슬관절 통증 환자의 한의학적 치료 호전도와 BMI 와의 상관성 (The Correlation between Korean Medical Treatment on Knee Joint Pain and BMI)

  • Objectives : This study was designed to assess the general distribution, clinical effectiveness of Korean medical treatment on knee pain and the correlation between Korean medical therapy on knee pain and BMI. Methods : This is an observational study. 65 patients admitted to Daejeon Jaseng Hospital of Korean Medicine with musculoskeletal disorders were observed from July, 2014 to July, 2015. They were analyzed according to sex, age, pain lesion, body mass index(BMI) and treatment efficacy. All patients received a combination of treatments during hospitalization, including acupuncture, pharmacopuncture, herbal medicines and physical therapy. A zero to ten numerating rating scale (NRS) assessing pain, Western Ontario and McMaster Universities Arthritis Index(WOMAC) index and Range of Motion(ROM) was used before and after treatments. Statistical correlations among assessment measurements were evaluated by examining the paired t-test and the Pearson's correlation coefficients. Results : The average BMI of all patients was $24.03{\pm}2.95kg/m^2$. According to clinical definition of obesity by Korean Society for the Study of Obesity, 33.8% of all patients was within normal weight, 1.5% was underweight, 33.8% was overweight, 24.6% was obesity and 6.2% was morbid obesity. For knee pain patients, NRS (Numeric Rating Scale) decreased from $6.28{\pm}1.63$ to $3.94{\pm}2.03$(p<0.001). WOMAC index decreased from $48.14{\pm}17.63$ to $40.37{\pm}18.28$(p<0.001). But, there were no significant correlations in statistics among BMI, knee pain index and knee pain index improvement. Conclusions : Korean medical combination treatment might be effective in reducing pain and improving functional disorders for patients with knee pain. This study further confirmed the efficacy of Korean medical treatment on knee pain. But, more studies on correlation between knee pain and obesity and development of assessment measurement are needed.

요통을 호소하는 ALS환자에 대한 한의학적 치료 효과 증례 보고 (The Clinical Case of Oriental Medical Treatment at Tender Point for Patient with Lower Back Pain Suggesting of Amyotrophic Lateral Sclerosis)

  • ALS를 진단받았으며 2010.5.11부터 2010.6.10까지 ${\bigcirc}{\bigcirc}$한방병원에 입원한 요통을 호소하는 남환에 대하여 화침치료, 전침치료, 침치료, 구치료, 약물치료, 이학요법 등을 시행한 결과 K-ODI, VAS, K-ALSFRS-R 등의 평가 항목에 대하여 호전 경향을 보였다. ALS는 기본적으로 진행성 경과를 밟는 질환이기 때문에, 병의 진행을 늦추는 것과 대증치료를 통해 삶의 질을 높이는 것이 중심이 된다고 볼 수 있다. 본 환자의 경우 상기 한의학적 처치에 의해 일정한 효과가 나타났다. ALS에서 수반되는 요통의 호전에 의해 기능수행능력이 향상된 결과라고 볼 수 있다. 이는 한의학적 치료 방법이 ALS 환자의 치료에 있어서 통증을 감소시키고 일상생활에서의 기능수행능력을 향상시킴으로써 삶의 질을 높이는 대중치료의 역할을 수행할 수 있다는 단서라고 할 수 있다. 치료효과의 검증을 위해서는 추후 지속적인 증례 연구가 필요할 것으로 사료된다.

근골격계 질환의 수기요법에 관한 경제성 평가: 체계적 문헌 고찰 및 근거 합성 연구 (Economic evaluation of manual therapy for musculoskeletal diseases: a systematic review and narrative synthesis of evidence)

  • 연구 목적 : 본 연구는 근골격계 질환에 대한 수기치료가 갖는 경제적 효과를 체계적으로 평가해 보기 위해 시행되었다. 연구 방법 : 2017년 2월 2일까지 국내외 관련 문헌을 체계적으로 검색 하였으며, 연관된 체계적 고찰 논문의 참고문헌을 조사하였다. 두 명의 연구자가 독립적으로 문헌을 선택하고, 비뚤림 위험 평가 및 경제성 평가 질 평가, 자료 추출을 시행하였다. 연구 결과 : 총 3,327개의 논문을 검토하여 최종적으로 18개의 무작위 대조 연구가 포함되었다. 경제성 평가는 수기요법과 다른 치료방법간의 유효성 비교를 통해 시행되었다. 요부 통증, 견관절 통증 및 외측 상과염 치료에 있어 침치료, 견인치료, 주사치료, 일반의 치료, 척추 안정화 기법 및 통증관리 치료보다 비용 효과적인 것으로 나타났다. 또한 또한, 수기치료는 경항통, 흉통, 고관절염 혹은 슬관절염, 경추신경병증 및 수부손상 치료에 있어 일반적 치료, 물리치료, 자가 치료 프로그램, 견인 치료에 비하여 효과적인 치료인 것으로 확인되었다. 결론 : 18개의 논문 중 10개의 논문에서 근골격계 질환 치료에 있어 수기치료가 경제적으로 효과가 있는 것으로 나타났다. 이러한 결과는 다른 치료방법에 비해 수기요법이 근골격계 질환에 있어서 경제적 가치가 있음을 시사하는 내용이다. 하지만, 근골격계 질환 수기치료의 경제성 평가에 대한 선행 연구는 전반적으로 부족한 실정으로, 보다 정확한 결과를 제시하고 정책결정을 위한 효과적인 제안을 위해서는 체계적인 추가 연구가 필요하다 사료되는 바이다.

A Retrospective Chart Review of 122 Inpatients with Knee Osteoarthritis Treated with Korean Medicine: An Analysis of the Effects of Treatment

  • Background: Korean medicine treatment was assessed in patients with knee osteoarthritis (OA) according to subgroups of: sex, age, cause of knee OA, body mass index, hospitalization period, history, OA compartment, phenotype, and comorbidity. Methods: A retrospective review was performed of 122 inpatients who were admitted to the Hospital of Korean Medicine for Korean medicine treatment of knee pain, and were diagnosed with knee OA based on magnetic resonance imaging findings. Analysis of patient subgroups (sex, age, cause of knee OA, body mass index, hospitalization period, history, OA compartment, phenotype, and comorbidity) was carried out and treatments including acupuncture, cupping, pharmacopuncture, herbal medicine, chuna therapy, medicinal steaming therapy, manual therapy, and extracorporeal shock wave therapy were listed. The numeric rating scale (NRS), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), and 5-level EuroQol- 5 Dimension (EQ-5D-5L) scores were measured before and after treatment to assess the effects of treatment on pain and quality of life. Results: Seventeen males and 105 females were included in this study. Most patients were in their 60s. In the total study population, NRS, WOMAC, and EQ-5D-5L scores were improved statistically significant when comparing before and after treatment. The NRS and WOMAC scores improved statistically significant in the medial, patellofemoral, medial + patellofemoral, medial + lateral + patellofemoral compartment. Conclusion: Korean medicine treatment significantly reduced pain, stiffness, and physical dysfunction, and improved the quality of life of patients with knee OA, suggesting that it may be an effective alternative to the current conservative treatments.

재발성 요추간판탈출증의 한의학적 치료에 대한 국내·외 연구 동향 체계적 문헌고찰 및 메타분석 (A Review on the Effect of Traditional Medicine Treatments in Korea and China for Recurrent Lumbar Disc Herniation: Systematic Review and Meta-Analysis)

  • Objectives This study aimed to examine the efficacy of Korean traditional medicine treatments for recurrent lumbar disc herniation by analyzing randomized controlled trials (RCTs). Methods The following 6 databases were used Korean studies Information Service System, Oriental Medicine Advanced Searching Integrated System, Research Information Sharing Service, ScienceON, PubMed, and China National Knowledge Infrastructure. Only RCTs were selected. The risk of bias was assessed by Cochrane's risk of bias tool. Results A total of 10 RCTs were analyzed, which utilized 8 types of interventions, including acupuncture, moxibustion, warm needling, electroacupuncture, herbal medicine, cupping, and chuna manual therapy. Acupuncture and moxibustion were the most frequently used treatments. There were 7 outcome measures, including the effective rate, visual analog scale, Japanese Orthopedic Association. The effective rate was assessed in all studies. Meta-analysis results indicated that warm needling therapy and traditional Chinese medicine interventions were more effective compared with the control. Conclusions In this study, warm needling therapy and herbal medicine interventions demonstrated beneficial effects. As the meta-analysis was conducted with only a subset of RCT studies, further large-scale, systematic RCT studies are needed.

추나요법이 턱관절 증후군에 미치는 영향: 체계적 문헌 고찰 (Chuna Manual Therapy for Temporomandibular Joint Disorder: Systematic Review)

  • Objectives To determine the effectiveness of Tuina for temporomandibular joint disorder. Methods We searched 10 electronic databases (Pubmed, CNKI, EMBASE, Cochrane Library, KISS, KISTI, NDSL, RISS, KMBASE, DBpia) up to May 2017. We included randomized controlled trials (RCTs) using Tuina for temporomandibular joint disorder. The methodological quality of each RCT was assessed by the Cochrane risk of bias tool. Results 14 RCT studies were eligible in our review. 14 studies were divided into 4 groups, and meta-analysised. The meta-analysis of 6 studies showed favorable results for the use of Tuina. High risk of bias were observed in 9 studies. Conclusions Although there are favorable results with meta-analysis, our systematic review are highly dependent on the single source of Chinese electrical database, CNKI. Now limited evidence is available to support Tuina for temporomandibular joint disorder and further well-designed RCTs should be encouraged.

Seizure after Subdural Hematoma Treated with Combination Western-Korean Medicine

  • Combined Western-Korean medicine treatments were given to a 67-year-old woman with late onset seizures who underwent surgical drainage of a subdural hematoma. Clonazepam and herbal medicine was prescribed. Acupuncture, moxibustion, cupping, chuna, and physical therapy were also performed. The frequency and intensity of seizures was assessed using the Chalfont Seizure Severity Scale. The seizure index score improved from 25 at admission to 0 at discharge. Pain in the right upper extremity reduced from 10 to 0 on the visual analogue scale and muscle strength increased from Grade 3 to 5 in Medical Research Council Scale for the Manual Muscle Test. At the time of hospitalization, the manual muscle strength tests for the affected shoulder, elbow, wrist, and grip strength were 30%, 60%, 10%, 5%, respectively, which improved almost 100% by discharge. Further studies using combined Western-Korean medicine for seizures after strokes are necessary to determine the most effective treatment.

  • The objective of this study is to report the clinical effects of applying an integrated rehabilitation protocol to four patients with traumatic cervical spinal cord injuries. The treatments applied included acupuncture, cupping and moxibustion, chuna manual therapy, functional electrical stimulation, and other physical therapies. The evaluation methods included American Spinal Injury Association Impairment Scale, functional independence measure (FIM), modified Barthel index (MBI), numeric rating scale (NRS) and other scales. All patients presented marked improvements in FIM, MBI, NRS and other scales, along with increased muscle strength in the manual muscle test. The application of our protocol resulted in clear clinical benefits and enhanced the recovery and quality of life for the patients in this study.

Appraisal of Guidelines for Research & Evaluation (AGREE) II를 이용한 교통사고 상해증후군의 국내·외 기개발 임상진료지침의 평가 (Appraisal of Guidelines for Research & Evaluation II Appraisal of Clinical Practice Guidelines for Traffic Injuries)

  • Objectives This study was aimed to evaluate clinical practice guidelines (CPGs) of traffic injuries, which has already been developed at domestic or outside of country, and to explore the Korean medical treatments included in the CPGs. Methods Twelve electronic databases (PubMed, Cochrane library, China National Knowledge Infrastructure [CNKI {Chinese Academic Journals, CAJ}], Research Information Sharing Service [RISS], Oriental Medicine Advanced Searching Integrated System [OASIS], KoreaMed, Korean Medical Guideline Information [KoMGI), National Guideline Clearinghouse [AHRQ], Core Outcome Measures in Effectiveness Trials Initiative Website [COMET], Turning Research into Practice [TRIP], The National Institute for Health and Care Excellence [NICE], and Medical Research Information Center [MedRIC]) up to July 2021 were searched. Only systematically developed CPGs for traffic injuries were selected and appraised. The appraisal was conducted by using Appraisal of Guidelines for Research & Evaluation (AGREE) II tool. Results Six CPGs were included and evaluated. All CPGs were appraised as highly recommended because they exceeded 60% in more than 4 among 6 domains of AGREE II, including domain of 'rigor of development', and 30% in the rest. Recommendations related to Korean medicine treatments such as on manual therapy related to Chuna were given in 6 CPGs, 4 for acupuncture, 1 for Qigong and 1 for Taping. Conclusions The 6 CPGs were developed up to now through a systematic development methodology and there were many recommendations for Korean medical treatments related to manual (Chuna) treatment, acupuncture and Qigong. However, the evidence for the side effects and risk factors of Korean medical treatments was scantly reflected in CPGs. Therefore, it is considered that balanced CPG with benefits and risks should be developed, covering Korean medical diagnosis, treatment and prognosis.

요통 환자 레지스트리 데이터를 통한 한의 보존적 치료 경향: 7개 한방병원 의무기록 분석 (Trends of Korean Medicine Conservative Treatment Using Low Back Pain Patients Registry Data: Analysis of Medical Records of 7 Korean Medical Hospital)

  • Objectives The purpose of this study is to investigate characteristics of low back pain patients and trends of Korean medicine conservative treatment effects of low back pain patients by registry data. Methods A total of 78 patients were analyzed, statistical analysis was performed using a generalized linear model. The comparison between groups was performed using independent two samples t-test or Wilcoxon rank sum test according to the distribution of normality. Before and after the treatment effects, ANOVA and Kruskal-Wallis method were used for the comparison, Sheffe method was used for the post test. Results The average age of hospitalized patients was 31.8 years old and out-patient department (OPD) patients was 44.9 years old. Lots of OPD patients and hospitalized patients were classified to 'Lumbago due to Coexist Qu Stagnation and Bloos Stasis'. Both OPD patients and hospitalized patients classified to 'Lumbago due to Coexist Qu Stagnation and Bloos Stasis' showed statistically significant decrease in RMDQ (Roland-Morris disability questionnaire) and NRS (numeric rating scale) on 1 month and 3 month after clinical research start date. Comparing the treatment effects of Chuna manual therapy, both Chuna group and non-Chuna group showed statistically significant decrease in RMDQ and NRS on discharge date, 1 month and 3 month after clinical research start date. Conclusions We expect that a larger scale registry study will be carried out to provide a higher level of basis for the Korean medical treatment of low back pain.
