Arachnoid Remodeling by Clipping Technique Facilitates Surgical Maneuverability during Transsphenoidal Surgery for Pituitary Macroadenoma

  • Objective : Pituitary adenomas frequently extend into the suprasellar space. After a suprasellar tumor is removed, the superiorly extended arachnoid becomes redundant and sinks down into the intrasellar space which often hiders visualization and accessibility to the hidden space behind the evaginated arachnoid. We introduced arachnoid remodeling by clipping technique, and evaluated its usefulness and safety during TSS. Methods : Total 223 patients who underwent arachnoid remodeling with our new clipping technique were included. Redundant arachnoid was clipped along the dural edge with multiple 2.6-mm titanium clips until the redundant arachnoid membrane no longer blocked the surgical route. To check for possible deterioration of hormonal function by this technique, we assessed anterior pituitary function of 166 patients who underwent arachnoid remodeling by clipping and compared this with those of other 429 control patients. Results : Our technique greatly enhanced the accessibility and visualization of intrasellar and parasellar spaces, both of which are generally hindered by redundant arachnoid during transsphenoidal surgery (TSS). We found no difference in anterior pituitary function between a clip-assisted arachnoid remodeling group and the control group, implying that this technique does not result in hypopituitarism. Conclusion : During TSS for pituitary adenomas with suprasellar extension, arachnoid remodeling by clipping technique is very useful and convenient for the management of the redundant arachnoid membrane to enhance visualization and surgical accessibility.

Ecogeological Description of Sanyang Gotjawal, Jeju Island, Korea

  • BACKGROUND: Gotjawal forest on Jeju Island is characterized by uneven topography that developed as a result of freezing-thawing weathering process and irregular substrates caused by numerous lava-flow collapses. Sanyang Gotjawal, located in southwest of Jeju Island, is a well-developed forest with a long history. In addition to photographs, there is a need for a good way to describe the unique features of Gotjawal, including its geology and vegetation. METHODS AND RESULTS: We illustrated the area's natural features using Clip Studio Paint 1.12.0. To reveal its complexity, we separated the vegetation and geological features in the illustrations. CONCLUSION(S): We drew a cross-section of Gotjawal's unique layers, including lava flows. In addition to an eco-geological approach, we described the dominant vegetation and geological characteristics of the three forest layers (trees, shrubs, and herbs) in Gotjawal.

Behavior Pattern Prediction Algorithm Based on 2D Pose Estimation and LSTM from Videos (비디오 영상에서 2차원 자세 추정과 LSTM 기반의 행동 패턴 예측 알고리즘)

  • This study proposes an image-based Pose Intention Network (PIN) algorithm for rehabilitation via patients' intentions. The purpose of the PIN algorithm is for enabling an active rehabilitation exercise, which is implemented by estimating the patient's motion and classifying the intention. Existing rehabilitation involves the inconvenience of attaching a sensor directly to the patient's skin. In addition, the rehabilitation device moves the patient, which is a passive rehabilitation method. Our algorithm consists of two steps. First, we estimate the user's joint position through the OpenPose algorithm, which is efficient in estimating 2D human pose in an image. Second, an intention classifier is constructed for classifying the motions into three categories, and a sequence of images including joint information is used as input. The intention network also learns correlations between joints and changes in joints over a short period of time, which can be easily used to determine the intention of the motion. To implement the proposed algorithm and conduct real-world experiments, we collected our own dataset, which is composed of videos of three classes. The network is trained using short segment clips of the video. Experimental results demonstrate that the proposed algorithm is effective for classifying intentions based on a short video clip.

The Effect of Experiential Value of AR Fashion Platform on Platform Preference (증강현실 패션플랫폼에 대한 경험적 가치가 플랫폼 선호도에 미치는 영향)

  • This study explores the effect of experiential value of AR fashion platform on platform preference. Based on the SOR theory, the effects of visual appeals, entertainment, service excellence, and efficiency on platform attitudes, mediated by intrinsic enjoyment and escapism, are tested. The participants respond to the questions after watching the video clip, using an AR fashion platform. A total of 252 data is analyzed using SPSS 26.0 for descriptive statistics and reliability analysis and AMOS 26.0 for confirmatory factor analysis and structural equation modeling. The results show visual appeals, entertainment, and efficiency influence platform preference, mediated by intrinsic enjoyment. Entertainment also influences platform preference, mediated by escapism. The moderating effect of the AR service pre-experience is also explored. For the group with the AR service experience, entertainment and efficiency has a positive effect on intrinsic enjoyment, leading to AR platform preference. For the group without an AR service experience, only entertainment influences the AR platform preference, mediated by intrinsic enjoyment; however, there are no factors which increase escapism. The findings of this study contribute to find the marketing directions for the AR service users, by offering appropriate experiential values based on their prior AR experience.

  • Endoscopic resection (ER) is widely utilized as a minimally invasive treatment for upper gastrointestinal tumors; however, complications could occur during and after the procedure. Post-ER mucosal defect leads to delayed perforation and bleeding; therefore, endoscopic closure methods (endoscopic hand-suturing, the endoloop and endoclip closure method, and over-the-scope clip method) and tissue shielding methods (polyglycolic acid sheets and fibrin glue) are developed to prevent these complications. During duodenal ER, complete closure of the mucosal defect significantly reduces delayed bleeding and should be performed. An extensive mucosal defect that comprises three-quarters of the circumference in the esophagus, gastric antrum, or cardia is a significant risk factor for post-ER stricture. Steroid therapy is considered the first-line option for the prevention of esophageal stricture, but its efficacy for gastric stricture remains unclear. Methods for the prevention and management of ER-related complications in the esophagus, stomach, and duodenum differ according to the organ; therefore, endoscopists should be familiar with ways of preventing and managing organ-specific complications.

  • Mammography has been the standard screening method for breast cancer. In women with suspicious calcifications and architectural distortion identified on mammography or digital breast tomosynthesis only without detected on breast US, stereotactic biopsy and mammography-guided preoperative localization is one of the method for pathologic diagnosis. This review aims to describe the indication, contraindication, technique of stereotactic biopsy, clip placement after stereotactic biopsy, and digital breast tomosynthesis-guided stereotactic biopsy. In addition, this article reviews mammography-guided preoperative localization using a wire or non-wire device.

Risk of Stroke with Temporary Arterial Occlusion in Patients Undergoing Craniotomy for Cerebral Aneurysm

  • Objective : This study was performed to elucidate the technical and patient-specific risk factors for postoperative ischemia in patients undergoing temporary arterial occlusion (TAO) during the surgical repair of their aneurysms. Methods : Eighty-nine consecutive patients in whom TAO was performed during surgical repair of an aneurysm were retrospectively analyzed. The demographics of the patients were analyzed with respect to age. Hunt and Hess grade on admission, Fisher grade of hemorrhage, aneurysm characteristics, timing of surgery, duration of temporary occlusion, and number of temporary occlusive episodes. Outcome was analyzed at the 3-month follow-up, along with the occurrence of symptomatic and radiological stroke. Results : In overall, twenty-seven patients (29.3%) had radiologic ischemia attributable to TAO and fifteen patients (16.3%) had symptomatic ischemia attributable to TAO. Older age and poor clinical grade were associated with poor clinical outcome. There was a significantly higher rate of symptomatic ischemia in patients who underwent early surgery (p=0.007). The incidence of ischemia was significantly higher in patients with TAO longer than 10 minutes (p=0.01). In addition, patients who underwent repeated TAO, which allowed reperfusion, had a lower incidence of ischemia than those who underwent single TAO lasting for more than 10 minutes (p=0.011). Conclusion : Duration of occlusion is the only variable that needs to be considered when assessing the risk of postoperative ischemic complication in patients who undergo temporary vascular occlusion. Attention must be paid to the patient's age, grade of hemorrhage, and the timing of surgery. In addition, patients undergoing dissection when brief periods of temporary occlusion are performed may benefit more from intermittent reperfusion than continuous clip application. With careful planning, the use of TAO is a safe technique when used for periods of less than 10 minutes.

  • Ever-evolving diverse communication tools bring numerous changes and improvements into the lives of humans. It is extremely important to provide visual information when communicating with consumers in the commercial arena because humans acquire over eighty percent of the information around them through sense of sight. One cannot compete with just sheer quality in today's world. Therefore, the applications of digital technology in the visual merchandising became crucial, for it can dramatically improve the market value. Among these applications is digital signage, a new media for esthetic experience. I strongly believe that this would be a new marketing tool for the brand's distinctive and unique publicity. This research has been conducted in order to find out how the fast-growing digital signage, defined by the contents, has become not just a mere digital video clip but an esthetic merchandising tool that can produce various effective marketing strategies; and to discover the characteristics that this application has brought out in the visual merchandising field of fashion stores' global SPA brands. Based on the market case study, analyses have been made for the digital signage types and strategies regarding the visual merchandising expressive elements and for the effects the digital signage application will have on publicity. According to the results, the digital signage has proven to be informative, diversely applicable and contributes greatly to the increase in sales and to the improvement of company and brand image. Thus, it is much more than just a media tool for advertisement.

Design of Smart Phone-Based Braille Keyboard System for Visually Impaired People (시각 장애인을 위한 스마트폰 기반 점자형 문자 입력 시스템설계)

  • Recently, as the smart phone market grows explosively, there exists a large difference of utilization of the smart phone between an usual people and the visually impaired people. In this paper, We propose the character and numer ic input system based on the low cost of braille film and braille for the visually impaired people in order to reduce to information literacy gap for the visually impaired people. The proposed system is designed with the braille film and keyboard layout based on the braille character layout. Additionally, the telephone, the SMS, the abbreviated dial and location transfer function for the emergency situation are designed. The character which inputs using the braille keyboard can be automatically copied in the clip board and used in the other applications. Hence, the visually impaired people can easily use the smart phone for getting plentiful information on the device and network.

  • In recent years many people are changing their mobile phones from feature phones to smart phones because huge number of useful smart phone applications are released onto the market everyday. Development of smart phone applications could be very helpful educational contents for technologically gifted children who has lots of curiosity and creativity. For this reason this paper deals with development of smart phone application UI(User Interface) layout education programs for the technologically gifted children. These education programs which can operating under Android-based smartphone, have been developed by using JAVA and Eclipse. Survey from the technologically gifted children education experts has been analyzed by Delphi method to figure out effectiveness of the developed education programs.
