A Case of Imatinib-mesylate associated Hypersensitivity Pneumonitis (Imatinib-mesylate에 의한 과민성 폐렴 1예)

  • Imatinib-mesylate (Gleevec, Glivec) is a protein-tyrosine kinase inhibitor that inhibits the Bcr-Abl tyrosine kinase created by the Philadelphia chromosome abnormality in CML. Imatinib is also used to treat patients with c-kit (CD 117)-positive unresectable tumors, or metastatic malignant gastrointestinal stromal tumors, or both. Imatinib is a welltolerated drug with few side effects. However, it has been associated with gastrointestinal irritation, fluid retention and edema, skin rashes, depigmentation, hepatotoxicity, hemorrhage, and hematological toxicity (anemia, neutropenia, and thrombocytopenia). In addition, imatinib has been associated with dyspnea and cough, which are mainly secondary to the pleural effusion and pulmonary edema, which represent local or general fluid retention. These events appear to be dose related and are more common encountered in the elderly. However, there has been no report of hypersensitivity pneumonitis associated with imatinib-mesylate in Korea. We report a case of 51-year old woman who developed hypersensitivity pneumonitis that might have been induced by imatinib-mesylate during the treatment of a gastrointestinal stromal tumor.

Inhibitory Effect of Curcumin on MDR1 Gene Expression in Patient Leukemic Cells

  • When patients with cancers are treated with chemotherapeutic agents a long time, some of the cancer cells develop the multidrug resistance (MDR) phenotype. MDR cancer cells are characterized by the overexpression of multidrug resistance1 (MDR1) gene which encodes P-glycoprotein (Pgp), a surface protein of tumor cells that functions to produce an excessive efflux and thereby an insufficient intracellular concentration of chemotherapeutic agents. A variety of studies have sought potent MDR modulators to decrease MDR1 gene expression in cancer cells. Our previous study has shown that curcumin exhibits characteristics of a MDR modulator in KB-V1 multidrug-resistant cells. The aim of this study was to further investigate the effect of curcumin on MDR1 gene expression in patient leukemic cells. The leukemic cells were collected from 78 childhood leukemia patients admitted at Maharaj Nakorn Chiang Mai Hospital, Chiang Mai, Thailand, in the period from July 2003 to February 2005. There were 61 cases of acute lymphoblastic leukemia (ALL), 14 cases of acute myeloblastic leukemia (AML), and 3 cases of chronic myelocytic leukemia (CML). There were 47 males and 31 females ranging from 1 to 15 years old. Bone marrows were collected. The leukemic cells were separated and cultured in the presence or absence of $10{\mu}M$ curcumin for 48 hours. MDR1 mRNA levels were determined by RT-PCR. It was found that curcumin reduced MDR1 gene expression in the cells from 33 patients (42%). Curcumin affected the MDR1 gene expression in 5 of 11 relapsed cases (45%), 10 of 26 cases of drug maintenance (38%), 7 of 18 cases of completed treatment (39%), and 11 of 23 cases of new patients (48%). The expression levels of MDR1 gene in leukemic patient cells as compared to that of KB-V1 cells were classified as low level (1-20%) in 5 of 20 cases (25%), medium level (21-60%) in 14 of 32 cases (44%), and high level (61-100%) in 14 of 20 cases (70%). In summary, curcumin decreased MDR1 mRNA level in patient leukemic cells, especially in high level of MDR1 gene groups. Thus, curcumin treatment may provide a lead for clinical treatment of leukemia patients in the future.

Molecular genetic characterization of multiple antimicrobial resistant Salmonella spp. isolated from pigs and cattle (소와 돼지유래 다제내성 Salmonella속 균의 분자유전학적 특성)

  • At the present study, it was aimed to explore the molecular genetic characterization of multiple antimicrobial resistant Salmonella spp. isolates from pigs and cattle. A total of 138 Salmonella Typhimurium (S. Typhimurium) isolates were typed with phage, among them, 83.3% of S. Typhimurium tested could divide into a 10 phage types. Definitive type 193 (DT193) (25.4%) and DT195 (24.6%) were exhibited as the dominant types. DT104 and U302 were found from pigs and cattle. On the other hand, S. Enteritidis had 6 phage types, of them, phage type 21 (PT21) and PT11b were the popular types. In the plasmid profiles, 135 of S. Typhimurium isolates were exhibited 1 to 6 plasmid bands which molecular weight ranged from 90 to 2kb. 35 isolates (25.4%) harbored a 90kb plasmid which is thought to be the serotype specific virulence plasmid. Two of twenty five S. Enteritidis had common plasmids at 2 and 1.5kb. With multiplex polymerase chain reaction, virulence genes (invA and spvC) were detected from all Salmonella spp. from 167 of S. Typhimurium, S. Enteritidis and chloramphenicol resistant S. Schwarzengrund, but some drug resistant genes, such as PSE-1, cml/tetR and flo were not determined but other drug resistant genes, for example TEM and int were found. The detection rates of spvC, TEM and int gene was 35.3%, 29.3% and 72.5%, respectively. The TEM gene was highly popular in S. Typhimurium, which was detected from ampicillin and amoxicillin resistant strains as 95.9%. int gene was able to detect from all the isolates identified as multidrug resistsnt (MDR), particularly DT193 was thought as the most prevalent virulence and multidrug resistance isolate. The major plasmid profile and drug resistance pattern of DT193 were 90, 40, 10.5, 6.3, 3.0kb and ACCbDNaPSSuT, respectively. MDR was commonly found in other phage types, particularly DT104, U302 and DT203.

Corni Fructus Inhibits Wrinkle Formation by Reduced Advanced glycation end product (AGEs) (산수유의 최종당화산물 억제로 인한 주름 개선효과)

  • Objectives : Corni Fructus (CF) is traditional herbal medicine used on polyuria, low back pain, and tinnitus. This study aimed to evaluate inhibits skin wrinkle formation effect of CF. Methods : To evaluate the produce inhibition effect of CF, SD-rats were distributed into four groups; normal rats (Nor), AGEs (advenced glycation end product)-induced rats (Con), AGEs-induced rats treated with 100mg/kg CF (CF). To induce AGEs, streptozotocin (50mg/kg) was administered intraperitoneally and after 3 days oral administrated 100mM methyl glyoxal for 3 weeks. Results : The oral administration of CF suppressed the reactive oxygen specis (ROS) in serum. The AGEs in skin tissues was significantly reduced through treatment of CF. Furthermore, the expressions of AGEs related proteins such as polyclonal anti-$N^e$-(carboxymethyl)lysine (CML), anti-$N^e$-(carboxyethyl)lysine (CEL), AGE receptors (RAGE) were decreased in CF treated group compared with the control group in skin tissues. Inflammation-related proteins such as Nuclear factor kappa-light-chain-enhancer of activated B cells (NF-kB), monocyte chemoattractant protein-1 (MCP-1), interleukin-6 (IL-6) reduced in CF treatment group than control group. AGE-induced rats exhibited that the significant decreased collagen however, CF treatment (100mg/kg of body weight) up regulated collagen by improved the expression levels of skin fibril-related genes such as Matrix metalloproteinase (MMP-1). Conclusion : Taken together, our study suggests that CF regulates ROS to prevent accumulation of AGEs and inhibits skin wrinkles. Our finding indicate that CF may be an effective agent for inhibits AGEs formation, and improved skin wrinkle.

Anti-skin-aging Effect of Mori Folium through decreased Advanced glycation end product (AGEs) (최종당화산물 억제를 통한 상엽(桑葉)의 항피부노화 효과)

  • Objectives : Recently, numerous studies reported that excessive generation of advanced glycation end products (AGEs) stimulated expression of skin wrinkle related proteins. This study aimed to evaluate inhibits skin wrinkle formation effect of Mori Folium (MF) through decreased AGEs. Methods : To evaluate the skin wrinkle inhibition effect of MF, SD-rats were divided into three groups; normal rats (Nor), AGEs-induced rats (Con), AGEs-induce rats treated with MF at dose of 100mg/kg body weight (MF). To induced AGEs, streptozotocin (50mg/kg) was injected intraperitoneally, and after 3 days, 100mM methyl glyoxal was administered orally for 3 weeks. After the experiment, the animal's dorsal skin tissues and serum were separated and tested. Results : The oral administration of MF suppressed the AGEs level in serum. Also, the AGEs in skin tissues was significantly reduced through treatment of MF compared with control group. Moreover, the expressions of AGEs related proteins such as polyclonal anti-$N^e$-(carboxymethyl) lysine (CML), anti-$N^e$-(carboxyethyl)lysine (CEL), AGE receptors (RAGE) were reduced in MF group compared with the control group in kidney and skin tissues. The matrix metallo proteinase-1 (MMP-1) reduced by MF treatment with the result that collagen type 1 alpha 2 (COL1A2) was improved that reduced by accumulation of AGEs. Conclusion : The evidence of this study indicate that oral administration of MF reduces the levels of AGEs in serum, skin, and kidney tissues. In conclusion, MF inhibit skin wrinkle formation, suggesting the potential of anti-wrinkle material.

Studies on the Main Level-Grading Factors for Establishment of LFQC (Liquid Fertilizer Quality Certification) System of Livestock Manure in Korea (가축분뇨 액비품질인증제도 구축을 위한 목표요소에 관한 연구)

  • Establishment of the LFQC (Liquid Fertilizer Quality Certification) system is very urgent issue for recycling livestock manure as renewable resources in Korea faced with environmental problem of manure application to land due to intensive livestock farming. In this study, we investigated relevant laws and regulations on livestock manure fertilizer, certifications of eco-friendly agricultural products, government policies on livestock manure management to establish reasonable direction of Korean LFQC (Liquid Fertilizer Quality Certification) system. As a result from this study, the liquid fertilizers in 'LFQC' system could be classified as three levels according to the usage patterns in field; 1st. Individual Farm Level (IFL), 2nd. Joint Farm Level (JFL), and 3rd. Commercial Level (CML). And finally, we found some characteristics in 'Main Level-Grading Factors' of liquid fertilizer such as fertilizing value, harmfulness, stability, uniformity, economic effect, storage potential, commercial value, functionality. Those items were considered to be the key factors for the establishment of 'LFQC' system. More research on 'Evaluation Standards' for concrete guideline and on the 'Main Level-Grading Factors' be needed to complete Korean LFQC system.
