Chronic ankle instability is a very common abnormality of the ankle, but there is still controversy regarding its evaluation criteria. The stress view has difficulties in reflecting the patient's symptoms and treatment progress. Therefore, this study examined the relationship between the center of pressure (COP) measured by a pedobarograph and the symptoms of the patient. Materials and Methods: Thirty patients with chronic ankle instability from February to August 2018 were included. Each patient was surveyed with the foot and ankle outcome score (FAOS). The COP was measured with a foot pressure scanner, and the travel distance and ellipse area of the COP were calculated. Each patient was measured on one foot and on two feet with his or her eyes closed and open. The relationship between the COP measurement and FAOS score was analyzed using the Pearson correlation coefficient. Results: The participants were consisted of 21 male and nine female, with a mean age of 30 years, mean weight of 72 kg, and mean foot size of 259 mm. With the eyes open, the correlation coefficient between the FAOS and travel distance of the affected side was -0.394 (p<0.05) and that between the FAOS and the ellipse area of the affected side was -0.425 (p<0.05). On the other hand, no significant correlations were found between the travel distance and ellipse area of the affected side when patients closed their eyes. Conclusion: Measurement of the COP using foot pressure scanner could evaluate objectively patients with chronic ankle instability, with measurements in patients with their eyes open being more significant. Based on the findings of this study, an analysis of the COP with the patients with their eyes open and standing on one foot may help determine the management strategy and assess the progress of the patients.

경사로 오르기와 내리기 동안 압력중심 이동경로와 족저압 비교 (Comparison of Pathway of COP and Plantar Foot Pressures while Ascending and Descending a Slope)

  • Purpose: The purpose of this study was to compare the pathway of COP and plantar foot pressure and to determine the correlation between plantar regions during the ascending and descending of a ramp. Methods: Fifteen healthy adults who had no musculoskeletal problems participated in our study. They were asked to walk on a level surface and on an ascending and descending ramp in their bare feet. Pathway of COP and plantar foot pressures were recorded using the Matscan system (Tekscan, Boston, USA). For pressure measurements, the plantar foot surface was divided into seven regions: two toe regions, three forefoot regions, a midfoot region, and a heel region. To determine whether there was a statistically significant difference between pathway of COP and plantar foot pressures during walking, we used repeated measuremes ANOVA. Results: In comparison to results for a level walking, pathway of COP while ascending a ramp had a tendency to be shifted medially in the forefoot and became longer till the big toe. Pathway of COP while descending a ramp also was shifted medially, but ended in the forefoot. Plantar foot pressure of the second and third metatarsal head and the fourth and fifth metatarsal heads was significantly decreased while descending the ramp. Conclusion: These results indicated that plantar foot pressure is changed while ascending and descending a ramp and demonstrated that ramp walking can affect the structure and function of the foot. Therefore, gait environment is associated with significant changes in foot characteristics, which contribute to altered plantar loading patterns during gait.

정적직립자세에서 여성고령자의 COP와 BBS, SPPB, TUG와의 상관관계연구 (Study of Correlation between BBS, SPPB, TUG and COP during Quiet Standing in Elderly Women)

  • 본 연구의 목적은 여성고령자의 균형과 관련된 평가항목 BBS(Berg balance scale), SPPB(short physical performance battery), TUG(timed up and go)를 측정하여 COP와의 상관관계를 알아보고자 하였다. 이를 위해 여성고령자 10명을 대상으로 신체기능검사와 정적직립자세 동안 지면경사(평지, 내리막, 오르막경사)에 따른 COP 이동범위와 평균속도를 산출하였다. 여성고령자들의 COP 좌우와 전후 값 비교에서 평지와 내리막, 오르막 모두 전후 값이 유의하게 크게 나타났으며, 좌우와 전후 각 방향에서 지면경사에 따른 유의한 차이는 나타나지 않았다. 신체 기능 점수와 COP 상관관계에서 BBS와 SPPB 점수가 높을수록 COP 이동범위와 평균속도 값은 감소하였으며 TUG 값이 클수록 COP 값이 크게 나타나는 유의한 상관관계를 나타냈다. 고령자들에게 균형과 관련된 신체기능검사와 COP는 지면경사에 따라 유의한 상관관계를 가지므로 임상에서 치료사들은 고령자들의 균형평가 시 COP 변수뿐만 아니라 BBS, SPPB, TUG를 통한 균형평가 활용이 가능하다고 본다.

예측된 그리고 예측되진 않은 갑작스런 상지로의 부하 적용시 요통 환자와 정상인의 압력 중심 이동 및 근활성 개시에 미치는 영향 (The Effect of Center of Pressure Displacement and Muscle Activation Onset during Expected and Unexpected Sudden Upper Limb Loading in Subjects with Low Back Pain and Healthy Subjects)

  • Purpose: This study was to compare the effect of center of pressure(COP) displacement and muscle activation onset during expected and unexpected sudden limb loading in subjects with low back pain and healthy control subjects. Most studies of COP displacement and muscle activation onset on postural task focused on sudden trunk loading or gross limb movements. Investigation of the COP displacement and muscle activation onset during expected and unexpected sudden upper limb loading deserves similar attention. Methods: For this study, 14 subjects with low back pain and 12 healthy control subjects are participated. Force plate and surface EMG measures were used to determine COP displacement and muscle activation onset under expected and unexpected sudden upper limb loading. Results: COP displacement and muscle activation onset under unexpected sudden upper limb loading were similar in subjects with low back pain and healthy control subjects. However, COP displacement and muscle activation onset under expected sudden upper limb loading were shortened in healthy control subject but not among the subjects with low back pain. Conclusion: The results provide evidence for impaired feed-forward control in subjects with low back pain.

A Study on the Cost-Effective Personalized Plantar Pressure Measurement System

  • Plantar pressure data can be used not only for walking patterns in daily life, but also for eating, health care, and disease prevention. For this reason, the importance of plantar pressure measurement has recently increased. However, most systems that can measure both static and dynamic plantar pressure at the same time are expensive, not portable, and not universal. In this study, we propose a system that effectively reduces the number of sensors in plantar pressure system. Through this, we want to increase the economics and practicality by reducing the size and weight of the system, as well as the power consumption. First, for static plantar pressure and dynamic plantar pressure, the values measured by existing precision instruments are analyzed to determine how many measurement parts the insole is divided into. Next, for the divided measuring parts, the position of the sensor is determined by calculating the Center of Pressure (COP) for each part with the values of all dynamic and static plantar pressure sensors. Finally, in order to construct a personalized plantar pressure measurement system, we propose a weighting method for the static plantar pressure COP and the dynamic plantar pressure COP for each part.

편평족 노인의 계단 하강 보행 시 아치 지지형 인솔 종류에 따른 족저압력 및 균형성 평가 (Comparison of plantar pressure and COP parameters in three types of arch support insole during stair descent in elderly with flatfoot)

  • 본 연구의 목적은 평발을 가진 여성 노인의 계단 하강 보행 시 일반인솔 및 아치 지지형 인솔을 적용에 따른 족저압력 및 압력중심점 변인들의 차이를 조사하는데 목적이 있다. 족저압력 분석장비(Pedar-X, Novel, Germany)를 사용하여 14명의 평발 노인을 대상으로 3종류(일반인솔, A형 인솔, B형 인솔)의 인솔착용 후 최대족저압력, 평균족저압력, 접촉면적, 압력중심점의 이동거리, 변위 및 최대범위의 평균값을 산출하였으며, 일원변량분석(one-way ANOVA)를 이용하여 인솔 종류간 평균을 비교하였다. 족저 압력 변인 중 최대족저압력은 중족부의 M3, 평균족저압력은 M2, M3, M4 영역에서, 접촉면적은 M2, M3, 그리고 M6 영역에서 통계적으로 유의한 차이를 보였으며, 압력중심점 변인 중 전후축과 좌우축에서 압력중심점의 이동거리에서 통계적으로 유의한 차이를 보였다. 족저압력 평가결과 아치 지지 기능을 가진 인솔을 삽입한 A와 B형 인솔에서 족궁지지 영역인 M3의 최대압력은 B형 인솔과 A형 인솔이 일반인솔과 비교했을 때 높게 나타났다. 좌우축, 전후축 압력중심점의 이동거리는 A형과 B형 인솔 모두 일반 인솔에 비해 짧게 나타났다.

Immediate Effects of Lumbar Rotational Mobilization on the One-Legged Standing Ability in Healthy Individuals: A Randomized Controlled Trial

  • The original focus of this study was to investigate the immediate effects of lumbar rotational mobilization on the one-legged standing ability. Fifteen subjects (6 men and 9 women, mean age = 22.77 (SD = 1.21), mean height = 165.46cm (SD = 11.65), mean weight = 61.46kg (SD = 8.29) volunteers from healthy individuals were recruited and randomized to a lumbar rotational mobilization (LRM) group and a trunk rotational exercise (TRE) group. Mobilization (grade 3 or 4) was applied to the LRM group on the lumbar spine (L1 to L5) in a side-lying, and trunk twist exercise (left and right side) was applied the to the TRE group with lunge position. Center of pressure (COP) and the velocity of the center of pressure (VCOP) of each participant were measured as a balance ability through one leg standing position. Results are as follows. In within-group difference, the COP of the LRM group reduced during standing with the right foot, but the VCOP change of the LRM was not statistically significant. In between-groups difference, COP of TRE group was decreased compared with LRM group only during left leg standing in the eyes (p <.05). The results of this study suggest that LRM is more effective than TRE in improving balance ability.

허벅다리걸기 동작 시 발목 관절의 테이핑 처치가 하지 관절과 압력 중심 요인들에 미치는 영향 (Effect of ankle joint taping treatment on lower extremity joint and center of pressure factors during the Uchi-mata)

  • 본 연구의 목적은 유도 허벅다리걸기 기술 발휘 시 발목 관절의 테이핑 처치가 하지 관절과 압력중심(COP, center of pressure)에 미치는 영향을 분석하는데 있었다. 대학 유도 선수 20명(연령, 20.9±0.8세; 신장, 168.6±7.4cm; 체중, 73.5±11.6kg; 신체질량지수, 25.7±2.6kg/㎡)이 참여하였으며, 발목 관절 테이핑 처치 전과 후의 2가지 조건에서 허벅다리걸기 기술 발휘 시 지지다리 관절의 각도와 압력중심 요인을 분석하여 다음의 결과를 도출하였다. E2(t=2.411, p=.027) E4(t=2.388, p=.029)시점에서 발목 관절의 각도는 테이핑 처지 전에 비해 처치 후 발목의 각도가 통계적으로 적은 수치를 나타내었고, E2(t=-2.343, p=.032) E3(t=-4.531, p=.000)시점에서 힙 관절의 각도는 통계적으로 크게 나타났다. 그리고 발목 관절 테이핑 처치 후 좌·우의 COP 이동은 상대를 메치는 P3 국면에서 통계적으로 크게 나타났으며(t=2.670, p=.016), 전·후의 COP 이동은 상대의 무게 중심을 기울이는 P1 국면에서 통계적으로 적은 수치를 나타내었다(t=2.846, p=.011). 그러므로 허벅다리걸기를 특기로 사용하는 유도 선수들은 발목 관절 테이핑으로 인해 발생하는 지지관절의 운동 기능과 COP의 이동범위를 고려하여 사용하는 것에 대하여 제안하고자 한다.
